Survey Analysis of the Farm Machinery Accidents

  • A review of accidents involving farm machinery, which occurred in Korea during 2002, shows that $90.7\%$ of farm-work accidents and $9.3\%$ of traffic accidents were linked to agricultural machinery. The frequency of accidents per annum amounts to 72.2 out of 10,000 vehicles, $94.4\%$ of which relates to tillers and tractors. An analysis of the causes of the accidents revealed that $49.1\%$ of farm-work accidents were due to the tiller driver's fault or error. In the case of traffic accidents, $83\%$ of farm machinery mishaps were found to be due to violation of traffic regulations while $60.4\%$ of the farm machinery damage resulted from faulty driving or lack of safe driving practices. The average casualty count of farm machinery-related accidents is 1.39 persons per case, which consist of 0.07 deaths, 0.74 serious injuries, and 0.58 slight injuries.

우리나라 주택 내에서 발생하는 비의도적 손상의 양상 (Patterns of Unintentional Domestic Injuries in Korea)

  • Objectives: To investigate the patterns of unintentional home injuries in Korea. Methods: The study population was 12,382,088 people who utilized National Health Insurance services due to injuries (main diagnosis codes S00 to T28) during 2006. Stratified samples(n=459,501) were randomly selected by sex, age group and severity of injury. A questionnaire was developed based on the International Classification of External Causes of Injury and 18,000 cases surveyed by telephone were analyzed after being projected into population proportionately according to the response rates of their strata. Domestic injury cases were finally included. Results: Domestic injuries (n=3,804) comprised 21.1% of total daily life injuries during 2006. Women were vulnerable to home injuries, with the elderly and those of lower income (medical-aid users) tending to suffer more severe injuries. Injury occurred most often due to a slipping fall (33.9%), overexertion (15.3%), falling (9.5%) and stumbling (9.4%), with severe injury most often resulting from slipping falls, falls and stumbles. Increasing age correlated with domestic injury-related disability. Conclusions: The present findings provide basic information for development of home injury prevention strategies, with focus on the elderly.

Imaging of Abusive Head Trauma : A Radiologists' Perspective

  • Abusive head trauma (AHT) is the most common and serious form of child abuse and a leading cause of traumatic death in infants and young children. The biomechanics of head injuries include violent shaking, blunt impact, or a combination of both. Neuroimaging plays an important role in recognizing and distinguishing abusive injuries from lesions from accidental trauma or other causes, because clinical presentation and medical history are often nonspecific and ambiguous in this age group. Understanding common imaging features of AHT can increase recognition with high specificity for AHT. In this review, we discuss the biomechanics of AHT, imaging features of AHT, and other conditions that mimic AHT.

졸음운전과 교통사고 (Drowsy Driving and Traffic Accidents)

  • Drowsy driving is a major cause of automobile crashes and can lead to more serious injuries than other causes of traffic accidents. Factors increasing the risk of drowsy driving and related crashes include sleep loss, late night driving, untreated or unrecognized sleep disorders, use of sedating medications and consumption of alcohol. Young people, especially young males, shift workers, and people with untreated sleep apnea syndrome and narcolepsy are well known as the population groups at highest risk. To prevent drowsy driving and its consequences, getting adequate and quality sleep is both easier and much more successful than any remedial measure. Other helpful behaviors include avoidance of alcoholic beverages and limiting late night driving. Taking a short nap or consuming caffeine can make a short-term difference in driving alertness. In addition, information should be actively provided to the public about the importance of sleep disorders and their consequences. To reduce injuries and death caused by drowsy driving, it is a prerequisite to increase public awareness that drowsy driving can cause serious automobile crashes and has morbidity and mortality rates as high as those of drunk driving.

외상성 십이지장 손상의 수술적 치료: 단일 기관 연구 (Surgical Management of Duodenal Traumatic Injuries: A Single Center Study)

  • Purpose: Abdominal trauma rarely causes injuries involving duodenum. But, it is associated with higher rate of the complication and mortality than other abdominal injuries. There are many options for the management of duodenal injuries. Herein we are to review our experiences and find out the risk factors related to the morbidity and the mortality in traumatic duodenal injuries. Methods: The medical records of total 25 patients who managed by surgical managements and survive more than 48 hours were conducted from January 2006 to December 2012. The clinical characteristics, treatments, and outcomes are reviewed. Results: Among 25 patients, most of them (n=17, 68.0%) were managed by the pyloric exclusion and the gastrojejunostomy. The $3^{rd}$ portion is the most injured site (n=15, 60.0%), and the majority exhibited grade 2 severity (n=14, 56.0%). Most of patients had blunt abdominal traumas (n=23, 92.0%) so that many of them (n=14, 56.0%) had other combined abdominal injuries. The mean ISS is $11.5{\pm}6.2$. The surgery related mortality rate was 28.0%. There was no statistical significance between each factors and the mortality except leakage (p=0.012). But, we could find some trends about traumatic duodenal injuries in this study. The mortality rates of them who older than 55 years were higher than others. And, all 3 patients who delayed the operation more than 24 hours after the trauma had some complications or died. Also, the patients who had the $2^{nd}$ portion injury, grade 3 injury, or combined abdominal injury were less survived. Conclusion: Duodenal injury is related to high rate of morbidity(47.8%) and mortality(28.0%). Age, portion of injury, OIS grade, ISS>15, combined intra-abdominal operation, and trauma to operation time over 24 hrs have some trend with attribution to mortality. Especially leakage of duodenal injury is related to mortality.

  • Recently, industrial accidents have been getting great damage to an enterprise management directly and indirectly, the industrial injuries of conventional type are decreasing : on the other hand, musculoskeletal injuries are trending to a rapid increase. This shows that most of carrying works have been performing in almost all production process and convey objects, machine equipment and work method. Then, they are made by unfitted design which doesn't consider physical condition of workers, so it causes them to bring about forceful motion. In this paper, it was used NIOSH standard the data of spot. The ergonomic design of machine equipment and the evaluation of biomechanical compression force at theL_5/S_1$and back power, intend to provide the basis which can be applied, compared, and analyzed between before process improvement and after.

만성 발목 불안정증 수술 후의 지속적인 발목 통증 (Residual Pain after Operative Treatment for Chronic Ankle Instability)

  • Patients with chronic lateral ankle instability often experience a range of associated injuries. The well-known comorbidities include intra-articular pathologies (osteochondral lesion, soft tissue or bony impingement, and loose body), peroneal tendon pathologies, neural injuries, and other extra-articular pathologies. Surgeons should have a high index of suspicion for these associated pathologies before operative intervention, correlate the clinical findings, and plan the treatment. Despite the restoration of ankle stability following ligament repair or reconstruction surgery, a high prevalence (13%~35%) of postoperative residual pain has been reported. This pain can negatively affect the clinical outcomes and patient satisfaction. This study examined the causes of persistent pain after surgical treatments for chronic ankle instability.

흉부손상 76례에 대한 임상적 관찰 (A clinical evaluation of 76 chest injuries)

  • A clinical evaluation was performed on 76 cases of chest injury experienced at department of Chest Surgery, Capital Armed Forces General Hospital during the past 3 years period from January 1981 to August 1983. 1.The most common cause of the chest trauma was gun shot by which 26 cases were injured among 44 cases [57.9%] of penetrating injury. Remaining 32 cases [42.1%] were injured by non-penetrating blunt trauma. 2.Hemopneumothorax was observed in 60 cases [78.9%], those were caused by both penetrating [65%] and non-penetrating [35%] injuries. 3.Rib fracture was found in 58.7% of total cases and with rib fracture, clavicle fracture was combined at 19.6% and sternal fracture, at 8.7%. 4.Most common symptoms were chest pain and dyspnea, and most common signs were breath sound diminution and subcutaneous emphysema. 5.Common site of rib fracture was from 4th rib to 8th rib [69.4%]. 6.In 58 cases [76.3%], patients were treated with operation including open thoracotomy [25 cases]. 7.Overall mortality was 5.3%[4 cases] and causes of death were septic shock and respiratory failure.

악안면 골절에 관한 임상적 고찰 (Clinical Study of facial bone fracture)

  • The purpose of this study was to investigate the fractures of facial skeleton ; mandible, maxilla, zygoma Clinically, we observed 413 patients with facial bone fractures treated at the department of dentistry, Koryo General Hospital from Jan. l989 to Dec. I991. This results ere as follows : 1. The most common fracture was occured in the mandible (63.7%) and the symphysis was occured most freguently(30.4%) 2. The most prevalent age was twenties(37.5%) 3. Main causes was traffic accident(21.7% ) 4. The ratio of male to female was about 9.3 : 1 5. Most combined injuries was head injuries. (54.6%) 6. The most frequent job was a company employee. 7. There were the highest frequency in March, and the lowest frequency in February.

한국 노인의 넘어짐과 연계된 인체손상에 대한 이해와 예방: 체계적 문헌 고찰 (Understanding and Prevention of Fall-related Injuries in Older Adults in South Korea: A Systematic Review)

  • Background: Fall-related injuries in older adults are a major health problem, and the risks and mechanisms of these injuries should be affected by race, culture, living environment, and/or economic status. Objects: Research articles have been systematically reviewed to understand fall-related injuries in older adults in South Korea. Methods: 128 published research papers have been found through the Korea Citation Index and the Korean Studies Information Service System, and reviewed in various perspectives, including incidents, fall death rates, medical costs, causes, injury sites and types, locations where falls occurred, prevention strategies, scholarly fields interested in fall injuries, and the role of physical therapy. Results: Fall-related injuries were found to be more common in women than in men, and the number of incidents increased with age, with the highest rate found in individuals over 85 years old. Risk of fall injury was associated with education level, comorbidities, and fear of falling. Common places where falls occurred included the bathroom, living room, stairs, and hallway. Common types of injury included bruises, fractures, and sprains in the lower extremities. Intervention strategies included exercise programs, education, and protective clothing. Scholarly fields interested in fall-related injuries in older adults included medicine, nursing, physical therapy, occupational therapy, physical education, pharmacology, oriental medicine, biomedical engineering, design, clothing, and textiles. Physical therapy intervention using proprioceptive neuromuscular facilitation has been used to improve one's balance. Conclusion: Any movement during the activities of daily living can lead to a fall. Physical therapists are highly educated to analyze human movements and should be involved in more research and practices to solve fall-related injuries in older adults.
