The Effect Analysis on the Container Terminal Productivity according to Combination of YT Pooling and Dispatching Rules (이송장비 풀링(Pooling)과 우선순위 규칙(Dispatching rule) 조합에 따른 컨테이너 터미널 생산성 효과분석)

  • Today, container terminals are fiercely competing to attract an increasing number of containers. As a way to improve terminal productivity, this study proposes two dispatching rules for yard truck allocation priorities. First, Multi-Attribute Dispatching Rule(MADR) is an allocation method to calculate the weighted sum of multiple factors affecting container terminal productivity and priority them. Especially, the workload of the quay crane was considered one of the factors to reduce the residence time of the ship. Second, Cycling Dispatching Rule(CDR) is the effective way to increase the number of double cycles that directly affect terminal productivity. To identify the effects of combinations of pooling and dispatching, a comparative experiments was performed on 8 scenarios that combined them. A simulation environment has been developed for experiments and the results have demonstrated that the combination of terminal level pooling and Multi-attribute Dispatching could be an excellent combination in KPIs consisting of GCR and delayed departure of ships, etc.

Association between Thyroid-Related Hormones and Cognitive Function in Patients with Alzheimer's Disease and Mild Cognitive Impairment (알츠하이머병 및 경도인지장애 환자에서 갑상선 관련 호르몬과 인지기능과의 상관관계)

  • Objectives : The association between thyroid-related hormones and cognitive function has been controversial. The purpose of this study is to compare the levels of thyroid-related hormones in patients with Alzheimer's disease (AD) and mild cognitive impairment (MCI). Furthermore, we investigated the relationship between thyroid-related hormones and cognitive function. Methods : From January 2011 to December 2018, we retrospectively reviewed 105 patients who were diagnosed with AD and MCI by visiting a dementia clinic at Ilsan Paik Hospital. Thyroid-related hormones [triiodothyronine (T3), thyroxine (T4), and thyroid stimulating hormone (TSH)] was measured using chemiluminescent immunoassay. An independent sample t-test was performed to analyze the mean value of thyroid-related hormones in patients of AD and MCI. To investigate whether thyroid-related hormones correlate significantly with Global deterioration scale (GDS), Clinical dementia rating (CDR) and scores of each The Korean version of the consortium to establish a registry for Alzheimer's disease items, we conducted a partial correlation analysis with geriatric depression scale-Korean version (GDS-K) scores as covariates. Results : There was no significant difference in the mean serum T3, T4 and TSH levels between patients of the AD and the MCI, but the Construction Praxis Test (CPT) showed a significant positive correlation with the serum TSH concentration (p-value=0.004). Conclusions : In our study, the positive correlation between serum TSH level and the CPT associated with executive function was found to be helpful in understanding the association between thyroid-related hormones and the pathophysiology of dementia. Prospective studies in regard of the pathophysiology of thyroid-related hormones on cognitive function will be necessary in the future.

  • The temporal aggregate in temporal databases is an extension of the conventional aggregate to include the time on the range condition of aggregation. It is a useful operation for Historical Data Warehouses, Call Data Records, and so on. In this paper, we propose a structure for the temporal aggregation with multiple selection predicates, called the ITA-tree, and an aggregate processing method based on the structure. In the ITA-tree, we transform the time interval of a record into a single value, called the T-value. Then, we index records according to their T-values like a $B^+$-tree style. For possible hot-spot situations, we also propose an improvement of the ITA-tree, called the eITA-tree. Through analyses and experiments, we evaluate the performance of the proposed method.

Improving Safety-Related Knowledge, Attitude and Practices of Nurses Handling Cytotoxic Anticancer Drug: Pharmacists' Experience in a General Hospital, Malaysia

  • Background: An increasing trend of cytotoxic drug use, mainly in cancer treatment, has increased the occupational exposure among the nurses. This study aimed to assess the change of nurses' safety-related knowledge as well as attitude levels and subsequently to assess the change of cytotoxic drug handling practices in wards after a series of pharmacist-based interventions. Materials and Methods: This prospective interventional study with a before and after design requested a single group of 96 nurses in 15 wards actively providing chemotherapy to answer a self-administered questionnaire. A performance checklist was then used to determine the compliance of all these wards with the recommended safety measures. The first and second assessments took 2 months respectively with a 9-month intervention period. Pharmacist-based interventions included a series of technical, educational and administrative support measures consisting of the initiation of closed-system cytotoxic drug reconstitution (CDR) services, courses, training workshops and guideline updates. Results: The mean age of nurses was $32.2{\pm}6.19$ years. Most of them were female (93.8%) and married (72.9%). The mean knowledge score of nurses was significantly increased from $45.5{\pm}10.52$ to $73.4{\pm}8.88$ out of 100 (p<0.001) at the end of the second assessment. Overall, the mean practice score among the wards was improved from $7.6{\pm}5.51$ to $15.3{\pm}2.55$ out of 20 (p<0.001). Conclusions: The pharmacist-based interventions improved the knowledge, attitude and safe practices of nurses in cytotoxic drug handling. Further assessment may help to confirm the sustainability of the improved practices.

Germinal Center-independent Affinity Maturation in Tumor Necrosis Factor Receptor 1-deficient Mice

  • Germinal centers (GCs) have been identified as site at which the somatic mutation of immunoglobulins occurs. However, somatic mutations in immunoglobulins have also been observed in animals that normally do not harbor germinal centers. This clearly indicates that somatic mutations can occur in the absence of germinal centers. We therefore attempted to determine whether or not GCs exist in TNFR1-deficient mice, and are essential for the somatic mutation of immunoglobulins, using (4-hydroxy-3-nitropheny)acetyl-ovalbumin (NP-OVA). Both wild-type and TNFR1-deficient mice were immunized with NPOVA, and then examined with regard to the existence of GCs. No typical B-cell follicles were detected in the TNFR1-deficient mice. Cell proliferation was detected throughout all splenic tissue types, and no in vivo immune-complex retention was observed in the TNFR1-deficient mice. All of these data strongly suggest that no GCs were formed in the TNFR1-deficient mice. Although TNFR1-deficient mice are unable to form GCs, serological analyses indicated that affinity maturation had been achieved in both the wild-type and TNFR1-deficient mice. We therefore isolated and sequenced several DNA clones from wild-type and the TNFR1-deficient mice. Eight out of 12 wild-type clones, and 11 out of 14 clones of the TNFR-1-deficient mice contained mutations at the CDR1 site. Thus, the wild-type and TNFR1-deficient mice were not extremely different with regard to types and rates of somatic mutation. Also, high-affinity antibodies were detected in both types of mice. Collectively, our data appear to show that affinity maturation may occur in TNFR1-deficient mice, which completely lack GCs.

Analysis of Immunoglobulin λ Light Chain Repertoire in Systemic Lupus Erythematosus (루푸스 환자의 면역글로불린 λ 경쇄 레파토리 분석)

  • Background: Immunoglobulin (Ig) light chain repertoire has been implicated as a critical determinant in regulation of autoreactive B cells and production of pathogenic anti-DNA antibodies in systemic lupus erythematosus (SLE). We analyzed the impact of Ig ${\lambda}$ chain repertoire on development of autoimmunity in patients with SLE. Methods: We obtained genomic DNA from individual peripheral CD19+ B cells of 3 untreated active SLE patients, and amplified $V{\lambda}$ rearrangements from each single cell by polymerase chain reaction. Results: A total number of 208 $V{\lambda}J{\lambda}$ rearrangements were analyzed. Analyzed sequences included 158 productive rearrangements and 50 nonproductive rearrangements. The differences in $V{\lambda}$ gene usage in the productive and nonproductive repertoire of SLE patients were found compared to the non-autoimmune individuals. $V{\lambda}$ gene, 9A was significantly overrepresented in nonproducative repertoire of SLE patients (P=0.016). In the productive repertoire, $V{\lambda}$ genes, 3L and 1E were found more often in the SLE patients (P=0.001, P=0.043). When the productive and the nonproductive repertoires were compared, 9A was found significantly less in the productive repertoire in the SLE patients (P=0.000). There were no significant differences in the $J{\lambda}$ gene usage between SLE patients and non-autoimmune individuals, but $J{\lambda}2/3$ gene was the most frequently used in SLE, whereas $J{\lambda}7$ gene was the most frequently used in the normal subjects. In the productive SLE $V{\lambda}$ repertoire, 9.4% of the total sequences employed identical CDR3. It was particularly striking to find 7 identical versions of the 1G-$J{\lambda}2/3$ $V{\lambda}J{\lambda}$ rearrangements from one patient and 3 of the same sequence from another patient. Notably, identical $V{\lambda}$ junctions in the SLE patients utilized significantly more homologous joining compared to $V{\lambda}$ junctions of the normal adults (P=0.044). Conclusion: These data demonstrate regulation of ${\lambda}$ light chain expression in the SLE patients by selection of unique $V{\lambda}$ genes. Also, biased selection and clonal expansion of particular $V{\lambda}$ rearrangements are apparent in the SLE ${\lambda}$ repertoire.

The Effects of Sleep Apnea and Variables on Cognitive Function and the Mediating Effect of Depression (수면무호흡증과 수면변수가 인지기능에 미치는 영향과 우울증의 매개효과)

  • Objectives: This study aimed to analyze causality among sleep apnea, depression and cognitive function in patients with obstructive sleep apnea. Methods: We reviewed the medical records of 105 patients with sleep apnea and snoring who underwent overnight polysomnography (PSG). We analyzed various biological data, sleep variables (sleep duration and percentage) and respiratory variables [arousal index (AI), periodic leg movement index (PLM index), snoring Index (SI), mean SpO2, minimum SpO2, apnea-hypopnea index (AHI), and respiratory disturbance index (RDI)]. We also analyzed various data by sleep, cognition, and mood related scales: Pittsburgh sleep quality index (PSQI), Epworth sleepiness scale (ESS), snoring index by scale (SIS), Montreal Cognitive Assessment-Korean (Moca-K), Mini-mental State Examination-Korean (MMSE-K), clinical dementia rating (CDR), and Beck Depression Inventory (BDI). We analyzed causation among sleep, and respiratory, mood, and cognition related scales in obstructive sleep apnea patients. We analyzed the mediating effects of depression on sleep apnea patient cognition. Results: As Duration N1 increased and Total sleep time (TST) decreased, MOCA-K showed negative causality (p < 0.01). As BDI and supine RDI increased, causality was negatively related to MOCA-K (p < 0.01). As PSQI (p < 0.001) and SIS (p < 0.01) increased and as MMSE-K (p < 0.01) decreased, causality was positively related to BDI. BDI was found to mediate the effect of age on MOCA-K in patients with obstructive sleep apnea. Conclusion: Duration N1, total sleep time, BDI, and supine RDI were associated with cognitive function in obstructive sleep apnea patients. Depression measured by BDI partially mediated cognitive decline in obstructive sleep apnea patients.

Comparison of rainfall-runoff performance based on various gridded precipitation datasets in the Mekong River basin (메콩강 유역의 격자형 강수 자료에 의한 강우-유출 모의 성능 비교·분석)

  • As the Mekong River basin is a nationally shared river, it is difficult to collect precipitation data, and the quantitative and qualitative quality of the data sets differs from country to country, which may increase the uncertainty of hydrological analysis results. Recently, with the development of remote sensing technology, it has become easier to obtain grid-based precipitation products(GPPs), and various hydrological analysis studies have been conducted in unmeasured or large watersheds using GPPs. In this study, rainfall-runoff simulation in the Mekong River basin was conducted using the SWAT model, which is a quasi-distribution model with three satellite GPPs (TRMM, GSMaP, PERSIANN-CDR) and two GPPs (APHRODITE, GPCC). Four water level stations, Luang Prabang, Pakse, Stung Treng, and Kratie, which are major outlets of the main Mekong River, were selected, and the parameters of the SWAT model were calibrated using APHRODITE as an observation value for the period from 2001 to 2011 and runoff simulations were verified for the period form 2012 to 2013. In addition, using the ConvAE, a convolutional neural network model, spatio-temporal correction of original satellite precipitation products was performed, and rainfall-runoff performances were compared before and after correction of satellite precipitation products. The original satellite precipitation products and GPCC showed a quantitatively under- or over-estimated or spatially very different pattern compared to APHPRODITE, whereas, in the case of satellite precipitation prodcuts corrected using ConvAE, spatial correlation was dramatically improved. In the case of runoff simulation, the runoff simulation results using the satellite precipitation products corrected by ConvAE for all the outlets have significantly improved accuracy than the runoff results using original satellite precipitation products. Therefore, the bias correction technique using the ConvAE technique presented in this study can be applied in various hydrological analysis for large watersheds where rain guage network is not dense.

Hydrothermal Gold mineralization of the trabong district, vietnam : Mineralogical and geochemical study (베트남 짜봉(Trabong) 지역의 열수 금 광화작용 : 광물 및 지화학적 연구)

  • Hydrothermal gold deposits of the Trabong district in Vietnam occur as single-stage quartz $\pm$ calcite veins (0.3-1.2 m thick) which fill fault fractures in graphite-bearing gneiss and schist of the Chulai Complex and Kham Duc Formation of the Proterozoic age. Ore grades are 1.3 to 92.4 g/ton Au. Ore mineralogy is very simple, consisting mainly of pyrite with minor amounts of base-metal sulfides and electrum. Gold grains occur in two assemblages as follows: (1) early, Fe-rich (7.2-10.4 mole % FeS) sphalerite + electrum (50.4-64.3 atom % Au) assemblage occurring as inclusions in pyrite; (2) late, Fe-poor «4.7 mole % FeS) sphalerite + galena + electrum (47.6-81.7 atom % Au) assemblage occurring along fractures of pyrites. Based on fluid inclusion data and thermochemical considerations of ore mineral assemblages, ore minerals were formed at high temperatures (about $230^{\circ}C$ to $420^{\circ}C$) from $H_{2}O-CO_{2}(-CH_{4})$-NaCI fluids with the sulfur fugacity of about $10^{-6}$ to $10^{-10}$ atm. Fluid inclusion data also indicate that ore mineralization occurred mainly as a result of fluid unmixing accompanying $CO_2$ effervescence. Calculated oxygen and measured hydrogen isotope compositions of mineralizing waters (${\delta}^{18}O_{V-SMOW}$ values = 5.3 to 8.6$\textperthousand$, ${\delta}D_{V-SMOW}$ values = - 60 to - 52$\textperthousand$), along with the sulfur isotope compositions of vein sulfides (${\delta}^{34}S_{CDR}$ values = - 1.2 to 2.8$\textperthousand$) and carbon isotope compositions of inclusion $CO_2$ (${\delta}^{13}C_{PDB}$ values = - 4.7 to - 2.0$\textperthousand$) indicate that the high temperature (mesohypothermal) gold mineralization formed from a magmatic fluid.

Prostate Imaging-Reporting and Data System: Comparison of the Diagnostic Performance between Version 2.0 and 2.1 for Prostatic Peripheral Zone

  • Objective: To compare the diagnostic performance between Prostate Imaging-Reporting and Data System version 2.0 (PI-RADSv2.0) and version 2.1 (PI-RADSv2.1) for clinically significant prostate cancer (csPCa) in the peripheral zone (PZ). Materials and Methods: This retrospective study included 317 patients who underwent multiparametric magnetic resonance imaging and targeted biopsy for PZ lesions. Definition of csPCa was International Society of Urologic Pathology grade ≥ 2 cancer. Area under the curve (AUC), sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy for csPCa were analyzed by two readers. The cancer detection rate (CDR) for csPCa was investigated according to the PI-RADS categories. Results: AUC of PI-RADSv2.1 (0.856 and 0.858 for reader 1 and 2 respectively) was higher than that of PI-RADSv2.0 (0.795 and 0.747 for reader 1 and 2 respectively) (both p < 0.001). Sensitivity, specificity, PPV, NPV, and accuracy for PI-RADSv2.0 vs. PI-RADSv2.1 were 93.2% vs. 88.3% (p = 0.023), 52.8% vs. 76.6% (p < 0.001), 48.7% vs. 64.5% (p < 0.001), 94.2% vs. 93.2% (p = 0.504), and 65.9% vs. 80.4% (p < 0.001) for reader 1, and 96.1% vs. 92.2% (p = 0.046), 34.1% vs. 72.4% (p < 0.001), 41.3% vs. 61.7% (p < 0.001), 94.8% vs. 95.1% (p = 0.869), and 54.3% vs. 78.9% (p < 0.001) for reader 2, respectively. CDRs of PI-RADS categories 1-2, 3, 4, and 5 for PI-RADSv2.0 vs. PI-RADSv2.1 were 5.9% vs. 5.9%, 5.8% vs. 12.5%, 39.8% vs. 56.2%, and 88.9% vs. 88.9% for reader 1; and 4.5% vs. 4.1%, 6.1% vs. 11.1%, 32.5% vs. 53.4%, and 85.0% vs. 86.8% for reader 2, respectively. Conclusion: Our data demonstrated improved AUC, specificity, PPV, accuracy, and CDRs of category 3 or 4 of PI-RADSv2.1, but decreased sensitivity, compared with PI-RADSv2.0, for csPCa in PZ.
