Purpose: The development of assistive devices has allowed for the performance of capsule endoscopy in children. Anticipating the capsule's transit time could affect the efficacy of the investigation and potentially minimize the fasting period. This study determined the predictors of small bowel transit time for small-bowel capsule endoscopy in children and adolescents with inflammatory bowel disease. Methods: We retrospectively examined children and adolescents with inflammatory bowel disease who underwent capsule endoscopy by the age 18 at a Japanese tertiary care children's hospital. Small bowel transit time predictors were analyzed using multiple regression with explanatory variables. Results: Overall, 92 patients, aged 1-17 years, with inflammatory bowel disease (63 Crohn's disease and 29 ulcerative colitis cases) were examined for factors affecting small bowel transit time. In the simple regression analysis, diagnosis, age, height, weight, serum albumin, general anesthesia, and small intestine lesions were significantly associated with small bowel transit time. In the multiple regression analyses, serum albumin (partial regression coefficient: -58.9, p=0.008), general anesthesia (partial regression coefficient: 127, p<0.001), and small intestine lesions (partial regression coefficient: 30.1, p=0.037) showed significant associations with small bowel transit time. Conclusion: Hypoalbuminemia, the use of general anesthesia for endoscopic delivery of the capsule, and small intestine lesions appeared to be predictors of prolonged small bowel transit time in children and adolescents with inflammatory bowel disease. Expecting the finishing time may improve examination with a fasting period reduction, which benefits both patients and caregivers.
In a patient with blunt abdominal trauma with small bowel injury, early diagnosis is clinically challenging due to unclear clinical symptoms and signs in the early stage of an injury. On the other hand, a delay of diagnosis of bowel disruption may lead to increased complication and mortality. The diagnostic laparoscopy is very useful for the evaluation of the small bowel injury. Laparoscopy can reduce unnecessary open surgery in a patient with blunt abdominal trauma with subtle symptoms and imprecise findings on abdominal computed tomography. Also it can prevent delay of treatment and be converted immediately to open surgery as soon as bowel damage is revealed. Furthermore, extracorporeal repair of small bowel via mini-laparotomy after a single incision diagnostic laparoscopy in a patient with traumatic small bowel perforation was a feasible and safe alternative to conventional laparoscopy. We are pleased to introduce successfully treated cases by extracorporeal repair of small bowel via mini-laparotomy after a single incision diagnostic laparoscopy in a patient with small bowel perforation after blunt trauma.
Background: The incidence of inadequate bowel preparation before elective colonoscopy in this region has never been studied. Materials and Methods: The authors prospectively described the incidence and factors related to inadequate bowel preparation in Thammasat university hospital. Bowel preparation quality was accessed by using Aronchick scale. Factors associated with inadequate bowel preparation were also recorded. Results: Two hundred patients undergoing elective outpatient colonoscopy were enrolled. Inadequate and fair bowel preparation was documented in 9 and 43%, respectively. Factors associated with inadequate bowel preparation were incomplete cleansing agent ingestion (odds ratio 7.7; 95%CI 1.62-36.64) and patient's unrecognization of vegetable avoidance (odds ratio 3.26; 95%CI 1.14-9.28). Conclusions: Compared with previous reported, inadequate bowel preparation was seen less in our study, however, more patients with fair bowel preparation was documented. Further study aiming at investigating the type and amount of fiber contained in diet before elective colonoscopy should be commenced.
Purpose: The purpose of this study was to evaluate the effects of an educational video program on bowel preparation for a colonoscopy. Methods: The study used a non-equivalent control group and non-synchronized design as a quasi-experimental research involving 101 participants undergoing bowel preparation for a colonoscopy (experimental group 51, control group 50 subjects) at W. university hospital, from Aug. 7 to Oct. 31, 2013. The control group received verbal education with an explanatory note while the experimental group received education using a video program. To measure knowledge of diet restrictions and compliance with ingesting bowel preparation solutions, a questionnaire, based on The Korean Society of Gastrointestinal Endoscopy's Guide (2003), developed by Sam-Sook You, was used after revisions and supplementation was done. To measure bowel cleanness, the 'Aronchick Bowel Preparation Scale' was adopted. Data were analyzed using the SPSS WIN 12.0 program. Results: A higher proportion of the experimental group showed a positive change in knowledge level on diet restrictions (U=1011.50, p =.035) and ingestion of bowel preparation solutions (U=980.50, p =.019), a higher level of compliance with diet restrictions (U=638.50, p <.001), ingesting bowel preparation solutions (U=668.00, p <.001) and the level of bowel cleanness (${\chi}^2$=17.00, p <.001) than the control group. Conclusion: The results of this study indicate that a video educational program for patients having a colonoscopy can improve knowledge, level of compliance with diet restrictions, ingestion of bowel preparation solutions, and bowel cleanness. Therefore video educational program should be used with this patient group.
Short bowel syndrome in children, most commonly results after extensive bowel resection for necrosis of the bowel. It may be caused by several intestinal catastrophes such as volvulus, hernia and necrotizing enterocolitis. The risk factors on short bowel syndrome are the remaining length of the bowel, the age of onset, the absence of the ileo-cecal valve and the time after resection. Macro and micronutritional deficiencies are the most significant complications of short bowel syndrome. We report a 5 year-old girl, who had a strangulated congenital transmesenteric hernia leading to short bowel syndrome accompanied by iron deficiency anemia.
Objectives: Irritable bowel syndrome is a functional disorder of the gastrointestinal tract marked by chronic abdominal pain and altered bowel habits. Irritable bowel syndrome is experienced by 11% of the global population. Although antidepressants and antispasmodic agents are currently used as therapeutic methods, they have side effects, so safer treatment agents are needed. Hence, I report the case of an outpatient who suffers from irritable bowel syndrome. Methods: The patient visited a public health center 14 times over four weeks and was treated with acupuncture and herbal medicine (Sosiho-tang and Jakyakgamcho-tang). Treatment progress was assessed using the Irritable Bowel Syndrome Severity Scoring System. Results: After 28 days of treatment, the Irritable Bowel Syndrome Severity Scoring System score decreased from 410 to 30 points without adverse events. The patient was highly satisfied. Conclusion: Sosiho-tang and Jakyakgamcho-tang with acupuncture might become recommended therapeutic options for irritable bowel syndrome patients.
Objectives : The purpose of this study was to present the clinical guidelines for Korean medical treatment of Irritable Bowel Syndrome. Methods : Reports for Korean medical treatment of Irritable Bowel Syndrome were collected, analyzed and summarized from the Electronic journal search since 2000. Results : In Korean medicine treatment of irritable bowel syndrome used herbs, acupuncture, moxibustion, herbal acupuncture, and auricular acupuncture. Herb medicines were administered according to the patient's case by a prescription of considering physical condition and symptoms. Acupuncture and moxibustion therapies stimulate the median points of LR(足厥陰肝經), ST(足陽明胃經), LI(手陽明大腸經), SI(手太陽小腸經). The assessment items for Korean medical treatment of Irritable Bowel Syndrome were abdominal symptoms, defecation, autonomic nervous system changes, stress, Live blood condition and mental health. Conclusions : Through continued research need to develop better treatment strategies for Korean treatment of Irritable Bowel Syndrome.
Purpose: The purpose of this study was to build and verify a structural model that could predict the severity of irritable bowel syndrome in university students. Methods: Participants were 205 students enrolled in college with irritable bowel syndrome using the irritable bowel syndrome module of the ROME IV Adult Questionnaire. The data were collected using online questionnaires in AprilMay 2019. The data were analyzed using the SPSS WIN 25.0 and AMOS 20.0 programs. Results: 1) The symptom severity that participants experienced were mild (14.6%), moderate (45.4%), and severe (40%). 2) Fit indices of the model were x2= 79.66 (df = 52, p= .009), CFI= .94, TLI= .96, RMSEA= .05, RMR= 1.59, GFI= .94, and TLI= .96.3). The severity of irritable bowel syndrome was influenced directly by anxiety and sleep, and indirectly by family history, perfectionism, social support, coping, and stress. The severity of irritable bowel syndrome was indirectly affected by the following: family history through anxiety; perfectionism through stress, anxiety, and sleep; social support through coping, stress, anxiety, and sleep; coping through stress and anxiety; and stress through anxiety and sleep. Conclusion: Based on the results of this study, a nursing intervention is needed to reduce the anxiety and stress and improve the quality of sleep to improve the health of the college students and manage the symptoms of patients with irritable bowel syndrome.
Background & Object : Postprandial epigastric fullness is one of common symptoms in functional dyspepsia, but its pathophysiological mechanism has still been unknown. In this study, its association between postprandial epigastric fullness and pyloric valve disturbance was studied through analysis of bowel sounds and electrogastrography. Methods : Bowel sounds and electrogastrography were recorded together for fasting 15 min and postprandial 40 min. Parameters of bowel soundincluding motility index, sound to sound interval, standard deviation of sound to sound interval, sounds per minute, % of bowel sound, sound length, intensity, dominant frequency of sounds, and postprandial / fasting ratio of dominant frequency of sounds were analyzed with a specialized program. By electrogastrography, regularity of slow waves and power ratio were obtained. Results : Significances of bowel sounds appeared in motility index (p=0.046), dominant frequency of fasting (p=0.048), postprandial (p=0.003), and the ratio of postprandial/fasting (p=0.000); those of EGG parameters were shown in postprandial regularity of slow waves (p=0.006) and power ratio (p=0.011). Conclusion : Pyloric valve disturbance was a cause of postprandial epigastric fullness in patients with functional dyspepsia. Analysis of bowel sound might be useful in diagnosing its existence.

Researchers have endeavored to identify the etiology of inflammatory bowel diseases, including Crohn's disease and ulcerative colitis. Though the pathogenesis of inflammatory bowel diseases remains unknown, dysregulation of the immune system in the host gastrointestinal tract is believed to be the major causative factor. Omics is a powerful methodological tool that can reveal biochemical information stored in clinical samples. Lipidomics is a subset of omics that explores the lipid classes associated with inflammation. One objective of the present systematic review was to facilitate the identification of biochemical targets for use in future lipidomic studies on inflammatory bowel diseases. The use of high-resolution mass spectrometry to observe alterations in global lipidomics might help elucidate the immunoregulatory mechanisms involved in inflammatory bowel diseases and discover novel biomarkers for them. Assessment of the characteristics of previous clinical trials on inflammatory bowel diseases could help researchers design and establish patient selection and analytical method criteria for future studies on these conditions. In this study, we curated literature exclusively from four databases and extracted lipidomics-related data from literature, considering criteria. This paper suggests that the lipidomics approach toward research in inflammatory bowel diseases can clarify their pathogenesis and identify clinically valuable biomarkers to predict and monitor their progression.
