Autogenous Low Heat Treated Bone Graft for Bone Reconstruction in Bone and Soft Tissue Tumors (골연부 종양에서 저온 열처리한 자가골을 이용한 재건술)

  • Although autoclaved autogenous bone reconstruction is one of the established procedures, it may have some problems in bone regeneration and mechanical property. The purpose of this study is to evaluate the efficacy of more biologic and anatomical reconstruction where allograft is not readily available. From Aug.1991 to Feb. 1996 the authors analyzed 32 cases of reconstruction with autogenous low heat treated bone. Autogenous graft sites were humerus 4, tibia 4, pelvis 9, and 15 femur. Average follow-up period was 23(range;12-51) months. There were 49 graft-host junctional sites. Diaphysis was 22, metaphysis 10, and flat bone 17. Average duration of healing for the 38 united sites was 7 months. Average union time for each anatomical area 8 months in 19 diaphysis, 12 months in 7 metaphysis, and 12.7 months in 12 flat bone(pelvis). Eleven nonunion sites consisted of 3 diaphysis(3/22), 3 metaphysis(3/10), and 5 flat bone(5/17). Complications other than nonunion were local recurrence(4), bone resorption(3), graft fracture(2), osteomyelitis(1), metal failure(2), and wound infection(1). Initial bone quality and stable fixation technique was important for union rate. Plate and screw is a good method for diaphyseal lesion. Metaphyseal and flat bone are weak area for rigid fixation and one stage augmentation with iliac bone graft can be a salvage procedure.

Comparative Study of Arthroscopic ACL Reconstruction Using Bone-Patellar Tendon-Bone Versus Four-Stranded Semitendinosus-Gracilis Tendon (관절경적 전방십자인대 재건술시에 골-슬개건-골과 네가닥 반건양건-박건을 이용한 방법의 비교연구)

  • This study is retrospective analysis of 31 patients treated by arthroscopically assisted ACL reconstruction, from September 1995 to September 1996. ACL reconstructions using autogenous bone patellar tendon bone (B-PT-B) were done in 18 patients, and using hamstring tendon were done in 13 patients. We used four-stranded hamstring tendon grafts and fixed the grafts using Endobutton and screw. The mean postoperative Lysholm knee score was 87.2 points in B-PT-B. group, and 89.0 points in hamstring tendon group. There were no clinically significant results between two groups in Telos test, quadriceps atrophy, Lachman test, anterior drawer test and pivot shift test, but the incidences of anterior knee pain were lower in hamstring group. Four-stranded hamstring tendon graft showed enough stability and good functional outcome similar to that of patellar tendon graft, and had an advantage of quicker return of quadriceps function and less donor site morbidity.

Finite element analysis on the stress of supporting bone by diameters and lengths of dental implant fixture (유한요소법을 이용한 치과 임플란트 고정체의 직경과 길이에 따른 지지골의 응력 분석)

  • Purpose: The dental implant should be enough to endure chewing load and it's required to have efficient design and use of implant to disperse the stress into bones properly. This study was to evaluate the stress distribution on a supporting bone by lengths and diameters of the implant fixture. Methods: The modeling and analysis of stress distribution was used for the simple molar porcelain crown model by Solidworks as FEM program. It was designed on applying with tightening torque of 20 Ncm of a abutment screw between a cement retained crown abutment and a fixture. The fixtures of experimental model used 10, 13mm by length and 4, 5mm by diameter. A external vertical loading on the two buccal cusps of crown and performed finite element analysis by 100 N. Results: The maximum von Mises stress(VMS) of all supporting bone models by fixture length and diameter were concentrated on the upper side of supporting compact bone. The maximum stress of each model under vertical load were 164.9 MPa of M410 model, and 141.2 MPa of M413 model, 54.3 MPa of M510 model, 53.6 MPa of M513 model. Conclusion: The stress reduction was increase of fixture's diameter than it's length. So it's effective to use the wider fixture as possible to the conditions of supporting bone.

Surface Characteristics and Electrochemical Behaviors of TiN and ZrN Coated Orthodontic Mini-screw (ZrN 및 TiN 코팅된 치과교정 용 미니나사의 표면특성과 전기화학적 거동)

  • The dental orthodontic mini-screw requires good mechanical properties and high corrosion resistance for implantation in the bone. The purpose of this study was to investigate the electrochemical characteristics of TiN and ZrN coated orthodontic mini-screws, mini-screws were used for experiment. Ion plating was carried out for mini-screw using Ti and Zr coating materials with nitrogen gas. Ion plated surface of each specimen w as o bserved with f ield emission scanning e lectron microscopy ( FE-SEM), e nergy dispersive x-ray spectroscopy (EDX), and electrochemical tester. The surface of TiN and ZrN coated mini-screw were more smooth than that of other kinds of non-coated mini-screw due to dercrease of machined defects. The corrosion current density of the TiN and ZrN coated mini-screw decreased compared to non-coated sample. The corrosion potential of TiN and ZrN coated mini-screw were higher than that of non-coated mini-screw in 0.9% NaCl solution. The pitting corrosion resistance increased in the order of ZrN coated, TiN coated and non-coated wire. Pitting potential of ZrN coated mini-screw was the highest in the other specimens.

Comparision of the Two Groups between Autologous Bone Chips and Cage as Posterior Lumbar Interbody Fusion in Spondylolisthesis Patients (요추전방전위증 환자들에서 후방요추체간유합술로 자가골편 또는 Cage를 사용한 두 군간의 비교)

  • Objective : Posterior lumbar interbody fusion(PLIF) with transpedicular screw fixation(TPSF) have many merits in the treatment of spondylolisthesis. The aim of this study was to compare cage PLIF group(PLIF using cage and TPSF) with chip PLIF group(PLIF using autologous bone chips and TPSF) as surgical treatment of spondyloisthesis. Methods : PLIF and TPSF were performed in 44 patients with spondylolisthesis from January 1994 to December 1998. The surgical methods were divided into two groups. One group was cage PLIF(20 patients), and the other group was chip PLIF(24 patients). We analyzed the change of anterior translation, change of intervertebral space height, fusion rate, clinical outcomes, and postoperative complications in two groups. Result : There was no significant difference in reduction and maintenance of anterior translation between two groups. Intervertebral space height was increased in the two groups at immediate postoperative state. At last followup, it was decreased compared to preoperative height in chip PLIF group. In cage PLIF group, last follow-up height was decreased compared to immedate postoperative height, but it was significantly increased compared to preoperative height. Fusion rates were 70.9% and 90% in chip PLIF group and cage PLIF group, respectively. Excellent and good clinical outcomes were 79.2% in chip PLIF group and 85% in cage PLIF group, but there was no statistical significance. Complications were screw fracture(1 case), CSF leakage(1 case) in chip PLIF group and screw loosening and retropulsion of cage(1 case), CSF leakage(2 cases) in cage PLIF group. Conclusion : PLIF using cage is better than PLIF using autologous bone chips in the maintenance of intervertebral space height and fusion rate. But there is no statistical difference of the clinical outcomes between the two groups. Further studies, especially on long term follow-up, should be considered.

Stress analysis according to the vertical bone level in the implant placement (임플란트 매식 시 수직골 높이에 따른 응력분석)

  • The purpose of this study was to compare the distributing pattern of stress on the finite element models with the different vertical bone level of implant fixture. The two kinds of finite element models were designed according to vertical bone level around fixture ($4.0mm{\times}11.5mm$). The cemented crowns for mandibular first and second molars were made. Three- dimensional finite element model was created with the components of the implant and surrounding bone. Vertical loads were applied with force of 200N distributed within 0.5mm radius circle from the center of central fossa and distance 2mm and 4 mm apart from the center of central fossa. Von-Mises stresses were recorded and compared in the supporting bone, fixtures, abutment screws, and crown. The results were as following : (1) In vertical loading at the center circle of central fossa on model 1 and 2, the difference from vertical bone in implant placement did not affect the stress pattern on all components of implant except for crown. (2) With offset distance incerasing and the bone level of implant decreasing, the concentration of stress occured in the buccal side of long crown, around the buccal crestal bone, and on the fixture- abutment interface. As a conclusion, the research showed a tendency to increase the stress on the supporting bone, fixture and screw under the offset loads when the vertical level of bone around fixture was different. Since the same vertical bone bed has more benefits than the different bone bed around fixtures, it is important to prepare a same vertical level of bone bed for the success of implants under occlusal loads.

  • Conventional compression bone fracture plates sometimes cause osteoporosis under the plate due to their high rigidity which in turn transfer physiological load mostly through the plates and screws. In order to prevent the osteopenia we have designed a system which have a viscoelastic washer between plate and screw head. The washer is made of a biocompatible ploymer (untra high molecular weight polyethylene, UHMWPE). Three-dimensional finite element meshes of the human femur with the conventional and new concept bone plate ere generated and the comparative stress analyses are performed with static half-stance loading condition. The results of analyses showed that could reduce the stress shielding effect compared with the conventional plate.

Histomorphometric Analysis of Two Types of Coated Implants : a Preliminary Study Using the Rabbit Tibia Model

  • Purpose : The purpose of this pilot experiment was to evaluate early bone response in two types of coated implants using the rabbit tibia model. Materials and Methods : Screw type titanium implants manufactured with a calcium metaphosphate (CMP) coating and hydroxyapatite (HA) coating were placed in the tibiae of 3 New Zealand White rabbits. The bone responses at 2 weeks after insertion were evaluated and compared by histomorphometry. Results : There was no significant difference in bone-to-implant contact between the groups (P>.05). However, some qualitative differences on histologic views were found. Conclusions : CMP-coating is suggested to be the preferred candidate for fast osseointegration over HAcoating.

  • This is a case of the core reamer breakage during anterior cruciate ligament reconstruction using bone-patellar tendon-bone autograft. A 19-year-old man with acute anterior cruciate ligament rupture had reconstruction surgery. During tunneling in the proximal tibia, the core reamer have been cracked open and broken. Rest of the procedure was performed routinely except the tibial sided fixation of bone block which was performed with larger interference screw than usual. The patient followed the accelerated rehabilitation program. The result was satisfactory at 2 years after surgery.

  • Dental implant is increasingly used to recover the mastication function of tooth. Several types of implant were designed to give an optimal stress distribution in surrounding bony regions. In this study, six types of implant were investigated using finite element method and it was studied i) how the variation of cortical bone thickness affects the stress distribution in surrounding bony regions depending upon implant types, ii) which type gives the best characteristics in the sence of stress distribution and stability. The hybrid-type implant with cylinder and screw gave the optimum properties in view of stability and response to the variation of cortical bone thickness.
