완전 방실 차단으로 우측 쇄골하 정맥을 통해 영구심박동기를 삽입한 52세 여자환자가 영구심박동기 발생장치(generator) 삽입 부위의 반복적인 감염을 주소로 내원하였다. 환자는 24년 전 유방암으로 좌측 변형근치유방절제술(Modified radical mastectomy, MRM)과 방사선 치료로 주변 조직의 심한 변화가 있었다. 이로 인해 정중 흉골 절개는 불가능할 것으로 판단하여 우측 개흉 술을 계획하였고, 개흉 후 체외순환 없이 우심방을 절개한 후 심박동기 와이어를 제거하고 새로운 심외막 와이어로 영구심박동기를 삽입하였다.
This 32 year old female patient underwent left radical mastectomy due to ductal carcinoma on May 1990, and treated with FAM (5-fluorouracil, Adriamycin and Mitomycin C) regimen postoperatively. However, right cervical Iymph node enlargement and facial edema progressively developed since December 199). On April 1994, operation was performed, and findings were as followes; x4$\times$5$\times$7 to 1 : 1 $\times$ 1 cm sized multiple enlarged and hyperemic Iymph nodes were scatterred throughout submandibular area to the junction of superior vents cave and pericardium, and partially invaded both anterior segmental lobe, sternum and both distal tip of clavicles. After radical dissection of the nodes of neck and mediastinal nodes, and wedge resection of both anterior segments of lung, and partial resection of both clavicle tips and total sternum. The both innominate veins and superior vena cava were partially obstructed by invaded cancer SVC reconstruction was done with preclotted 10$\times$ 10$\times$ 18mm Y shap d woven Dacron graft, which was anastomosed to the point of the junction of subclavian vein and jugular vein after cross clamping both veins and 2cm above the pericardial junction with one arm clamp. After maintaining blood drainage to the SVC from the right side, left innominate vein was anastomosed with 4-0 Prolene continuous running suture. Bone cement was used for resected sternal portion and clavicular ends were fixed to postal portion with 18 Gauge wires. The patient was treated with radiation and chemotherapy after discharge, and there were no evidence of regrowing of the mass nor obstruction of the graft inspite of no antithrombotic therapy.
Objective : The authors analyzed the effectiveness and therapeutic response of Novalis shaped beam radiosurgery for metastatic brain tumors, and the prognostic factors which influenced the outcome. Methods : We performed a retrospective analysis of 106 patients who underwent 159 treatments for 640 metastatic brain lesions between January 2000 and April 2008. The pathologies of the primary tumor were mainly lung (45.3%), breast (18.2%) and GI tract (13.2%). We classified the patients using Radiation Therapy Oncology Group Recursive Partitioning Analysis (RPA) and then analyzed the survival and prognostic factors according to the Kaplan Meier method and univariate analysis. Results : The overall median actuarial survival rate was 7.3 months from the time of first radiosurgery treatment while 1 and 2 year actuarial survival estimates were 31% and 14.4%, respectively. Median actuarial survival rates for RPA classes I, II, and III were 31.3 months, 7.5 months and 1.7 months, respectively. Patients' life spans, higher Karnofsky performance scores and age correlated closely with RPA classes. However, sex and the number of lesions were not found to be significantly associated with length of survival. Conclusion : This result suggests that Novalis radiosurgery can be a good treatment option for treatment of the patients with brain metastases.
본 연구에서 Phantom에 고 밀도 물질이 삽입 된 CT 영상을 재구성 하여 방사선 치료 계획 시 선량 분포에 대한 평가를 하고자 하였다. Gammex 467 Tissue Characterization Phantom을 사용하여 인체 조직과 유사한 영상을 획득하였고 Titanium을 삽입하여 금속물로 인한 인공허상을 발생시켜 영상을 획득하였다. 획득한 영상은 Metal Artifact Reduction for Orthopedic Implants (O-MAR)를 이용하여 영상을 재구성 하였고 전산화 치료계획 시스템을 이용하여 체적을 분석 하고 선량 분포를 추출하였다. $MapCHECK^{TM}$을 이용하여 선형가속기의 광자선 선량 분포를 측정하여 계획한 선량 분포와 비교 분석 하였다. 비교 분석 결과 Titanium으로 인한 인공허상이 발생 되었을 때 O-MAR를 적용한 체적은 BR-12 Breast는 16.8 % 그리고 LV 1 Liver는 40.2 % 증가하였고 선량 분포는 O-MAR를 적용하기 전의 선량 분포 보다 1.4 에서 1.6 % 높게 나타났다. 결론적으로 금속물로 발생된 인공허상 O-MAR를 적용하여 가능한 제거하고 치료계획에 이용해야 오류를 줄일 수 있을 것으로 사료된다.
목적 : 유방암의 수술 후 방사선조사 시 폐나 심장 등 정상 증기에 대한 합병증을 줄이고자 흉벽에 대한 전자선 치료 시 조사야 내 전체 흉벽 두께를 균일하도록 보상할 수 있는 개별화된 조직보상체를 제작하였으며, 3차원 입체조형치료계획을 통하여 유용성을 평가하고자 하였다. 대상 및 방법 :유방전절제술 후 방사선치료를 받는 10명의 환자를 대상으로 하였다 우측 유방암 환자가 3명, 좌측유방암 환자가 7명이었다. 모든 환자는 조사야를 결정하기 위한 모의치료를 시행한 후 치료계획용 컴퓨터단층촬영을 하였으며, 이를 바탕으로 1 cm$^{2}$ 간격으로 흉벽 두께를 세밀히 측정하였다. 이후 주로 내유방림프절 근방인 가장 두꺼운 흉벽 부위를 기준으로 그 곳의 전방 흉막면에 80% 선량이 조사될 수 있는 방사선에너지를 설정하고, 이 부위를 기준으로 보다 얇은 흉벽을 보상하기 위한 개별화된 조직보상체를 제작하였으며, 제작된 조직보상체를 적용하여 다시 치료계획용 컴퓨터단층촬영을 시행하였다. 이후 각 환자의 영상자료를 이용하여 3차원 치료계획용 프로그램으로 설계하였다. 매 환자에서 조직보상체 적용 전후로 등선량곡선 분포 및 선량체적히스토그램을 비교하였고, 정상조직합병증발생률(normal tissue complication probability, NTCP)의 변화 및 기타 선량통계값도 분석 비교하였다. 결과 : 조직보상체를 적용하였을 때 모든 예에서 처방선량의 80% 등선량곡선의 깊이가 흉벽 두께와 거의 일치하였다. 조직보상체를 사용하지 않았을 때는 90% 이상의 등선량 곡선이 전방 흉막면을 지나 폐 실질 부위에 깊이 걸쳐 있는 경우가 많았으며, 특히 좌측 유방암의 경우에는 심장에도 불필요하게 높은 선량이 조사됨을 관찰할 수 있었다. 선량체적히스토그램을 조직보상체 적용 전후로 동측 폐, 반대측 폐 및 심장에 대하여 각각 비교하였는데 모든 예에서 조직보상체를 사용하였을 때 동측 폐의 선량체적히스토그램이 크게 향상된 소견을 보였으만 심장의 경우 좌측 유방암 환자에서 특히 두드러진 향상을 보였다. 동측 페의 경우 조직보상체를 적용하지 않았을 때 평균NTCP 값이 80.2${\pm}$3.43%이고, 조직보상체를 사용한 경우에는 평균 NTCP 값이 47.7${\pm}$4.61%로 개별화된 조직보상체의 사용으로 24.5~40.5%의 정상조직합병증발생률을 줄일 수 있었다. 동측 폐와 심장에 대해서 평균 선량, V$_{50}$ (처방선량 50% 이상의 선량이 조사되는 체적의 백분율), V$_{95}$ (처방선량 95% 이상의 선량이 조사되는 체적의 백분율), 최대 선량, 최소 선량 등을 구하여 보았을 때 평균 선량, V$_{50}$, V$_{95}$은 조직보상체 적용 전후에 두드러진 변화를 보였으나 최대선량 및 최소 선량값은 별다른 차이를 보이지 않았다. 결론 : 조직보상체를 적용하였을 때 적용하지 않은 경우에 비해 등선량곡선분포, 선량체적히스토그램, Lymankutcher 모델에 의한 정상조직합병증발생률 및 기타 선량통계값 등 모든 면에 있어서 우월성을 확인할 수 있었다. 향후 이러한 결과가 임상에서 실질적인 합병증 발생률 감소와 잘 연계되는지 계속적인 추적관찰 및 연구가 필요할것으로 생각된다.
방사성옥소는 갑상선암의 핵의학적 영상과 방사성치료에 널리 그리고 성공적으로 사용되어 왔다. 최근 세포의 옥소섭취를 담당하는 운반체로서 Na/I symporter (NIS)의 분자세포학적 특성이 규명되고 그 유전자가 클로닝되면서 앞으로는 갑상선암 이외의 각종 암에도 NIS 유전자를 외부에서 전달함으로써 방사성옥소 치료를 적용하는 새로운 암치료 기술이 가능할 것으로 기대되고 있다. 방사성옥소를 이용한 암치료의 성공을 위해서는 NIS를 통한 표적세포의 옥소 섭취를 극대화 시키는 것이 핵심이다. TSH는 갑상선 세포의 NIS 발현을 항진시키고 retinoic acid는 갑상선암과 유방암 세포의 NIS발현을 증가시키는 효과가 있다. 또 일반 암세포에는 NIS 유전자를 전달하여 발현 시킬 수 있다. 그러나 NIS 발현 만으로는 원하는 수준의 방사성옥소 섭취를 충분히 얻지 못할 수 있다. 이는 세포의 옥소 섭취가 NIS 단백질의 총량이 아니라 세포막에 위치한 NIS의 양에 의해 결정되기 때문이다. 즉, 옥소를 섭취하려는 전사된 NIS단백질이 세포막으로 이동하여 정상적으로 기능하게 하는 조절 기전이 중요하다. NIS의 세포막 이동 기전은 아직 밝혀져 있지 않으나 다른 운반체와 유사하게 단백질의 전사후 glycosylation이나 phosphorylation이 관여할 것으로 생각된다. 본 연구진은 NIS 유전자를 전달한 암세포에서epidermal growth factor를 통한 extracellular signal regulated kinase 신호경로의 활성화가 방사성옥소 섭취를 항진시킴을 관찰하여 NIS의 전사외 기능조절 기전을 조사하고 있다. 앞으로 NIS기능에 대한 조절기전이 보다 자세하게 밝혀지면 방사성옥소 치료기술과 NIS유전자 영상기술의 개선과 발전에 도움이 될 것으로 기대된다.
Salivary duct carcinoma(SDC) is rare malignancy, accounting for approximately 1-3% of all malignant salivary gland tumors. Systemic chemotherapy has been used for stage IV SDC, but there is no consensus on the standard treatment. SDC is histologically similar to ductal carcinoma of breast and often overexpress HER2/neu, hence HER2/neu targeted therapy could be one of treatment options. A 75-year-old Arabian man was diagnosed as SDC of right parotid gland with extensive metastases. He received oral 5-FU as palliative chemotherapy, but he was intolerable to oral chemotherapy due to severe oral mucositis. Considering immunohistochemical stain of tumor tissue showing strong positive for HER2/neu, we decided to administer an anti-HER2/neu antibody, trastuzumab. Follow-up CT scans before the third dose of trastuzumab demonstrated remarkable regression of multiple metastases as well as primary tumor. This case suggests that HER2/neu targeted therapy may be a potential therapeutic option for the SDC patient with overexpression of HER2/neu.
Objective : Brainstem metastases are rarely operable and generally unresponsive to conventional radiation therapy or chemotherapy. Recently, Gamma Knife Radiosurgery (GKRS) was used as feasible treatment option for brainstem metastasis. The present study evaluated our experience of brainstem metastasis which was treated with GKRS. Methods : Between November 1992 and June 2010, 32 patients (23 men and 9 women, mean age 56.1 years, range 39-73) were treated with GKRS for brainstem metastases. There were metastatic lesions in pons in 23, the midbrain in 6, and the medulla oblongata in 3 patients, respectively. The primary tumor site was lung in 21, breast in 3, kidney in 2 and other locations in 6 patients. The mean tumor volume was $1,517mm^3$ (range, 9-6,000), and the mean marginal dose was 15.9 Gy (range, 6-23). Magnetic Resonance Imaging (MRI) was obtained every 2-3 months following GKRS. Follow-up MRI was possible in 24 patients at a mean follow-up duration of 12.0 months (range, 1-45). Kaplan-Meier survival analysis was used to evaluate the prognostic factors. Results : Follow-up MRI showed tumor disappearance in 6, tumor shrinkage in 14, no change in tumor size in 1, and tumor growth in 3 patients, which translated into a local tumor control rate of 87.5% (21 of 24 tumors). The mean progression free survival was 12.2 months (range, 2-45) after GKRS. Nine patients were alive at the completion of the study, and the overall mean survival time after GKRS was 7.7 months (range, 1-22). One patient with metastatic melanoma experienced intratumoral hemorrhage during the follow-up period. Survival was found to be associated with score of more than 70 on Karnofsky performance status and low recursive partitioning analysis class (class 1 or 2), in terms of favorable prognostic factors. Conclusion : GKRS was found to be safe and effective for management of brainstem metastasis. The integral clinical status of patient seems to be important in determining the overall survival time.
Renal cell carcinoma (RCC) is the most frequent urological malignant tumor in adults and it occurs mostly between the fifth and the sixth decades of life. The male-female ratio is 3:1 and it is more common in smokers. It accounts for approximately 3% of adult malignancies; 90~95% of neoplasms of the kidney. The classic triad of presenting symptoms of RCC is hematuria, back pain and a mass in the flank. More than 50% of RCCs show metastasis to breast, lung and regional lymph nodes, and 15% present in the orofacialmaxillary region. This case is about a 66 year-old man who was treated for painless swelling in the left masseteric area. The mass was surgically excised and sent for biopsy. It was diagnosed as RCC and two weeks later nephronectomy of the left kidney was performed. Metastasis to other organs was detected and the patient received radiation therapy. In this case we were able to find the primary lesion by the metastatic lesion.
Objectives: This study is aimed to analyze the trend of research on nutrition, pain and fatigue of cancer patients in Korea, suggest direction for future research of nutrition, pain and fatigue of cancer patients. Methods: 74 studies published from 1991 to 2001 were examined according to the year of publication, types of journal, research design, types of disease, care methods, major concepts, tool and research findings. Results: 1) The number of studies related nutrition, pain and fatigue of cancer patients had increased rapidly since the 1996's(78.4%). 2) 42 nutrition, pain and fatigue of cancer patients studies(56.1%) were done for a thesis for a degree and 32 were nondegree research studies(43.9%). 3) 70 studies(94.7%) were quantitative study, which included in 40 descriptive studies(54.1%), 22 experimental studies (29.75), 5 correlative studies(6.8%), 2 comparative studies(2.7%) and 1 case study(1.4%), and 4 studies(5.3%) were qualitative study, which included in 3 content analysis studies(3.9%) and 1 phenomenological study(1.4%). 4) Researches about cancer more than 2 were the most by 48 studies (66.1%), following leukemia researches were 8 studies(10.8%), breast cancer researches were 7 studies(9.5%), gastric cancer researches were 4 studies(5.4%), pediatric cancer researches were 3 studies(4.1%), uterine cancer researches were 2 studies(2.7%). 5) Researches about chemotherapy were the most by 39 studies(52.6%), following analgesic researches were 14 studies(18.9%), researches that do not present treatment method were 9 studies(12.2%), radiation researches were 7 studies(9.5%). 6) In 22 experimental studies, the effects of 13 types of nursing interventions were tested. Research findings were effective almost but muscle relaxation therapy to decrease nausea and vomiting was no effect. Conclusion: We need more researches about research of nutrition, pain and fatigue of cancer patients, especially need to prove the effect of intervention or program for nutrition, pain and fatigue of cancer patients by experimental research designs and need more qualitative researches to identify indepth the meaning of nutrition, pain and fatigue of cancer patients.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
