유방암 환자에서 근치적 유방 절제술 후 재발 발견에 대한 CA 15-3의 역할 (The Role Of Tumor Marker CA 15-3 in Detection of Breast Cancer Relapse After Curative Mastectomy)

  • 목적 : 유방암 환자에서 근치적 유방 절제술 후 재발의 발견에 대한 CA 15-3의 역할을 평가하고 재발된 환자에서 전이 부위에 따른 CA 15-3의 관계를 비교하였다. 대상 및 방법 : CA 15-3 값이 측정된 환자 중 원발성 유방암으로 근치적 유방 절제술을 시행 받았으며 분석하고자 하는 모든 임상적, 병리학적 인자에 대한 정보가 의무 기록에 기재되고 최소한 3개월간 외래 추적이 가능하였던 202명을 대상으로 분석하였다. CA 15-3은 IRMA 15-3 키트(CIS BIO INTERNATIONAL, France)로 정량적으로 측정하여 정상 상한값을 30 U/ml으로 정하였다. 결과: 근치적 유방 절제술 후 재발을 진단하는 CA 15-3의 진단 성적은 예민도 31%(5/16), 특이도 100%(186/186), 양성 예측도 100%(5/5), 음성 예측도 94%(186/197)이었다. 재발 환자에서 전이 부위에 따른 CA 15-3의 진단 성적을 비교하면 간전이가 관찰된 환자의 CA 15-3값은 모두 상승하여 간전이의 진단에 대한 CA 15-3의 예민도는 100%로 매우 좋았다. 그러나 골전이 또는 국소지역전이만 관찰된 환자의 CA 15-3 값은 정상으로 골전이 또는 국소지역전이의 진단에 대한 CA 15-3 검사는 유용하지 않았다. 결론: 유방암 환자에서 근치적 유방 절제술 후 재발의 발견에 대한 CA 15-3의 진단 성적은 매우 좋은 특이도를 보였으나 예민도는 낮았다. 그러나 비교적 예후가 불량하다고 알려진 간전이로 첫 재발한 환자의 CA 15-3 값은 모두 증가되어 유방암 환자에서 CA 15-3의 측정은 간전이 발견에 매우 유용하였다.

Changes in Breast-tumor Blood Flow in Response to Hypercapnia during Chemotherapy with Laser Speckle Flowmetry

  • Development of a biomarker for predicting tumor-treatment efficacy is a matter of great concern, to reduce time, medical expense, and effort in oncology therapy. In a preclinical study, we hypothesized that the blood-flow parameter based on laser speckle flowmetry (LSF) could be a potential indicator to estimate the efficacy of breast-cancer treatment. To verify this hypothesis, a 13762-MAT-B-III rat breast tumor was grown in a dorsal skinfold window chamber applied to a nude mouse, and the change in blood flow rate (BFR) - or the speckle flow index (SFI) is used together as the same meaning in this manuscript - was longitudinally monitored during tumor growth and metronomic cyclophosphamide treatment. Based on the daily LSF angiogram, several BFR parameters (baseline SFI, normalized SFI, and △rBFR) were compared to tumor size in the normal, treated, and untreated tumor groups. Despite the incomplete tumor treatment, we found that the daily changes in all BFR parameters tended to have partially positive correlation with tumor size. Moreover, we observed that the changes in baseline SFI and normalized SFI responded one day earlier than the tumor shrinkage during chemotherapy. However, daily variations in the hypercapnia-induced △rBFR lagged tumor shrinkage by one day. This study would contribute not only to evaluating tumor vascular response to treatment, but also to monitoring blood-flow-mediated diseases (in brain, skin, and retina) by using LSF in preclinical settings.

Vascular Morphometric Changes During Tumor Growth and Chemotherapy in a Murine Mammary Tumor Model Using OCT Angiography: a Preliminary Study

  • To develop a biomarker predicting tumor treatment efficacy is helpful to reduce time, medical expenditure, and efforts in oncology therapy. In clinics, microvessel density using immunohistochemistry has been proposed as an indicator that correlates with both tumor size and metastasis of cancer. In the preclinical study, we hypothesized that vascular morphometrics using optical coherence tomography angiography (OCTA) could be potential indicators to estimate the treatment efficacy of breast cancer. To verify this hypothesis, a 13762-MAT-B-III rat breast tumor was grown in a dorsal skinfold window chamber which was applied to a nude mouse, and the change in vascular morphology was longitudinally monitored during tumor growth and metronomic cyclophosphamide treatment. Based on the daily OCTA maximum intensity projection map, multiple vessel parameters (vessel skeleton density, vessel diameter index, fractal dimension, and lacunarity) were compared with the tumor size in no tumor, treated tumor, and untreated tumor cases. Although each case has only one animal, we found that the vessel skeleton density (VSD), vessel diameter index and fractal dimension (FD) tended to be positively correlated with tumor size while lacunarity showed a partially negative correlation. Moreover, we observed that the changes in the VSD and FD are prior to the morphological change of the tumor. This feasibility study would be helpful in evaluating the tumor vascular response to treatment in preclinical settings.

  • Breast cancer is a major cause of death in women worldwide. Mammary tissue expressing xenobiotic metabolizing enzymes metabolically activate or detoxify potential genotoxic breast carcinogens. Deregulation of these xenobiotic metabolizing enzymes is considered to be a major contributory factor to breast cancer. The present study is focused on the expression of the xenobiotic metabolizing gene, CYP1A1, in breast cancer and its possible relationships with different risk factors. Twenty five tumors and twenty five control breast tissue samples were collected from patients undergoing planned surgery or biopsy from different hospitals. Semi-quantitative reverse transcriptase polymerase chain reaction (RT-PCR) and western-blotting were used to investigate the expression of CYP1A1 in breast cancer control and disease samples. mRNA expression of CYP1A1 was down-regulated in 40% of breast tumor samples. Down-regulation was also observed at the protein level. Significnat relations were noted with marital status and tumour grade but not histopathological type. In conclusion, CYP1A1 protein expression was markedly reduced in tumor breast tissues samples as compared to paired control tissue samples.

Regulation of tumor-associated macrophage (TAM) differentiation by NDRG2 expression in breast cancer cells

  • Macrophages are a major cellular component of innate immunity and are mainly known to have phagocytic activity. In the tumor microenvironment (TME), they can be differentiated into tumor-associated macrophages (TAMs). As the most abundant immune cells in the TME, TAMs promote tumor progression by enhancing angiogenesis, suppressing T cells and increasing immunosuppressive cytokine production. N-myc downstream-regulated gene 2 (NDRG2) is a tumor suppressor gene, whose expression is down-regulated in various cancers. However, the effect of NDRG2 on the differentiation of macrophages into TAMs in breast cancer remains elusive. In this study, we investigated the effect of NDRG2 expression in breast cancer cells on the differentiation of macrophages into TAMs. Compared to tumor cell-conditioned medium (TCCM) from 4T1-mock cells, TCCM from NDRG2-over-expressing 4T1 mouse breast cancer cells did not significantly change the morphology of RAW 264.7 cells. However, TCCM from 4T1-NDRG2 cells reduced the mRNA levels of TAM-related genes, including MR1, IL-10, ARG1 and iNOS, in RAW 264.7 cells. In addition, TCCM from 4T1-NDRG2 cells reduced the expression of TAM-related surface markers, such as CD206, in peritoneal macrophages (PEM). The mRNA expression of TAM-related genes, including IL-10, YM1, FIZZ1, MR1, ARG1 and iNOS, was also downregulated by TCCM from 4T1-NDRG2 cells. Remarkably, TCCM from 4T1-NDRG2 cells reduced the expression of PD-L1 and Fra-1 as well as the production of GM-CSF, IL-10 and ROS, leading to the attenuation of T cell-inhibitory activity of PEM. These data showed that compared with TCCM from 4T1-mock cells, TCCM from 4T1-NDRG2 cells suppressed the TAM differentiation and activation. Collectively, these results suggest that NDRG2 expression in breast cancer may reduce the differentiation of macrophages into TAMs in the TME.

전이성 유방암 환자의 통합 암 치료에 대한 증례보고 (A Case Report of Metastatic Breast Cancer Patient Treated with Integrative Cancer Treatment)

  • Objective : The purpose of this study is to report and observe effects of Integrative Cancer Treatment (ICT) on metastatic breast cancer patient. Method : A left breast cancer patient diagnosed with recurrence on liver and bones April 2018. The patient had received paclitaxel chemotherapy for two months and discontinued it because of tumor progression and side effects. The patient has been treated with ICT since March 2018 and has been taking capecitabine since October 2018. The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (CEA, CA15-3). The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (CEA, CA15-3), liver function test (AST, ALT), Eastern Cooperative Oncology Group performance status (ECOG PS), and numeric rating scales (NRS). Results : After the ICT, tumor size was partially decreased accompanying by reducing the levels of tumor markers. Major clinical symptoms induced by paclitaxel chemotherapy were improved. There were no severe adverse events induced by ICT based on National Cancer Institute Common Terminology Criteria for Adverse Events (NCI CTCAE) version 5.0. Conclusion : This case suggests that ICT may have synergistic effect for the treatment of metastatic breast cancer patient in conjunction with capecitabine.

Neighborhood 러프집합 모델을 활용한 유방 종양의 진단적 특징 선택 (A Diagnostic Feature Subset Selection of Breast Tumor Based on Neighborhood Rough Set Model)

  • 특징선택은 데이터 마이닝, 기계학습 분야에서 가장 중요한 이슈 중 하나로, 원본 데이터에서 가장 좋은 분류 성능을 보여줄 수 있는 특징들을 찾아내는 방법이다. 본 논문에서는 정보 입자성을 기반으로 한 neighborhood 러프집합 모델을 이용한 특징선택 방법을 제안한다. 제안된 방법의 효과성은 5,252명의 유방 초음파 영상으로부터 추출된 298가지의 특징들 중에서 유방 종양의 진단과 관련된 유용한 특징들을 선택하는 문제에 적용되었다. 실험결과 19가지의 진단적 특징을 찾을 수 있었고, 이때에 평균 분류 정확성은 97.6%를 보였다.

A Prototype System for Early-Stage Breast Cancer Detection

  • The early detection of breast cancer is very important. The final goal of this study is to develop an inexpensive portable device for simply detecting the existence of breast cancer using microwaves. In this study, we proposed a detection algorithm and the system configuration of such a device. The feasibility of the proposed method was investigated by numerical simulation and actual measurement. A prototype sensing module was fabricated, and the measurement results for tumor phantoms were presented. The measurement results showed that the prototype sensing module worked well, and the simulation results revealed that the detection rate of the device could be about 60% for a 4 mm tumor and almost 100% for a 6 mm tumor. The proposed device can be used for the self-diagnosis of breast cancer. Further study is required to reduce the size of the sensing module.

유방 초음파 영상에서 질감 특성을 이용한 악성종양 분석 (Analysis of Malignant Tumor Using Texture Characteristics in Breast Ultrasonography)

  • 조기 유방암을 진단하기 위해서는 유방초음파 판독이 매우 중요하다. 초음파 검사는 초음파장비에 따라 화질의 차이가 심하게 나타날 뿐만 아니라 검사자의 경험과 숙련 정도에 따라 진단의 차이가 크게 나타난다. 따라서 정확한 진단과 치료를 위하여 객관적인 판단기준이 필요하다. 이에 본 연구에서는 GLCM(Gray Level Co-occurrence Matrix) 알고리듬을 적용하여 질감 특성을 분석하고 특징파라미터들을 추출하여 신경망분류기를 이용하여 유방암을 진단하였다. 유방초음파 영상은 정상 조직과 양성, 악성 종양으로 분류하여 질감 특성 파라미터 6가지를 추출하였다. 유방초음파검사로 진단된 정상 영상, 악성 및 양성종양 영상 각각 14증례를 대상으로 추출된 6개의 파라미터들을 적용하여 다층 퍼셉트론 신경망구조 역전파 학습방법으로 학습을 시켰다. 학습된 모델에 정상 유방 영상 51증례, 양성종양 영상 62증례, 악성종양 영상 74증례의 영상을 사용하여 분류한 결과 95.2%의 분류율을 나타내었다.

Socio-economic Factors Influencing Tumor Presentation and Treatment Options in Chinese Breast Cancer Patients

  • The impact of income and education level on the clinical and pathologic characteristics, implementation of clinical breast examination (CBE), and treatment patterns of a small population of Chinese female breast cancer patients was studeied in order to provide a theoretical basis and statistical reference for further nationwide research. We included 484 pathologically confirmed female primary breast cancer inpatients of the First Affiliated Hospital of Medical College of Xi'an Jiaotong University from February 2003 to January 2004. All cases were reviewed and relevant information was collected using a designed case report form (CRF). Chisquare tests, rank-sum tests, and Fisher's exact tests were used in the analysis. Our analysis showed that: (1) women in different occupation groups had significant differences in tumor size, pre-operative mammography, surgical options, post-operative estrogen receptor (ER), progestin receptor (PR) and human epidermal growth factor receptor 2 (Her2) status, and post-operative radiotherapy and chemotherapy (P < 0.05); and (2) women with different education levels had statistically significant differences in tumor size, post-operative ER, PR and Her2 status, and post-operative chemotherapy, radiotherapy, and endocrine therapy (P < 0.05). In Xi'an, China, women in low-income occupations or with low education levels are more likely to have advanced tumor stages at presentation, lower implementation rate of clinical breast examination, and less treatment.
