Healing outcomes of root canal treatment for C-shaped mandibular second molars: a retrospective analysis

  • Objectives: This study aimed to evaluate the healing rate of non-surgical endodontic treatment between C-shaped and non-C-shaped mandibular second molars. Materials and Methods: Clinical records and radiological images of patients who had undergone endodontic treatment on mandibular second molars between 2007 and 2014 were screened. The periapical index scoring system was applied to compare healing outcomes. Information about preoperative and postoperative factors as well as the demographic data of the patients was acquired and evaluated using chi-square and multinomial logistic regression tests. Results: The total healing rate was 68.4%. Healing rates for the mandibular second molar were 70.9% in C-shaped canals (n = 79) and 66.6% in non-C-shaped ones (n = 117). The difference was not statistically significant. Conclusions: The presence of a C-shaped canal in the mandibular second molar did not have a significantly negative effect on healing after treatment. Instead, proper pulpal diagnosis and final restoration were indicated as having significantly greater influence on the healing outcomes of C-shaped and non-C-shaped canals, respectively.

  • 무한 도체 평면에 H-형태와 C-형태의 소형 개구가 놓여져 있는 경우에 평면파가 입사될 때 투과 효율의 척도인 투과 단면적의 특성을 분석하였다. 동일한 개구 면적에서 C-형태 개구가 H-형태 개구보다 더욱 큰 투과 단면적이 나타남을 알 수 있었다. 두 개의 C-형태 개구가 인접해 놓여 있을 때 투과 단면적의 특성 관점에서 상호 결합 효과를 연구하는데 보다 관심을 두었고, 투과 단면적이 더욱 증대된다는 관점을 두고 두 개의 C-형태 개구가 x축으로 놓여있는 평행 배열 구조와 C-형태 개구가 y축으로 놓여있는 동일선상 배열 구조를 조사하였다.

  • 목적: 이 후향적 연구의 목적은 한국인에서 콘빔CT를 이용하여 상악 대구치에서 C-형 치근 및 근관의 발현빈도와 형태를 분류하기 위함이다. 연구 재료 및 방법: 총 357개 상악 대구치(제1대구치 186개, 제2대구치 171개)의 콘빔CT 영상을 두 명의 치과보존과 전문의가 평가하였다. 결과: 8가지의 C-형 치근 형태와 5가지의 C-형 근관형태로 분류할 수 있었으며, C-형 치근 및 근관의 발현빈도는 각각 21.0%와 5.3%였다. 결론: 이러한 해부학적 변이는 제1대구치보다 제2대구치에서 많이 발견되었다. 상악 대구치의 성공적인 근관치료를 위해 다양한 형태의 C-형 치근 및 C-형 근관에 대한 해부학적 지식이 필요하다.

한국인의 영구 하악 제2대구치의 C형 근관 빈도와 형태 : Cone-Beam CT 자료 분석 (ORIGINAL ARTICLE - The incidence and types of C-shaped canal of permanent mandibular second molar in Korean sub-population: Cone-Beam CT data analysis)

  • Objective: The aim of this study was to investigate the incidence of the C-shaped canal of permanent mandibular second molar (PMSM) in Korean sub-population using Cone-Beam CT (CBCT) data and analyze the types of C-shaped canal. Materials & Methods: The protocol for this study was approved by the Institutional Review Board at the Pusan National University Hospital (E-2011039). Among the CBCT images taken of patients who visited the St. Bennedict Dental Hospital (Busan, Korea) from May 2008 to April 2011 for implant surgery and surgical removal of impacted teeth, high-quality CBCTs from 705 patients (361 male and 342 female) were screened and 607 PMSMs of 383 patients were evaluated retrospectively. All PMSMs were anatomically analyzed in detail by using image viewer software (EasyDent; Vatech). PMSMs were evaluated in the axial plane to investigate the shape of root and canals. The C-shaped canals were classified into five types. The total incidence, gender ratio, bilateral and unilateral appearance. and the correlation between right-side and left-side occurrences of C-shaped PMSMs were computed and compared statistically using the chi-square test. Results: Among the 607 PMSMs of 383 CBCTs of 187 females and 196 males, 198 PMSMs(32.6%) had C-shaped root and 158 PMSMs(26.0%) had C-shaped canals. The shape of C-shaped root canals at the furcalion level did not have significant change at the level of mid root (P<0.0001). Female had more prevalence of C-shaped root canals than male (P<0.0001). The prevalence of bilateral occurrence of C-shaped root canals was higher than unilateral occurrence. Conclusions: The occurrence of C-shaped PMSMs among a Korean population was 32.6% and was higher than other countries and ethnicities. Understanding the prevalence of PMSMs with a C-shaped root and/or canal in a Korean population may be useful for successful endodontic treatments.

Monotonic behavior of C and L shaped angle shear connectors within steel-concrete composite beams: an experimental investigation

  • Shear connectors are essential elements in the design of steel-concrete composite systems. These connectors are utilized to prevent the occurrence of potential slips at the interface of steel and concrete. The two types of shear connectors which have been recently employed in construction projects are C- and L-shaped connectors. In the current study, the behavior of C and L-shaped angle shear connectors is investigated experimentally. For this purpose, eight push-out tests were composed and subjected to monotonic loading. The load-slip curves and failure modes have been determined. Also, the shear strength of the connectors has been compared with previously developed relationships. Two failure modes of shear connectors were observed: 1) concrete crushing-splitting and 2) shear connector fracture. It was found that the L-shaped connectors have less shear strength compared to C-shaped connectors, and decreasing the angle leg size increases the shear strength of the C-shaped connectors, but decreases the relative ductility and strength of L-shaped connectors.

CT사진을 이용한 한국인의 C형 치근관의 분포에 대한 연구 (The distribution of C-shaped canal system in Korean population with CT image)

  • Purpose: To investigate the incidence and prevalence of C-shaped root canal using computed tomographic images of head and neck in Korean population. Materials and Methods : Regardless of each examination purpose, randomly selected 268 examples which have serial axial computed tomographic images with 8 normal molars in maxilla and mandible were selected and investigated. Defined C-shaped root canal and we could get 82 proper image examples in view of this definition. These were detected and investigated of incidence and prevalence of C-shaped root canal. Results: C-shaped root canals were found in 82 examples among 268 (30.6%) and 147 teeth. They were only found on molar area, and the highest incidence was 37.41% of left mandibular second molars, and the lowest was 2.04% of right maxillary second molars. On prevalence of tooth position, mandibular second molar was the highest, 65% of C-shaped root canal teeth and maxillary second molar was 6% the lowest. Conclusion: 31% of randomly selected CT images no considering check-up reason have C-shaped root canals. The C-shaped root canals of mandibular second molar are found most frequently and they are also detectable on maxilla. On Korean population, C-shaped root canals are one of conditions that have to concern with dental treatment for it appears at a high prevalence relatively. Analysis of C-shaped root canal configuration in maxillary molars in a Korean population using cone-beam computed tomography

  • Objectives: The purpose of this study was to investigate the incidence of root fusion and C-shaped root canals in maxillary molars, and to classify the types of C-shaped canal by analyzing cone-beam computed tomography (CBCT) in a Korean population. Materials and Methods: Digitized CBCT images from 911 subjects were obtained in Chosun University Dental Hospital between February 2010 and July 2012 for orthodontic treatment. Among them, a total of selected 3,553 data of maxillary molars were analyzed retrospectively. Tomography sections in the axial, coronal, and sagittal planes were displayed by PiViewstar and Rapidia MPR software (Infinitt Co.). The incidence and types of root fusion and C-shaped root canals were evaluated and the incidence between the first and the second molar was compared using Chi-square test. Results: Root fusion was present in 3.2% of the first molars and 19.5% of the second molars, and fusion of mesiobuccal and palatal root was dominant. C-shaped root canals were present in 0.8% of the first molars and 2.7% of the second molars. The frequency of root fusion and C-shaped canal was significantly higher in the second molar than the first molar (p < 0.001). Conclusions: In a Korean population, maxillary molars showed total 11.3% of root fusion and 1.8% of C-shaped root canals. Furthermore, root fusion and C-shaped root canals were seen more frequently in the maxillary second molars.

C-shaped root canals of mandibular second molars in a Korean population: a CBCT analysis

  • Objectives: The purpose of this study was to investigate the C-shaped root canal anatomy of mandibular second molars in a Korean population. Materials and Methods: A total of 542 teeth were evaluated using cone-beam computed tomography (CBCT). The canal shapes were classified according to a modified version of Melton's method at the level where the pulp chamber floor became discernible. Results: Of the 542 mandibular second molars, 215 (39.8%) had C-shaped canals, 330 (53%) had 3 canals, 17 (3.3%) had 2 canals, 12 (2.2%) had 4 canals, and 8 (1.7%) had 1 canal. The prevalence of C-shaped canals was 47.8% in females and 28.4% in males. Seventy-seven percent of the C-shaped canals showed a bilateral appearance. The prevalence of C-shaped canals showed no difference according to age or tooth position. Most teeth with a C-shaped canal system presented Melton's type II (45.6%) and type III (32.1%) configurations. Conclusions: There was a high prevalence of C-shaped canals in the mandibular second molars of the Korean population studied. CBCT is expected to be useful for endodontic diagnosis and treatment planning of mandibular second molars.

연소화염이 존재하는 봉대평판 갭에서 봉전극 형상에 따른 공기의 방전 특성 (Discharge Characteristics of Air according to the Shapes of Rod Electrode in the Rod-Plane Gap having Flame on the Plane Electrode)

  • In this paper, discharge characteristics of air in the vertical arrangement of three rods(dome-shaped, cylinder-shaped, and needle-shaped rod) and plane gap having propane flame on the plane electrode are examined under the application of a.c. and d.c. high-voltages. As the result of the experiment, flashover voltages in the presence of the flame are substantially lowered than those in the absence of flame, and relative a.c. flashover voltages in the dome-shaped, cylinder-shaped, and needle-shaped rod gaps are found 32.5%, 32.1%, and 26.4% respectively. The polarity effects with the d.c. voltages are shown, and flashover voltages in negative polarity are much lowered than those in positive one. The negative flashover voltages in the dome-shaped and cylinder-shaped gaps are found 18.3% and 18.7% respectively, but the positive ones are found 53.5% and 43.2% respectively. In the needle-shaped rod gaps, meanwhile, the flame is extinguished by corona wind, and the quenching voltages are found 28.4% under the application of a.c. voltages. The ion winds of corona and the reduction of air density are the main factors of flashover reduction owing to the flame in the case of a.c. voltages, but the electrifications of the positive ion by thermal ionization besides the above factors are remarkably found in the case of d.c. voltages.

Endodontic treatment of a C-shaped mandibular second premolar with four root canals and three apical foramina: a case report

  • This case report describes a unique C-shaped mandibular second premolar with four canals and three apical foramina and its endodontic management with the aid of cone-beam computer tomography (CBCT). C-shaped root canal morphology with four canals was identified under a dental operating microscope. A CBCT scan was taken to evaluate the aberrant root canal anatomy and devise a better instrumentation strategy based on the anatomy. All canals were instrumented to have a 0.05 taper using 1.0 mm step-back filing with appropriate apical sizes determined from the CBCT scan images and filled using a warm vertical compaction technique. A C-shaped mandibular second premolar with multiple canals is an anatomically rare case for clinicians, yet its endodontic treatment may require a careful instrumentation strategy due to the difficulty in disinfecting the canals in the thin root area without compromising the root structure.
