Mammary Paget's disease without underlying malignancy of the breast

  • Mammary Paget's disease (MPD) is usually accompanied by underlying breast malignancy; however, a few cases have been reported as only skin lesions without any evidence of malignancy of the breast on imaging tests and microscopic examination of surgical specimen. Here, we describe a 47-year-old woman who visited our hospital who had an eczematous lesion on right nipple and areola for over 10 years. The lesion was diagnosed as Paget's disease by punch biopsy; however, imaging studies demonstrated no breast malignancy or lymph node metastasis. The patient underwent surgery of on the nipple and areola including underlying breast tissue. No underlying malignancy was found upon microscopic examination, except for Paget's disease. Immunohistochemical stains revealed that the tumor cells were positive for cytokeratin 7, and negativity for p63, cytokeratin 5/6, estrogen receptor, progesterone receptor, and human epidermal growth factor receptor 2. We report a case of MPD without underlying malignancy. To the best of our knowledge, this is the third case reported in Korea.

DLC-1 Expression Levels in Breast Cancer Assessed by qRT-PCR are Negatively Associated with Malignancy

  • Objective: The aim of this study was to explore the expression of DLC-l in breast carcinoma and any association with tumor metastasis. Methods: 51 surgical specimens of human breast carcinoma, divided into high invasive and low invasive groups according to their clinicopathological features, 30 cases of adjacent normal tissue and 28 benign breast lesions were examined by qRT-PCR for expression of DLC-1. Results: Expression level of DLC-1 in adjacent normal tissue and benign breast lesion specimens was higher than that in breast carcinoma (P<0.0001); the values in the high invasive group with synchronous metastases were also lower than in the low invasive group (P=0.0275). The correlation between DLC-1 expression level and tumor progression and metastasis of breast cancer was negative. Conclusion: As an anti-oncogene, DLC-1 could play an important part in breast carcinoma occurrence, progression, invasiveness and metastasis. Detecting the changes of the expression of DLC-1 in the breast carcinoma may contribute to earlier auxiliary diagnosis of invasiveness, metastasis and recrudescence.

  • Fine needle aspiration (FNA) cytology of breast disease is recognized to be highly accurate and cost effective, especially when this is used in combination with clinical examination and imaging as part of a triple approach. A probabilistic/categorical approach is used for the classification of breast FNA specimens. Criteria are defined from the perspective of the likelihood of making a definitive diagnosis of cancer on excision. This approach is an accurate way of classifying breast FNA specimens, and this can be reliably applied regardless of the level of experience of the pathologist for interpreting the case. When a definitive diagnosis of malignancy is made, the next step is to determining the specific histologic types of the malignancy according to their cytological features. In order to make an accurate diagnosis of carcinoma and for correct typing a tumor, an adequate, correctly sampled aspirate without any other artifacts is required.

Risk of Malignancy Associated with a Maternal Family History of Cancer

  • This study was conducted in order to obtain a screening and early detection reference for children whose mothers had been diagnosed with cancer. Data for 276 mother-child pairs with malignant tumors were analyzed. The distribution of cancers in affected families was generally similar to that of the general Chinese population, and correspondingly breast cancer was the most common malignancy amongst daughters whose mother had cancer (32.7%). The most prevalent cancer amongst sons with affected mothers was gastric cancer, rather than lung cancer. Daughters were more likely to have the same kind of malignant tumor as their mother (P<0.05), and were more likely to develop breast cancer than any other malignant disease if their mother had a breast tumor (P<0.0001). Likewise, if the mother was diagnosed with breast or gynecological cancer, the daughter was more likely to be diagnosed with breast or gynecological cancer than any other cancer (P<0.01). Daughters and sons developed malignant diseases 11 and 6.5 years earlier than their mothers, respectively (P<0.0001).Women with a mother who suffered cancer should be screened for malignancy from 40 years of age especially for breast, lung, and gynecological cancers. For men with affected mothers, screening should start when they are 45 years old focusing particularly on lung and digestive system cancers.

유방암 환자의 MRI에서 발견된 병변의 악성 예측을 위한 점수체계: 진단적 능력과 이차 초음파 결정에 미치는 영향 (Scoring System to Predict Malignancy for MRI-Detected Lesions in Breast Cancer Patients: Diagnostic Performance and Effect on Second-Look Ultrasonography)

  • 목적 유방암 환자의 MRI에서 발견된 추가적 병변의 악성 예측을 위한 점수체계를 설계하고자 하였다. 대상과 방법 68명 유방암 환자의 86개 MRI 발견 병변(64 양성, 22 악성)이 후향적으로 연구되었다. 스튜던트 t 검정, Fisher 정확검정, 로짓 회귀분석을 이용해 영상적 소견과 조직학적 결과의 상관관계를 분석했다. 의미 있는 악성 시사 소견을 기반으로 한 점수체계를 설계하고 그 것의 진단적 능력을 Breast Imaging-Reporting and Data System (이하 BI-RADS) category와 비교하였다. 결과 병변 크기 ≥ 8 mm (p < 0.001), 주 병소와 동일한 사분면에 위치(p = 0.005), 지연기의 고원형 조영 증강(p = 0.010), T2 등신호(p = 0.034) 혹은 저신호 강도(p = 0.024), 불규칙한 종괴 모양(p = 0.028)이 악성과 관련 있었다. 이 소견들과 의심스러운 비종괴 내부 조영 양상을 바탕으로 한 점수체계는 BI-RADS의 진단적 능력을 향상시켰고(area under the curve, 0.918 vs. 0.727), 3개의 위음성 케이스를 방지할 수 있었다. 또한, 22개의 불필요한 2차 초음파 검사(22/66, 33.3%)를 줄일 수 있었다. 결론 병변 크기, 주 병소와의 상대적 위치, 지연기의 조영 증강 양상, T2 신호강도, 종괴의 모양 및 비종괴 내부 조영 양상을 기반으로 한 점수체계는 유방암 환자의 MRI 발견 병소를 평가하는데 있어 정확도를 높여 줄 수 있다.

Evaluation of Common Risk Factors for Breast Carcinoma in Females: a Hospital Based Study in Karachi, Pakistan

  • Background: Breast malignancies are one of the leading causes of deaths in females worldwide. There are a number of risk factors associated with breast cancer but in Karachi Pakistan there are insufficient data available. Materials and Methods: A case control study was conducted on females in age group between 30-80 years. This study was accomplished by retrospective data collection in Aga Khan University Hospital Karachi, Pakistan. A total of 108 females with primary malignancy of breast were included along with 108 matched controls. Relationship of various factors with disease was studied using logistic regression to calculate odds ratios with 95 % confidence intervals. Results: A total of 14 variables were analyzed and based on and 7 were found to be risk factors: old age, family history of breast cancer, family history of other carcinomas, personal history of breast carcinoma, early age of menarche, older age of mother at first delivery and lower number of children. Five factors, parity, breast feeding, history of oral contraceptive pills intake, past history of oophorectomy and hysterectomy showed protective associations. One variable, use of hormonal replacement therapy, showed a controversial link and one other, marital status, was not significant in this study. Conclusions: It is concluded that most of the well-known risk factors for breast cancer are also associated with the disease in the female population of Karachi, Pakistan. High risk patients should be the focus with the help of this study so that screening can be more effective for early diagnosis before clinically evident breast malignancy.

Texture Analysis for Classifying Normal Tissue, Benign and Malignant Tumors from Breast Ultrasound Image

  • Breast ultrasonic reading is critical as a primary screening test for the early diagnosis of breast cancer. However, breast ultrasound examinations show significant differences in diagnosis based on the difference in image quality according to the ultrasonic equipment, experience, and proficiency of the examiner. Accordingly, studies are being actively conducted to analyze the texture characteristics of normal breast tissue, positive tumors, and malignant tumors using breast ultrasonography and to use them for computer-assisted diagnosis. In this study, breast ultrasonography was conducted to select 247 ultrasound images of 71 normal breast tissues, 87 fibroadenomas among benign tumors, and 89 malignant tumors. The selected images were calculated using a statistical method with 21 feature parameters extracted using the gray level co-occurrence matrix algorithm, and classified as normal breast tissue, benign tumor, and malignancy. In addition, we proposed five feature parameters that are available for computer-aided diagnosis of breast cancer classification. The average classification rate for normal breast tissue, benign tumors, and malignant tumors, using this feature parameter, was 82.8%.

  • Regardless of advances in treatment modalities with the invention of newer therapies, breast cancer remains a major health problem with respect to its diagnosis, treatment and management. This female malignancy with its tremendous heterogeneous nature is linked to high incidence and mortality rates, especially in developing region of the world. It is the malignancy composed of distinct biological subtypes with diverse clinical, pathological, molecular and genetic features as well as different therapeutic responsiveness and outcomes. This inconsistency can be partially overcome by finding novel molecular markers with biological significance. In recent years, newer technologies help us to indentify distinct biomarkers and increase our understanding of the molecular basis of breast cancer. However, certain issues need to be resolved that limit the application of gene expression profiling to current clinical practice. Despite the complex nature of gene expression patterns of cDNAs in microarrays, there are some innovative regulatory molecules and functional pathways that allow us to predict breast cancer behavior in the clinic and provide new targets for breast cancer treatment. This review describes the landscape of different molecular markers with particular spotlight on vitamin D signaling pathway and apoptotic specific protein of p53 (ASPP) family members in breast cancer.

Beyond BI-RADS: Nonmass Abnormalities on Breast Ultrasound

  • Abnormalities on breast ultrasound (US) images which do not meet the criteria for masses are referred to as nonmass lesions. These features and outcomes have been investigated in several studies conducted by Asian researchers. However, the term "nonmass" is not included in the American College of Radiology (ACR) Breast Imaging Reporting and Data System (BI-RADS) 5th edition for US. According to the Japan Association of Breast and Thyroid Sonology guidelines, breast lesions are divided into mass and nonmass. US findings of nonmass abnormalities are classified into five subtypes: abnormalities of the ducts, hypoechoic areas in the mammary glands, architectural distortion, multiple small cysts, and echogenic foci without a hypoechoic area. These findings can be benign or malignant; however, focal or segmental distributions and presence of calcifications suggest malignancy. Intraductal, invasive ductal, and lobular carcinomas can present as nonmass abnormalities. For the nonmass concept to be included in the next BI-RADS and be widely accepted in clinical practice, standardized terminologies, an interpretation algorithm, and outcome-based evidence are required for both screening and diagnostic US.
