지역사회중심 재활에서 물리치료사의 참여 (A Participation of Physical Therapist for Community Based Rehabilitation)

  • The physical therapist's participation in community based rehabilitation(CBR) is necessary, in a variety of ways, to ensure the disabled quality service. Although CBR needs the Physical Therapist's help, participation is limited due to unstable CBR policy, and because there is a lack of financial support, skilled Physical Therapist's are usually not hired. Physical Therapist's themselves do not seem to completely understand this. The experts active participation is needed for effective rehabilitation service. Therefore, the trained Physical Therspist's participation is absoutely necessary is CBR policy if it is even to provide completely effective service.

  • The percentage of postnatal disabled people and elderly population is rapidly increased by the industrialization, and the improvement of medical technology. The disparity between the city and rural area is in the way to disabled people who need rehabilitation. To solve these problems, Community Based Rehabilitation(CBR) is suggested by WHO. In this study, at first, the CBR model considering the specific characteristics of AnSan City are presented. Secondly, the first, the second, and the third rehabilitation facilities in AnSan City are classified by the CBR model. Thirdly, the estimation of optimum space for physical therapy and occupational therapy at rehabilitation center and public health center are sugested, which are the second level rehabilitation facilities. There are some differences in the distribution of population between new and old area in AnSan City. In other words, the subject population of CBR program in old area is relatively higher than that of new area. So, the CBR program for AnSan City must reflect their regional characteristics. In this study, the therapeutic boundaries of second level rehabilitation facilities is decided through the survey of physical therapists and occupational therapists in general hospitals and nursing homes.

부산·울산·경남 작업치료사의 지역사회중심재활에 대한 인식도 조사 및 활성화 방안 (A Study on Recognition and Activation Plan of Occupational Therapists for Community Based Rehabilitation in Pusan·Ulsan·Gyeongnam Province)

  • 목적 : 부산 울산 경남 지역의 장애인복지관과 병원에서 근무하는 작업치료사를 대상으로 지역사회중심재활에 대한 인식도와 필요성을 조사하여 향후 지역사회중심재활에서 작업치료사의 역할을 구축하고, 지역사회중심재활 참여에 대한 활성화 방안을 모색하고자 한다. 연구방법 : 2010년 12월 1일부터 2011년 5월 4일까지 한국장애인복지관협회에 등록된 부산 울산 경남 지역에 소재한 장애인복지관에 근무하는 작업치료사 23명과 부산 지역에 소재한 병원에서 근무하는 작업치료사 49명을 대상으로 설문을 실시하였다. 통계분석은 SPSS12.0 통계 패키지 프로그램을 이용하였다. 결과 : 지역사회중심재활에 대한 작업치료사의 인식도와 필요도는 두 집단 모두 높은 것으로 나타났고 두 집단 간 유의한 차이는 없었다(p>.05). 또한 지역사회중심재활 중 작업치료사가 필요하다고 생각되는 영역에 대해 재활상담 및 정보제공, 가정방문재활치료, 직업재활, 재활기구대여, 장애예방교육으로 조사되었으며, 지역사회중심재활에서 작업치료사의 활성화를 위해서는 인력 수급과 국가적 차원의 법적/제도적 장치 마련과 관련 전문가와의 팀웍 조성 등이 필요한 것으로 나타났다. 결론 : 본 연구는 지역사회중심재활에 대한 작업치료사들의 인식도와 작업치료사들의 필요성은 높은 것으로 나타났으나, 현재 시행되는 기관은 부족한 것으로 나타났다. 따라서 지역사회중심재활에 있어 작업치료의 활성화를 위한 법적/제도적 장치 마련이 시급하다.

지역사회중심재활에 대한 전국 보건소 물리치료사들의 인식 조사 (An Investigation on the Consciousness for the Community Based Rehabilitation by Public Health Center's Physical Therapy in Korea)

  • The community based rehabilitation(CBR ) of public health center(PHC) has been considered as the one of the alternative medical services which is adequate to our realities. The purpose of this study is to serve basic data in order to strengthen the CBR of PHC. This study was investigated on 191 samples of 95 physical therapists, 35 physicians. and 61 nurses who work at PHC. Data were collected for 40 days from January 10 to February 20,2001. The results are as follows: First, the recognition level of CBR of sample specialists was very high as about 85% of the total number studied. Also the number of physical therapists at PHC, who had intention of participation to CBR, was very high as almost 92%. Second, specialists more than a half of sample group thought that two therapists and one assistance per one PHC were needed for serving with sufficient CBR services. finally, various additional strategies were proposed by sample specialists. in order to activate the CBR of PHC. it is necessary to enact the related laws. to stimulate teamworks between rehabilitation specialists, and to establish so called 'public health therapists'. Also we must reinforce the CBR instruction for PHC therapists and the education for health center officer.

중심축은 무엇인가? 지역사회중심재활(CBR)을 중심으로 한 NGO 활동전략 (What is Pivot? Implementation Strategies in NGOs under CBR)

  • 많은 NGO 단체들은 장애인들의 재활과 권리를 보장하기 위하여 오랜시간동안 중요한 역할을 수행해 왔다. 그러나 많은 단체들이 장애인들의 권익과 복지향상을 위한 장기간의 자발적인 노력에도 불구하고 개발도상국의 장애인들은 여전히 힘들고 열악한 상황에 있다. 이에 본 논문은 세계적으로 많은 NGO단체들이 활동하고 있는 인도를 중심으로, 그들의 NGO 활동이 보다 효과적인 결과를 가져오게 올 수 있도록 활동 전략을 제시하되, 지역사회중심재활(CBR) 패러다임을 적용하고자 한다. 인도와 같은 개발도상국의 장애인을 위한 재활전략으로 CBR이 효과적인라는 많은 연구들이 있어 왔다. 이를 위하여 본 논문은 우선 인도의 장애인에 대한 사회적 인식과 주요 현황, 그리고 재활서비스에 대한 정보를 제공하였다. 그리고 그들의 권리를 지속적으로 보장하기 위한 중요한 전략 중 하나인 CBR을 중심으로, NGO 실무자 및 자원봉사자의 역할, 지역사회의 리더 및 장애인 당사자와 관련단체들의 역할에 대한 몇가지 제언을 하였다.

Is the Legal Basis for Physical Therapist's Home-Based Rehabilitation Appropriately Prepared?

  • Purpose: This study examined the legal basis related to a physical therapist's home-based rehabilitation. Methods: The policy data were referenced to the comprehensive plan for national health promotion and the guidebook Community Health Promotion Project issued by the Korea Health Promotion Institute and other institutes. The legal data were referenced to the Bill information system on the National Assembly website. Results: The physical therapist's home-based rehabilitation did not have a legal basis in the community-based rehabilitation project (CBR) of the HP 2020 project. On the other hand, according to the Home health care part of the community health promotion project that began in 2013, physical therapists were allowed to play a role from 2020 under Article 16-2 (Public Officials in Exclusive Charge of Visiting Health Management) of the REGIONAL PUBLIC HEALTH ACT. Conclusion: Policies and laws are being developed in the field of healthcare, but the necessary policies and laws in the field of rehabilitation are still insufficient. A bill to rehabilitate in a variety of fields and spaces by modifying the scope of work of physical therapists will be needed.

Effects of Community-based Rehabilitation Programs on Activities of Daily Living, Quality of Life and Assistive Technology Satisfaction for Disabled Adults in South Korea

  • The purpose of this study was to examine the effects of community-based rehabilitation (CBR) program on activities of daily livings (ADLs), quality of life and assistive technology satisfaction for disabled adults in South Korea. Fourteen community-dwelling subjects were participated in a home-based rehabilitation program which performed once a month for 2 months. The outcome measures included Modified Barthel Index (MBI) for assessing ADLs, EuroQol five-dimensions questionnaire (EQ-5D) for quality of life (QoL) and Quebec User Evaluation of Satisfaction with assistive Technology 2.0 (QUEST 2.0) for assistive technology. Significant improvements in ADLs, quality of life and assistive technology satisfaction were observed after the program. This study indicated that community-based rehabilitation program is an effective method for improving ADLs, quality of life and assistive technology satisfaction for adults with disabilities.

경기도 보건소 물리치료사들의 지역사회심재활에 대한 인식 (A Recognition for Community Based Rehabilitation by Public Health Center Physical Therapists in Kyoungki Province)

  • To survey the recognition of Community Based Rehabilitation (CBR) by Public Health Center(PHC) Physical Therapists in Kyoungki Province. This study analysed general characteristic of Physical Therapists and congnition, participation, education, problems and requireanents for CBR. The objects of this study were 37 therapists who work at 27 of 39 PHC in Kyoungki province and data was collected for 20days from Jan. 12 to 31. 1998 and the results computed from 32 responses. First, Female therapists outnumber males. The age category 30 to 35 old represneted $56.2\%$ of the total number studied. $59.4\%$ of therapists had worked more than 7 years and therapists usually$(46.9\%)$ worked from 6 month to 2 years at the PHC. Junior college graduates were $84.4\%$, and respondents with family responsibilirt were $65.6\%$. Second, Recently pain control has become the major requirment of patiants at PH$(53.1\%)$. If they offered expaneded services - a rehabilitation program $(53.1\%)$, home vistation $(31.3\%)$. when asked about a working guide book, nobody had one. Third, $68.7\%$ of therapists were aware of the present level of CBR, $40.9\%$ of them had gotten an information from educated colleagues, and $68.7\%$ of therapists were aware of the present level of CBR, $86.4\%$ of therapists felt responsible to provide rehabilitation Fourth, For CBR, $93.7\%$ had positive ideas in which they could participate. $73.3\%$ wished to composed a team of various rehabilitational specialists. $73.3\%$ wished for the meeting day to be flexible and $86.6\%$ desired to meet once or twice a week. Fifth, Almost all$(96.9\%)$ desired more educational opportunity about CBR. Example, once or twice a year$(56.2\%)$ and training period of 4-7 days $(43.7\%)$. They wanted to learn theory and practice together$(74.2\%)$ and $74.2\%$ wanted the mational multi-rehabilitation center as a educational managing organization. Sixth, The most important requirment for accomplishing CBR by physical therapists was job security and an adequate number of therapists$(84.4\%)$ Required number of physical therapists in the PHC was 3 $(43.7\%)$. The cooperative agency should be the district goverment organization $(56.2\%)$. Factors inhibiting the execution of CBR were lack of administrative support, physical therapists, and equipments for pain control and for therapeutic exercise.

Evaluation of the Home-based Rehabilitation Program Based on CBR Model Through In-depth Interviews With Visiting Physical Therapists

  • Purpose: This study was conducted in order to explore home visiting therapists' self-perceived role, problems, and supplement points of the home-based rehabilitation (HR) program based on the community-based rehabilitation (CBR) model. Methods: Four home visiting physical therapists, who conducted the HR program, participated in this study. After completion of the HR program, in-depth interviews were conducted using a semi-structured questionnaire for participants to explore their self-perceived role, problems, and supplement points of the HR program. Results: Participants regarded the role of the HR program as for "the linkage between recipient & society", "maintenance & improvement of recipients' physical function", and "education of recipient about the way of self rehabilitation". The problems and supplement points were derived from all phases of the HR program, including "human & material resources", "training program for human resources for HR service", "selection of recipient of HR service", "contents of the intervention for HR service", and "duration of HR service". Conclusion: These findings indicate that participants well recognized the intention of the HR program based on the CBR model, and suggest that high-quality human resources with rich expertise and experiences, a training program for HR service led by experts, and selection of recipients led by experts are necessary for an effective HR program. In addition, strategies and capacities specified to HR service should be identified, and applied to the training program for human resources for HR service. The results of this study could provide useful information when the government decides on the next guideline for home visiting health service.

지역사회중심재활에 대한 거점 보건소 물리치료사들의 인식도 조사 (A Recognition Research of Physical Therapist in the Strong Point Public Health Center about Community Based Rehabilitation)

  • This research is due to know the transformation after Physical Therapist(PT) was educated the Comunity Based Rehabilitation(CBR) at the strong point by Public Health Center(PHC) of the CBR. It was the education of physical therapist of strong point by PHC of the CBR from April 17 to April 29 of 2000 in National Rehabilitation Center. The following is the result of Questionnaire educated 39 peoples. First. male and female are same rate. average age, 33 years old, In the position, regular position $86.3\%$. irregular position $13.2\%$. Second, the $76.3\%$was only working PHC one PT, thc main patients were $73.7\%$ of chronic degeneration and over 65 years old who are treated. they are treating $60.5\%$ under 20person in each day. Third. We asked three Questionnaire. these are, first need CBR education. second, participating of the PT of the CBR projects, third. suggesting the CBR the another in the PHC. The most of $97.4\%$ was answered the Positive. $89.5\%$ agreed that they can offer physical therapy to handicapped man without responsibility. And all of these answered $100\%$ that they want to ewer the physical therapy when they meet the Handicapped. Are you can aviable to assess the part of handicapped they answered $77.1\%$, to therapeutic exercise $52.6\%$, to transfer and family education $47.4\%$, to gait training and family education $15.3\%$. In the welfare policy. introduce job and rehabilitation information side, having answer $44.7\%$. It was negative. In the filed of social welfare also is it possible for them to educate for the another medical staffs and volunteers is PHC. It was $52.6\%$. In Conclusion, needs and participant's rate was high all the people but it was the less in professional physical therapy.
