Purpose: To compare radiation dose of the brain and lens among various conventional whole brain radiotherapy (WBRT) techniques. Materials and Methods: Treatment plans for WBRT were generated with planning computed tomography scans of 11 patients. A traditional plan with an isocenter located at the field center and a parallel anterior margin at the lateral bony canthus was generated (P1). Blocks were automatically generated with a 1 cm margin on the brain (5 mm for the lens). Subsequently, the isocenter was moved to the lateral bony canthus (P2), and the blocks were replaced into the multileaf collimator (MLC) with a 5 mm leaf width in the craniocaudal direction (P3). For each patient plan, 30 Gy was prescribed at the isocenter of P1. Dose volume histogram (DVH) parameters of the brain and lens were compared by way of a paired t-test. Results: Mean values of $D_{max}$ and $V_{105}$ of the brain in P1 were 111.9% and 23.6%, respectively. In P2 and P3, $D_{max}$ and $V_{105}$ of the brain were significantly reduced to 107.2% and 4.5~4.6%, respectively (p<0.001). The mean value of $D_{mean}$ of the lens was 3.1 Gy in P1 and 2.4~2.9 Gy in P2 and P3 (p<0.001). Conclusion: WBRT treatment plans with an isocenter located at the lateral bony canthus have dosimetric advantages for both the brain and lens without any complex method changes.

The purpose of this study was to investigate the effects of green tea catechin on change of bone tissue in long-term cadmium treated rats. Sprague-Dawley male rats weighing 100$\pm$10 g were randomly assigned to one normal group and three cadmium treated groups. Cadmium groups were classified to catechin free diet group (Cd-0C group), 0.25% catechin diet group (Cd-0.25C group) and 0.5% catechin diet group (Cd-0.5C group) according to the levels of catechin supplement. Animals were raised for 20 weeks. Cadmium were supplied as drinking water of 50 ppm Cd$^{2+}$. Effects of catechin were analyzed on changes of bony tissue in long-term cadmium treated rats by determining the accumulated cadmium in bone and bone mineral density and micro- photographs of bony tissue. The cadmium accumulation of tibia and femur were higher in Cd-treated groups than in normal group, but they was lowered by catechin supplementation. The bone mineral density (BMD) of tibia and femur in Cd-0C group was significantly lower than in normal group, but it of catechin supplemetation group was similar to normal group. Microphological changers were appeared under a light microscope and an electro microscope reveal no structural changes in bony spicules, marrow cell distribution and cellular morphology in all groups. The bone weight and length tend to decrease in Cd-0C groups. Catechin supplementation in long-term cadmium treated rats depressed the cadmium accumulation in bony tissue that led to improve the bone mineral density in tibia and femur.r.

Despite of the successful clinical performance of implants, it is still lacking of the knowledge of changes in implant occlusion. The purpose of this study was to evaluate the changes of infraocclusal contact after clinical occlusal function of implant. Twenty patients(38 implants) were recalled during 10 months after placement of implant prosthesis. Occlusion changes were investigated at placement, placement 1 months, 4 months and 10 months serially with silicone bite material and T-Scan II sensor. Bony changes were also evaluated with periapical radiographs. The changes of silicone thickness and T-Scan II sensored areas were statistically analyzed with repeated measured ANOVA and the Scheffe's post-hoc test at the 95% significance level. The following results have been made based on this study: 1. Alveolar bone loss was within 0.20mm and it was generally concluded within physiologic level. 2. There were no statistically significant differences in the thickness changes of silicone material at 1 month and 4 months of occlusal function. However, there was statistically significant difference at 10 months of occlusal function (p<0.05). 3. There was no statistically significant difference in changes of occlusal contact area in T-Scan II at 1 month and 4 months of occlusal function, but there was statistically significant difference at 10 months of occlusal function (p<0.05). Conclusively, as time goes by, implant occlusion to be formed infraocclusion was to be far close and increased occlusal contact. However, it was not observed destructive bone resorption in periapical radiographs and any other side effects.
Purpose: To assess bony trabecular changes potentially caused by loading stress around dental implants using fractal dimension analysis. Methods: Fractal dimensions were measured in 48 subjects by comparing radiographs taken immediately after prosthesis delivery with those taken 1 year after functional loading. Regions of interest were isolated, and fractal analysis was performed using the box-counting method with Image J 1.42 software. Wilcoxon signed-rank test was used to analyze the difference in fractal dimension before and after implant loading. Results: The mean fractal dimension before loading ($1.4213{\pm}0.0525$) increased significantly to $1.4329{\pm}0.0479$ at 12 months after loading (P<0.05). Conclusions: Fractal dimension analysis might be helpful in detecting changes in peri-implant alveolar trabecular bone patterns in clinical situations.
Seventeen rigid screw fixation and sixteen nonrigid wire fixation cases of mandibular sagittal slit ramus osteotomy were selected to compare postoperative dental and skeletal changes. A constructed horizontal plane was drawn seven degrees under sella-nasion plane and detailed cephalometirc assessment was applied to serial radiographic films taken before surgery($T_0$), immediately after surgery($T_1$), and at least six months after surgery($T_2$). Linear and angular positional changes were measured and analyzed statistically using paired t-test method and percent of positional changes(amount of post-op change/amount of intra-op change)${\times}100$. The results were as follows; 1. It was 29.4% in rigid fixation cases and 37.5% in nonrigid fixation cases comparing the postoperative positional change of more than 2mm at point B. So rigid fixation method was slightly more stable. 2. In nonrigid fixation cases, the positional change might be caused by incomplete bony union at the osteotomy site and soft tissue tension acting on this site. 3. In rigid fixation cases, the positional change might be caused by interaction between relapse tendency of protracted condyle-proximal segment and neighboring soft tissue tension.

In cases of severe alveolar bony resorption in the edentulous posterior maxillae, implant placement is limited anatomically due to maxillary sinus. If the ridge is atrophic, the various bone grafting methods are required for the ridge augmentation. But the result of the onlay grafting procedure is not always promising. On the posterior maxilla, maxillary sinus mucosa lifting and bone grafting into the sinus floor, subantral augmentation(SA) technique are recommended. Various SA procedures have been developed for implant installation. We perfer to simultaneous block bone graft and implant installation through the residual alveolar ridge into the grafted block bone to fix the grafted bone and to gain the primary stability of the installed fixture. When a sagittal skeletal discrepancy in present due to the severe alveolar bony resorption of the maxilla, the advancement of the maxilla by Le Fort I osteotomy simultaneously with installation of implant fixtures combined with sinus lifting and interpositional bone graft procedure can be indicated. We applied various SA techniques for implant installtion to the 46 edentulous posterior maxillae, and total 154 implants were installed at our department from 1992 to 1999. Various SA techniques were classified in detail and the indications of each techniques were discussed. The changes of residual bony height following SA procedure were studied. The results were as follows. 1. The SA procedure combined with bone graft and simultaneous fixture installation were performed in 41 cases, 126 fixtures were installed and 5 fixtures were removed out of them. Le Fort I osteotomy procedure combined with sinus lifting and interpositional bone graft simultaneous with fixture installation were performed in 5 cases. Total 28 fixtures were installed and 2 fixtures were removed so far. 2. Autogenous block bone graft into sinus floor were performed in 35 cases, autogenous particulated marrow cancellous bone(PMCB) graft in 9 cases, and demineralized human bone powder in 2 cases. The donor site for bone graft were anterior iliac bone in 39 cases, posterior iliac bone in 3 cases and mandibular symphysis in 1 case and mandibular ramus in 1 case. 3. In 9 cases with which SA procedure had been performed with the block bone graft, the change of pre- and postoperative residual bony height were measured using MPR(multiplanar reformatted)-CT. The mean residual bony height was 8.0mm preoperatively, 20.2mm at 6 months following up operation and we gained average 12.2mm alveolar bony height. So, we can recommend this one-stage subantral augmentation and fixture installation technique as a time conserving, safe and useful method for compromised posterior edentulous maxilla.

The purpose of this study was to evaluate the specialized Orthopantomograms and standard films compared with Waters' views in the diagnosis of maxillary sinusitis. The author analysed the types of mucosal thickening, the types of bony wall change, the relation between the types of chief complaints and mucosal thickening on the Waters' views of 156 patients with 206 maxillary sinuses demonstrating radiographic changes. And the pathologic change of antral floor was observed on 187 standard films and total amount of agreement in the diagnosis of maxillary sinusitis was evaluated at 156 specialized Orthopantomograms and 187 standard films. The results were obtained as followings. 1. Among 206 mucosal thickenings classified by 6 types, generalized increased radiopacity was 69 cases (33.5%) and the entire thickening of mucosa except an ostium area was 66 cases (32.0%), 2. The bony wall change was seen in 128 cases (62.1%) and the incidence of indistinct antral wall was the highest as 55 cases (43.0%) among 5 types. 3. Among 148 chief complaints, pain was 71 cases (48.0%), swelling was 19 cases (12.8%) and discomfort was 18 cases (12.2%). 4. The most frequent change of antral floor was indistinct antral floor as 25 cases (13.4%) and elevation of antral floor was 23 cases (12.3%). And the first molar presented the most frequent involvement of antral floor as 47 cases (57.3%). 5. In 156 specialized Orthopantomograms, 101 cases (49.0%) presented apparent increased radiopacity and 72 cases (35.0%) showed suspicious increased radiopacity in comparison with Waters' view. In 187 standard films, 47 cases (25.1%) presented apparent increased radiopacity and 53 cases (28.4%) showed suspicious increased radiopacity in comparison with Waters' views.
There has been recent interest in radiation-induced bone injury in clinical conditions, especially for pelvic insufficiency fracture (PIF). A PIF is caused by the effect of normal or physiological stress on bone with demineralization and decreased elastic resistance. Pelvic radiotherapy (RT) can also contribute to the development of a PIF. A PIF has been regarded as a rare complication with the use of megavoltage equipment. However, recent studies have reported the incidence of PIFs as $8.2{\sim}20%$ after pelvic RT in gynecological patients, an incidence that was higher than previously believed. The importance of understanding a PIF lies in the potential for misdiagnosis as a bony metastasis. If patients complain of pelvic pain after whole-pelvis radiation therapy, the presence of a PIF must be considered in the differential diagnosis. The use of multibeam arrangements and conformal RT to reduce the volume and dose of irradiated pelvic bone can be helpful to minimize the risk of fracture. In addition to a PIF, osteonecrosis and avascular necrosis of the femoral head can develop after radiation therapy. Osteoradionecrosis of the pelvic bone is a clinical diagnostic challenge that must be differentiated from an osseous metastasis. A post-radiation bone sarcoma can result as a long-term sequela of pelvic irradiation for uterine cervical cancer.

Objectives: The purpose of this retrospective study is to find the differentiating characteristics of cystic and cystic-appearing lesions that involve the impacted mandibular third molar by analyzing panoramic radiographs and computed tomography images, and to aid the preoperative diagnosis. Materials and Methods: Eighty-one patients who had a mandibular cystic or cystic-appearing lesion that involved impacted mandibular third molar and underwent cyst enucleation were included in the study. The preoperative panoramic radiograph and computed tomography findings were analyzed in accordance to the histopathologic type. Results: Most of the cystic lesions containing the mandibular third molar were diagnosed as a dentigerous cyst (77.8%). The occurrence of mesio-distal displacement of the third molar was more frequent in the odontogenic keratocyst (71.4%) and in the ameloblastoma (85.7%) than in the dentigerous cyst (19.1%). Downward displacement was primarily observed in each group. Odontogenic keratocyst and ameloblastoma showed more aggressive growth pattern with higher rate of bony discontinuity and cortical bone expansion than in dentigerous cyst. Conclusion: When evaluating mandibular cystic lesions involving the impacted mandibular third molar, dentigerous cyst should first be suspected. However, when the third molar displacement and cortical bone absorption are observed, then odontogenic keratocyst or ameloblastoma should be considered.

Thoracic outlet syndrome is actually a collection of syndromes brought about by abnormal compression of the neurovascular bundle by bony, ligamentous or muscular obstacles between the cervical spine and the lower border of the axilla. First of all a syndrome is defined as a group of signs and symptoms that collectively characterize or indicate a particular disease or abnormal condition. The neurovascular bundle which can suffer compression consists of the brachial plexus plus the C8 and T1 nerve roots and the subclavian artery and vein. The brachial plexus is the network of motor and sensory nerves which innervate the arm, the hand, and the region of the shoulder girdle. The vascular component of the bundle, the subclavian artery and vein transport blood to and from the arm. the hand. the shoulder girdle and the regions of the neck and head. The bony, ligamentous, and muscular obstacles all define the cervicoaxillary canal or the thoracic outlet and its course from the base of the neck to the axilla or arm pit. Look at the scheme of this region and it all becomes more easily understood. Compression occurs when the size and shape of the thoracic outlet is altered. The outlet can be altered by exercise, trauma, pregnancy, a congenital anomaly, an exostosis, postural weakness or changes. Thoracic outlet syndrome has been described as occurring in a diverse population. It is most often the result of poor or strenuous posture but can also result from trauma or constant muscle tension in the shoulder girdle. The first step to beginning any treatment begins with a trip to the doctor. Make a list of all of the symptoms which seem to be present even if the sensations are vague. Make a note of what activities and positions produce or alleviate the symptoms and the time of day when symptoms are worst. Also, note when the symptoms first appeared. This list is important and should also include any questions one may have.
