Full mouth rehabilitation of a severely worn dentition using intraoral scanner and the CAD/CAM double scanning technique (중증도의 치아 마모 환자를 구내 스캐너와 이중스캔을 이용해 수복한 완전구강회복 증례)

  • With the evolution of the computer-aided design/computer-aided manufacturing (CAD/CAM) technology, the intraoral scanners are playing an increasingly important role, as they are the first step towards a completely digital workflow. The CAD/CAM double scanning technique has been used to transfer the information from provisional restorations to definitive restorations. In this case, a 67-year-old male with esthetically compromised anterior teeth, generalized severe attrition of teeth, and reduced vertical dimension was treated with full mouth rehabilitation including a re-establishment of the lost vertical dimension of occlusion assisted by the crown lengthening procedure. The provisional restorations were fabricated using an intraoral scanner and the CAD/CAM double scanning technique. After the period of adaption, the definitive monolithic zirconia restorations were delivered. The CAD/CAM double scanning technique successfully transferred the occlusal and morphological characteristics, obtained from the provisional restorations, to the definitive restorations.

  • Over the last 30 years, the use of chairside computer-aided design (CAD) and computer-aided manufacturing (CAM) systems has evolved and has become increasingly popular in dentistry. Although CAD/CAM restorations have been used in the anterior dentition, satisfying the esthetic requirements of clinicians and patients, where the restorations are limited to the chairside, remains a challenge. To reproduce multi-shades of CAD/CAM restorations in the clinic, a preliminary experiment to express several shades on A2 lithium disilicate (LS2) blocks using a staining kit was performed. After measurement of the CIE L*a*b* value of specimens, it was compared with that of the commercial shade guide. This report presents two cases with individual customization of shade and surface characterization of the CAD/CAM restorations using predictable methods based on the preliminary experimental data. The anatomical shape of restoration was obtained from 'copy and paste technique' and 'mirror image acquisition technique'. All treatment procedures and fabrication of restorations performed in this report were executed in the clinic itself.

  • Full-mouth reconstruction of a patient using dental implants is a challenge if there is vertical and horizontal bone resorption. Therefore, it is should be cautious in making the fixed prostheses that restore the function and the esthetics of the gingiva and teeth. In full mouth rehabilitation, CAD/CAM system makes it possible to fabricate restorations with high precision, regardless of span of the restoration. Recently, Palladium-silver (Pd-Ag) alloy which is highly biocompatible and millingable has been developed to compensate for the shortcomings of the titanium or zirconia. This clinical report presents the reconstruction of a maxillary arch with a cement retained implant supported fixed prosthesis using a Pd-Ag alloy generated by CAD/CAM system on eleven osseointegrated implants. The occluding surfaces were made of Pd-Ag alloy, to decrease the risk of chipping or fracture. The prostheses were esthetically pleasing, and no clinical complications have been reported after two years.

  • The implant-supported fixed dental prosthesis in irradiated maxilla needs meticulous treatment planning due to low bone healing capacity. All-on-4 concept implantation can reduce the number of implants to be placed avoiding bone grafting procedure. Conventionally, prefabricated angled abutments for tilted implants have been used. However, in this case, it was replaced with computer-aided design and computer-aided manufacturing (CAD/CAM) abutment. This case report described all-on-4 concept implantation and fabrication of CAD/CAM zirconia fixed dental prostheses using CAD/CAM titanium abutments.

  • Comprehensive prosthetic treatment requires considerations from various points of view. The anterior guidance is important factor in prosthodontic treatment of anterior teeth. Lingual surface contour of anterior restoration is so critical that a small mistake of laboratory or clinical process can cause discomfort of patient and disharmony of entire dentition. There are no guidelines for lingual surface contour that fit all patients. Therefore the lingual surface of provisional restoration is most accurately described as a customized one. The dentist transfers the exact information of anterior guidance that has made through long term provisional restoration to the technician. This case introduce that the duplication technique of CAD/CAM system to reproduce the anterior guidance of provisional restoration. This method can improve satisfaction of both patient and dentist.

  • Using computer-aided design and manufacturing technique improve quality of treatment in many aspect. This case reports the complete mouth rehabilitation of a patient with amelogenesis imperfecta utilizing digital technology. Clinical examination revealed loss of mastication due to insufficient occlusal stop, missing teeth, interdental spacing due to microdontia, insufficient overbite, and etc. Full veneer crowns for teeth were selected, followed by a fixed partial denture and implant placement was done using CAD-CAM guide template with bone graft for partially edentulous space. Definitive restorations were duplicated by double scanning provisional restorations and successfully delivered to the patient. These full mouth rehabilitation procedures resulted in satisfactory outcomes for the patient functionally and aesthetically.

  • In edentulous mandible, implant supported overdenture was considered as a first treatment option. Konus type attachment supplies rigid support and cross arch stabilization so that more favorable force transmission and distribution can be attained. In the dentistry, computer aided design-computer aided manufacturing (CAD-CAM) system makes it possible to fabricate restorations with high precision and effectiveness. Recently, Palladium-silver (Pd-Ag) alloy which is millable has been developed. This article presents that application of CAD-CAM Konus type attachment can be provide satisfactory stability and function on four-implant supported mandibular overdenture.

  • 치과보철학 분야에서 컴퓨터를 이용한 기술의 발달로 치료 계획, 수술, 보철물 수복에 이르기까지 CAD-CAM의 광범위한 적용이 가능해지고 있으며, 비용과 시간의 절약으로 효율적인 진료가 가능하다는 점을 인지한 많은 회사들의 투자로 다양한 프로그램과 기계들이 소개되고 있다. 그러나, 이렇게 활발한 제품 개발에도 불구하고 아직 실제 임상으로의 적용은 보편화되었다고 보기는 어려운데, 가장 큰 이유는 정확도에 대한 의심과 복잡하다고 느껴지는 초기의 숙련 과정에 있다고 하겠다. 제작회사들의 보고에 따르면 컴퓨터를 이용하여 디자인하고 절삭하는 것이 기존의 주조 기술보다 더 정교하다고 하였으나, 실제 임상에서는 미세한 오차를 간과한 데서 오는 최종 결과물과의 차이를 얘기하는 임상가들이 있는 것이 현실이다. 또, 그 사용 재료에 있어서도 초기 티타늄 금속 재료에 이어 심미적 요구를 위한 알루미나, 보다 향상된 강도를 가진 지르코니아에 이르기까지 다양한 재료들이 쏟아져 나오고 있다. 이에 본 논문에서는 다양한 CAD-CAM 시스템 중 대표적인 것 중의 하나인 Nobel Biocare사의 $Procera^{(R)}$ 시스템을 중심으로 지르코니아 수복물에 관한 최신 정보를 제공하며, 그 다양한 치과적 적용을 누구나 쉽고 정확하게 사용할 수 있는 임상 증례와 함께 소개함으로써 실제 임상 적용에 도움이 되고자 한다.

  • An accurate implant placement with ideal location is significant for long-term success of the implant. An exact evaluation of nearby anatomic structures such as quality of residual bone, an inferior alveolar bone and a maxillary sinus is required. For a prosthetic-driven treatment, planned surgery, precise prosthesis and communication with the patient are significant requisites especially for full-mouth rehabilitation. In this case, the patient with severe alveolar bone resorption had a CT guided surgery supported by CT data and the data from scanning diagnostic wax-up. Afterward, edentulous area was restored by full mouth implant-supported prosthesis by using monolithic zirconia and CAD/CAM technique. This paper reports the outcome of the procedure which was remarkable both esthetically and functionally.
