A Study for the Development of Standardized Management Manuel in Sanhujoriwon - Centered on the Management of Women & Newborn - (산후조리원의 표준화 관리 지침을 위한 연구 - 산모와 신생아 관리현황을 중심으로 -)

  • The purpose of this study was to develop the frame for standardized manual for Sanhujoriwon by finding out the status of the management of women & newborn in Sanhujoriwon, like postpartum care center. The subjects were 95 staffs of 22 Sanhujoriwon agreed on oral consent, in Seoul and Bundang, Korea. Data were collected from Sep. 15 to Oct. 24, 2001. The instrument used for this study was a structured questionnaire consisted of 16 items of general characteristics & educational characteristics about postpartum care, 31 items of degree of management of women's postpartum care (Cronbach's $\alpha$.93 ), 24 items of degree of management of newborn's care(Cronbach's $\alpha$.94 ), 10 items of methods of management of women's postpartum care, 8 items of methods management of newborn's care. The data were analyzed by the SPSS/PC+ program using frequency, percentage, mean, standard deviation, t-test, ANOVA and Pearson correlation coefficient. The results of the study were as follows ; 1. The mean of the degree of management for postpartum women was 4.1. The mean score for the degree of management of general postpartum care for women was 4.00 & traditional postpartum care(Sanhujori) was 4.20. The degree of management of traditional postpartum care was higher than general postpartum care for women. 2. The degree of management for newborn : The mean score for the degree of management of newborn was 4.37. 3.There was a strong positive correlation between general postpartum care and traditional postpartum care($r=.744^{**}$), and postpartum mother care and newborn care($r=.798^{**}$). 4.The basic frame for the management of the women and newborn in Sanhujoriwon. 1) For women: Integrated postpartal care Physical management : Vital sign & BP check, contraction of uterus, form and amount of lochia, management of personal hygiene, management of breast & breast-feeding, management of postpartum exercise, prevention of infection, symptom & sign of high risk and prevention & management of high risk condition; Emotional-psychological management: assessment and management of mother-baby attachment, emotional state; Educational management : education of vaccination schedule, urinary incontinence, rearing infant, breast-feeding ; and Environmental management : temperature, humidity, disinfection, cleaning, light, infection control integrated with 6 principles of Sanhujori. 2) For newborn Physical management : check of vital sign, management of umbilical cord, jaundice, prevention of infection, management of diaper rash; Emotional-psychological management : assessment of sleep, crying, activity, response of mother-baby attachment; and Environmental management : temperature, humidity, disinfection, cleaning, light, infection control integrated with 6 principles of Sanhujori. In conclusion Sanhujoriwon must be health care center for the postpartum women and newborn. Therefore, the establishment of various laws and regulations in such a way to meet the realistic needs of Sanhujoriwon as a health care center for women and infants future health should be done. The standardized management manual based on the results is absolutely required above all.

The Effects of Individual Education on Self-Care Knowledge and Performance for Breast Cancer Patients Receiving Chemotherapy (항암화학요법에 관한 개별교육이 유방암환자의 지식과 자가 간호수행에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of an individual education program on breast cancer patients who received chemotherapy. Methods: The quasi-experimental design was employed with a nonequivalent control group pre-post test. The study was performed from January 1 to July 30, 2010 for the patients who were firstly treated by A C (Adriamycin, Cyclophosphamide) chemotherapy in a university hospital. Control group was recruited first to prevent treatment diffusion. The experimental group was recruited and received an individual education during 30 minutes. The collected data were analyzed using SPSS 16.0 version. ${\chi}^2$-test, t-test, and the Pearson correlation were implemented in order to examine the effect of the individual education among the patients who are treated by chemotherapy. Results: The experimental group had a greater improvement in self-care knowledge (t=-5.236, p=.001), and self-care performance (t=-6.543, p=.004) than did control group. Moreover, the experimental group had significantly positive correlation between self-care knowledge and self-care performance (r=.494, p=.006) as compared to the control group. Conclusion: The individual education program on cancer patients who were treated by chemotherapy could be utilized as an effective nursing intervention.

  • Background: Cancer genetics have gained public attention dramatically. Everyday physicians and nurses use the knowledge of cancer genetics in their practices. Despite the importance of the knowledge of the health care providers on cancer genetics, there is a paucity of literature investigating the levels of knowledge about cancer genetics among health care providers. Purpose: This study aimed to describe the Korean physicians' current knowledge level about hereditary breast and ovarian cancer (HBOC). Method: This nation-wide survey used a 15-item questionnaire which is modified from the Breast Cancer Genetic Counseling Knowledge Questionnaire originally developed by Erblich et al.(2005). One hundred and forty-four physicians participated in this survey from October 1, 2006 to March 31, 2007. Result: Physicians' knowledge level about hereditary breast cancer was 11.94 (S.D=2.46). Physicians with short-term careers (less than one year), the position of Intern, or who were involved in breast or ovarian cancer care demonstrated the highest knowledge about HBOC. Conclusion: Genetic courses in medical education seemed to contribute to the high level of physicians' genetic knowledge. Also, nursing discipline needs to incorporate genetics or genetic counseling courses into the formal educational curriculum in order to deliver up to date cancer care services which are sensitive to ever-changing cancer genetic information.

Use of Complementary and Alternative Medicine Among Breast Cancer Survivors in Taiwan

  • Aims: The purpose of this study was to investigate complementary and alternative medicine use among breast cancer survivors in Taiwan. Materials and Methods: This study employed a descriptive research design approach to detail the CAM use among the target population. Convenience sampling was used along with a structured questionnaire. Results: A total of 230 breast cancer survivors completed the use CAM scale. Prayer, reading books, taking antioxidants, eating various grains, and maintaining a vegetarian diet proved to be the five most frequently used CAM practices among patients in our study. More than 50.0% of the participants reported praying occasionally. More than 40.0% of participants read books occasionally, and 38.7% stated that they occasionally take antioxidants. Conclusions: These results provide more insight into CAM use for nurses who care for breast cancer patients.

The Effect of Health Care Program for Newly Married Couple in a Community Health Care Center (일 보건소의 신혼부부 건강프로그램의 효과)

  • Purpose: The research purpose was analysis of the effect of a health education program for newly-married executed from 2003 to 2005 as a part of health care service from community health center and providing the fundamental source for health education needed during newly-married. Method: The data collection with structured questionnaires was conducted during October to December in 2006. From the health center 106 brides who participate the program and 130 brides who didn't participate. Health care program for newly married couple were composed health examinations and health educations. Health examinations were CBC, LFT and Ag & Ab prevallence rates of hepatitis B, rubella. Health educations were done two times, the first individual health education was done at enrolled in health center for establishment of desirable couple relationship, family planning, contraception method and management of pre-pregnancy. The second individual health education was done for explanation of blood examination results, prevention of the congenital deformity and the vertical transmission of hepatitis B, management for pregnancy, breast feeding method, introduction to safety delivery method and encouraging self-study using by materials made by health center after two weeks at revisiting health center. SPSS/PC(ver. 12.0) and $X^2$-test, t-test was used to analyze the collected and tabulated data. Socio-demographic characteristics and regional characteristics of residence area of two groups shows no significant difference. Result: As a variables of experimental effect. The natural childbirth rates of participants group was significantly higher than Non-participants group(p=.012). Breast feeding rates for participants group was also statistically significant higher than Non-participants group. Contraception rates showed no significant difference. The rubella vaccination rates (p=.001) and The hepatitis B vaccination rates (p=.012) shows statistical differences. Conclusion: Therefore health care program for newly married couple in a community health care center was effective generally. We supposed that explore participation ways for nonparticipants, expand the program to community health care center in whole country.

The Effects of Antenatal Breast Care on Breast Feeding practice (임신중 유방간호가 산모의 모유수유 실천에 미치는 영향)

  • The purpose of this research was to study the effects of antenatal breast care on decreases in breast discomfort and increases in the breast feeding rate during the postpartum period. A nonequivalent control group posttest research design was used. The experimental group consisted of fifty -one pregnant women(primigravida) who were receiving antenatal care in the OBGYN clinics of four hospitals between March 5 and May 30, 1991. The control group was made up of seventy - five postpartum women who delivered at two hospitals OBGYN clinic and one midwife clinic between May 4 and June 15, and between September 5 and October 15, 1991. Data were collected via telephone interviews on the seventh postpartum day and at the end of the second month. Data analysis methods used frequencies and the x$^2$- test. The results were as follows : 1. The rate of breast feeding practice at two months was higher in the experimental group(70.6%) than in the control group(25.3%) (p<.01). 2. Nipple soreness in the early breast feeding period was lower in the experimental group(14.6%) than in the control group(25.3)(p<.01). 3. Severe breast discomfort in the early breast feeding period was lower in the experimental group (12.5%) than in the control group(39.2%)(P<.01). 4. There was a significant relationship between the breast feeding practice and the planned feeding method(p<.05), and between breast feeding practice and nipple soreness(p<.01) in the experimental group, and the presence of a job(p<.01), the sex of the infant(p<.05), and the first feeding time(p<.05) in the control group. 5. The reasons for unsuccessful breast feeding were a deficiency of breast milk (66.7% in the experimental group, 30.4% in the control group), poor sucking on the part of the baby(13.3% in the experimental group, 21.4% in the control group).

Mammography Screening according to Breast Cancer Disease and Social Network Characteristics of Married Korean Women (기혼여성의 유방암과 사회연결망 특성에 따른 유방촬영술 수검행위)

  • Purpose: This study was done to examine differences in mammography screening according to breast cancer and social network characteristic. Methods: Data were collected from 187 married women 35 years and older who were using public health centers, health promotion centers, cultural centers, obstetrics and gynecology hospitals or other relevant community sites. Data were collected between October 24 and December 4, 2008. Data were analyzed using the SPSS/WIN 15.0 program. Results: The participation rate for mammography screening was 35.3%. The following general and breast cancer characteristics showed statistically significant differences: religion, family incomes, regular medical-care, general health examinations during past 2 years, and history of breast disease. The following social network characteristics showed statistically significant differences: social norms and subjective norms. Using logistic regression analysis, regular medical-care, breast cancer risk appraisal, social norm, and subjective norms were highly predictive of subsequent mammography. Conclusion: The results of this study indicate that it is important to develop and provide tailored intervention programs through integrated socially mediated programs. By consciously including social network and support systems, breast cancer detection efforts would not end as a one-time event, but naturally build on network structure of adults women, thus facilitating regular mammography screening.

Effect of Breast-feeding Education and Follow-up care on the Breast-feeding Rate and the Breast-feeding Method - Focused on Home Visit and Phone Counselling - (모유수유교육과 추후간호방법이 산모의 모유수유실천율과 모유수유방법에 미치는 효과 - 가정방문과 전화상담을 중심으로 -)

  • This was a pre-experimental study to confirm the breast-feeding ability and effect of follow up care on the breast-feeding rate and the breast-feeding method by the mother-infant breast-feeding assessment tool for the mothers who got the breast-feeding education. The subjects were 46 mothers who experienced normal delivery of infants at a college hospital in K-city, Kyungsangbuk-do, from July 1 to October 21, 2000. The instrument for data obtainment were The Mother-Infant Breast-Feeding Assessment Tool of Johnson et al. (1999), and The Breast- Feeding Method Measurement Tool of Jeong, Geum-hee(1997). This instrument was reliable, showing Cronbach $\alpha$.751. This study classified them into 3 groups: at high risk for breast-feeding failure, at risk for breast-feeding problems, and at low risk for breast-feeding failure by the mother-infant breast-feeding assessment tool on the day of discharge from the hospital after delivering individual breast-feeding education to the subjects. This study investigated the breast-feeding rate and the breast-feeding method through mail questionnaire at the four week after childbirth, and through the phone counselling and the home visit for follow up care at the first week and the second week after childbirth. The sixth week after childbirth, this study investigated the breast-feeding rate by phone. The data analyzed the hypothesizes by $x^2$-test, paired t-test, ANOVA, Wilcoxon signed rank test, Wilcoxon rank sum test and trend analysis using SPSS/PC+ WIN 10.0 program. The results were as follows : 1) Hypothesis 1-1, "there won't be any difference the breast-feeding rate of a group at risk for breast-feeding failure by the time elapsed" was supported through constant the breast-feeding rate, because changes in the breast-feeding rate by the time elapsed after childbirth wasn't statistically significant(t= -1.501, p=.270). Hypothesis 1-2, "there won't be any difference the breast-feeding rate of group at low risk for breast-feeding failure by the time elapsed" was supported through constant the breast-feeding rate, because changes in the breast-feeding rate by the time elapsed after childbirth wasn't statistically significant(t=-1.732, p=.225). 2) Hypothesis 2-1, "there won't be any difference between the breast-feeding method of group at risk for breast-feeding failure for four weeks after childbirth and just after childbirth” was rejected, because the mean point of post test appeared to be higher than that of pre test(t=-7.267, p=.000). Hypothesis 2-2, "there won't be any difference between the breast-feeding method of the group at low risk for breast-feeding failure for four weeks after childbirth and just after childbirth" was rejected, because the mean point of post test appeared to be higher than that of pre test(t=-2.501, p=.012). 3)The 3rd hypothesis, "there won't be any difference between breast-feeding method of groups at risk for breast-feeding problems and at low risk for breast-feeding failure at the 4th week after childbirth and just after childbirth" didn't show any difference between the breast-feeding method of groups at risk for breast-feeding problems and at low risk for breast-feeding failure in the advance test(t=-1.521, p=.130) but there was difference between them in post test (t=-2.012, p=.044). As a result, the 3rd hypothesis was supported by pre test, but it was rejected by post test. In conclusion, this study confirmed breast- feeding education and follow up care just after childbirth were effective for the breast-feeding rate and method. Accordingly, it is proposed that successful nursing intervention of breast-feeding to be necessary by continuously providing follow up care through the mother-infant breast-feeding assessment tool as well as to execute individual breast-feeding education to mothers just after childbirth.

Perception of Breast Health amongst Malaysian Female Adolescents

  • Background: Breast cancer is the most common cancer among women in Malaysia, about one in 19 women being at risk. This study aimed to investigate knowledge and practice of breast self-examination (BSE), as well as knowledge of risk factors for breast cancer amongst female adolescents in Malaysia. Subsequently, relationships between demographic characteristics and knowledge level of BSE, risk factors for breast cancer and BSE practice were assessed. Materials and Methods: A descriptive, cross sectional survey was conducted using a sample of 500 Malaysian adolescents from the age of 15 to 19 years. A self-administered questionnaire was used to gather socio-demographic characteristics, knowledge of BSE, knowledge of risk factors for breast cancer and BSE practices. Results: The findings of this study indicated that female adolescents in Malaysia demonstrated an inadequate knowledge level of BSE and risk factors for breast cancer. Only 27.8% of female adolescents performed BSE regularly. BSE practice, knowledge of BSE and knowledge of risk factors for breast cancer showed significant positive relationships. Conclusions: The study highlighted the importance of planning and implementing breast health education programs for female students in secondary schools in Malaysia. It will also provide the health care providers an avenue to stress on the importance of imparting breast health education to adolescents.

Symptom Experience and Quality of Life in Breast Cancer Survivors (유방암 생존자의 신체적 증상, 정서적 증상과 삶의 질)

  • The purposes of this study were to evaluate symptom experience and quality of life (QOL) and to identify the predictors of QOL among breast cancer survivors. Methods: A cross-sectional study was conducted on 200 disease-free breast cancer survivors at two hospitals between December 2007 and July 2008. Functional Assessment of Cancer Therapy Scale-B, Memorial Symptom Assessment Scale-short Form and The Linear Analogue Self Assessment Scale were used to assess symptom experience and QOL in these patients. Data were analyzed using the Pearson correlation, t-test, ANOVA, and stepwise multiple regression with SPSS/WIN 12.0. Results: The mean score of QOL for breast cancer survivors was 95.81 (${\pm}18.02$). The highest scores among physical and psychological symptoms were sexual interest and anxiety. Year since treatment completion was significantly associated with QOL in sociodemographic variables. Physical and psychological symptoms have a significant negative association with QOL. The results of the regression analyses showed that physical and psychological symptoms were statistically significant in predicting patients' QOL. Conclusion: Symptom experience and QOL are essential variables that should be acknowledged when delivering health care to breast cancer survivors. More attention to the reduction and management of psychological distress could improve QOL among breast cancer survivors.
