PURPOSE. To determine the extent of treatment traces, the roughness depth, and the quantity of titanium nitride (TiN) removed from the surface of CAD/CAM abutments after treatment with various instruments. MATERIALS AND METHODS. Twelve TiN coated CAD/CAM abutments were investigated for an in vitro study. In the test group (9), each abutment surface was subjected twice (150 g vs. 200 g pressure) to standardized treatment in a simulated prophylaxis measure with the following instruments: acrylic scaler, titanium curette, and ultrasonic scaler with steel tip. Three abutments were used as control group. Average surface roughness (Sa) and developed interfacial area ratio (Sdr) of treated and untreated surfaces were measured with a profilometer. The extent of treatment traces were analyzed by scanning electron microscopy. RESULTS. Manipulation with ultrasonic scalers resulted in a significant increase of average surface roughness (Sa, P<.05) and developed interfacial area ratio (Sdr, P<.018). Variable contact pressure did not yield any statistically significant difference on Sa-values for all instruments (P=.8). Ultrasonic treatment resulted in pronounced surface traces and partially detachment of the TiN coating. While titanium curettes caused predominantly moderate treatment traces, no traces or detectable substance removal has been determined after manipulation with acrylic curettes. CONCLUSION. Inappropriate instruments during regular plaque control may have an adverse effect on the integrity of the TiN coating of CAD/CAM abutments. To prevent defects and an increased surface roughness at the transmucosal zone of TiN abutments, only acrylic scaling instruments can be recommended for regular maintenance care.

PURPOSE. The aim of this study was to evaluate the interface accuracy of computer-assisted designed and manufactured (CAD/CAM) titanium abutments and implant fixture compared to gold-cast UCLA abutments. MATERIALS AND METHODS. An external connection implant system (Mark III, n=10) and an internal connection implant system (Replace Select, n=10) were used, 5 of each group were connected to milled titanium abutment and the rest were connected to the gold-cast UCLA abutments. The implant fixture and abutment were tightened to torque of 35 Ncm using a digital torque gauge, and initial detorque values were measured 10 minutes after tightening. To mimic the mastication, a cyclic loading was applied at 14 Hz for one million cycles, with the stress amplitude range being within 0 N to 100 N. After the cyclic loading, detorque values were measured again. The fixture-abutment gaps were measured under a microscope and recorded with an accuracy of ${\pm}0.1{\mu}m$ at 50 points. RESULTS. Initial detorque values of milled abutment were significantly higher than those of cast abutment (P<.05). Detorque values after one million dynamic cyclic loadings were not significantly different (P>.05). After cyclic loading, detorque values of cast abutment increased, but those of milled abutment decreased (P<.05). There was no significant difference of gap dimension between the milled abutment group and the cast abutment group after cyclic loading. CONCLUSION. In conclusion, CAD/CAM milled titanium abutment can be fabricated with sufficient accuracy to permit screw joint stability between abutment and fixture comparable to that of the traditional gold cast UCLA abutment.

All-on-4 개념에 의한 임플란트 식립은 골이식을 피하면서 임플란트를 매식할 수 있어 유리하다. 일반적으로 경사된 임플란트에는 기성 경사형 지대주를 사용해왔다. 본 임상증례에서는 computer-aided design and computer-aided manufacturing (CAD/CAM)으로 제작된 지대주를 사용하였다. 본 증례는 all-on-4 개념에 의해 임플란트를 매식하고 CAD/CAM titanium 지대주를 제작한 다음 CAD/CAM zirconia 고정성 보철물로 수복하여 좋은 결과를 얻었기에 보고하고자 한다.

PURPOSE. The objective of the study was to analyze the impact of cement, bonding pretreatment, and ceramic abutment material on the overall color results of CAD-CAM ceramic crowns bonded to titanium-based hybrid abutments. MATERIALS AND METHODS. For single implant restoration of a maxillary lateral incisor a total of 51 CAD-CAM-fabricated monolithic lithium disilicate crowns were fabricated and subsequently bonded onto 24 lithium disilicate Ti-base abutments, 24 zirconia Ti-base abutments and 3 resin abutment replicas as a control group. The 48 copings were cemented with three definitive and one provisional cement on both grit-blasted and non-blasted Ti-bases. The color of each restoration and surrounding artificial gingiva was measured spectrophotometrically at predefined measuring points and the CIELAB (ΔEab) color scale values were recorded. RESULTS. The color outcome of ceramic crowns bonded to hybrid abutments and soft tissues was affected differently by cements of different brands. Grit-blasting of Ti-bases prior to cementing CAD-CAM copings affected the color results of allceramic crowns. There was a significant difference (P = .038) for the median ΔE value between blasted and non-blasted reconstructions at the cervical aspect of the crown. Full-ceramic crowns on zirconia Ti-base abutments exhibited significantly lower ΔE values below the threshold of visibility (ΔE 1.8). In all subcategories tested, the use of a highly opaque temporary cement demonstrated the lowest median ΔE for both the crown and the artificial gingiva. CONCLUSION. Various cements, core ceramic materials and airborne particle abrasion prior to bonding can adversely affect the color of Ti-base supported ceramic crowns and peri-implant soft tissue. However, zirconia CAD-CAM copings and an opaque cement can effectively mask this darkening.
In the past, restoration of implant crown, ready-made abutment produced by implant manufacturer could only be used. Using straight, angled abutment, there was a limit in adaptation multiple implants. Recently, with the development of implant and CAD/CAM technology, CAD/CAM customized abutment use has become possible which is different from the past when restoration was possible with only prefabricated abutment. Not only it makes emergence profile possible which is similar to natural teeth, but also it makes insertion path possible on CAD in multiple implant restorations. However, on anterior teeth which dental esthetics is very important, another restorations which are formed with natural colored gingiva area could be required. Titanium-based zirconia prostheses which have titanium connection and zirconia structure from 1mm above fixture platform are alternative. Therefore, the purpose of this review is to analyze the characteristics, advantages and disadvantages of the abutment which is used in multiple implant restorations, and to choose right abutment when clinical trials.
연구 목적: 본 연구는 국내 개발된 맞춤형 임플란트 지대주와 기성 임플란트 지대주의 반복 하중 전과 후의 나사 풀림 토크 값을 측정, 비교하여 나사 결합부 안전성에 차이가 있는지를 알아보고자 하였다. 연구 재료 및 방법: 총 12개의 임플란트 고정체(Implantium, Dentium Co., Seoul, Korea)를 알루미늄 원통에 레진으로 고정하고, 그것을 각각 4개씩, 3개의 군으로 나눠서 각 군별로 기성 티타늄 지대주(Implantium, Dentium Co., Seoul, Korea), 캐드캠 맞춤형 티타늄 지대주(Myplant, Raphabio Co., Seoul, Korea), 그리고 금속 연결부가 있는 캐드캠 맞춤형 지르코니아 지대주(Zirconia Myplant, Raphabio Co., Seoul, Korea)를 제작하여 임플란트에 연결하였다. 금속관은 지대주에 맞춰서 티타늄을 밀링해서 제작한뒤 지대주에 합착하였다. 반복 하중을 가하기 전에 지대주 나사를 30 Ncm 토크로 조이고 약 30분 후 나사의 초기 풀림 토크값을 측정하였다. 그 후 반복 하중을 30 N에서 120 N의 싸인 곡선을 이루는 압축력으로 2 Hz의 빈도로 50만 싸이클을 가하고 하중 후 풀림 토크값을 측정하였으며, 풀림 토크 상실률을 구하여서 군끼리 비교하였다. 나사의 풀림 토크값의 상실률의 비교를 위해 Kruskal-Wallis test를 이용해서 유의수준 .05에서 분석하였다. 결과: 반복하중 후 나사 풀림 상실률에서 기성 티타늄 지대주 군에 비해 맞춤형 티타늄 지대주는 높은 값을 보였으나 유의성 있는 차이를 보이지 않았다($P$>.05). 반면, 맞춤형 지르코니아 지대주는 기성 티타늄 지대주에 비해 유의하게 높은 값을 보였다($P$=.014). 결론: 본 실험의 한계 내에서 캐드캠 맞춤형 티타늄 임플란트 지대주는 기성 임플란트 지대주 에 비해 나사 안정성이 떨어진다고 볼 수 없다. 반면, 캐드캠 맞춤형 지르코니아 지대주 는 기성 임플란트 지대주에 비해 나사 안정성이 다소 떨어진다고 볼 수 있다.
시멘트 유지 임플란트 지지 수복물은 치과 치료에 일상적으로 사용된다. 임플란트와 지대주 나사연결의 안정적인 견고함과 골유합 임플란트의 높은 생존율로 인해 높은 강도를 가진 시멘트의 사용이 점차 늘어나고 있다. 그러나 CAD/CAM을 이용하여 제작한 금속 코핑의 유지력에 대해 이용할 수 있는 임상 자료가 없다. 네 종류의 시멘트를 이용하여 티타늄 지대주와 CAD/CAM을 이용하여 제작한 금속 코핑의 유지력을 평가하는 것과 시멘트 유지력에 대한 sandblasting의 효과를 알아보고자 함이다. 40개의 티타늄 지대주 블록(Innovium, 세라젬바이오시스, 대한민국)을 제작하여 네 그룹으로 나누었다. 40개의 금속 코핑을 CAD/CAM을 이용하여 제작하고 금속 코핑의 교합면 상방부에 유지력 테스트를 위해 구멍을 형성하였다. 네 종류의 시멘트는 Fujicem(Fuji, Japan), Maxcem Elite(Kerr, USA), Panavia F2.0(Kurarary, Japan), Superbond C&B(Sunmedical, Japan)이다. 금속 코핑과 티타늄 지대주를 제조사의 지시대로 시멘트 혼합하여 합착하고 100% 습도 하에 37도에서 24시간 동안 보관 후 유지력 테스트하였다. 유지 실패가 발생하는 힘을 newton으로 기록하고 실패의 양상 또한 기록하였다. 유지력의 평균과 표준 편차를 ANOVA와 Paired t-test 로 통계분석하였다. Panavia F2.0이 Fujicem과 Maxcem Elite보다 통계적으로 유의하게 높은 유지력을 보였다(p<0.05). Sandblasting이 모든 시멘트에서 유의하게 유지력을 증가시켰다(p<0.05). 유지력 실패의 양상은 대부분의 시멘트가 금속 코핑 내부에 남아있는 접착성 실패였다. 본 연구의 한계 내에서 Panavia F2.0이 Fujicem이나 Maxcem Elite보다 통계적으로 유의하게 큰 유지력을 보였고(p<0.05), Fujicem과 Maxcem Elite는 유지력의 차이를 보이지 않았다. Sandblasting처리는 모든 실험군에서 유지력의 향상을 보였다. 따라서 임플란트 지대주에 합착된 금속 코핑의 유지력은 표면 거칠기와 시멘트의 종류에 영향을 받는다.
PURPOSE. To evaluate the effects of different abutment geometries in combination with varying luting agents and the effectiveness of different cleaning methods (prior to re-cementation) regarding the retentiveness of zirconia copings on implants. MATERIALS AND METHODS. Implants were embedded in resin blocks. Three groups of titanium abutments (pre-fabricated, height: 7.5 mm, taper: $5.7^{\circ}$; customized-long, height: 6.79 mm, taper: $4.8^{\circ}$; customized-short, height: 4.31 mm, taper: $4.8^{\circ}$) were used for luting of CAD/CAM-fabricated zirconia copings with a semi-permanent (Telio CS) and a provisional cement (TempBond NE). Retention forces were evaluated using a universal testing machine. Furthermore, the influence of cleaning methods (manually, manually in combination with ultrasonic bath or sandblasting) prior to re-cementation with a provisional cement (TempBond NE) was investigated with the pre-fabricated titanium abutments (height: 7.5 mm, taper: $5.7^{\circ}$) and SEM-analysis of inner surfaces of the copings was performed. Significant differences were determined via two-way ANOVA. RESULTS. Significant interactions between abutment geometry and luting agent were observed. TempBond NE showed the highest level of retentiveness on customized-long abutments, but was negatively affected by other abutment geometries. In contrast, luting with Telio CS demonstrated consistent results irrespective of the varying abutment geometries. Manual cleaning in combination with an ultrasonic bath was the only cleaning method tested prior to re-cementation that revealed retentiveness levels not inferior to primary cementation. CONCLUSION. No superiority for one of the two cements could be demonstrated because their influences on retentive strength are also depending on abutment geometry. Only manual cleaning in combination with an ultrasonic bath offers retentiveness levels after re-cementation comparable to those of primary luting.
하악 제 1대구치에 티타늄과 지르코니아 소재의 기성 지대주와 맞춤형 지대주를 이용해 지르코니아 크라운으로 수복한 경우를 3차원 유한요소법을 통해 응력 분포를 분석해 보고, 피로 파절 강도 및 파절 형태를 고찰해 보았다. 정상교합자의 CT scan을 재구성한 하악골 모형상에 제 1대구치 임플란트 지지 지르코니아 크라운으로 수복한 6가지 유한요소 모형을 제작하고, 협측 교두 중앙에 수직과 30도 각도로 100 N의 하중을 가하는 조건으로 응력분포를 분석하였다. 현재 임상적으로 사용되고 있는 4가지 지대주를 이용하여, 군당 10개의 시편을 제작하고, ISO 14801에 따라 피로파절 실험을 실시하였다. 지대주나 고정체의 재질에 따른 응력분포의 차이는 없었고 형태에 따른 응력크기의 차이만 보였다. 맞춤형 지대주 군이 전반적으로 낮은 크라운 응력을 나타내었으며, 지대주 일체형 지르코니아 임플란트는 계면 골조직의 응력이 가장 낮게 나타났다. 피로수명의 평균값은 7군이 가장 높았고, 1군, 2군, 3군의 순서로 낮게 나타났으며(P<0.05), 기성 지대주 군은 피로수명의 편차가 적게 나타났다. 구치부의 임플란트 수복 시에는 맞춤형 지대주의 사용이 상부 보철물의 파절저항성 향상에 유리하며, 심미적 요구도가 높은 경우에는 고정체와의 접합부를 티타늄으로 제작한 지르코니아 맞춤형 지대주의 사용을 고려해 볼 만 하다.
PURPOSE. Cement-retained implant prostheses can lack proper retrievability during repair, and residual cement can cause peri-implantitis. The purpose of this in vitro study was to evaluate the influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with lingual slots, known as retrievable cement-type slots (RCS). MATERIALS AND METHODS. We fabricated six types of titanium abutments (10 of each type) with two different heights (4 mm and 6 mm), three different convergence angles ($8^{\circ}$, $10^{\circ}$, and $12^{\circ}$), a sloped shoulder margin (0.6 mm depth), a rectangular shape ($6mm{\times}6.5mm$) with rounded edges, and a rectangular ledge ($2mm{\times}1mm$) for the RCS. One monolithic zirconia crown was fabricated for each abutment using a dental computer-aided design/computer-aided manufacturing system. The abutments and crowns were permanently cemented together with dual-curing resin cement, followed by 24 hours in demineralized water at room temperature. Using a custom-made device with a slot driver and torque gauge, we recorded the torque ($N{\cdot}cm$) required to remove the crowns. Statistical analysis was conducted using multiple regression analysis and Mann-Whitney U tests (${\alpha}=.05$). RESULTS. Removal torques significantly decreased as convergence angles increased. Multiple regression analysis showed no significant interaction between the abutment height and the convergence angle (Durbin-Watson ratio: 2.186). CONCLUSION. Within the limitations of this in vitro study, we suggest that the retrievability of cement-retained implant prostheses with RCS can be maintained by adjusting the abutment height and convergence angle, even when they are permanently cemented together.
