경두개 직류자극을 결합한 목 안정화 운동이 경추성두통환자의 근육특성과 기능에 미치는 영향 (Effects of Neck Stabilizing Exercise Combined with Transcranial Direct Current Stimulation on Muscle Characteristics and Function in Patients with Cervicogenic Headache)

  • Purpose: The purpose of this study was to provide an effective method of exercise therapy for patients with cervicogenic headache. Methods: The subjects were divided into the following three groups according to the intervention received: cervix-stabilizing exercise (n=12, group 1), transcranial direct current stimulation (n=12, group 2), and cervix-stabilizing exercise combined with transcranial direct current stimulation (n=12, group 3). The intragroup and intergroup differences in muscle characteristics and neck disability index were compared and analyzed. Results: The comparison and analysis of the changes in muscle tone and post hoc analysis revealed statistically significant intragroup decreases in the upper trapezius and suboccipitals in groups I and III, and statistically significant intergroup differences in the upper trapezius, with greater changes in group III than in group II, and in the suboccipitals, with greater changes in groupIII than in groups Iand II. The comparison and analysis of the change in muscle stiffness and post hoc analysis revealed a statistically significanti ntra group decrease in the upper trapezius in group Iand suboccipitals in group III, and a statistically significant intergroup difference in both muscles, with greater change in group III than in group II. The comparison and analysis of change in neck disability index and post hoc analysis revealed a statistically significant intragroup decrease in all the three groups and a statistically significant intergroup difference, with greater change in group III than in groups I and II. Conclusion: The neck-stabilizing exercise and transcranial direct current stimulation were shown to be effective in decreasing the tone of the cervical muscles by stabilizing the cervical bone and improving muscle durability, and in improving the movement and limitation of joint range of motion by decreasing muscle tone and stiffness.

Unilateral Augmented Pedicle Screw Fixation for Foraminal Stenosis

  • Objective: The purpose of this study is to evaluate the effectiveness of unilateral decompression and pedicle screw fixation for the unilateral symptomatic foraminal stenosis. Methods: The study group comprises consecutive 16 patients who underwent unilateral decompression and bone cement augmented pedicle screw fixation from May 2003 to January 2006. The patients were evaluated by visual analog scale (VAS) for pain and the scoring system of the Japanese Orthopedic Association (JOA) for low back pain. The result of surgery was also evaluated with McNab's classification. Excellent or good outcome was considered as successful. The patients were followed at postoperative 1 month, 3 month, 6 month, and 1 year with standing AP and lateral films. Results: The average VAS and JOA score of the 16 patients were 7.8(range, 6-9) and 5.8(range, 3 - 10) before surgery and 2.2(range, 0 - 5)and 12.3(range, 9 - 15) at the time of last follow up. Both VAS and JOA score improved significantly after the surgery (p<0.05, t-test). All patients improved after the operation and no revision surgery was required. No metal failure or pseudoarthrosis was observed during the follow-up. The success rate was 87.5%. Conclusion: Our data suggest that unilateral decompression and pedicle screw fixation for the unilateral symptomatic foraminal stenosis is an effect method for obtaining satisfactory clinical outcome. Its possible advantage is shorter operation time and reduced surgical extent. We believe that the reduced stiffness of unilateral fixation was compensated by pedicle screw augmentation and interbody fusion.

  • 1. 저자들은 1986년 11월부터 1988년 2월까지 경골간부 골절 8례를 나사못 맞물림법을 이용한 골수강내 금속정을 사용하여 치료하였다. 2. 술 후 방사선 소견상 골유합 기간은 평균 약 15주였다. 3. 환자의 입원기간은 평균 5.5주로서 비교적 짧은 입원기간을 보여주었으며 아울러 사회복귀 및 적응이 빨랐다. 4. 술 후 조기운동이 가능하였고 평균 4.3주 경과 후 목발보행을 시작하였다. 5. 술 후 관절강직, 근위축등의 합병증은 거의 없었다. Hamza등에 의한 술후 평가방법에 따르면 Excellent 7례, Good 1례로 결과는 만족스러웠다.

Metronomic Chemotherapy with Toceranib Phosphate for a Disseminated Histiocytic Sarcoma in a Miniature Schnauzer Dog

  • A 15-year-old spayed female Miniature Schnauzer was presented for unilateral foreleg lameness and pain. On physical examination, left elbow joint swelling and stiffness were identified. On a computed tomography (CT) scan, a periosteal reaction of the left humerus from the distal metaphysis to the epiphysis and cortical destruction of the medial condyle was observed. Based on blood tests, histopathology, and immunohistochemistry, it was concluded as a skeletal histiocytic sarcoma. Since the patient's pain was not controlled despite application of a fentanyl patch, a left forelimb amputation was decided upon as part of the palliative therapy. Metronomic chemotherapy with toceranib phosphate and pamidronate was initiated. Toceranib was administered for 3 months without the development of any adverse effects except mild neutropenia. However, 3 months after initiating treatment, the toceranib was discontinued due to moderate gastrointestinal disturbances. Over the next 2 months, a left mandibular bone mass and cortical bone destruction in the bilateral tibia and tarsal joint were identified on CT. The patient became unwilling to eat and was noted to have severe skeletal pain. The anorexia and lethargy were progressively worsening and the owner decided to euthanize the patient. A necropsy was performed and the patient was definitively diagnosed with disseminated histiocytic sarcoma based on histopathologic and immunohistochemical analyses. This report describes a Miniature Schnauzer dog with DHS managed with surgical removal and metronomic chemotherapy with toceranib that survived with an improved quality of life for 7 months.

Comparative finite element analysis of mandibular posterior single zirconia and titanium implants: a 3-dimensional finite element analysis

  • PURPOSE. Zirconia has exceptional biocompatibility and good mechanical properties in clinical situations. However, finite element analysis (FEA) studies on the biomechanical stability of two-piece zirconia implant systems are limited. Therefore, the aim of this study was to compare the biomechanical properties of the two-piece zirconia and titanium implants using FEA. MATERIALS AND METHODS. Two groups of finite element (FE) models, the zirconia (Zircon) and titanium (Titan) models, were generated for the exam. Oblique (175 N) and vertical (175 N) loads were applied to the FE model generated for FEA simulation, and the stress levels and distributions were investigated. RESULTS. In oblique loading, von Mises stress values were the highest in the abutment of the Zircon model. The von Mises stress values of the Titan model for the abutment screw and implant fixture were slightly higher than those of the Zircon model. Minimum principal stress in the cortical bone was higher in the Titan model than Zircon model under oblique and vertical loading. Under both vertical and oblique loads, stress concentrations in the implant components and bone occurred in the same area. Because the material itself has high stiffness and elastic modulus, the Zircon model exhibited a higher von Mises stress value in the abutments than the Titan model, but at a level lower than the fracture strength of the material. CONCLUSION. Owing to the good esthetics and stress controllability of the Zircon model, it can be considered for clinical use.

공진 주파수 분석법에 의한 임플랜트의 안정성 측정에 관한 연구 (A STUDY ON THE MEASUREMENT OF THE IMPLANT STABILITY USING RESONANCE FREQUENCY ANALYSIS)

  • Statement of problem : Successful osseointegration of endosseous threaded implants is dependent on many factors. These may include the surface characteristics and gross geometry of implants, the quality and quantity of bone where implants are placed, and the magnitude and direction of stress in functional occlusion. Therefore clinical quantitative measurement of primary stability at placement and functional state of implant may play a role in prediction of possible clinical symptoms and the renovation of implant geometry, types and surface characteristic according to each patients conditions. Ultimately, it may increase success rate of implants. Purpose : Many available non-invasive techniques used for the clinical measurement of implant stability and osseointegration include percussion, radiography, the $Periotest^{(R)}$, Dental Fine $Tester^{(R)}$ and so on. There is, however, relatively little research undertaken to standardize quantitative measurement of stability of implant and osseointegration due to the various clinical applications performed by each individual operator. Therefore, in order to develop non-invasive experimental method to measure stability of implant quantitatively, the resonance frequency analyzer to measure the natural frequency of specific substance was developed in the procedure of this study. Material & method : To test the stability of the resonance frequency analyzer developed in this study, following methods and materials were used : 1) In-vitro study: the implant was placed in both epoxy resin of which physical properties are similar to the bone stiffness of human and fresh cow rib bone specimen. Then the resonance frequency values of them were measured and analyzed. In an attempt to test the reliability of the data gathered with the resonance frequency analyzer, comparative analysis with the data from the Periotest was conducted. 2) In-vivo study: the implants were inserted into the tibiae of 10 New Zealand rabbits and the resonance frequency value of them with connected abutments at healing time are measured immediately after insertion and gauged every 4 weeks for 16 weeks. Results : Results from these studies were such as follows : The same length implants placed in Hot Melt showed the repetitive resonance frequency values. As the length of abutment increased, the resonance frequency value changed significantly (p<0.01). As the thickness of transducer increased in order of 0.5, 1.0 and 2.0 mm, the resonance frequency value significantly increased (p<0.05). The implants placed in PL-2 and epoxy resin with different exposure degree resulted in the increase of resonance frequency value as the exposure degree of implants and the length of abutment decreased. In comparative experiment based on physical properties, as the thickness of transducer increased, the resonance frequency value increased significantly(p<0.01). As the stiffness of substances where implants were placed increased, and the effective length of implants decreased, the resonance frequencies value increased significantly (p<0.05). In the experiment with cow rib bone specimen, the increase of the length of abutment resulted in significant difference between the results from resonance frequency analyzer and the $Periotest^{(R)}$. There was no difference with significant meaning in the comparison based on the direction of measurement between the resonance frequency value and the $Periotest^{(R)}$ value (p<0.05). In-vivo experiment resulted in repetitive patternes of resonance frequency. As the time elapsed, the resonance frequency value increased significantly with the exception of 4th and 8th week (p<0.05). Conclusion : The development of resonance frequency analyzer is an attempt to standardize the quantitative measurement of stability of implant and osseointegration and compensate for the reliability of data from other non-invasive measuring devices It is considered that further research is needed to improve the efficiency of clinical application of resonance frequency analyzer. In addition, further investigation is warranted on the standardized quantitative analysis of the stability of implant.

콩 이소플라본이 난소를 절제한 성장기 흰쥐의 골격대사에 미치는 영향 (Effects of Soy Isoflavone on the Bone Metabolism in Ovariectomized Growing Rats)

  • 본 연구에서는 대두 이소플라본이 난소를 절제한 성장기 흰쥐의 골격 발달에 미치는 영향을 알아보기 위하여 3주령 된 Wister 암컷 쥐를 난소절제한 후 8주 동안 genistein을 경구 투여하여 골대사에 미치는 일반지표 및 생화학지표를 관찰하였다. 1. 체중 증가는 OVX군$(230.2{\pm}25.15\;g)$이 SH군$(170.6{\pm}20.51\;g)$에 비해 유의적으로 높게 나타났으나 (p<0.05), OVX+GEN군과 OVX+ES군의 체중 증가는 OVX군에 비해 유의적인 차이를 보이지 않았다. 2. 체중 당 간과 자궁의 무게는 난소를 절제한 모든군에서 SH군에 비해 유의적으로 낮았으며(p<0.05), 신장의 무게 또한 SH군에 비해 난소를 절제한 모든 군에서 유의적으로(p<0.05) 감소하였으나 난소절제군 사이에서 유의적인 차이는 없었다. 3. 대퇴골의 무게는 모든 군간에서 유의적인 차이는 없었으나 대퇴골의 길이는 SH군에 비해 OVX군에서 길게 나타났다. 4. 뇨 중 creatinine의 양은 난소 절제군에서 모두 증가하였으나 유의적인 차이는 나타나지 않았다. 또한 hydroxyproline 함량도 모든 군간에서 차이가 나타나지 않았다. 5. 혈청 ALP 활성은 모든 군에서 차이가 없었으며 대퇴골, 변, 혈청, 뇨에서의 Ca의 함량도 모든 군간에서 유의적인 차이가 없었다. 6. 대퇴골의 골밀도는 SH군에 비해 난소를 절제한 군에서 낮았으며 genistein 투여로 다시 증가하는 경향을 보였으나 유의적인 차이는 없었다. 파단력과 stiffiness의 경우 난소 절제 및 genistein 처리에 의한 영향을 받지 않았다. 본 결과를 통하여 genistein에 의한 투여는 난소를 절제한 성장기의 흰쥐에서도 최대골질량을 유지하고 골질량의 손실을 억제할 수 있는 효과가 있어 성장기 동안의 섭취가 골다공증 예방에 효과가 있을 것으로 예상된다.

Freiberg병의 수술적 치료 (Surgical Treatment of Freiberg's Disease)

  • Freiberg's disease is a pathologic condition of the second or third metatarsal head, rarely the forth or fifth metatarsal head, and it becomes abnormally enlarge due to avascular necrosis of subchondral cancellous bone. From Nov. 1982 to Sep. 1994, we treated surgically 10 cases of the disease who complained the continuous symptoms inspite of proper conservative management. Metatarsal head excision was done in 8 cases and resurfacing of the cartilagenous portion of the metatarsal head in 2 cases. During the average follow up of 55 months, the pain was relieved in all patients who were underwent surgical intervention but the stiffness of the metatarsophalangeal joint was remained in most of cases. It seems to be a logical treatment of choice in younger and active patients that conservative management is the initial treatment but more ablative procedure is needed for the continued symptoms. We can propose the metatarsal head excision or resurfacing of the involved joint is effective treatment method with simple procedure and minimize morbid period than other procedures such as corrective osteotomy.

족부에 발생한 활막 연골종증을 동반한 유선상 과오종: 2예 보고 (Melorheostosis with Synovial Chondromatosis of the Foot: A Report of Two Cases)

  • Melorheostosis is a rare disease, belonging to the sclerotic bone dysplasia group. Initially described by Leri and Joanny in 1922, its etiology remains unknown. Onset is usually insidious, with deformity of the extremity, pain, limb stiffness, and limitation of motion in the joints. The typical radiographic appearance consists of irregular hyperostotic changes of the cortex, resembling melted wax dripping down one side of a candle. Treatment is usually symptomatic and conservative; however, conservative treatment is unsatisfactory due to functional issues when involving the distal extremity. We report on two cases of melorheostosis with synovial chondromatosis of the foot treated by mass excision.

Mechanical Properties and Microstructures in WC-12%Co/Low Carbon Steel Metal Matrix Composites(MMC) Welding Overlay

  • Metal matrix composites(MMC) consist of metal matrix into which is distributed a second solid phase. The normal intension is to develop a material with superior mechanical properties (for example increased toughness, stiffness and wear resistance) compared to those inherent in the matrix component. In this study, WC-12%Co/low carbon steel MMC overlays have been prepared by Gas Metal Arc Welding(GMAW) according to feeding rate of WC-12%Co grit. The macro and microstructures were examined using optical microscopy (OM) and scanning electron microscopy(SEM) each other. The characteristics of hardness and wear resistance have been investigated. WC-12%Co/low carbon steel MMC overlays which have been taken good beads without porosity and cracks were manufactured by method of GMAW. Matrix of overlayed surface was seen as fish bone and faceted dendrite structures. It was known that structures were iron tungsten carbides, Fe$_{6}$W$_{6}$C which have been occurred by melting of WC-12%Co grits. After MMC had been tested by block-roll wear test it was known that WC-12%Co/low carbon steel MMC has a excellent wear resistance by exiting Fe6w6c and WC-12%Co grit. The consequence was that region of overlay with Fe$_{6}$W$_{6}$C phase has been showed a model of adhesive wear, but region of overlay with WC-12%Co grit was restrained as a result of mechanism that wear of WC-12%Co grit is not adhesive but fracture.racture.
