스네이크를 이용한 뇌 자기 공명 영상에서 종양의 경계선 추출 (Tumor boundary extraction from brain MRI images using active contour models (Snakes))

  • 본 연구는 스네이크를 이용하여 뇌의 자기 공명 영상에서 자동 혹은 반자동으로 종양 또는 병변의 정확한 윤곽선을 찾기 위함이다. 본 연구에서 기존의 스네이크가 가지고 있는 에너지 최적화 문제를 동적 프로그래밍을 이용하여 개선하였고, Image Force로 Canny Edge Detector의 값을 이용하여 스네이크가 잡음에 덜 민감하도록 하였다. 병변의 윤곽선이 추출되면, 병변의 면적, 중심 좌표, 둘레 등을 계산하도록 하였다 또한 병변에 대한 다수의 2차원 단면 영상을 합성하여 3차원으로 재구성하여 병변의 입체적인 모양을 볼 수 있도록 하였다. 본 연구에서 제안된 방법은 뇌종양 환자의 치료 계획 수립 뿐 아니라 경과를 평가하는데 유용하게 활용될 것으로 기대된다.

Intracranial Pial Arteriovenous Fistulas

  • Intracranial pial arteriovenous fistula (AVF) is a rare cerebrovascular lesion that has only recently been recognized as a distinct pathological entity. A 41-year-old woman (Patient 1) presented with the sudden development of an altered mental state. Brain CT showed an acute subdural hematoma. A red sylvian vein was found intraoperatively. A pial AVF was revealed on postoperative angiography, and surgical disconnection of the AVF was performed. A 10-year-old boy (Patient 2) presented with a 10-day history of paraparesis and urinary incontinence. Brain, spinal MRI and angiography revealed an intracranial pial AVF and a spinal perimedullary AVF. Endovascular embolization was performed for both lesions. The AVFs were completely obliterated in both patients. On follow-up, patient 1 reported having no difficulty in performing activities of daily living. Patient 2 is currently able to walk without assistance and voids into a diaper. Intracranial pial AVF is a rare disease entity that can be treated with surgical disconnection or endovascular embolization. It is important for the appropriate treatment strategy to be selected on the basis of patient-specific and lesion-specific factors in order to achieve good outcomes.

Aneurysmal Bone Cyst of the Orbit : A Case Report with Literature Review

  • Aneurysmal bone cyst (ABC) is benign vascular lesion destructing the cortical bone by the expansion of the vascular channel in the diploic space that usually involve long bone and spine. Orbital ABC is rare and the clinical symptoms deteriorate rapidly after initial slow-progression period for a few months. A 12-year-old female patient visited ophthalmologist due to proptosis and upward gaze limitation of the right eye, and orbital mass was noted in the upper part of right eye on orbital MRI. Five months later, exophthalmos was worsened rapidly with other features of ophthalmoplegia. Orbital mass was enlarged on MRI with intracranial extension. Surgery was done through frontal craniotomy and intracranial portion of the tumor was removed. Destructed orbital roof and mass in the orbit was also removed, and surrounding bone which was suspected to have lesion was resected as much as possible. Histopathological diagnosis was aneurysmal bone cyst. Postoperative course was satisfactory and the patient's eye symptoms improved. Authors report a rare case of orbital ABC with review of the literature. Exact diagnosis by imaging studies is important and it is recommended to perform surgical resection before rapid-progressing period and to resect the mass completely to prevent recurrence.

Cavernous Sinus Metastasis of Non-Small Cell Lung Cancer

  • Progressive ptosis and headache developed in a 50-year-old woman with non-small cell lung cancer. Although brain magnetic resonance imaging showed improved cerebellar metastasis after prior radiotherapy without any other abnormality, the follow-up examination taken 6 months later revealed metastasis to the cavernous sinus. The diagnosis of metastasis to the cavernous sinus is often difficult because it is a very rare manifestation of lung cancer, and symptoms can occur prior to developing a radiologically detectable lesion. Therefore, when a strong clinical suspicion of cavernous sinus metastasis exists, thorough neurologic examination and serial brain imaging should be followed up to avoid overlooking the lesion.

발성실행증 사례연구 (Apraxia of Phonation: a Case Report)

  • Apraxia of phonation (AOP) has often been described as a feature of apraxia of speech or of severe non-fluent type of aphasia. Pure AOP is rare and, to our knowledge, only two cases have been reported. Brain lesion sites of the reported cases were not those sites known to be responsible for apraxia of speech. This study presents a case of AOP which resulted from the secondary stroke in the left corona radiata immediately following the first stroke in the left temporoparietal lobe. A 61-year old right-handed man shwoed a global type of aphasia after the first cerebral infarction, but was able to generate spontaneously some short fragments of speech. On the day after the first infarction, he suffered from the secondary infarction, leaving him a complete loss of voluntary phonation. He did not showed any significant change in language functions. Several occurrences of involuntary phonation were observed upon laughing or crying. He was also able to cough unintentionally. A vidoe-stroboscopic examination failed to reveal any evidence of structural and functional impairment in larynx. Although this case is not of pure form of AOP, AOP appeared after the secondary stroke without significant changes of language impairment since the first stroke. Therefore, AOP may be a consequence of the brain lesion from the secondary stroke even though we cannot rule out the possibility of an additive effect of the secondary to the first stroke.

가역적인 뇌기능장애를 보인 MELAS 증후군 (Reversible Brain Dysfunction in MELAS Syndrome)

  • The MELAS (Mitochondrial Encephalomyopathy with Lactic Acidosis, and Stroke-like episodes) syndrome is one of the inherited mitochondrial disorder. We have experienced a 16-year-old girl with headaches and left hemianopsia. Diagnosis of MELAS syndrome with multiple brain parenchymal lesions was confirmed by gene study. The stroke-like lesion of MELAS syndrome showed significant improvement in radiological follow up study. Therefore, MRI findings in MELAS could be interpreted as metabolic cellular dysfunction rather than ischemic vasculopathy.

  • This study was taken to clarify the histopathological changes of pigs naturally infected with hog cholera. Microscopic observations of the necrotic lesion and inclusion body in the lymphoid organs were carried out in the natural cases of hog cholera and experimental cases inoculated with ALD virus and isolated virus strains. Electron microscopic findings of the intranuclear inclusion bodies in the reticular cell of spleen and lymph node were also observed in the experimental cases. The results obtained are as follow, As the histological findings necrosis of lymphoid organs was observed mainly in the lymph follicle. The necrotic lymphoid organs were found to contain 35.0% in the natural and 37.5% in the experimental cases. Intranuclear inclusion bodies were found mainly in the reticular cells of lymphoid organ, the epithelium of bronchiole and alevolus, and the vascular endothelium of brain. These inclusion bodies were seen in 40.0% of the natural cases and all of the experiment. The inclusion body was appeared to compose of activated nucleoli and chromatin granules (interchromatin and perichromatin) by electron microscopy.

두 개내 초급성 출혈 : EPI와 다른 MR 영상 기법의 비교 (Hyperacute Intracerebral Hemorrhage : Comparison of EPI and Other MR Sequence)

  • 목적 : Echo planar image (EPn와 다론 MR 영상기법에서 2시간 이내의 초급성 출혈의 발견율의 차이를 비교해 보고자 한다. 대상 및 방법 : 인위적으로 뇌출혈을 유발시킨 백서 10마리를 대상으로 EP1, 급속 스핀에코 (FSE) T2 강조영상, Fluid attenuated inversion recovery (FLAIR) 영상, 스펀에코 (SE) Tl 강조영상과 Tl 강조 Gradient echo (GE) 영상을 이용하여, 뇌출혈을 일으킨 2시간 후에 MR 촬영을 하였다. 추가적으로 3마리는 출혈 30분 후에, 6마리는 1시간 후에 각각 EPI와 FSE T2 강조영상을 얻었다. 얻어진 영상에 대한 주관적인 시각적 평가를 하기위하여 세명의 방사선과 의사가 출혈 병변이 어느 MR 영상 기법에서 제일 잘 보이는지를 평가하였다. 정량적인 평가 로는 병변-정상 뇌설질의 대조잡음비(contrast-to-noise ratio, CNR)를 측정하였다. 통계처리는 Wilcoxon-Ranks test를 이용하였다. 결과 : 시각적 평가에서 출혈 병변이 높은 신호강도로 나타난 EP1, FLAIR, FSE T2 영상 중 EPI 영상이 특히 높은 신호강도로 나타나 제일 쉽게 병변이 구별되었다. 정량적인 평가에서 출혈 병 변-정상 뇌설질의 CNR은 EPI와 FLAIR 영상에서 다른 세 영상기법에 비해 통계적으로 유의하게 우월하였다 (p<0.01). 그러나 EPI와 FLAIR 영상간의 CNR은 동계적으로 유의한 차이가 없었다 (p>0.10). 추가적으로 출혈 30분과 1l시간 후 촬영한 모두에서 EPI영상과 FSE T2 강조 영상은 정상에 비해 높은 신호강도를 보여 병변의 구별이 가능하였다. 결론 : 백서에서 유발시킨 초급성기 뇌출혈의 여러 기법의 MR 영상 중 EPI 영상이 영상획득 시간이 가장 빠르면서 시각적 평가와 정량적 평가에서 출혈의 발견율이 가장 높아 출혈의 선속한 진단에 유용할 것으로 생각된다.

  • Objectives : This study investigated stroke lesions and their relationship with warning signs and risk factors in stroke patients. Methods : Three hundred fifty-three stroke patients were recruited at the Department of Cardiovascular and Neurologic Diseases (stroke center) of Kyung Hee University Oriental hospital from October 2005 to July 2007. We evaluated their stroke lesions with brain MRI, their warning signs, and risk factors. Results : 353 subjects were included in the final analysis. The frequency of diabetes was found more in presence of brain stem lesion than without. The cortex lesion showed a close relationship with smoking, drinking and the frequency of one side paralysis or weakness. These were more common in males than in females. Conclusions : We observed stroke lesions and their characteristics in stroke patients. The subjects' brain lesions seemed affectedby the risk factors. A concrete conclusion can hardly be drawn from this study; additional research is necessary.

한방병원에 입원한 심인성 뇌색전증 환자에 대한 고찰 (A Review of Cardioembolic Stroke Patients Hospitalized in Hospital of Korean Medicine)

  • ■ Objectives Atrial fibrillation is the most common cause of cardioembolic stroke. Of the 44 ischemic stroke patients with atrial fibrillation who were hospitalized in hospital of Korean Medicine from July 1, 2014 to June 30, 2017, we selected 39 patients who have had Magnetic Resonance Angiography. We divided them into Atrial Fibrillation group with no stenosis or less than 50% stenosis in the ipsilateral artery of the lesion and Artery to Artery Embolism group with more than 50% stenosis or occlusion in the ipsilateral artery of the lesion. ■ Methods Clinical characteristics, examination and evaluation tools were collected from the patient's electronic medical records. CHADS2, Initial National Institutes of Health Stroke Scale, 8-item Stroke Scale and Improved 8-item Stroke Scale Number were checked. ■ Results & Atrial Fibrillation group showed differences in age, brain lesion location, vascular lesion, Conclusion initial National Institutes of Health Stroke Scale, initial 8-item Stroke Scale and progress compared to Artery to Artery Embolism group.
