Surgical Management of Edentulous Atrophic Mandible Fractures in the Elderly

  • Fractures of the mandible occur with a greater frequency in the elderly. This study reports three cases of edentulous atrophic mandible fracture in elderly patients treated with open reduction technique. Three patients who presented with edentulous atrophic mandible fractures underwent surgical management using open reduction and internal fixation. After treatment, clinical evaluations and postoperative complications were examined with postoperative x-ray. Patients were followed with clinical and radiographic examinations. In the postoperative clinical evaluation, two male patients healed well, but one female patient complained of pain and swelling. In radiographic examinations, no union delay or lack of fusion was observed in the edentulous area. Open reduction technique is a viable treatment option for the edentulous atrophic mandible fractures in geriatric patients.

Reconstruction of Large Femur and Tibia Defect with Free Vascularized Fibula Graft and Locking Plate

  • Purpose: The reconstruction of femur and tibia defects following tumor resection remains a surgical challenge. The clinical outcome of free vascularized fibula graft (VFG) reconstruction with locking plate for massive femur and tibia defects of more than 10 cm that were secondary to skeletal tumor resection is reported. Materials and Methods: Thirteen patients with a mean follow-up of 3.3 years were reviewed. Seven patients received vascularized fibula grafts in the femur and six in the tibia. The mean bony defect of the femur and tibia was more than 10 cm and the length of the grafted fibula was more than 15 cm. All defects were stabilized with long locking plates. Results: All patients were free of disease at final follow-up; All VFGs were transferred successfully. All patients had a successful outcome with bony union. Stress fractures of the grafted fibula had occurred but the locking plate stabilized the fracture and healed until the last follow-up. All patients were able to walk without a brace after a mean of 9 months postoperatively. Conclusion: VFG with locking plate is a reliable reconstructive procedure for massive femur and tibia defects.

Fracture of Proximal Humerus in the Lateral Anchor Site after Suture Bridge Repair - A Case Report

  • To report the fracture of proximal humerus in the lateral anchor site after suture bridge repair. A 57-year-old female patient with shoulder pain on the right-side was admitted through the emergency room following a car accident. Seven weeks before the accident, the patient had undergone surgery at a different hospital for the repair of supraspinatus tendon rupture on the right-side via suture bridge technique. Humerus surgical neck fracture was confirmed by X-ray, and proximal humerus fracture at the anchor site was confirmed by magnetic resonance imaging. Following 7 months of conservative treatment resulted in satisfactory bone union and motion of the shoulder joint. We report the need of close observation during and after the arthroscopic repair of the rotator cuff in patients with osteoporosis.

족관절 외과의 Danis-Weber B형 단독 골절에 있어서 활주 방지 금속판고정술과 외측 금속판고정술의 비교 (Comparison of Antiglide Plate Fixation and Lateral Plate Fixation for Danis-Weber Type B Isolated Lateral Malleolar Fractures)

  • Purpose: This study was performed retrospectively and to compare the clinical outcomes of the surgical management using Antiglide plating and lateral plating for Danis-Weber type B isolated lateral malleolar fractures. Materials and Methods: Between March 2004 and March 2009, 92 patients were followed up for more than 6 months after surgical treatment. 49 patients were treated with Antiglide plating, whereas the remaining 43 patients underwent lateral plating. The results of the two groups were analyzed both radiographically and clinically using Ankle scoring system. Results: Both groups were comparable for age, sex distribution, time to surgery, and operation time. Bone union was noted at average of 8 weeks in all fractures. The mean ankle score was similar in both groups (94% versus 93%). Conclusion: Antiglide plating for Danis-Weber type B isolated lateral malleolar fracture is one of the good methods which has some advantages such as early ankle motion, ambulation and smaller operative incision.

Internal Fixation Using Clavicle Hook Plates for Distal Clavicle Fractures

  • Background: To report the radiological and clinical outcomes of internal fixation using distal clavicle hook plates for distal clavicle fractures. Methods: From April 2008 to December 2012, 32 patients with distal clavicle fractures underwent surgery using an AO hook plate. The reduction was qualified and evaluated according to the radiological findings. The evaluation of the clinical outcomes was performed with the University of California at Los Angeles (UCLA) score, the Korean Shoulder score, and the visual analogue scale (VAS) pain score. Results: By radiological evaluation, we found that 31 of 32 patients showed anatomical reduction and solid bone union. Although we obtained satisfactory UCLA scores, Korean Shoulder Scale scores, and VAS pain scores, 12 cases of complications were present. We found 4 cases of osteolysis of the acromion, 1 case of nonunion, 3 cases of periprosthetic fractures, 3 cases of subacromial pain, and 1 case of skin irritation. We performed re-operations in 2 patients. Conclusions: To avoid complications associated with clavicle hook plates, choosing the appropriate hook size and bending of the hook according to the slope of the acromion undersurface is critical. Also, we believe that early removal of clavicle plates may help reduce complications.

원위 대퇴골 골절에 대한 고령 환자군의 치료 결과 비교 -임상적, 방사선학적 비교- (Clinical and Radiological Outcome of Distal Femoral Fracture in Elderly Patient Group)

  • Purpose: To report the postoperative Clinical and Radiological outcomes following distal femoral fractures in elderly patients compared with young patients. Methods: From March, 1996 to March, 2012, 83 patients who received surgical treatment for fractures of the distal femur were enrolled in this retrospective study. Ages more than 65 was named group A. Group A was 49 cases and mean age is 72.5 year(65~91year). Group B was 49 cases and mean age is 45.7 year(16~61 year). Surgical methods are retrograde IM nail, locking compression plate, cannulated screw and postoperative rehabilitation is no difference between two groups. Clinical results were evaluated using Neer scores, radiographic results and the presence of clinical complications. Results: The mean union period was 18.4(12-40) weeks in group A and 17(10-24) weeks in group B. Neer functional scores are no significant statistical difference between two groups. There are 5 cases metal breakage in group A and 1case in group B. There are 3 cases nonunion in group A and 1 case in group B. Conclusion: In the case of fractures of the distal femur in elderly patients, locking plate using minimally invasive percutaneous periosteal osteosynthesis (MIPPO) technique may be one of the most effective methods and preoperative bone stock evaluation in important.

  • Tae-Geuk-Guan'(太極拳) Tae-Geuk(太極) is arranged Yum-Yang-O-Hang(陰陽五行) and created from Mu-geuk(無極). Mu-geuk(無極) means great chaos. Jin-sik Tae-Geuk-Guan(陳式太極拳) has tenth essential theory. There are Li(理), Qi(氣), Samjul(三節), Sacho(四稍), Ojang(五臟), Samhap(三合), Yukjin(六進), Sinbup(身法), Bobup(步法), Gangyou(剛柔). Li(理) is Rules of all things. Qi(氣) is body. Body is divided into Samjul(三節), Sacho(四稍). Ojang(五臟) is inside body and make human'body and Qi(氣). Samhap(三合) is union of mind & intention, Qi & power, muscle & bone. Yukjin(六進) is movement of body. Sinbup(身法) has Chong(縱), Hoeng(橫), Ko(高), Jeo(低), Jin(進), Toe(退), Ban(反), Chuk(側). Bobup(步法) has Jin(進), Toe(退), Ban(反), Chuk(側). Yang-sik Tae-Geuk-Guan(揚式太極拳) has tenth essential theo. There are Songyo, Heolungjungkyung(虛靈頂勁), Hamhungbalbae(含胸拔背), Chimgyunsuju, Bunheosil(分虛實), Yongeuibulyonglyuk(用意不用力), Sanghasangsu(上下相隨), Naeoeisanghap(內外相合), Sangyunbudan(相連不斷), Dongchungguchung(動中求靜).

Proximal Interphalangeal Joint (PIPJ) Arthrodesis for Treating PIPJ Osteoarthritis in a Horse

  • A 2-year-old, 440 kg, thoroughbred gelding showed a lameness grade of 3 out of 5, as well as swelling and heating on the proximal interphalangeal joint (PIPJ). Periarticular new bones due to the arthritis were observed on performing radiography and ultrasonography. PIPJ arthrodesis was performed under general anesthesia. With the PIPJ open, the articular cartilage was removed by using curette, chisel, and mallet. Arthrodesis was performed with a PIP plate and two additional transarticular cortex screws inserted in lag fashion. A cast was placed over the limb distal from the proximal metatarsal bone. At 6 months after surgery, complete union of the PIPJ was confirmed by radiography, and the incision site was well closed. Based on the outcome, a PIP plate with two additional transarticular cortex screws inserted in lag fashion is recommended as a method for treating osteoarthritis of the PIPJ in horses.

Temporomandibular Joint Ankylosis Caused by Osteoarthritis: A Case Report Based on Cone Beam Computed Tomography Images

  • Temporomandibular joint (TMJ) ankylosis is a condition in which condylar movement is restricted because of fibrous or bony union between the mandibular condyle and temporal bone. TMJ ankylosis is most often caused by trauma, followed by systemic or local infection, and secondary to unknown causes. Diagnostic imaging plays a vital role in diagnosing TMJ ankylosis and establishing a treatment plan. Computed tomography (CT) or cone beam computed tomography (CBCT) is currently the imaging technique of choice to accurately demonstrate preoperative ankylosing masses and other surgically important findings, such as the shape of the mandibular condyle and the pathological changes in the joint. The osseous changes in the mandibular condyle are easily identified in the coronal and sagittal sections of CT or CBCT images. This report describes the case of a middle-aged woman who developed TMJ ankylosis of the left TMJ while undergoing repeated treatment for TMJ disease. We report the findings observed on radiographic and CBCT images through continuous observation.

요추부 협착에서의 피질골 궤도 나사못 고정의 초기 실패 사례에 대한 고찰 (Early Failure of Cortical-Bone Screw Fixation in the Lumbar Spinal Stenosis)

  • 목적: 퇴행성 요추부 척추 질환에 대한 수술적 치료로 척추 경 나사못(pedicle screw) 삽입술이 전통적으로 사용되어왔다. 이에 대한 대안으로 제시된 피질골 궤도 나사못(cortical-bone trajectory screw)은 덜 침습적인 후방 요추부 고정이 가능한 점과 몇몇의 생체 역학 연구에서 보고한 우수한 기계적 안정성을 장점으로 가지고 있다. 본 연구의 목적은 후방 감압술 후 후방 고정술 및 유합을 시행한 환자에 있어 피질골 궤도 나사못 고정술의 초기 실패에 관한 사례의 임상적 및 방사선적 결과를 알아보고자 함에 있다. 대상 및 방법: 요추의 퇴행성 척추 협착 및 척추 전방 전위에 대한 전통적인 척추 경 나사못 고정을 대안하여 피질골 궤도 나사못을 사용하여 2013년부터 2018년까지 수술적 치료를 시행한 311명의 환자를 대상으로 하였다. 수술 후 조기 고정 실패는 수술 후 6개월째, 외래 추시에서 시행한 컴퓨터 단층촬영(computed tomography) 및 방사선 사진상 나사못의 이완, 이탈 및 파손과 같은 고정 실패가 일어날 시로 정의하였다. 결과: 조기 고정 실패는 311예 중 46예(14.8%), 나사못 이완이 46예(14.8%), 이탈이 12예(3.9%), 파손이 4예(1.3%)에서 발생하였다. 해당 고정 실패가 일어난 부위 분석 시, L1이 7예(15.2%), L2가 3예(6.5%), L3가 4예(8.7%), L4가 4예(8.7%), L5가 4예(8.7%), 그리고 S1이 24예(52.2%)였다. 주로 말단 피질골 궤도 나사못 중에서도 S1 나사못에서 이완, 이탈 및 파손과 같은 고정 실패가 주로 발생하였다. 결론: 피질골 궤도 나사못 고정은 기존의 척추 경 나사못 고정과 비교했을 때 동등한 임상 결과 또는 적은 합병증을 나타낼 수 있는 하나의 대안이 될 수 있으나 골다공증이 있거나 특히 L5-S1 유합부위에서 전방지지구조가 되지 않는 조건일 때, 이완, 이탈, 파손 등의 조기 고정 실패가 나타나는 결과가 있었다.
