Sleeve Resection of Lung Cancer - A report of 5 cases- (폐암 환자에서의 기관지 성형술 -5례 보고-)

  • Sleeve resection is safe, effective, and appropriate treatment for a wide range of endo-bronchial lesions including neoplasms of low grade malignant potential and selected cases of bronchogenic carcinoma. Five cases of bronchoplastic procedures were performed for primary bronchogenic carcinoma patients at Department of Thoracic and Cardiovascular Surgery, Chonbuk National University Hospital from Aug. 1983 to Oct. 1987. Of the 5 patients, four were male and one patient was female and ages ranged from 51 years to 66 years old. Histopathologically, 4 cases were squamous cell carcinoma and one case small cell carcinoma. Operative procedures of the 5 patients were as follows: Right upper sleeve lobectomy, 1 case; Left upper sleeve lobectomy, 1 case; left lower sleeve lobectomy, 1 case; Left lower lobe and lingular segment sleeve resection, 2 cases. The early and late postoperative complications of the above operations were pneumonia, atelectasis, bronchopleural fistula, empyema, brain metastasis, and local recurrence of primary tumor.

  • A hiccup is an involuntary, spasmodic contraction of the diaphragm accompanied by a sudden closure of the glottis, which is reported commonly in patients with brain stem disease such as ischemic stroke, dolichoectatic basilar artery, tumor, encephalitis, and multiple sclerosis. 1) Intractable hiccup is an uncommon, chronic and incapacitating disturbance defined as a hiccup bout lasting more than 48hours or recurring despite various treatments and affecting male subjects more than female. 2) Constipation and hiccup are common symptoms in stroke patients and purgation therapy has been often used. We discovered two patients who had a hiccup symptom after purgation therapy(diarrhea) and so reported course and result of treatment.

  • Purpose: To evaluate the white matter fiber connectivity of normal human using diffusion tensor MRI. Method: Normal young healthy volunteers (2 women and 1 man) and 3 brain tumor patients participated in this study. All studies were performed using a 1.5T Philips Gyroscan Intern system. Diffusion weighted imaging was performed using single-shot echo planar imaging, with navigator echo phase correction and SENSE. Diffusion weighting was performed along six independent axes, using diffusion weighting of b=800s/$\textrm{mm}^2$. 128matrix, 23cm FOV, 2.5mm slice thickness were used for Imaging parameters. Data were processed on a Window-2000 PC equipped with IDL and PRIDE (Philips Medical System). Corticospinal tract was traced from mid-pons level via posterior limb of internal capsule. Corpus callosum, cerebellar peduncles and frontal fibers were traced by fiber tractography.

The Anthocyanin Components and Cytotoxic Activity of Syzygium cumini (L.) Fruits Growing in Egypt

  • Four anthocyanins were isolated from the acidic alcoholic extract of Syzygium cumini fruits growing in Egypt: Pelargonidin-3-O-glucoside, pelargonidin-3,5-O-diglucoside, cyanidin-3-O-malonyl glucoside, and delphenidin-3-O-glucoside. They were identified by the chromatographic, TLC and PC, and spectral analyses, UV, $^1$H-NMR and FAB/MS. The fruits were found to contain 0.03 gm % anthocyanins calculated on fresh weight basis calculated by spectrophotometric assay. Cytotoxic activity of total alcoholic extract of the fruits was performed against several types of tumor cell lines using the SRB assay. The tested extract exhibited significant cytotoxic activity for MCF7 (breast carcinoma cell line) (IC$_{50}$= 5.9 ${\mu}$g/mL), while the IC$_{50}$ was > 10 ${\mu}$g/mL for both Hela (Cervix carcinoma cell line), HEPG2 (liver carcinoma cell line), H460 (Lung carcinoma cell line) and U251 (Brain carcinoma cell line).

  • Hereditary syndromes cause approximately 5 to 10% of overall cancer cases. Cancer related with genetic syndromes are found elsewhere, including stomach, breast, colorectum, ovary, brain and so on. Because hereditary cancers are due to germline mutations, these patients have unique clinical features distinct from sporadic cancer. Generally these features include (i) early age-of onset of cancer, (ii) frequent association with synchronous or metachronous tumors, (iii) frequent bilateral involvement in paired organs (iv) frequent association with other site tumors or characteristic clinical manifestation specific to each genetic syndrome. Due to these differences, the management strategy for patients with hereditary cancer is quite different from that for sporadic cancer. Additionally, there are important screening and surveillance implications for family members. Genetic counselling is prerequisite to these families for risk assessment by pedigree analysis, and guidance to clinical or genetic testing. The genes responsible for these syndromes has recently identified, as a result, genetic testing has become important determining factor in clinical decisions.

A Case of Gliosarcoma Associated with Neurofibromatosis Type 1 (제 1 형 신경섬유종증과 동반된 교육종)

  • A rare case of gliosarcoma with neurofibromatosis type I is presented. The patient was a 33-year-old woman who had headache and vomiting for one week. Multiple neurofibromas over her whole body with many cafeau- lait spots were present since childhood. At admission, she had no focal neurological deficit and ophthalmologic examination revealed bilateral Lisch nodules. Brain CT and MRI revealed a heterogeneously enhancing mass in the left fronto-parietal region with marked peritumoral edema and mass effect. The tumor was removed gross totally and a gliosarcoma was diagnosed histopathologically. Post operative course was uneventful with resolution of symptom, followed by radiotherapy with 60 Gy. A brief overview is given of this rare case together with the pertinent literature.

  • Intradiploic epidermoid cysts, like epidermoid cysts in other cranial locations, are rare. Approximately 100 intradiploic epidermoids have been reported, involving all of the cranial bones in proportion to their relative sizes. Over half erode through both tables of the cranium, creating variably sized areas of unprotected brain beneath the soft tumor. We report a case of an intradiploic epidermoid cyst of the right parietal bone that was found after minor head trauma.

Intra-Suprasellar Schwannoma Originating from the Diaphragma Sellae

  • A 49-year-old woman presented with headache, vomiting and visual disturbance. Neurological examination revealed bitemporal hemianopsia with poor visual acuity. Magnetic resonance imaging showed a bulky intra-suprasellar mass, which was isointense with brain parenchyma on T1-weighted images, and slightly hyperintense on T2-weighted images. After gadolinium administration, the mass was homogeneously enhanced. The mass was partially removed by the endonasal transsphenoidal approach and then the remnant mass was totally removed by the transcranial approach five months later. We found a yellowish mass which was attached to the diaphragm sellae in operation field. Histopathological examination of the tumor revealed the characteristic features of a schwannoma. We report an unusual case of an intra-suprasellar schwannoma resembling a non-functioning pituitary macroadenoma both clinically and radiologically.

Multimodality and Non-rigid Registration of MRI' Brain Image

  • Registering different kinds of clinical images widely used in diagnostic and surgery planning. However, cause of tumor growth or effected by gravity, human tissue has plenty of non-rigid deformation with clinically. Non-rigid registration allows the mapping of straight lines to curves. Therefore, such local deformation makes registration more complicated. In this work, we mainly introduce intra-subject, inter-modality registration. This paper mainly studies the nonlinear registration method of 2D medical image registration. The general medical image registration algorithm requires manual intervention, and cost long registration time. In our work to reduce the registration time in rough registration step, the barycenter and the direction of main axis of the image is calculated, which reduces the calculation amount compared with the method of using mutual information.
