영역평균 기반의 지오데식 동적 윤곽선 모델에 의한 뇌실 분할 (Segmentation of Brain Ventricle Using Geodesic Active Contour Model Based on Region Mean)

  • 본 논문에서는 지오데식 동적 윤곽선 모델을 이용하여 뇌실 영역을 검출하기 위하여 기존의 에지지시함수를 대신한 영역 기반의 곡선진행억제 함수를 제안하였다. 제안한 곡선 진행 억제 함수는 뇌실 영역의 검출에 매우 효과적이었으며, 이 함수는 MRI 영상에서 밝게 나타나는 뇌실 영역의 평균 밝기를 기반으로 한다. 본 논문에서는 제안한 방법이 기존의 방법보다 뇌실 영역을 잘 검출할 수 있음을 다양한 척도를 이용하여 수치적으로 비교하였다. 실제 정상과 뇌종양에 의한 뇌질환 영상에 적용시켜 뇌실 검출 과정을 시각적으로 비교하여 우수성을 검증하였다.

  • 본 논문에서는 MRI 뇌 영상에서 뇌실 영역을 검출하기 위하여 자동적으로 임계치를 설정하는 방법을 제안하였다. 웨이브릿 변환 후 수평 및 수직 신호의 상세 신호 크기를 이용하여 물체의 윤곽선에서 상세신호의 크기 평균을 의미하는 에지 선예도를 계산하였다. 영역 성장을 위한 임계치를 반복적으로 증가시켜 에지 선예도가 최대일 때 최적 임계치를 설정하여 뇌실 영역을 검출하였다. 본 논문에서는 제안한 방법과 지오데식 동적 윤곽선 모델을 수치적으로 비교하였으며, 실제 MRI 뇌영상에 적용시켜 제안한 알고리즘의 유효성을 검증하였다.

Cystic Hemangiopericytoma in the Third Ventricle

  • Primary intracranial hemangiopericytoma is rare and resemble meningioma on imaging study. It shows meningeal attachment, and is usually isointense with gray matter on T1-weighted MR image with heterogeneous enhancement and prominent vascular flow voids on T2-weighted image. Cystic type of hemangiopericytoma is very rare and only 3 cases have been reported in the literature which arised in the middle fossa, cerebellum, and occipital area. Ventricular hemangiopericytomas were reported in 9 cases, and all of them were solid type. Authors experienced a peculiar case of cystic hemangiopericytoma in the 3rd ventricle and report it with review of the literature.

Expanded Endoscopic Transnasal Approach to the Chordoid Glioma of the Third Ventricle : The First Case Ever Reported

  • Chordoid glioma of the third ventricle is a rare and challenging tumor to surgery because of its unique anatomical location and its close juxtaposition to the neurovascular structures and hypothalamus. The authors report a case of chordoid glioma of the third ventricle in a 43-year-old woman, who presented with headache and somnolence. The tumor was approached by endoscopic transnasal technique with a favorable result. Histopathologic examination disclosed a neoplastic tissue composed of eosinophilic epithelioid cells, mucinous, periodic acid Schiff-diastase positive, extracellular matrix, and scattered lymphoplasmacytic infiltrates. The best treatment option remains controversial. Customarily, the surgical route to remove chordoid glioma is transcranial; however, the undersurface of the optic chiasm and optic nerves preclude an adequate surgical visualization. In contrast, an expanded endoscopic transnasal approach provides a direct midline corridor to this region without any brain retraction.

Evaluation of the lateral ventricle using MRI in normal micropigs

  • This study was undertaken to assess the lateral ventricle, which was some portion of brain and related to congenital anomalies, from 1, 2, 4, and 8 months of age in healthy micropigs. They were induced general anesthesia and performed magnetic resonance imaging (MRI) with a 0.3 Tesla magnet. Each age group was evaluated by three subjects such as lateral ventricular volume, ventricular volume ratio and asymmetry. T1 weighted transverse images were acquired for calculation of lateral ventricular and corresponding brain parenchyma areas. The ratio of bilateral ventricle areas used to analyze the asymmetry. The mean ventricular volumes of each month were $676.74{\pm}25.58mm^3$ (1 month-old), $630.64{\pm}143.84mm^3$ (2 month-old), $992.12{\pm}106.03mm^3$ (4 month-old) and $1172.62{\pm}237.57mm^3$ (8 month-old), respectively. The ventricular volume ratio was the smallest at 2 month-old and re-increased from that age. The ratio was significantly different between 2 month-old and other age groups (p < 0.05). The value of bilateral area ratio showed within 1.5 in all experimental animals. Consequently the lateral ventricle showed a positive correlation with aging and symmetric shapes in both sides. The developmental pattern of the lateral ventricle provides basic data in micropigs as an experimental animal model for physiological and neurosurgical approach.

  • Various grading systems and surgical techniques have been developed for the treatment of intraventricular hemorrhage (IVH); however, little attention has been paid to the fourth ventricle hematoma. Nonetheless, hemorrhagic dilation of the fourth ventricle may lead to catastrophic consequences for patients with massive IVH. We present two cases of massive IVH accompanied by massive fourth ventricle hematoma which was successfully removed with combination of suboccipital craniotomy for fourth ventricle hematoma and intraventricular fibrinolysis for supratentorial hematoma.

Chordoid Glioma of the Third Ventricle with Unusual MRI Features

  • Chordoid glioma is a rare primary brain tumor of the third ventricle, and was recently characterized as a novel tumor entity. Typical radiological features of chordoid gliomas include; a solid, round-to-ovoid, well-circumscribed, contrast-enhancing mass of the hypothalamus and anterior third ventricle. Despite being classified as WHO grade 2, the third ventricular chordoid glioma has been reported to have a poor clinical outcome because of its anatomical location. The authors report a case of chordoid glioma of the third ventricle in a 48-year-old man with unusual radiological features of cystic component and ill defined circumscription.

뇌교에 발생한 악성 핍지신경교종 1례 (A Case of Brain Stem Anaplastic Oligodendroglioma with Exophytic Growth)

  • Primary anaplastic oligodendroglioma in brain stem is extremely rare. The authors present a case of a anaplastic oligodendroglioma arising from pons. A 29 year-old male patient was admitted because of cranial nerve palsy and visual disturbance. Neurological examination revealed bilateral sixth and left seventh cranial nerve plasies. Near-total resection of tumor mass was performed through midline suboccipital appraoch. Tumor was not related with choroid plexus and major vessels but it was firmly attached to the fourth ventricle floor. Tumor was considered to be arised from the tegmental portion of pons, growing dorsally into the 4th ventricle. Hitopathological exmination revealed primary anaplastic oligodendroglioma. Postoperative course was uneventful. The authors believe that this type of tumor with dorsally growing pattern can be successfully resected without major neurological deficit.

뇌실로 파열되어 있는 뇌농양에 대한 뇌정위적 내시경하 제거술 - 증례보고 - (Stereotactic Endoscopic Treatment of Brain Abscess Ruptured into Ventricle - Case Report -)

  • The major indication of endoscope in neurosurgical field is intraventricular procedures. Recently, it can be used selectively in the intraaxial mass lesion associated cavity. For example, cystic mass, with liquefied necrosis, and blood clot can be approached with this technique. The authors present its usage in brain abscess ruptured into lateral ventricle. The neuroendoscope was introduced into abscess cavity through stereotactic guidance, the pus was then removed through continuous irrigation and suction under direct video visualization. The intraventricular pus was also cleaned through gentle, direct endoscopic irrigation and suction. The postoperative clinical course was uneventful. Brief overview is given for this intraaxial neuroendoscopic procedure.

경도인지장애를 동반한 파킨슨병 환자의 서울신경심리검사와 뇌 자기공명영상에서 뇌실 체적 변화에 대한 상관관계 (Correlation between the Seoul Neuropsychological Screening Battery of the Parkinson's Disease Patient with Mild Cognitive Impairment and Change of the Cerebral Ventricle Volume in the Brain MRI)

  • 본 연구는 경도인지장애가 동반된 파킨슨병 환자에 대하여 인지 기능 평가를 위한 서울신경심리검사와 뇌 자기공명 영상에서 영역별 뇌실에 대한 체적 변화를 분석하여 경도인지장애가 동반된 파킨슨병에 대한 진단적 기준을 판단할 수 있는 가이드라인을 제시하고자 하였다. 이를 위하여 파킨슨병으로 진단 받은 환자(경도인지장애군: 34명, 비인지장애군: 34명)를 대상으로 주의력, 언어, 기억력, 시공간, 그리고 전두엽 집행 기능에 대한 서울신경심리검사와 뇌 자기공명영상 검사를 시행하였다. 또한 영역별 뇌실에 대한 체적 변화를 비교하기 위하여 정상인 32명(정상 대조군)을 추가 선정하여 추가적으로 뇌 자기공명영상 검사를 시행하였으며 영역별 뇌실에 대한 체적 분석은 프리서퍼(Freesurfer Ver. 5.1, Boston MA, USA)를 통해 수행되었다. 결과적으로 경도인지장애군은 시공간 및 기억력에 대한 수행 능력이 비인지장애군과 비교하여 통계적으로 유의하게 감소되었다(p<0.05). 영역별 뇌실에 대한 체적 변화는 좌 우측뇌실, 좌 우하측뇌실, 제3뇌실에서 통계적으로 유의한 체적 변화를 나타내었으며 객관화된 비교를 위하여 정규화한 백분율을 적용한 체적의 변화는 비인지장애군보다 경도인도장애군에서 확장되어 나타났다. 특히 경도인지장애를 동반한 파킨슨병 환자의 좌 우측뇌실의 확장은 서울신경심리검사에서 시공간 및 기억력 영역에 대하여 뚜렷한 양적 선형관계를 보였다(r>0.5, p<0.05). 따라서 뇌 자기공명영상에서 프리서퍼를 이용하여 영역별 뇌실의 체적 변화를 관찰하고 서울 신경심리검사와 상관관계를 분석함으로써 경도인지장애를 동반한 파킨슨병 환자의 진단적 기준을 마련할 수 있을 것으로 판단되었다.
