The purpose of this study was to measure the skin dose using the glass dosimeter and diode and to compare those measurements to the planned skin dose from the treatment planning system. For the reproducibility of the glass dosimeter (ASAHI TECHNO GLASS CIRPORATION, Japan), the same dose was irradiated to 40 glass dosimeters three times, among which 28 with the reproducibility within 3% were selected for the use of this study. For each of 27 breast cancer patients, the glass dosimeters and diodes were attached to 4 different locations on the skin to measure the dose during treatment. All the patients received one fraction of 180 cGy each. The maximum difference of measurements between the glass dosimeter and diode at the same location was 3.2%. Comparing with the planned skin dose from the treatment planning system (Eclipse v6.5, Varian, USA), the dose measured by the glass dosimeter and the diodeshowed on an average 3.4% and 2.3% difference, respectively. The measured doses were always less than the planned skin dose. This may be due to the specific errors of both detectors. Also, the difference may be caused by the fact that since the skin where the detectors were attached is pretty moveable, it was not fix the detectors on the skin.
Effects on skin caused by the dose from linear accelerator (LINAC) opposing portal irradiation and TomoDirect 3-D modeling treatment according to the radiation devices and treatment methods were measured, and a comparative analysis was performed. Two groups of 10 patients each were created and measurements were carried out using an optically stimulated luminescence dosimeter. These patients were already receiving radiation treatment in the hospital. Using the SPSS statistical program, the minimum and maximum average standard deviations of the measured skin dose data were obtained. Two types of treatment method were selected as independent variables; the measured points and total average were the dependent variables. An independent sample T-test was used, and it was checked whether there was a significance probability between the two groups. The average of the measured results for the LINAC opposing portal radiation was 117.7 cGy and PDD 65.39% for the inner breast, 144.7 cGy and PDD 80.39% for the outer breast, 143.2 cGy and PDD 79.56% for the upper breast, 151.4 cGy and PDD 84.11% for the lower breast, 149.6 cGy and PDD 83.11% for the axilla, and 141.32 cGy and PDD 78.51% for the total average. In contrast, for TomoDirect 3-D conformal radiotherapy, the corresponding measurement values were 137.6 cGy and PDD 76.44%, 152.3 cGy and PDD 84.61%, 148.6 cGy and PDD 82.56%, 159.7 cGy and PDD 88.72%, and 148.6 cGy PDD 82.56%, respectively, and the total average was 149.36 cGy and PDD 82.98%. To determine if the difference between the total averages was statistically significant, the independent sample T-test of the SPSS statistical program was used, which indicated that the P-value was P=0.024, which was 0.05 lower than the significance level. Thus, it can be understood that the null hypothesis can be dismissed, and that there was a difference in the averages. In conclusion, even though the treatment dose was similar, there could be a difference in the dose entering the body surface from the radiation treatment plan; however, depending on the properties of the treatment devices, there is a difference in the dose affecting the body surface. Thus, the absorbed dose entering the body surface can be high. During breast cancer radiotherapy, radiation dermatitis occurs in almost all patients. Most patients have a difficult time while undergoing treatment, and therefore, when choosing a radiotherapy treatment method, minimizing radiation dermatitis is an important consideration.

We present three cases of patients with breast cancer and psoriasis who received radiotherapy following breast-conserving surgery. One patient developed an extensive flare-up of psoriasis during chemotherapy. After discontinuing chemotherapy, she received conventional radiotherapy to the ipsilateral whole breast, axillary, and supraclavicular lymph nodes with 50.4 Gy in 28 fractions and boosted with 10 Gy in five fractions to the tumor bed. Two patients received hypofractionated whole-breast radiotherapy at a total dose of 40.05-42.4 Gy in 15-16 fractions. In all three cases, there was no flare-up of psoriatic events at least 6 months after the completion of radiotherapy. We hypothesized that there is a close relationship between psoriatic events and the extent of trauma rather than the daily dose of the fraction. Therefore, we confirmed that the effect of radiotherapy on psoriatic events is minimal if the radiation field size does not exceed that of the whole breast.

Purpose : We recently perform the latest radiotheraphy (3D conformal, IMRT,etc.) with the development of 3D CT planning technology. However, in case patients with breast cancer, it is difficult to acquire the CT image with the limitation of CT hole size and tilting of breast immobilization device. The Ewha Breast Device(EBD) was constructed to improve the problem in the treatment of patients with breast cancer and we are intend to introduce the procedure of the EBD construction in this study and compare the EBD with conventional breast device in the view point of usefulness. Materials and Methods : We have constructed the EBD with acryl, analyze the skull size with CT data, consider the skin folder in SCL field and evaluated the EBD usefulness from the view point of set-up reproducibility, dose distribution, skin reaction in comparison with conventional breast device. Results : In the case of patients set-up error analysis, the EBD is superior to conventional device in portal film repetition($\%$) check (80pt.), equal to that in simulation & CT image coincidence check(5pt.). There is no difference between the two systems in dose distribution and skin reaction in SCL field is better the EBD than conventional device. Conclusions : The construction of the EBD enable us to perform the latest radiotheraphy in breast treatments, relieve the pains in simulation, and reduce, the skin reaction. In the future, we expect that modification of the EBD is useful in treating for patients with breast cancer.
Whole Spine Scanography (WSS) using the Digital Radiography (DR) system is an examination that requires whole body X-ray exposure, which involves more exposure to radiation for patients than other general radiographies. This can affect the occurrence of breast cancer. This research measured radiation dose when breasts were shield and not shield using the Auto Exposure Control (AEC) mode. The radiation dose without a shield was 1.540 mGy, and that using a collimator was measured 0.506 mGy. Moreover, 0.733 mGy was measured when 1 shield (0.3 mm) was used, and $0.523{\mu}Gy$ when 5 of them (1.5 mm) were used. The results showed that the radiation dose with 5 shields and the radiation dose with a collimator were similar. Moreover, 0.233 mGy was measured when 8 shields (2.4 mm) were used. The standard deviation were 0.081 when using collimator and 0.014 when 5 shields were used. Also, when 8 shields were used, it was found to be 0.002. Most patients who go under a scoliosis test are children or young people who are highly sensitive to radiation. In the research results, the case where the organs sensitive to radiation, women's breasts, were shielded showed more distinct differences compared to without shields. It is considered that using shields can provide more constant shield than using a collimator and lower the risk of breast cancer caused by exposure to radiation.

Methods and Materials: The data collected consisted of a measurement taken with solid water, a Humanoid Alderson Rando phantom and patients. The radiation doses at the ipsllateral breast and skin, contralateral breast and skin, surrounding peripheral soft tissue, and Ipsllateral lung and heart were compared using the physical wedge and virtual wedge and the radiation dose distribution and DVH of the treated breast were compared. The beam-on time of each treatment technique was also compared Furthermore, the doses at treated breast skin, contralateral breast skin and skin 1.5 cm away from 4he field margin were also measured using TLD in 7 patients of tangential breast Irradiation and compared the results with phantom measurements. Results: The virtual wedge showed a decreased peripheral dose than those of a typical physical wedge at 15$^{\circ}$, 30$^{\circ}$, 45$^{\circ}$, and 60$^{\circ}$. According to the TLD measurements with 15$^{\circ}$ and 30$^{\circ}$ virtual wedge, the Irradiation dose decreased by 1.35$\%$ and 2.55$\%$ In the contralateral breast and by 0.87$\%$ and 1.9$\%$ In the skin of the contralateral breast respectively. Furthermore, the Irradiation dose decreased by 2.7$\%$ and 6.0$\%$ in the Ipsllateral lung and by 0.96$\%$ and 2.5$\%$ in the heart. The VW fields had lower peripheral doses than those of the PW fields by 1.8$\%$ and 2.33$\%$. However the skin dose Increased by 2.4$\%$ and 4.58$\%$ In the Ipsliateral breast. VW fields, In general, use less monitor units than PW fields and shoriened beam-on time about half of PW. The DVH analysis showed that each delivery technique results In comparable dose distribution in treated breast. Conclusion: A modest dose reduction to the surrounding normal tissue and uniform target homogeneity were observed using the VW technique compare to the PW beam in tangential breast Irradiation The VW field is dosmetrically superlor to the PW beam and can be an efficient method for minimizing acute, late radiation morbidity and reduce 4he linear accelerator loading bV decreasing the radiation delivery time.

Purpose: In breast cancer radiotherapy, brass mesh bolus has been recently studied to overcome disadvantage of conventional bolus. The purpose of this study is to investigate the stability of first introduced the brass mesh in the country, and evaluate the skin surface dose of that. Materials and Methods: The measurement of skin surface dose was evaluated to verify similar thickness of the Brass mesh bolus that compared conformal tissue equivalent bolus with 5 mm thickness. We used 6 MV photons on an ELEKTA VERSA linear accelerator and optically stimulated luminescent dosimeter (OSLD). In addition, two opposed beam using IMRT phantom was applied to comparative study of brass mesh bolus between tissue equivalent bolus. Results: The results showed that similar thickness of the Brass mesh bolus was 3 mm compared with 5 mm tissue equivalent bolus by measuring the skin surface dose of solid phantom. The surface dose for IMRT thorax phantom using 3 mm brass mesh bolus was about 1.069 times greater than that using tissue equivalent bolus. Conclusion: In this study, we found that the brass mesh bolus improved better reduction of skin sparing effect and dose uniformity than tissue equivalent bolus. However evaluation for various clinic cases should be investigated.

Purpose: The purpose of this study is to evaluate the dosimetric effects of couch attenuation and air gaps using 3D phantom for prone breast radiation therapy. Materials and method: A 3D printer(Builder Extreme 1000) and computed tomography (CT) images of a breast cancer patient were used to manufacture the customized breast phantom. Eclipse External Beam Planning 13.6 (Varian Medical Systems Palo Alto, CA, USA) was used to create the treatment plan with a dose of 200 cGy per fraction with 6 MV energy. The Optically Stimulated Luminescence Detector(OSLD) was used to measure the skin dose at four points (Med 1, Med 2, Lat 1, Lat 2) on the 3D phantom and ion-chamber (FC65-G) were used to perform the in-vivo dosimetry at the two points (Anterior, Posterior). The Skin dose and in-vivo dosimetry were measured with reference air gap (3 cm) and increased air gaps (1, 2, 3, 4, 5, 6 cm) from reference distance between the couch and 3D phantom. Results: As a result, measurement for the skin dose at lateral point showed a similar value within ${\pm}4%$ compared to the plan. While the air gap increased, skin dose at medial 1 was reduced. And it was also reduced over 7 % when the air gap was more than 3 cm compared to radiation therapy plan. At medial 2 it was reduced over 4 % as well. The changes of dose from variety of the air gap showed similar value within ${\pm}1%$ at posterior. As the air gap was increased, the dose at anterior was also increased and it was increased by 1 % from the air gap distance more than 3 cm. Conclusion: Dosimetrical measurement using 3D phantom is very useful to evaluate the dosimetric effects of couch attenuation and air gaps for prone breast radiation therapy. And it is possible to reduce the skin dose and increase the accuracy of the radiation dose delivery by appling the optimized air gap.
In radiation treatment, it is unavoidable to block the influence of scattered ray on a skin and prevent internal normal organs from being exposed to radiation. It is fair to say that radiation therapy aims to reduce an absorbed dose of normal tissues. In particular, in radiation therapy of left-sided breast cancer, the internal neighboring organs are normal breast tissues, the heart, and the lung. The side effects on the heart include cardioplegy and myocardial infarction. This study tried to observe changes in the volume and dose of the heart in general radiation therapy plan and respiratory control based radiation therapy plan for patients with left-sided breast cancer, and to find the heart's volume and dose generated by respiration. According to the 4D computer tomography (CT), a volume of the heart had $12.8{\pm}8.7cc$ on average, and its dose had $17.3{\pm}12.1cGy$ on average. The differences in the volume and dose may cause side effects in radiation treatment. Therefore, it is necessary to apply respiratory control technique to establish the radiation treatment plan based on an accurate position of the heart.

Purpose: The purpose of this study is to compare the dosimetry of electron beam (EB) plans and three-dimensional helical tomotherapy (3DHT) plans for the patients with left-sided breast cancer, who underwent breast conserving surgery. Materials and Methods: We selected total of 15 patients based on the location of tumor, as following subsite: subareolar, upper outer, upper inner, lower lateral, and lower medial quadrants. The clinical target volume (CTV) was defined as the area of architectural distortion surrounded by surgical clip plus 1 cm margin. The conformity index (CI), homogeneity index (HI), quality of coverage (QC) and dose-volume parameters for the CTV, and organ at risk (OAR) were calculated. The following treatment techniques were assessed: single conformal EB plans; 3DHT plans with directional block of left anterior descending artery (LAD); and 3DHT plans with complete block of LAD. Results: 3DHT plans, regardless of type of LAD block, showed significantly better CI, HI, and QC for the CTVs, compared with the EB plans. However, 3DHT plans showed increase in the $V_{1Gy}$ at skin, left lung, and left breast. In terms of LAD, 3DHT plans with complete block of LAD showed extremely low dose, while dose increase in other OARs were observed, when compared with other plans. EB plans showed the worst conformity at upper outer quadrants of tumor bed site. Conclusion: 3DHT plans offer more favorable dose distributions to LAD, as well as improved target coverage in comparison with EB plans.
