갈근해기탕(葛根解肌湯)의 기원(基源), 변천(變遷), 구성(構成)에 대한 고찰(考察) (The Study on The Origin, Changes, and Composition of Galgeulhaegi-tang)

  • 1. Objectives : This paper was written in order to understand the origin and changes of Galgeunhaegi-tang. 2. Methods : We analysis Galgeunhaegi-tang through pathology and new prescription in "Donguibogam(東醫寶鑑)", "Chobongwon(草本卷)", "Gabobon(甲午本)" and "Sinchukbon(辛丑本)" of "Dongyi Suse Bowon". 3. Results and Conclusions : We can find the first origin of Galgeunhaegi-tang in "Sinchukbon(辛丑本)" in "Donguibogam(東醫寶鑑)" which invelves Galgeunhaegi-tang in the categories of pestilence(瘟疫) and Yangmyeonggyeongbyeong(陽明經病). Also, we can find the origin in Jugoeng(朱肱)' Jojung-tang. Seungeumgalgeun-tang in the "Chobongwon(草本卷)" developed by a Galgeunhaegi-tang of "Gabobon(甲午本)", and Galgeunnabokja-tang in the "Chobongwon(草本卷)" developed by a Galgeunhaegi-tang of "Sinchukbon(辛丑本)". It is considered that Galgeunhaegi-tang of "Gabobon(甲午本)" solves heat depression(鬱熱), due to not throwing off, by using Galgeun(葛根) and Daehwang(大黃), and accompanied dryness(燥) by using Hangin(杏仁) and Sanjoin(酸棗仁) which moisten dryness(燥). It is considered that Galgeunhaegi-tang of "Sinchukbon(辛丑本)" solves heat depression(鬱熱) by using Galgeun(葛根) and accompanied dryness(燥) by using Gobon(藁本) which sets free dryness(燥).

  • In oriental medicine, Byun-jo(煩躁) is including in insomnia, restless, uneasiness, chest discomfort, irritation et all. Byun-jo is one of symptom in Soyangin(少陽人)(one of human type in Sasang constitutional medicine(四象醫學) the lower chest-overheated disease(胸膈熱證). In the book 'dongyi soose bowon(東醫壽世保元)', Soyangin Yanggueksanwha-tang(凉膈散火湯) is used at Soyangin the lower chest-overheated disease. One patient usually who had suffered from chest discomfort, insomnia of Byun-jo, after cerebral infarction the symptom of Byun-jo had grown worse, the patient was classified as Soyangin by somatotype, personality, therefore the patient treated with Soyangin Yanggueksanwha-tang for 10days, and the patient improved in clinical symptoms. so we report it for the better treatment.

소음인(少陰人) 약리(藥理)의 형성과정(形成過程) 관한 연구(硏究) -갑오본(甲午本)과 신축본(辛丑本)을 중심으로- (The Study on the Formative Process of Soeumin Pharmacology -Focused on Gabobon and Sinchukbon-)

  • 1. Objectives This paper was written in order to understand the formative process of Soeumin pharmacology. 2. Methods Souemin pharmacology was analysed with pathology and new prescription in Gabobon(甲午本) and Sinchukbon(辛丑本) of ${\ulcorner}$Dongyi Suse Bowon${\Ircorner}$. 3. Results and Conclusions Soeumin is charactrized to much output of kidney and a little input of speen in sight of ingestive food(水穀). So deficiency of YangQi is a peculiarity of pathology and ascending-Qi is a basic pharmacology. The pharmacology in the exterior disease of Soeumin is built up to base on the old prescription of previous text in Gabobon. Ascending-Yang is the basic pharmacology of Sinchukbon and prescription is summarized to base on the Kyuji-tang. So new prescriptin of Chungoongkyuji-tang and Hwangkikyuji-tang is made by combine Kyuji-tang with Koongkihyangso-san and Bojoongikki-tang. The pharmacology in the interior disease of Soeumin is built up to devide to the weakness of Stomach-Qi, dyspepsia and invasion of cold-Qi. Descending Yin is the basic pharmacology of Sinchukbon and prescription is summarized to base on the Yijoong-tang. Sokunjoong-tang's pharmacology of abdominal pain is newly added and applided to Baekhaoyijoong-tang and Kwankyuboojayijoong-tang. The discourse of the symptoms and diseases at the Sasang Constitutional Medine is built up to base on the previous text in Gabobon and base on clinical experience in Sinchukbon. So clinical experience is the power of summarizing the pharmacology and escaping the previous pharmacology.

요로감염(尿路感染)으로 인한 소음인(少陰人) 발열(發熱) 치험례(治驗例) (A Case Study of the Febrile Soeumin Patient Caused by UTI(Urinary Tract Infection))

  • 1. Objectives In the western Medicine, UTI generally bring on fever, and treated by antibotics. In the traditional way of korean Medicine, our predecessors used the herb for the febrile disease that has the property of coldness. However, Lee Je-ma tried to make the Yang ascend with the herb, considering thar the febrile illness in Soeumin is made by the Exterior Heat not ascending. In the aspect of the Sasang Consititutional Medicine(SCM), Hwanggigyeji-tang(황지계지탕(黃芷桂技場)) & Palmoolgunja-tang (팔물군자탕(八物君子湯)) belongs to the kind of the warm medication. This study is reported to evaluate rhe effected of Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), Palmulgoonja tang(팔물군자탕(八物君子湯)) to the febrile patient. 2. methods To heal the febrile patient who was diagnosed as UTI & Soeumin exterior disease, we used soeumin Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), Palmulgoonja tang(팔물군자탕(八物君子湯)). 3. Results This parient's main symptoms are fever, sweating impotently feeling, anorexia. So we treated the patient with Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), (팔물군자탕(八物君子湯)) in ${\ulcorner}$Dongyi Suse Bowon(동의수세보원(東醫壽世保元))${\lrcorner}$ and improved the patient?s symptoms. 4. Conclusions We suggest that Soeumin Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), (팔물군자탕(八物君子湯)) are effective significantly to the patient with febrile illness, bur then we consider that the studies on the case like this are more needed to convince that Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), (팔물군자탕(八物君子湯))are very much effective.

인삼(人蔘)과 부자(附子)의 배합에 관한 고찰(考察)(동의보감(東醫寶鑑)을 중심(中心)으로) (Applications of Prescriptions Containing Ginseng Radix and Aconiti Lateralis Preparata Radix in Dongeuibogam)

  • Objectives : This study was performed to investigate the applications of 47 prescriptions containing Ginseng Radix and Aconiti Lateralis Preparata Radix in Dongeuibogam. Methods : 47 prescriptions containing Ginseng Radix and Aconiti Lateralis Preparata Radix which have been used separately or concurrently in Oriental Medicine for a long time as a treatment for various disease in Dongeuibogam were studied through order of frequency, symptoms, dosages, etc. Results : 1. 17.02 % prescriptions are used in cold chapter with the 8 frequency, 14.89% in fatigue chapter with the 7 frequency, 10.64% in stool and wind chapter each with the 5 frequency, and 6.38% in children chapter with the 3 frequency. 2. Cold-diarrhea, Yang deficiency treating herbs, General fatigue treating herbs are most used chapters and articulation pain, severe cold, convulsion chapters are following frequency among 38 symptoms in prescriptions including Ginseng Radix and Aconiti Iateralis Preparata Radix. 3. In the prescriptons which has used more Ginseng Radix than Aconiti Lateralis Preparata Radix, the dosage ratio between Ginseng Radix and Aconiti Lateralis Preparata is ranged from 2.5 puns:1.5 puns ~ 1 jeon:7 puns. Among the range, 1 jeon:5 puns is recorded 2 times(33.33%), the rests are 1 time(16.67%). 4. In the prescriptons which has used same amount Ginseng Radix and Aconiti Lateralis Preparata Radix, the dosage ratio Ginseng Radix and Aconiti Lateralis Preparata is ranged from 2 puns ~ 1 nyangs each. Among the range, 1 jeon is recorded 12 times(41.38%), 1.25 jeon and 5 jeons are 3 times(10.34%) each, 0.5 jeon, 0.7 jeons and 3 jeons each are recorded 2 times (6.90%), the rests are 1 time(3.45%). 5. In the prescriptons which has used less Ginseng Radix than Aconiti Lateralis Preparata Radix, the dosage ratio between Ginseng Radix and Aconiti Lateralis Preparata is ranged from 3 puns:1 jeon ~ 1 nyang:3 nyangs. Among the range, 5 jeons:1 nyang is recorded 2 times(25.00%), the rests are 1 time(12.50%). 6. 5 jeons:1 piece are used 2 times(50.00%), the rests are 1 time(25.00%) each used among 3 prescriptions including Ginseng Radix and Aconiti Lateralis Preparata Radix. Conclusions : The 47 prescriptions including Ginseng Radix and Aconiti Iateralis Preparata Radix in Dongeuibogam are mainly composed of Samboo-tang, Sayeok-tang, Eejoong-tang, Samool-tang, Sagoonja-tang and Gobangpoong-tang, Bowon-tang, etc.

소양인처방(少陽人處方) 활용(活用)에 대한 임상적(臨床的) 연구(硏究) (A Clinical Study on Soyangin Prescription)

  • 1. Objectives Due to a lack of detail guidelines for prescriptions to ordinary and pathological symptoms in ${\ulcorner}$DongyiSuseBowon${\lrcorner}$, doctors depend on their own decision when give a prescription. This study aimed to compare ordinary symptoms and pathological symptoms in regards to six prescriptions being applied to Soyangin to utilize them as guidelines in practices. 2. Methods Date were collected from September, 2003 to August, 2004 at the Sasang Constitutional clinic of Pohang Oriental Medicine Hospital. Patients were first asked to fill Out a questionnaire of symptoms that they had at the time of visiting. Data from 273 Soyangin patients whose chief complains got alleviated after taking the prescribed drug were analyzed to examine if there were differences in prescriptions according to ordinary or pathological symptoms complained. 3. Results and Conclusions 1. Hyungbangpaedog-san(Paedog) was given more to the patients having good digestive function than Hyungbangjihwang-tang(Hyungji) and Hyungbangdojeok-san (Dojeok) ; Dojeok was given for bloating symptom compared to Hyungbangsabaek-san(Sabaek). Yangkyuksanhwa-tang(Yangkyuk) and Dojeok were more like for patients with stomachache compared to Paedog. Doghwaljihwang-tang(Dogji) was prescribed to patients complaining frequent voiding compared to Sabaek while Sabaek was given for patients who prefer cold water and feel their body warm compared to other prescriptions. Yangkyuk and Dojeok were prescribed more likely to patients with feeling of suffocation than Sabaek. 2. In symptom comparisons between one prescription and the group of the rest of 5 prescriptions, Paedog was found to be prescribed when patients had good digestive function while Dojeok was given in symptoms cases of indigestion, bloating, constipation and dizziness. Sabaek was applied more for patients having tendency of preferring cold water and feeling of heat body as well as good digestive function. Hyungji was used for patients complaining a symptom of fatigue, in addition Yangkyuk was given to patients who were nervous, sensitive, or impatient when compared to Other prescriptions. Dogji was applied to frequent voiding, insomnia, tingling of extremities, backache or leg pain, or in case of tendency of catching a common cold.

태음인(太陰人) 간수열(肝受熱) 이열병론(裡熱病論)을 통해 살펴본 과거의학(過去醫學)과 동의수세보원(東醫壽世保元)의 음양관(陰陽觀)의 차이(差異) (A study of the difference of Dongeui-Suse-Bowon and past Oriental-Medicine appeared in the argument of Interior-overheating-sympton of the Tae-Eum-In caused by liver's receiving heat)

  • 사상의학(四象醫學)은 과거의학(過去醫學)에 비해 간명한 변증체계로 모든 병증을 포괄하고 있다. 그것은 음양변증(陰陽辨證)과 태소변증(太少辨證)으로 이루어지는 사상변증(四象辨證)이 음양변화를 정확히 사분(四分)하여 줌으로써, 과거의학의 팔망변증(八網辨證)이나 장부변증(臟腑辨證)이 지닌 변증의 혼잡성을 극복해주기 때문이다. 이러한 사상변증의 장점은 과거의학에서 제대로 파악하지 못했던 태음인 태양인의 호산(呼散)-흡취강약(吸聚强弱)의 병증 파악에서 보다 선명하게 드러난다. 이에 본 논고에서는 첫째, 동의수세보원(東醫壽世保元)의 전편에 걸쳐 서술된 승강취산론(升降聚散論)을 살펴보고, 둘째 태음인(太陰人) 간수열(肝受熱) 이열병론(裏熱病論)의 양독증(陽毒症)과 조열병(燥熱病)에 인용된 과거 병론에서 병증의 음양 인식이 어떻게 변화해 왔는가를 조사하며, 셋째, 태음인(太陰人) 이열병(裏熱病)의 처방들이 어떻게 발전해왔는가를 조사함으로써, 태양인(太陽人) 이열병론(裏熱病論)에 나타난 동무의 음양관을 파악해 보았으며, 다음과 같은 결론을 얻었다. 동의수세보원(東醫壽世保元)에서 비신(脾腎)은 승양(升陽)-강음(降陰), 간폐(肝肺)는 호산(呼散)-흡취(吸聚)의 짝운동을 하는 것으로 본다. 따라서 소음인, 소양인의 병증은 음양승강(陰陽升降)의 병리로 분석되며, 태음인, 태양인의 병증은 음양취산(陰陽聚散)의 병리(病理)로 분석된다. 이러한 승강취산론(升降聚散論)은 과거의학의 승강부침론(升降浮沈論)과 개념은 같으나, 논리가 일관되지 못했던 과거의학과 달리 병리해석과 치법에 이르기까지 일관되게 체계화되어 있다. 예를 들어 태음인(太陰人) 이열병(裏熟病)은 욕화(欲火)로 인해 폐의 호산지기(呼散之氣)를 고갈시켜서 태음인의 장부특성인 '흡취지기(吸聚之氣) 태과(太過)-호산지기(呼散之氣) 부족(不足)'을 심화시킴으로써 오므로, 그 욕심을 놓고 폐(肺)의 호산지기(呼散之氣)를 회복시키는 약재를 쓰면 낫는다고 치법을 제시하였다. 내경(內經), 상한론(傷寒論) 시대에는 태음인(太陰人) 이열병(裏熱病)을 열증(熱證)으로만 인식하였다. 송원명(宋元明)의 의가(醫家)들은 태음인(太陰人) 이열병(裏熱病)의 병리(病理) 기전(機轉)을 '양기독성(陽氣獨盛) 음기폭절(陰氣暴絶)'로 해석하였으며, 치법(治法)을 '용산고지약(用酸苦之藥) 영음기복(令陰氣復) 이대한해(而大汗解)'로 제시하였다. 여기서의 음양의 뜻은 표리나 한열이아니라 기(氣)를 쓰는 것을 양, 기(氣)를 저축하는 것을 음으로 보는 포괄적인 음양의 개념이다. 따라서 흡취지기(吸聚之氣) 과다(過多)로 인한 태음인(太陰人) 간열증(肝熱證)과 하강지기(下降之氣)가 막혀서 오는 소양인(少陽人) 위열증(胃熱證)이 정확히 분별되지 못했다. 태음인(太陰人) 조열병(燥熱病)은 내경(內經)에서부터 인식되었으며, 열결(熱結)로 인해 조증(燥證)이 발생한다는 병리로 파악하였으나, 역시 소양인(少陽人) 위열병(胃熱證)의 소갈병(消渴病)과 정확히 구별하지 못하였다. 소양인(少陽人) 위열증(胃熱證)은 음기하강(陰氣下降)이 막혀 중상초(中上焦)에 병증이 나타나고, 태음인(太陰人) 간열증(肝熱證)은 호산지기(呼散之氣)가 고갈되어 중하초(中下焦)에 병증이 나타난다. 동무는 이러한 병증 분석을 통해 양독증(陽毒症)과 조열병(燥熱病)을 태음인 병증으로 인식했다고 판단된다. 의학사(醫學史) 초기에는 태음인 리열병증에 소양인(少陽人) 위열증(胃熱證) 약재와 태음인(太陰人) 간열증(肝熱證) 약재가 혼합된 청열방(淸熱方)들을 많이 썼다. 태음인(太陰人) 이열병방(裏熱病方)은 상한론에서 소음인 처방이나 소양인 처방에 갈근(葛根), 마황(麻黃), 승마(升麻) 등 태음인의 약재가 가미된 모습으로 출발하여, 주굉(朱肱)의 조중탕(調中湯)과 A 신(信)의 갈근해기탕(葛根解肌湯)으로 골격을 갖추었으며, 이를 모방(母方)으로 하여 태음인(太陰人) 갈근해기탕(葛根解肌湯)이 성립되었다. 그 외에 오행론(五行論)과 동의수세보원(束醫壽世保元)에서의 승강부침(升降浮沈)의 장부배속이 한 계절씩의 위상차이를 보이는 것은, 장부의 기능이 작용하여 현상으로 나타나는데 1/4주기의 시간차가 나는데 따른 것이라는 가설을 세워보았다.

And the maintenance of Yin-Qi is key point of Soyangin's pharmacology New prescription of Soyangin is made by regular rules in ${\ulcorner}$Sinchukbon${\lrcorner}$. Dongmu say that Hyungkae(荊芥) Bangpung(防風) Kanghwal(羌活) Dokhwal(獨活) is herbs for descending-Yin, and Sukjihwang(熟地黃) Sansuyoo(山茱萸) Bokryung Taeksa(澤瀉) or Saengjihwang(生池黃) Sukgo(石膏) Jimo(知母) is herbs for ascending-Yang And all new prescription except Sukjihwangkosam-tang(熟地黃苦蔘湯) is composed of asneding-Yang and descending-Yin's herbs. New prescription of the exterior disease contained all descending-Yin's herbs and that of the interior disease contained all ascending-Yang's herbs completly. Namely when ${\ulcorner}$Gabobon${\lrcorner}$ was written Soyangin pharmacology is the maintenance of Yin-Qi by selecting prescription of cold character. And when ${\ulcorner}$Sinchukbon${\lrcorner}$ was written, base sprit of Soyangin pharmacology was not changed. Pharmacology was added ascending-Yang and descending-Yin at a time.
