폐경 후 여성의 영양소섭취수준에 따른 골대사 지표물질의 관련성 분석 (Relationship between Nutrient Intake and Biochemical Markers of Bone Metabolism in Korean Postmenopausal Women)

  • To delineate the relationship between the nutrient intake from diet and the serum biochemical markers of bone metabolism, 56 postmenopausal women of 50 to 77 years of age were recruited. The biochemical markers including osteocalcin, calcium, phosphorus, estradiol and free testosterone were measured in fasting blood. Bone mineral density(BMD) was measured also by dual energy X-ray absorptiometry, and the nutrient intake of earth individual subject was estimated by 24-hour recall of 3 days. The age of the subjects was 64.8 $\pm$ 7.7 years, and the BMDs of the subject were 0.86 $\pm$ 0.26g/$cm^2$(Lumbar spine), 0.60 $\pm$ 0.10g/$cm^2$ (Femoral neck), 0.49 $\pm$ 0.10g/$cm^2$(Trochanter), and 0.41 $\pm$ 0.14g/$cm^2$(Ward's triangle). There were no significant differences among age and nutrient intake level groups due to the small sample size. The biochemical markers showed certain degree of relationship with nutrient intake levels. The results were compared among 3 groups with different nutrient intake level classified by the percentage of Recommended Daily Allowances(RDA) for Koreans as follows low < 75% RDA, 75% RDA $\leq$ adequate < 125% RDA, high $\geq$ 125% RDA. The low energy and low riboflavin groups showed significantly higher serum osteocalcin levels than those of the high intake groups(p < 0.05). On the other hand, there was a trend for serum Ca level to be higher with high nutrient intake. In this case, protein and thiamin were the only nutrients that reached a statistical significance(p < 0.05). And the groups with low intake for protein and Ca showed significantly lower serum free testosterone levels than that of other intake groups(p < 0.05). This study suggests an important role of nutrient intake levels on blood biochemical markers of bone metabolism.

Effect of soy isoflavones supplement on climacteric symptoms, bone biomarkers, and quality of life in Korean postmenopausal women: a randomized clinical trial

  • BACKGROUND/OBJECTIVES: Soy isoflavones are expected to improve menopausal symptoms and osteoporosis in women. However, their efficacy is still inconclusive, and there was limited data for postmenopausal women in South Korea. We examined the effects of soy isoflavones on climacteric symptoms, bone biomarkers, and quality of life in Korean postmenopausal women. SUBJECTS/METHODS: A randomized, double-blind study design was used. Eighty-seven participants who had undergone natural menopause were randomly administered either 70 mg/day isoflavones (n = 43) or placebo (n = 41) for 12 weeks. We assessed the Kupperman index for climacteric symptoms and the menopause-specific quality of life (MENQOL) questionnaire for quality of the life. Biomarkers of bone metabolism were also measured in serum bone-specific alkaline phosphatase (BALP), osteocalcin (OC), N- and C-terminal cross-linking telopeptides of type Ι collagen (NTx, CTx), and urine-deoxypyridinolin (u-DPD). RESULTS: Scores of the Kupperman index were decreased in both the isoflavones group ($-7.0{\pm}15.8$, P = 0.0074) and placebo group ($-6.3{\pm}14.6$, P = 0.0064) during the intervention, but no significant difference was noted between the groups. Regarding the bone formation markers, the level of serum BALP increased by $6.3{\pm}4.1%$ (P = 0.004) and OC increased by $9.3{\pm}6.2%$ (P < 0.001), meanwhile those of the placebo were not changed. For the bone resorption markers, NTx, CTx, and u-DPD were not significantly different in either group. MENQOL was significant decreased in the isoflavone group ($-0.6{\pm}0.5$) and placebo group ($-0.6{\pm}0.4$), with a significant difference between groups (P = 0.0228). CONCLUSIONS: Our study suggests that 70 mg isoflavones supplement has beneficial effects on bone formation markers; however, it showed no benefit compared to the placebo on climacteric symptoms or quality of life.

Characterizations of Cell Lineage Markers in the Bone Marrow Cells of Recloned GFP Pigs for Possible Use of Stem Cell Population

  • Two piglets and one juvenile pig were used to investigate closely what types of cells express green fluorescent protein (GFP) and if any, whether the GFP-tagged cells could be used for stem cell transplantation research as a middle-sized animal model in bone marrow cells of recloned GFP pigs. Bone marrow cells were recovered from the tibia, and further analyzed with various cell lineage markers to determine which cell lineage is concurrently expressing visible GFP in each individual animal. In the three animals, visible GFP were observed only in proportions of the plated cells immediately after collection, showing 41, 2 and 91% of bone marrow cells in clones #1, 2 and 3, respectively. The intensity of the visible GFP expression was variable even in an individual clone depending on cell sizes and types. The overall intensities of GFP expression were also different among the individual clones from very weak, weak to strong. Upon culture for 14 days in vitro (14DIV), some cell types showed intensive GFP expression throughout the cells; in particular, in cytoskeletons and the nucleus, on the other hand. Others are shown to be diffused GFP expression patterns only in the cytoplasm. Finally, characterization of stem cell lineage markers was carried out only in the clone #3 who showed intensive GFP expression. SSEA-1, SSEA-3, CD34, nestin and GFAP were expressed in proportions of the GFP expressing cells, but not all of them, suggesting that GFP expression occur in various cell lineages. These results indicate that targeted insertion of GFP gene should be pursued as in mouse approach to be useful for stem cell research. Furthermore, cell- or tissue-specific promoter should also be used if GFP pig is going to be meaningful for a model for stem cell transplantation.

  • Bisphosphonates are widely used in the treatment of many medical conditions, such as osteoporosis, multiple myeloma, Paget's disease, etc. However, side effect has been documented in the published data during the past years, osteonecrosis of the jaw in patients receiving long-term bisphosphonate therapy. Although pathogenesis of BRONJ(bisphophonate-related osteonecrosis of the jaw) is not yet fully understood, it is currently known to be a disease associated with suppressed bone turnover by bisphopbonate. Recent literature has indicated a similar association with nonbisphosphonate drugs used in cancer therapy including monoclonal antibodies denosumab and bevacizumab and multikinase inhibitor sunitinib. Accordingly, many studies have been carried out on the biochemical markers examination to assess the risk for BRONJ. The treatment of BRONI is reported with a review of the relevant literature. However, there is still a controversial discussion about the adequate treatment. It is necessary to accumulate further studies in order to establish more useful biochemical markers and effective treatment for BRONJ.

Effects of Calcium and Vitamin D Supplementation on Bone Mineral Density and Biochemical Markers in Osteoporotic Postmenopausal Women

  • It has been reported that taking a proper amount of calcium and vitamin D helps to increase bone mineral density (BMD) and is effective in decreasing the risk of osteoporosis. This study investigated the supplementary effects of calcium and vitamin D on postmenopausal women who had osteoporosis and used calcium and vitamin D supplements. The study subjects consisted of osteoporotic postmenopausal women who were recruited from the Department of Orthopedics in a university-affiliated hospital. Sixty-seven study subjects were orally administrated 1,000 mg of calcium (calcium carbonate) and 2.5 mg of active vitamin D (1-$\alpha$ hydroxyvitamin D) (cholecalciferol 250 IU) twice a day for a year and a half. BMD and biochemical markers were evaluated and repeated every six months. One year after the intervention test, the bone mineral density of the lumbar spine was significantly increased as compared to the baseline. Six months after supplement administration, the level of serum alkaline phosphatase began to decrease, and afterwards a significant difference was maintained Concentration of 1, 25-dihydroxy-vitamin D at 1.5 years was higher than that of the baseline. In comparison with that of the baseline, the level of urinary hydroxyproline in the study subjects over six months was significantly decreased This study continued that effects such as BMD improvement and changes in biochemical markers appeared at least one year after administration of supplements.

Comparison of immunohistochemical analysis on sinus augmentation using demineralized tooth graft and bovine bone

  • Objectives: The purpose of this animal research was to compare bone regeneration in augmented rabbit maxillary sinuses treated with demineralized particulate human-tooth graft and anorganic bovine bone by immunohistochemical analysis. Materials and Methods: Piezoelectric bilateral sinus augmentation was performed in eight adult rabbits. In the control group, anorganic bovine was grafted in the maxillary sinus following elevation of the sinus membrane. In the experimental group, demineralized human particulate tooth bone was grafted in the sinus. Bone regeneration in augmented sinuses was evaluated by immunohistochemical analysis using various markers of osteoprogenitor cells. Results: The number of bromodeoxyuridine-labeled cells was significantly higher in the experimental group than in the control group at eight weeks. The immunoreactivity of proliferating-cell nuclear antigen was increased slightly in the experimental group relative to the control group at eight weeks. Other bone markers were expressed equally in the two groups. Conclusion: In the rabbit maxillary sinus, higher osteoinduction was correlated with demineralized human particulate tooth bone grafting than with anorganic bovine grafting.

응급센터를 내원한 대퇴골 골절 환자에서 골밀도와 생화학적 표지자들의 의의 (Bone Mineral Bensity and Bone Turnover Markers in Patients with Femur Fracture Who Visited the Emergency Department)

  • Background: The most frequent fracture seen at the emergency department (ED) in the elderly is a femur fracture whereas they do not know the degree of osteoporosis. We analyzed the degree of osteoporosis in patients with femur fractures and compared patients with only femur fractures (FX) to patients with femur and vertebral fractures (VX) by examining the clinical features, the bone mineral density (BMD), and biochemical markers. Methods: From January 2004 to December 2004, we enrolled prospectively 30 femur fracture patients who visited the ED. The bone mineral densities of the lumbar spine and the femur were examined. Total calcium, phosphate, alkaline phosphatase, osteocalcin, and serum C-terminal telopeptide (s-CTx) were measured. The patients with femur fractures were divided into two subgroups according to the presence of vertebral fracture. Results: All BMDs of the FX group showed osteoporosis. The s-CTx levels were higher than normal. The patients in the FX with VX were older than those in the FX only group, and had lower BMDs. There were no significant differences in markers between the subgroups, but the incidence of trochanteric fractures was higher in FX with VX group than in the FX only group. Conclusion: Femur fractures in the elderly were associated with osteoporosis. In our study, despite a considerable difference in BMD between patients with femur fractures and those with femur fractures combined with vertebral fractures, there was no difference in biochemical markers on bone formation nor in the those of bone resorption. We will further investigate the biochemical markers and BMD in the population of osteoporotic fractures. So those indicators should be helpful for planning treatment and for prevention of FX in the elderly.

  • The effects of mulberry-leaf powder Tofu(MPT) on anthropometric measurements, including bone mineral density(BMD) in the right carpus, biochemical bone turnover markers, serum levels of lipids and macrominerals, were investigated in 30 smoking male adults who lived in Choongnam were given MPT(100 g/day) for 4 weeks. The average ages, number of smoked cigarettes and packyear were 22.38 years, 15.12/day and 3.54 years, respectively. The nutrient contents per 100 g MPT were 86.10 kcal energy, 8.98 g protein, 0.53 mg fiber, 211.33 mg Ca and 1.59 g fat. Anthropometric measurements, including dietary intake using the 24-hours recall method, carpal BMD using DEXA, serum levels of protein, albumin and glucose, lipid profiles (cholesterol, triglyceride, HDL-cholesterol, LDL-cholesterol) with Al(atherosclerosis index), HTR, CRF, LHBt, some biomarkers of BMD(serum alkaline phosphatase activity, osteocalcin, urinary DPD), and serum macrominerals(Ca, Ca/P ratio, Mg) and Pb were analyzed before and after consumption of MPT. After MPT consumption, dietary intakes of plant protein, total Ca and plant Ca increased significantly, but there were no significant differences in anthropometric measurements, BMD with bone metabolism markers, serum levels of protein, albumin or glucose, lipid profiles with AI, HTR, LHR and CRF.

  • This study examined the effects of mulberry-leaf powder Tofu(MPT) on the anthropometric measurements, bone mineral density(BMD), biochemical markers of bone turnover, serum lipid levels of and macrominerals status in 30 female university students living in Choongnam with low dietary Ca intake was low(mean daily $intake=524.66{\pm}21.47$ mg/day). The subjects received the MPT supplemented(100 g/day) for 4 weeks. The nutrient content per 100 g MPT was 84.10 kcal(energy), 8.98 g(protein), 0.53 mg(fiber), 211.33 mg(Ca) and 1.59 g(fat). Anthropometric measurements, 24-hour dietary recall, carpus BMD using DEXA, select BMD biomarckers(serum alkaline phophatase activity, osteocalcin, urinary deoxypyridinoline), serum protein levels, albumin, and glucose levels, lipid profiles(cholesterol, triglyceride, HDL-cholesterol, LDL-cholesterol) and serum macrominerals(Ca, Ca/P ratio, Mg) were analyzed before and after MPT supplementation. Following MPT supplementation, there was no significant differences in anthropometric measurements, nutrient intakes, carpus BMD, serum alkaline phosphatase activity, osteocalcin, and urinary deoxypyridinoline, serum levels of protein, albumin, and glucose or lipid profiles with AI, HTR, LHR and CRF. However, there were significant increases in dietary calcium as well as decreases in serum triglycerides with MPT supplementation. Although further research is needed, these results that regular intake MPT could prevent bone and cardiovascular diseases.

Determining the Required Vitamin D Level for Bone Health Based on Bone Turnover Markers

  • To date, no clear threshold that has been established for defining an adequate store of vitamin D for bone health. Therefore, this study aims to determine the required level of vitamin D to maintain a healthy skeleton based on bone remodelling process among healthy adult population. This was a cross sectional study, involving a healthy adult population in Kota Bharu, Malaysia, aged 18~50 years. We measured serum 25(OH)D (vitamin D), serum parathyroid hormone (PTH), serum C-terminal telopeptide of type 1 collagen (CTX), and Procollagen 1 Intact N-Terminal (P1NP) in 120 healthy adults selected via multi stage sampling (64 males, 56 females) from 6 subdistricts in Kota Bharu. The mean level of 25(OH)D was 23.50 (${\pm}8.74$) nmol/L. There was a significant difference of the vitamin D level between genders ($26.81{\pm}8.3nmol/L$ vs $19.72{\pm}7.68nmol/L$ in males and females respectively) (p value<0.001). More than 50% of female subjects had 25(OH)D less than 20 nmol/L, while only 20.3% of male subjects had 25(OH)D below 20 nmol/L. Based on the LOESS plot, the bone turnover markers showed a plateauing result, at the 25(OH)D level of 35 nmol/L for CTX and 20 nmol/L for P1NP. Contrastingly, PTH showed a step rise in the 25(OH)D level of 20 nmol/L. Based on the LOESS plot for CTX, P1NP and PTH versus 25(OH)D, level of vitamin D between 20 to 35 nmol/L is recommended to maintain healthy skeleton.
