Tibial bone grafts provide an adequate volume of cancellous bone with cortical bone, high biologic value of bone, minimal gait disturbance and complications, and no special contraindications, and offer a superior clinical results than any other donor sites. Lateral appoach in tibial bone graft was used to gain large bone volume traditionally, but medial approach provides low morbidity associated with the tibial anatomic structure, simple and safety surgical procedure, and better comfortable to patients recently. We have undertaken clinical and retrospective studies on patients in Dept. of Oral and Maxillofacial Surgery, Inha University Hospital from April 2004 to January 2008. 50 patients have maxillofacial bony defect as resection of bening tumor, cyst enucleation, alveolar bone resorption, sinus pneumatization were received the tibial proximal autogenous particulated cancellous bone grafts. They were analyzed sex, age, diagnosis of recipient site, lesion size, dornor site, cortical bone repositioning, complications and we concluded favorable following results. 1. Medial approach for proximal tibia is safer and technically easier than lateral approach, associated with the proximal tibial anatomic structures, and short operative times. 2. Proximal tibia provides an adequate bone volume with predictability for oral and maxillofacial reconstruction. 3. Patients rarely complain of pain, swelling, discomfort and dysfunction such as gait disturbance. In conclusion, medial approach for proximal tibial graft seems to be a valuable tool for oral and maxillofacial reconstruction.

This study was performed to simulate various sizes of periapical lesions and condensing osteitis in bone and to clarify that condensing osteitis would give a greater radiographic contrast because of the more radiopacity immediately adjacent to the lesion and what the lamina dura on radiogram is in bone. For this study, two dry and wet human adult mandible and a cadaver were used. Each mandible was sectioned longitudinally to split lingual and buccal cortical plates to gain better access to the apex of tooth. Lesions were gradually enlarged and bordered with tooth ash that burned out at 950℃ for the reproduction of condensing osteitis in sequence. The serial radiograms were taken under the constantly maintained condition and compared with the actual lesions.

After extraction of tooth, alveolar ridge resorption is inevitable in most cases. Clinicians confront with horizontal and vertical resorption of alveolar bone. Without massive amount of bone and soft tissue graft, dental implant will be placed apically to gain stability. In those cases, not only white esthetic part, but also pink esthetic part should be restored with pink porcelain. The margin of prosthesis should be located apically to reproduce natural look with adjacent teeth. However, when the margin is located apically, it is always hard to remove remaining cement, thus complications may arise. In this report, a novel design of implant prosthesis is introduced to solve those issues. The novel design is consisted of zirconia framework with pink porcelain and separate crowns on top of the framework. It eliminates the possibility of cement remnants by bringing the crown margin coronally. Pink esthetic part is incorporated in abutment part instead of crown part and the screw hole is covered with separate crowns.
In recent years, a number of special treatment procedures have been introduced to reestablish new tooth supporting tissues with varying degrees of success including guided tissue regeneration(GTR), bone grafting(BG) and the use of enamel matrix derivative(EMD). EMD is an extract of enamel matrix and contains amelogenins of various molecular weights. Emdogain(EMD) might have some advantages over other methods of regenerating the tissue supporting teeth lost by gum disease, such as less postoperative complications. Emdogain contains proteins(derived from developing pig teeth) believed to regenerate tooth attachment. The decrease in probing depth after EMD treatment is achieved primarily by clinical attachment gain and bone regeneration and only to a minor extent by gingival recession. In conclsion, EMD seems to be safe, was able to regenerate lost periodontal tissues in previously diseased sites based on clinical parameters.
Comminuted fractures of the patella mostly occur at the inferior pole and require appropriate reduction and fixation to restore the extensor mechanism. Conventional methods such as tension-band wiring are not enough to gain proper fixation strength. Numerous methods have been reported, including circumferential cerclage wiring, osteosynthesis, and suture anchors depending on the fracture pattern. Herein, the author reports a relatively rare case of a comminuted fracture of the upper pole of the patella, for which we used augmented Krackow sutures in the quadriceps and fixation with tying of the suture limbs through patellar bone tunnels. Satisfactory results were obtained in terms of reduction and extensor mechanism recovery.

Cancer therapy for the head and neck malignoncy by surgery, radiotherapy, or combined modalities may cause substantial aesthetic and functional problems for the patient. The placement of osseointegrated implants into irradiated bone should only be performed when the predictability of achieving and maintaining osseointegration is high and the risk of developing of osteoradionecrosis is low. There are many benefits that irradiated patients may gain from the use of implants. A successful implant-retained prosthesis is dependent upon the implants attaining osseointegraton and then sustaining it during functional loads. The use of implants in irradiated patients requires high implant success rates that are acceptable to warrant their use. We report a case and review the literatures about implants in irradiated bone. In that case, the patient were undergone tumor resection and inner-table mandiblectomy due to squamous cell carcinoma of lower posterior gingiva. But 5 year later, the tumor were recurred, we resected the tumor and applied the radiation therapy. After then, we installed four IMZ implants after hyperbaric oxygenation, and made prosthesis using those implants. Until now they don't have any complications.

The aim of this study was to evaluate the effect of Lycii fructus (LF) on serum lipid and the collagen content of the connective tissues in ovariectomized rats. From day 2 until day 37 after the ovariectomy, Sprague-Dawley female rats were randomly assigned to the following groups : sham-operated rat (Sham), ovariectomized control rat (OVX-control), and ovariectomized rats supplemented with the LF 50mg/kg bw/day (OVX-LF). The LF ethanol extract were orally administrated 1ml per day. Body weight gain was not significantly different in groups. Although total-cholesterol and triglyceride were increased in the ovariectomized control, supplementation with the LF extract decreased the levels. Moreover, the serum HDL-cholesterol levels were significantly increased after supplementation with the LF extract(p<0.05). Supplementation with the LF extract prevented a decrease in the collagen level in bone and cartilage tissues. These results are consistent with the conclusions based on the estrogenic activities of LF. Therefore, it may be used to possibly improve the quality of life in menopausal women.
This study was performed to investigate the effects of lead poisoning and different levels of dietary lipid on lead and lipid metabolism in the rats. Forty eight male weanling rats of Sprague-Dawley strain weighing 73.1$\pm$11.1g were blocked into 6 groups according to body weight. Dietary lipid were given at the lev딘 of 5, 20 and 40% of total caloric intake and lead were either administered or not. The results obtained are summerized as follows ; 1) Food intake, weight gain, F.E.R. liver and epididymal. fat pad weights, weight and length of bone, hematocrit and hemoglobin content in Pb-adminstered groups were lower than these in Pb-free groups. 2) Plasma lipid and cholesterol levels were tended to be higher in Pb-administered groups than in Pb-free groups, while liver lipid and cholesterol levels were tended to be lower in Pb-adminstered groups. 3) Fecal dry matter excretion was tended to be higher in Pb-adminstered groups than in Pb-free groups, and were increased with increasing dietary lipid level. Daily fecal excretions of lipid and cholesterol were higher in high lipid groups than in low lipid groups and these levels were even higher in the animals exposed to Pb than in Pb-free groups. 4) Pb contents in blood, liver, kidney and bone were significantly higher in Pb administered groups than in Pb-free groups. Pb levels of blood, liver and bone did not show any significant difference among groups with different levels of dietary lipid, but Pb concentration in kidney of Pb-adminstered groups increased with increasing dietary lipid level.

It is difficult to manage the growing deformity of forearm bone caused by hereditary osteochondromatosis in children, because deformity and discrepancy of limb length is progressive. The are many treatment methods of these problems including excisio of osteochondroma, lengthening of ulna, shortening of radius, corrective osteotomy with or without lengthening apparatus. Among many treatment methods, we tried free vascularized epiphyseal transplantation with the proximal fibular epiphysis in 3 patients of hereditary osteochondromatosis for inducement of continuous bone growth and deformity correction. The average duration of follow up was 7 years and 1 month, the shortest duration being to 4 years and 5 months and the longest 10 years and 8 months. Serial radiologic and clinical evaluation were carried out during follow up and there were satisfactory length gain, deformity correction and improvement of adjacent joint motion in 2 cases. According to our follow up evaluation, free vascularized epiphyseal transplantation is valuable procedure in forearm deformity of hereditary osteochondromatosis although it needs skillful and experienced operative technique.
This study has investigated the bioavailability of calcium in eggshell powder (ESP) for the purpose of reutilizing eggshells as the calcium source. The experiment was designed 2 ${\times}$ 2 factorial method with two sources, CaCO$_3$ and ESP, and two levels, 0.2% and 0.4% calcium. Weanling SD rats were assigned randomly to one of 4 groups and provided by one of the isocaloric, 20% casein based 4 different experimental diets for 4weeks. Deionized water was given and environment was kept from any contamination of minerals. The body weight, diet intake, feed efficiency ratio (FER), bone growth, Ca contents of bones, and apparent absorption were measured. FER (0.38 - 0.40) and kidney weight were not different among groups and the weight and length, Ca content, strength of two bones Tibia and Femur were not affected by Ca sources except Femur Ca content. Ca content of Femur was greater in ESP groups than that of CaCO$_3$ groups. The body weight gain, bone growth, the Ca contents and strength of bones were significantly greater in 0.4% calcium groups suggesting 0.2% calcium is not sufficient for the optimum growth in the growing rats. These results indicate that ESP be a proper Ca source comparable or superior to CaCO$_3$. However the apparent absorption rate of final 3 days of feeding did not support the observed results showing lower rate in ESP than CaCO$_3$ groups. Further study be needed in the absorption aspect.
