A STUDY ON THE DENTAL RADIOGRAPHIC MANIFESTATIONS OF END-STAGE RENAL DISEASE (말기신장질환에서의 치과방사선학적 양상에 관한 연구)

  • For the assessment of changes in the bone architecture of the maxilla and mandible in renal osteodystrophy, 31 chronic renal failure patients who were undergoing hemodialysis therapy were selected. They were evaluated through clinical oral examination, radiographic and biochemical examination. The results were as follows: 1. In 17 cases (54.8%), there were evidences of bony change in jawbone. 2. The most common dental radiographic finding was decreased bone density (14 cases, 45.2%). 3. The second most common dental radiographic finding was total or partial loss of lamina alveolar dura (11 cases, 35.5%). 4. The third most common dental radiographic finding was total or partial loss of inferior canal wall (8 cases, 25.8%). 5 cases showed evidences of bony change only in jawbone, and 5 cases only in hand, and 12 cases in both. 6. Serum creatinine, urea nitrogen and alkaline phosphatase values in hemodialysis group were much higher than in control group. 7. There were statistically significant correlation between bone density and lamina dura, and inferior alveolar canal wall.

SKELETAL RELAPSE AFTER SAGITTAL SPLIT RAMUS OSTEOTOMY AND SCREW FIXATION (하악전돌증에서 하악지 시상분할골절단 및 Screw고정후 골성회귀에 관한 연구)

  • Skeletal and dental changes were examined in 38 patients of mandibular prognathism who been treated by a bilateral sagittal split osteotomy(SSRO) and internal fixation using titanium mini-screws. All patients were followed up for over 8 months after the surgeries, and postoperative cephalometric measurements were compared at 2 months and at 8 months. Linear measurements of the "Pog-most posterior screws" and angular measurementsts of "SN-Pog'were compared to figure out the change of bony fragments. The significancy of data were tested by unpaired T-test. The results were as follows : 1. The fixation screws were changed in cephalometric position as little as $0.32{\pm}2.51mm$ in SSRO and $0.15{\pm}1.00mm$ in SSRO & Le Fort I Osteotomy.(P<0.05) 2. Mandibular set-back over 5mm resulted in less stability of the fixation screws and higher relapse tendency. 3. The internal fixation using two screws along the inferior border and one on the superior ridge is considered to be very resistant to postoperative relapse of the repositioned bony segments.

Analysis of Dural-sac Cross Sectional Area Changes According to Vertical Impact rate (수직 충격률에 따른 척추 경막 단면적 변화 해석)

  • In this study the occlusion of dural-sac. the outer membrane of spinal cord in the lumbar region. was quantitatively analyzed using one motion segment finite element model. Occlusion was quantified by calculating cross sectional area change of dural-sac for different compressive impact duration (loading rate) due to bony fragment at the posterior wall of the cortical shell in vertebral body. Dural-sac was occluded most highly in the range of 8∼12 msec impact duration by the bony fragment intruding into the spinal canal. $\Delta$t = 400 msec case 4 % cross sectional area change was calculated. which is the same as the cross sectional area change under 6 kN of static compressive loading.

RADIOLOGIC STUDY OF THE MAXILLARY SINUSITIS (상악동염의 방사선학적 연구)

  • Radiographic features of maxillary sinusitis, types of mucosal thickening, types of bony wall change, and relationship between the radiographic features and symptoms of patients were observed and classified in the Waters' view of 360 patients with 441 maxillary sinuses demonstrating radiologic changes. And the classification of antral floor and pathologic change was observed in the 154 periapi- cal films. Finally, the value of orthopantomography in the diagnosis of maxillary sinusitis was evaluated in the 138 cases, The obtained results were as followings. 1. Among 441 maxillary sinuses examined by Waters' projection, mucosal thickening was seen in 56.7% and generalized opacification was seen in 36.3%. 2. Among 270 mucosal thickenings classified 8 types, overall regularly thickened type was the highest in incidence (36.3%). 3. Bony wall change was seen in 35.6% of all cases and the indistinct white line of the cortical plates was the highest in incidence (78.2%) among the 6 types. 4. Pain (41.9%) and pus discharge (21.0%) were the most frequent symptoms. And pus discharge, foul odor, and headache was more prevalent in type of generalized opacification than any other types. 5. Regular pneumatization involving all roots of premolars and molars was the highest in incidence (55.0%). The first molar presented the most frequent involvement (61.0%). 6. In orthopantomography, 46.4% showed apparent increased radiopacity and 23.9% showed suspicious increased radiopacity in comparison to Waters' projection.

Radiographic Evaluation on the Forefoot: Based on the Two Different Data (1982 and 2004) (전족부의 방사선학적 관찰: 1982년과 2004년의 차이를 중심으로)

  • Purpose: We compared the bony arrangements of the forefoot in 2 different years, 1982 and 2004, to determine any changes with time period of 22 years. Materials and Methods: The radiographs of 200 normal Korean adults, 100 male and 100 female volunteers, were evaluated both in 1982 and 2004. The radiographic results were evaluated with as follows; hallux valgus angle (HVA), intermetatarsal angle (IMA), morphology of metatarsal head and relative anterior protrusion of the metatarsals and the phalanges. The mean ages were 38 years (23-52years) in 1982, and 37 years (24-50years) in 2004. Results: The mean of HVA decreased from 15.6 degrees in 1982 to 14.3 degrees in 2004 (p=0.047), and the mean of IMA increased from 8.0 degrees in 1982 to 9.4 degrees in 2004 (p=0.031). The morphology of metatarsal head and relative anterior protrusion of the metatarsals and the phalanges were not different between the two study years. Conclusion: Comparing with those of 1982 measurements, we found an increase of IMA and a decrease of HVA. A prospective study may be needed to illuminate course of the changes.

The diagnosis of coronoid impingement using computed tomography

  • Coronoid impingement can cause limitation of mouth opening. In many cases, it appears to be related to the coronoid hyperplasia. We present a case of mouth opening limitation caused by coronoid impingement on the posterior surface of the zygomatic bone without coronoid hyperplasia. The bony changes in coronoid and zygoma including surface irregularity and discontinuity of the cortex and sclerotic change of inner medullary space were noted on computed tomography (CT) scans in different level of axial planes. Through another CT scans in open mouth position could demonstrate that those bony changes were caused by the contact of both surfaces against each other. In case coronoid impingement is suspected of the many possible causes, the open mouth CT scans will be needed to reveal the direct impingement of coronoid on zygoma even without coronoid hyperplasia.

Anatomical Reconstruction of the Medial Orbital Wall Fracture (안와내벽골절의 해부학적 복원술)

  • Purpose: In surgical treatment of the medial orbital wall fractures, restoring the original position of the orbital wall is difficult in some cases. Under such condition, the orbital wall is often reconstructed with synthetic material, without bony reduction, which is considered to be the conventional reconstruction. The purpose of this study is to compare the outcomes of anatomical reconstruction, which restores the bony wall to the anatomical position, from that of the conventional reduction in the isolated medial orbital wall fractures. Methods: Thirty patients, who underwent reconstruction surgery for the isolated medial orbital wall fractures from March 2007 to August 2011, were reviewed retrospectively. The surgical outcomes of two groups, the conventional reconstruction group (15 patients) and the anatomical reconstruction group (15 patients), were studied in 2 measurements, a one day before and 6 months after the surgery. The changes of orbital volume were calculated by the images from a computed tomography scan and enophthalmos was measured by a Hertel exophthalmometer. Results: The orbital volume ratio was decreased by an average of 1.05% in the conventional reconstruction group, while in the anatomical reconstruction group, the ratio decreased by 5.90% (p<0.05). The changes in the Hertel scale were 0.20 mm in the conventional reconstruction group, and 0.70 mm in the anatomical reconstruction group. However, the difference in the Hertel scale was statistically insignificant (p>0.05). Conclusion: In conclusion, the anatomical reconstruction technique of the isolated medial orbital wall fracture results in a better outcome than that of the conventional reconstruction, in terms of restoring of the original orbital volume and anatomic position. Thus, it can be considered as a useful method for the isolated medial orbital wall fractures.

Comparison of bony changes between panoramic radiograph and cone beam computed tomographic images in patients with temporomandibular joint disorders (측두 하악 관절 장애 환자의 파노라마 영상과 cone beam형 전산화 단층 영상의 비교)

  • Objective: This study was designed to assess the diagnostic validity of digital panoramic radiographs compared to cone beam computed tomography (CBCT) in patients with temporomandibular joint disorders. Methods: Panoramic radiograph and CBCT were taken from a total of 212 joints from 106 subjects. The joints were examined by two dentists and divided into the following six groups: normal, flattening, osteophyte formation, erosion, sclerosis, and unclassified. The sensitivity and specificity of each observer and inter-observer reliability were statistically analyzed. Results: The results showed relatively high intra-observer reliability in the diagnosis of both panoramic and CBCT images and the weighted Kappa indices of panoramic and CBCT images were 0.714 and 0.727, respectively. The sensitivities of panoramic images of observer A and B to CBCT images was 82.35% and 84.30%, respectively, while the specificity of observer A and B was 58.06% and 61.54%, respectively. However, guided diagnosis from panoramic and CBCT images were statistically different (p < 0.05). Conclusions: The present study suggests that the panoramic radiograph could be used as a primary diagnostic device to detect bony changes of temporomandibular joints in clinical orthodontics, because panoramic images showed relatively high sensitivity compared to CBCT images. However, CBCT images may be one of the best choices when a more accurate diagnosis is necessary.

A ROENTGENOCEPHALOMETRIC STUDY OF THE PHARYNX AND CRANIOFACIAL SKELETON IN KOREAN (인두와 두개안면골격에 관한 방사선학적 연구)

  • The aim of this study was to investigate the variation in shape, size and area of the pharynx and adenoids, and to analyze the relationship between pharyngeal cavity and upper facial cranium which effected on morphology of those parts in Korean. Age changes and sex differences in those areas were comprised in this study. Materials included 272 lateral cephalograms, which were divided into 4 groups by age; (1) 7-year-old group consisted of 29 males and 30 females, (2) 12-year-old group consisted of 30 males and 30 females, (3) 17-year-old group consisted of 30 males and 40 females, (4) 20-year-old group consisted of 37 males and 46 females. In subjects each variable was measured and evaluated statistically introducing 17 reference points and 17 reference lines respectively. Conclusions from this study were as follows. 1. Linear measurements of the bony nasopharynx revealed that the depth and height were larger in male than those in female in 17 and 20-year-old groups. 2. Linear measurements of the upper facial cranium were larger in male than those in female in all age groups. 3. Angular measurements of the bony nasopharynx and upper facial cranium did not show, on an average, sex differences in each age group. 4. As regards area of the bony nasopharynx, it increased gradually with age in both sexes. And the area was greater in male than that in female in 17 and 20-year old groups. 5. There were sex differences in area of the adenoids of which the area was larger in male than that in female in 17 and 20-year-old groups. And the area reached a peak at 17-year-old group in male and at 12 year-old group in female. 6. Area of the pharyngeal cavity increased gradually with age in both sexes, but no sex differences were noted in each age group. 7. Rate of area of the adenoids to that of the pharyngeal cavity decreased continually with age, and no sex differences were noted in all age groups. 8. In amounts and its differences of the growth, there were sex differences in the posterior upper facial height, and were not in area of the bony nasopharynx, pharyngeal cavity and adenoids in each age group.

A STUDY ON THE BONE FORMATION OF GRAFT MATERIAL CONTAINING DEMINERALIZED BONE MATRIX WITH A SIMULTANEOUS INSTALLATION OF IMPLANT (임프란트 식립시 이식된 탈회골기질을 함유한 이식재의 골형성에 대한 연구)

  • Purpose: The aim of the present study is to evaluate the effect of autogenous bone and allograft material coverd with a bioresorbable membrane on bone regeneration after a simultaneous installation of implant. Materials and methods: Twelve healthy rabbits, weighing about $3{\sim}4$ kg, were used in this experiment. Following impalnt(with 3.25 mm diameter and 8 mm length) site preparation by surgical protocol of $Oraltronics^{(R)}$, artificial bony defect, 5mm sized in height and depth, was created on femoral condyle using trephine drill(with 5 mm diameter and 5 mm length). Then implant was inserted. In the experimental group A, the bony defect was filled with autogenous particulated bone and coverd with $Lyoplant^{(R)}$ resorbable membrane. In the experimental group B, the bony defect was filled with allograft material(Orthoblast $II^{(R)}$) containing demineralized bone matrix and covered with $Lyoplant^{(R)}$. In the control group, without any graft materials, the bony defect was covered with $Lyoplant^{(R)}$. The experimental group A and B were divided into each 9 cases and control group into 3 cases. The experimental animals were sacrificed at 3, 6 and 8 weeks after surgery and block specimens were obtained. With histologic and histomorphometric analysis, we observed the histologic changes of the cells and bone formation after H-E staining and then, measured BIC and bone density with KAPPA Image $Base^{(R)}$ system. Results: As a result of this experiment, bone formation and active remodeling process were examined in all experimental groups and the control. But, the ability of bone formation of the experimental group A was somewhat better than any other groups. Especially bone to-implant contact fraction ranged from 12.7% to 43.45% in the autogenous bone group and from 9.02% to 29.83% in DBM group, at 3 and 8 weeks. But, bone density ranged from 15.67% to 23.17% in the autogenous bone group and from 25.95% to 46.06% in DBM group at 3 and 6 weeks, respectively. Although the bone density of DBM group was better than that of autogenous bone group at 3 and 6weeks, the latter was better than the former at 8 weeks, 54.3% and 45.1%, respectively. Therefore these results showed that DBM enhanced the density of newly formed bone at least initially.
