Effects of Minor Arginyl tRNA and Isoleucyl tRNA on the Expression of Clostridium botulinum Neurotoxin Light Chain in Escherichia coli

  • Botulinum neurotoxin type A (BONT/A) is an extremely potent toxin, which is produced by Clostridium botulinum. The light chain of this protein (BONT/A LC), which is known as a zinc endopeptidase, cleaves SNAP-25 involved in the exocytosis process. In this work, the expression of recombinant BoNT/A LC in E. coli is described. The BONT/A LC gene of C. botulinum contains a high frequency of the arginine AGA and isoleucine ATA codons that are rarely used in genes of E. coli, hampering the translation of recombinant protein. The argD and ilex tRNA genes were cloned into pACYC184 vector, resulting in pAAD131X plasmid. The translational stress of the toxin gene related to codon bias was reversed by fupplernentation of the AGA arginyl tRNA of T4 phage and AUA isoleucyl tRNA of E. coli. This system may be applicable for the expression of a variety of AT-rich heterologous genes in E. coli.

혹고니(Cygnus olor)의 보툴리눔독소증 (Botulism in a Mute Swan(Cygnus olor))

  • Many neurotoxigenic clostridia are found in soil. Among animals, birds are especially susceptible to botulism, perhaps because they feed on insects, invertebrate carcasses, and decayed feeds contaminated with spores of Clostridium (C.) botulinum. C. botulinum type C is mainly involved in avian botulism. In the summer of 2005, death of a mute swan (cygnus olor) living in the pond of large bird cage was found in Seoul Grand Park Zoo. The birds presented presumptive clinical signs of botulism, such as ruffled hackle feathers, abnormal posture of the head, weakness, and flaccid paralysis. At that time, pond water in the breeding facilities was drained for 7 days, but there were still remained water containing sediment of feed and feces. Therefore, botulism was suspected and an experimentation were made to detect C. botulinum in the dead mute swan. Gross post-mortem findings of a mute swan showed jelly-like hemorrhagic contents in the intestine, sands and vegetations in the stomach. C. botulinum was isolated from the liver, small intestine and large intestine samples. Botulism was also confirmed by mouse inoculation test with the organ samples. With PCR, a gene encoding C. botulinum type C toxin was detected for the several organs of the mute swan died. These results suggested that death of mute swan was caused by C. botulinum type C.

교근 부위의 보툴리눔 독소 주사가 저작근의 근전도 및 악기능에 미치는 영향 (Short-term Effect of Botulinum Toxin Injection on the Surface EMG of Masticatory Muscles and Jaw Function)

  • 저작근과 관련하여 나타나는 여러 운동장애의 치료나 심미적인 목적으로 교근 부위에 보툴리눔 독소를 주사하는 방법이 널리 이용되고 있다. 그러나 보툴리눔 독소의 교근 부위 주사가 다른 저작근의 근전도와 악기능에 어떠한 영향을 미치는 지에 대한 자료는 부족하다. 이에 본 연구에서는 측두하악관절장애 등 하악의 기능 이상을 가지고 있지 않는 건강한 성인남녀 14명을 대상으로 양측 교근에 각각 80 unit씩의 보툴리눔 독소 A(Dysport, Ipsen, Wrexham, UK)를 주사한 5 명의 실험군과 같은 위치에 같은 양의 생리식염수를 주사한 9 명의 대조군에서 주사 전과 주사 후 3 주까지 매주 교근과 전측두근의 표면 근전도를 측정하고, 국문판 악기능제한지수(Jaw Functional Limitation Scale) 설문지를 이용하여 악기능제한 정도를 평가하여 비교 분석하였다. 교근의 근전도는 실험군에서 주사 후 1주부터 감소하기 시작하여 3주 동안 지속적인 감소를 나타냈으며, 전측두근의 근전도는 유의한 변화를 나타내지 않았다. 악기능제한지수는 저작지수와 전반적 악기능 지수가 실험군에서 보툴리눔 독소 주사 후 1 주째에 증가한 뒤 점차 회복하는 양상을 보였으며, 개구지수와 대화 및 감정표현 영역 기능제한지수는 통계적으로 유의한 변화를 보이지 않았다. 이러한 결과로부터 교근에 시행하는 보툴리눔 독소 주사는 교근의 활성을 지속적으로 저하시키지만 전측두근의 활성에는 영향을 미치지 않았으며, 주관적 저작기능을 단기적으로 저하시키나, 근활성의 저하가 지속되는 과정에서도 주관적 저작기능은 짧은 기간 내에 회복됨을 알 수 있었다.

  • 보툴리눔 독소 주사는 이마, 눈가 주름치료 등 미용성형분야 뿐만 아니라 구강 악안면 분야에서는 만성 편두통(chronic migraine), 근육긴장이상(dystonia), 경직(spasticity), 측두하악장애(temporomandibular disorders, TMD)의 치료 등에 사용되어 왔다. 최근 보툴리눔 독소 주사가 운동신경에서 마비효과 뿐만 아니라 감각신경에서 말초감작과 신경원성염증과 관련되는 substance P, CGRP, glutamate 등 비콜린성 신경전달물질의 유리를 차단하여 통증신호를 차단하는 역할을 한다는 가설이 제기되고 있다. 따라서 본 연구는 쥐, 토끼 등 동물 신경 손상 모델에 대한 실험과 치과 치료 후 발생한 신경 손상 환자에서 보툴리눔 독소의 진통 효과 및 작용 기전을 살펴보고자 한다.

Screening for variations in anterior digastric musculature prior to correction of post-traumatic anterior open bite by injection of botulinum toxin type A: a technical note

  • It has recently been reported that long-standing post-traumatic open bite can be successfully corrected with botulinum toxin type A (BTX-A) injection into the anterior belly of the digastric muscle (ABDM). The report documented an individual with bilaterally symmetrical and otherwise unremarkable anterior digastric musculature. However, the existence of variant anterior digastric musculature is common and may complicate the management of anterior open bite with BTX-A injection. Screening for variant ABDM can be accomplished via ultrasound, computed tomography, and magnetic resonance imaging. Screening for variant ABDM should be performed prior to BTX-A injection in order to account for musculature that may exert undesired forces, such as inferolateral deviation, on the anterior mandible in patients with anterior open bite.

  • The use of botulinum toxin type A in the lower face has increasingly popular. And treatment of the depressor anguli oris muscle(DAO) and the mentalis muscle(MT), particularly in combination with filler substances, produces a remarkable improvement in the lower aged face. The aim of this study was to demonstrate the topographical anatomy of the DAO, MT, and their related structures, thereby providing critical information for determining the safest and most effective site for BTX-A injections. The most effective injection sites of DAO and MT were suggested based on the new anatomical knowledge of the lower face.

Correction of post-traumatic anterior open bite by injection of botulinum toxin type A into the anterior belly of the digastric muscle: case report

  • Post-traumatic anterior open bite can occur as a result of broken balance among the masticatory muscles. The superior hyoid muscle group retracts the mandible downward and contributes to the anterior open bite. Denervation of the digastric muscle by injection of botulinum toxin type A (BTX-A) can reduce the power of the digastric muscle and help to resolve the post-traumatic anterior open bite. A patient with a bilateral angle fracture had an anterior open bite even after undergoing three operations under general anesthesia and rubber traction. Although the open bite showed some improvement by the repeated operation, the occlusion was still unstable six weeks after the initial treatment. To eliminate the residual anterior open bite, BTX-A was injected into the anterior belly of the digastric muscle. Following injection of BTX-A, the anterior open bite showed immediate improvement. Complication and relapse were not observed during follow-up. Long-standing post-traumatic open bite could be successfully corrected by injection of BTX-A into the anterior belly of the digastric muscle without complication.

  • Asians tend to have prominent mandibular angle. The causes of wide lower third of the facial contour are obtuse mandibular angle and hypertrophy of masseter muscles. In cases of hypertrophy of masseter muscles, conventional treatment intends to the contraction of masseter muscle. Recently, volumetric reduction of masseter muscles using botulinum toxin type A injection and radiofrequency (RF) reduction have been introduced. The use of RF energy for masseter muscle reduction is known as a safe, simple, and effective method for aesthetic lower facial contouring. The purpose of this study is to present the effects of RF reduction applied to hypertrophy of masseter muscles, to review and to encourage RF practices in oral and maxillofacial region.

  • Botulinum toxin type A (BTX-A) inhibits muscle contraction, which leads to reversible muscle atrophy and paralysis. Therefore, BTX-A injection can be an effective treatment of facial asymmetry that originated from the uncoordinated muscle movement. A 52-year-old patient was referred from another hospital for the correction of post-traumatic sequelae. The patient had prominent scar in the mandibular symphysis area with asymmetric lower lip movement. The reason for this asymmetric lower lip movement was due to damage in the lower lip depressor muscle. After the injection of BTX-A on the lower lip depressors, asymmetric lip movement has been improved.

  • The neurotoxin of Clostridium botulinum type B was purified from a liquid culture. The purification steps consist of ammonium sulfate precipitation of whole culture, treatment of Polymin P(0.15%, v/v), gel filtration on Sephadex G-100 at pH5.6 and DEAE-Sephadex charomatography at pH8.0. The procedure recovered 17% of the toxin assayed in the starting culture. The toxin was homogeneous by sodium dodecyl sulfate(SDS)-polyacrylamide gel electrophoresis and had a molecular weight of 163,000. Subunits of 106,000 and 56,000 molecular weight were found when purified toxin was treated with a disulfide-reducing agent and electro phoresed on SDS-polyacrylamide gels.
