There can be little dissension that the ultimate goal of all physical therapy interventions with the elderly is to restore or maintain the highest level of function possible for the individual. Whenever physical therapists take on this challenge, they assist elders in maintaining their identities as competent adults. Advancing age is associated with profound changes in body composition, including increased fat mass, decreased fat-free mass(particularly muscle), decreased total body water and decreased bone density. Along with these changes in body compositions, and perhaps as a direct result of them, elderly people have lower energy needs, reduced strength and functional capacity and a greatly increased risk for such diseases as noninsulin-dependent diabetes mellitus and osteoporosis. Resistance training is considered a promising intervention for reversing the loss of muscle function and the deterioration of muscle structure that is associated with advanced age. This reversal is thought to result in improvements in functional abilities and health status in the elderly by increasing muscle mass, strength and power and by increasing bone mineral density. In the past couple of decades, many studies have examined the effects of Resistance training on risk factors for age-related diseases or disabilities. We have explored the positive and negative aspects of older adults' participation in resistance training programs. The benefits to older adults are reported to be increased strength, endurance, muscle capacity, and flexibility; more energy; and improved self-image and confidence. The negative aspects include some pain or stiffness and other nonspecific problems. The positive and negative aspects of resistance training are therefore very similar to those in younger populations. Scientific investigations over the past 10 years have demonstrated that resistance training can be safely and successfully implemented in older populations. Even the frail and very sick elderly can benefit and improve their quality of life. Proper design and progression of a resistance training program for older adults is vital to optimal benefits from resistance exercise. The results of data provided by this research on resistance training for health shows that there is enough existing evidence to conclude that resistance training, particularly when incorporated into a comprehensive fitness program, can offer substantial health benefits which can be obtained by persons of all ages. These benefits, including improvements in functional capacity, translate into an improved quality of life.
목적 : 어린이나 혹은 성인에 생긴 대퇴부 절단술이 필요한 슬관절 주위의 악성 및 침윤성 골종양에서, 분절 절제 및 회전 성형술을 시행한 치험례를 분석하여 보고하는 바이다. 대상 및 방법 : 1988년 2월부터 1994년 6월까지 슬관절 부위의 악성 및 침윤성 골종양으로 하지 분절 절제 및 회전 성형술을 시행 받은 2 6명을 대상으로 하였으며, 추시 기간은 악성종양의 경우 평균 57(6~120)개월이었고, 평균 연령은 21.4(5~37)세였다. 26례 중 stage IIB 이상의 골육종이 18례, 활막 육종이 2례, 병적골절 또는 재발된 거대 세포종이 6례였다. 결과 : 기능적 결과는 사망 또는 절단술을 받은 4례를 제외한 22례에 대하여 Shriner's rating system에 따라 실시하였으며, 18례에서 우수, 3례에서 양호, 1례는 보통이었다. 족관절의 평균 운동범위는 $-11^{\circ}$(족배굴)~$80^{\circ}$(족저굴)였으며, 보조기 착용 후 일상 생활의 보행이 가능하였다. 국소 재발은 2례, 원격 전이는 10례에서 관찰되었다. 초기 합병증은 혈전증 3례와 패혈증 1례가 있었다. 후기 합병증은 불유합 및 지연유합이 6례, 부정 회전이 2례, 족관절의 운동제한이 1례 있었다. 결론 : 약 10세 미만의 소아에서 많은 하지 부동이나 성장에 따른 골수강의 확장으로 종양 삽입물의 해리가 예상되는 경우와 악성 및 침윤성 골종양으로 절단술이 불가피한 경우에, 기능적으로 우수한 회전 성형술은 부분 사지 구제술로써 유효한 방법으로 사료된다.
Study was developed the metallic plate for fixation in the femur fracture and plates has a firm place in fracture treatment. Purpose: To analyze the outcome of metatarsal lengthening of first brachymetatarsia by callotasis using an external fixator. Materials and Methods: Between January 1998 and February 2004, 10 patients (17 cases) were reviewed. The mean age at operation was 17.3 years. Seven patients had bilateral first brachymetatarsia and eight patients had combined 4th brachymetatarsia. The operations were performed with a monoexternal fixator, and distraction was started at a rate of 0.75 mm/day after 7 days. The radiographic results were evaluated by lengthening amount and percentage, fixation time, and healing index. Complications and AOFAS score were evaluated. Results: The average lengthening amount was 17.7 mm and the average lengthening percentage was 43.4%. The external fixation time was 107 days and average healing index was 69.8 days/cm. The evaluation according to AOFAS score was excellent in 12 cases and good in 5 cases. Complications were 4 cases of hallux valgus, 4 of metatarsophalangeal joint stiffness, 3 of medial angular deformity, 3 of pes cavus, 2 of pin breakage, 2 of pin site infection, and 1 of skin hyperpigmentation. Conclusion: Callotasis for 1st brachymetatarsia is a very useful treatment method with high patient satisfaction, excellent healing rate and early ambulation without bone graft. Nevertheless, great care must be taken to minimize the various possible complications.
Purpose: We assessed the results of open treatment of anterior impingement syndrome of the ankle in elite level soccer players and concomitant injuries were idenfied. Materials and Methods: We retrospectively reviewed twenty one elite level soccer players diagnosed with anterior impingement syndrome who underwent open debridement between January 1997 and January 2002. All were men and the mean age at the operation was 21 years (range 16 to 27). The mean follow-up duration was 31 months (13 to 71). Concomitant abnormalities were idenfied through physical examination, bone scan and MRI. On a preoperative lateral radiograph, patients were classified according to McDermott's stage. Anteromedial or anterolateral approach was used at the operation and osteophyte was removed with osteotome and rongeur. When chronic ankle instability was accompanying, we performed Modified Brostrom-Gould procedure and for osteochondral lesion, multiple drilling was applied. The Ogilvie-Harris scoring system was used as a clinical scale to evaluate pain, swelling, stiffness and limitation of activity. The results were scored as excellent (15 to 16 points), good (13 to 14) and otherwise unsatisfactory. The time to return to full activity including sports activity was determined. Results: Eighteen of twenty one patients had an excellent outcome. Three patients were graded unsatisfactory and two of them abandoned their career due to the persistence of residual pain. Concomitant abnormalities were found including twelve cases of chronic ankle instability, three cases of osteochondral lesion and two cases of flexor hallucis longus tendinitis. Conclusion: Open debridement was successfully applied to the elite level soccer player with anterior impingement syndrome of the ankle. Considerable coexistence of other abnormalities such as chronic ankle instability may encourage us to consider additional operative procedure.
Background: The purpose of this study was to investigate the outcomes after fixation using a 3.5-mm locking compression plate (LCP) hook plate for isolated greater tuberosity (GT) fractures of the proximal humerus. Methods: We evaluated the postoperative radiological and clinical outcomes in nine patients who were followed up at least 1 year with isolated GT fractures. Using the deltopectoral approach, we fixed the displaced GT fragments with a 3.5-mm LCP hook plate (Synthes, West Chester, PA, USA). Depending on the fracture patterns, the hook plate was fixed with or without augmentation using either tension suture or suture anchor fixation. Results: All the patient showed successful bone union. The mean time-to-union was 11 weeks. The radiological and clinical outcomes at the final follow-up were generally satisfactory. The mean visual analogue scale for pain, the University of California at Los Angeles score, the American Shoulder and Elbow Surgeons score, and the subjective shoulder value were 1.4, 30.3, 84.3, and 82.2%, respectively. The mean active forward flexion, abduction, external rotation, and internal rotation of the shoulder were $156.7^{\circ}$, $152.2^{\circ}$, $61.1^{\circ}$, and the 10th thoracic vertebral level, respectively. Only one patient presented with a postoperative complication of shoulder stiffness. The patient was treated through arthroscopic capsular release on the 5th postoperative month. Conclusions: We conclude that fixation using 3.5-mm LCP hook plates for isolated GT fractures of the proximal humerus is a useful treatment method that provides satisfactory clinical and radiological outcomes.

Computational multibody models of the elbow can provide a versatile tool to study joint mechanics, cartilage loading, ligament function and the effects of joint trauma and orthopaedic repair. An efficiently developed computational model can assist surgeons and other investigators in the design and evaluation of treatments for elbow injuries, and contribute to improvements in patient care. The purpose of this study was to develop an anatomically correct elbow joint model and validate the model against experimental data. The elbow model was constrained by multiple bundles of non-linear ligaments, three-dimensional deformable contacts between articulating geometries, and applied external loads. The developed anatomical computational models of the joint can then be incorporated into neuro-musculoskeletal models within a multibody framework. In the approach presented here, volume images of two cadaver elbows were generated by computed tomography (CT) and one elbow by magnetic resonance imaging (MRI) to construct the three-dimensional bone geometries for the model. The ligaments and triceps tendon were represented with non-linear spring-damper elements as a function of stiffness, ligament length and ligament zero-load length. Articular cartilage was represented as uniform thickness solids that allowed prediction of compliant contact forces. As a final step, the subject specific model was validated by comparing predicted kinematics and triceps tendon forces to experimentally obtained data of the identically loaded cadaver elbow. The maximum root mean square (RMS) error between the predicted and measured kinematics during the complete testing cycle was 4.9 mm medial-lateral translational of the radius relative to the humerus (for Specimen 2 in this study) and 5.30 internal-external rotation of the radius relative to the humerus (for Specimen 3 in this study). The maximum RMS error for triceps tendon force was 7.6 N (for Specimen 3).
Objective: The purpose of this study is finding tendencies admission patients to orient medical hospital due to Traffic Accident. Method: We studied 34 cases who were admitted in Dong-eui University Oriental Hospital from 1st, December, 2003 to the 30st, May, 2005. And we came to some conclusion about clinical tendencies as follows. Results and Conclusions: 1. Women had occupied more than men, And 10 persons in 30th were the most distribution of age. 2. Most patients used western medical hospital as primary hospital and after several days they started to search for orient medical treatments. 3. In the diagnosis through radiological test, cervical spine sprain was most(19 cases, 55.6%). The others were lumbar spine sprain(14 cases, 41.2%), fracture of other bone{4 cases, 11.8%), fracture of spine(3 cases, 8.8%), Intercranial hemorrhage(3 cases, 8.8%). 4. In chief complaint, low back pain was most(19 cases, 55.9%), The others were neck stiffness(17 cases, 50.0%), headache(13 cases, 38.2%), lower limb pain(9 cases, 26.5%). 5. Simple Disease had higher than complex disese at effective rate. 6. Both $43{\sim}60$ days admission and $15{\sim}21$ days admission were the best effective. $8{\sim}14$ days admission was next effective. 7. Early visit made more effective.
본 연구에서는 인간중이의 소리전달특성 계산을 위한 유한요소모델을 개발하였다. 이소골의 형상을 얻기 위하여 한국인 사체에서 추출한 측두골을 마이크로 CT 촬영하여 3 차원 입체모델로 변환하였다. 유한요소모델은 이소골, 고막, 인대와 근육 등을 포함하여 구성하였다. 유한요소모델을 이용하여 고막에서 등골족판까지의 응답함수를 계산한 후 측정값을 갖는 선행연구와 비교하였고 그 결과 10 kHz 주파수 대역까지 소리전달특성을 잘 표현하고 있음을 보였다. 또한 유한요소 모델을 구성하는 주요 물성인자의 변화에 대한 소리전달특성의 변화를 살피고 침등골관절의 강성값이 중이의 소리전달특성에 큰 영향을 미침을 보였다.
본 논문은 정형외과영역인 대퇴부에 고정하여 사용하는 금속판으로 골 고정 골절치료의 유합술 골절치료를 할 수 있도록 구성하였는데, 사용되는 치료방법은 견고하고, 안정적이며, 역동적인 생물학적 금속판으로 고정 골수강 내 고정술을 적용되도록 견고한 골접촉 곡선형 시스템을 분석하였다. 금속판은 두 가지 유형으로 장형과 단형으로 구성되고, 금속판의 굴곡이 구조적이고 기하학적으로 경성 및 강도가 고루 분포하도록 최적화 하였다. 장 플레이트의 골접촉에 따른 곡선형으로 굽힙강도는 11,000N 이고, 단 플레이트의 골접촉에 따른 곡선형으로 굽힙강도는 6,525N 이며, 금속판에 골편간 압박을 주는 인장강도는 $1573N/m^2,\;1539N/m^2$정도이다. 금속판은 곡선부와 금속판부의 두 가지 부분으로 나뉘어져 있는데, 곡선부만 있는 단형과 밑 부분의 금속판이 달려있는 장형으로 진행되며, 곡선부의 단형은 전체적인 Profile이 낮고, 금속판이 달려있는 장형은 슬리브의 일체형으로 Profile보다 약간 높아져서 있다. 본 논문의 결과로 제공되는 것은 Hip Implant의 Revision case에 있어 보완뿐만 아니라 Hip Neck Fracture 경우에 사용되었던 Compression Hip Screw의 사용이 가능할 것으로 예상된다.
