흉곽부위 골절에 대한 골스캔의 유용성 고찰 (Availability of Bone Scan in Chest Trauma Patients)

  • 연구배경 : 많은 외상 환자에서 흉곽골이 손상에 노출되지만 흉곽골 외상 환자에서 단순 흉부 사진으로 흉곽부위 손상정도를 최종진단 하기는 어려운 경우가 많다. 특별히 흉곽골의 경미한 손상의 경우는 골주사 검사를 사용하여 명확한 진단을 할 수 있겠는데 감수성은 매우 높은 결과를 나타내었다. 재료 및 방법 : 680명의 흉곽 외상환자 중 골스캔을 환자를 대상으로 조사하였다. 결과 : 680례에서 감수성은 97.4%이고 위음성은 2.6%였다. 감수성에 있어서 검사 실시 시기가 가장 중요한 요소로 생각되며 손상 부위별로 늑골이나 흉골 골절의 경우는 1주 이후, 늑연골 접합부골절의 경우와 상기 부위골절들이 동반된 경우는 3일 이후면 언제나 우수한 결과를 보였다. 결론 : 흉부외상환자에서 골절에 대한 정확한 진단을 위하여는 적절한 시기의 골스캔이 필요하다.

골주사를 이용한 사교상(Snake bite) 환자의 경과 관찰 2예 보고 (Two Cases of Bone Scan in Snake Bite)

  • It is very difficult to check the severity and clinical course of the toxicity in snake bite patients by virtes of clinical manifestation and laboratory tests. And we observed the. findings of bone scan with 99mTc-MDP in two snake bite patients. First patient was bitten in the right ankle with local pain and swelling. The finding of bone scan of him was increased uptake of radionuclide in the soft tissue of right leg and thigh. Others were normal findings. Second patient was bitten in the right hand. But his symptom was severe and he complained local pain and swelling, nausea, blurred vision, and oliguria. The bone scan findings of second patient was; Increased uptake of radionuclide in the soft tissue of whole body. Decreased uptake in the bone tissue. Renal outline was not delineated. Follow up study 10 days after, revealed more improved findings in the scan.

류마티스 질환에서 골스캔의 역할 (Role of Bone Scan in Rheumatic Diseases)

  • Rheumatic diseases can be categorized by pathology into several specific types of musculoskeletal problems, including synovitis (e.g. rheumatoid arthritis), enthesopathy (e.g. ankylosing spondylitis) and cartilage degeneration (e.g. osteoarthritis). Skeletal radiographs have contributed to the diagnosis of these articular diseases, and some disease entities need typical radiographic changes as a factor of the diagnostic criteria. However, they sometimes show normal radiographic findings in the early stage of disease, when there is demineralization of less than 30-50 %. Bone scans have also been used in arthritis, but not widely because the findings are nonspecific and it is thought that bone scans do not add significant information to routine radiography. Bone scans do however play a different role than simple radiography, and it is a complementary imaging method in the course of management of arthritis. The Image quality of bone scans can be improved by obtaining regional views and images under a pin-hole collimator, and through a variety of scintigraphic techniques including the three phase bone scan and bone SPECT. Therefore, bone scans could improve the diagnostic value, and answer multiple clinical questions, based on the pathophysiology of various forms of arthritis.

Bone Scan 검사 시 실제 환자에게 투여되는 용량 계산을 위한 실험 (Experiment to Calculate the Dosage of Radiopharmaceutical Products during the Bone Scan Tests)

  • 본 실험에서는 조영제를 사용하는 CT 혹은 MRI 검사 후, 3-way를 제거하지 않고, Bone scan 검사를 위해 내원한 50명의 환자를 대상으로 방사성 의약품 99mTc-MDP가 표지된 동위원소를 주사 할 때, 주사기와 3-way에 남게 되는 방사능을 측정하여, 실제 환자에게 투여된 방사능을 확인했다. 핵의학과 Bone scan 검사를 위해 내원한 환자 중 CT 혹은 MRI 검사 후 3-way를 유지하고 있는 50명의 환자를 대상으로 3-way를 통해 99mTc-MDP로 표지된 동위원소를 주사 한 후, 주사기와 3-way에 남아있는 방사능을 검량기로 측정하여 실제 투여된 용량을 확인 했다. 방사성의약품을 투여하기 전의 주사기의 방사능과 투여한 후 주사기와 3-way의 방사능을 측정한 결과, 투여 전 주사기의 방사능은 평균 31.75 mCi, 투여 후 주사기에 남은 방사능은 평균 1.22 mCi, 투여 후 3-way에 남은 방사능은 평균 0.95 mCi으로 측정되었다. 실제 환자에게 투여된 방사능은 평균 29.57 mCi로 처음 환자에게 투여하고자 했던 용량보다 평균2.18 mCi 가 환자에게 덜 투여됨을 알 수 있었다. Bone scan 검사 시, 3-way와 실린지에 남게 되는 방사능을 감안하여 투여 용량을 결정한다면, 실제 환자에게 투여하고자 하는 목적 용량을 좀 더 정확하게 투여 할 수 있을 것이다.

The effect of mechanical properties of bone in the mandible, a numerical case study

  • Bone properties are one of the key components when constructing models that can simulate the mechanical behavior of a mandible. Due to the complexity of the structure, the tooth, ligaments, different bones etc., some simplifications are often considered and bone properties are one of them. The objective of this study is to understand if a simplification of the problem is possible and assess its influence on mandible behavior. A cadaveric toothless mandible was used to build three computational models from CT scan information: a full cortical bone model; a cortical and cancellous bone model, and a model where the Young's modulus was obtained as function of the pixel value in a CT scan. Twelve muscle forces were applied on the mandible. Results showed that although all the models presented the same type of global behavior and proximity in some locations, the influence of cancellous bone can be seen in strain distribution. The different Young's modulus defined by the CT scan gray scale influenced the maximum and minimum strains. For modeling general behavior, a full cortical bone model can be effective. However, when cancellous bone is included, maximum values in thin regions increase the strain distribution. Results revealed that when properties are assigned to the gray scale some peaks could occur which did not represent the real situation.

Spiral scanning imaging and quantitative calculation of the 3-dimensional screw-shaped bone-implant interface on micro-computed tomography

  • Purpose: Bone-to-implant contact (BIC) is difficult to measure on micro-computed tomography (CT) because of artifacts that hinder accurate differentiation of the bone and implant. This study presents an advanced algorithm for measuring BIC in micro-CT acquisitions using a spiral scanning technique, with improved differentiation of bone and implant materials. Methods: Five sandblasted, large-grit, acid-etched implants were used. Three implants were subjected to surface analysis, and 2 were inserted into a New Zealand white rabbit, with each tibia receiving 1 implant. The rabbit was sacrificed after 28 days. The en bloc specimens were subjected to spiral (SkyScan 1275, Bruker) and round (SkyScan 1172, SkyScan 1275) micro-CT scanning to evaluate differences in the images resulting from the different scanning techniques. The partial volume effect (PVE) was optimized as much as possible. BIC was measured with both round and spiral scanning on the SkyScan 1275, and the results were compared. Results: Compared with the round micro-CT scanning, the spiral scanning showed much clearer images. In addition, the PVE was optimized, which allowed accurate BIC measurements to be made. Round scanning on the SkyScan 1275 resulted in higher BIC measurements than spiral scanning on the same machine; however, the higher measurements on round scanning were confirmed to be false, and were found to be the result of artifacts in the void, rather than bone. Conclusions: The results of this study indicate that spiral scanning can reduce metal artifacts, thereby allowing clear differentiation of bone and implant. Moreover, the PVE, which is a factor that inevitably hinders accurate BIC measurements, was optimized through an advanced algorithm.

$^{18}$F-Fluoride-PET을 이용한 골격계 영상 ($^{18}$F-Fluoride-PET in Skeletal Imaging)

  • Bone scintigraphy using $^{99m}$Tc-labeled phosphate agents has long been the standard evaluation method for whole skeletal system. However, recent shortage of $^{99m}$Tc supply and advanced positron emission tomography (PET) technology evoked the attention to surrogate radiopharmaceuticals and imaging modalities for bone. Actually, fluorine-18 ($^{18}$F) was the first bone seeking radiotracer before the introduction of $^{99m}$Tc-labeled agents even though its clinical application failed to become pervasive anymore after the rapid spread of Anger type gamma camera systems in early 1970s. However, rapidly developed PET technology made us refocus on the usefulness of $^{18}$F as a PET tracer. Early study comparing $^{18}$F-Na PET scan and planar bone scintigraphy reported that PET has higher sensitivity and specificity in the diagnosis of metastatic bone lesions than planar bone scan. Subsequent reports comparing between PET and both planar and SPECT bone image also revealed better results of PET scan in similar study groups. Rapid clinical application of PET/CT also accumulated considerable amount of experiences in skeletal evaluation and this modality is known to have better diagnostic power than stand alone PET system as well as bone scan. Furthermore $^{18}$F-Na PET/CT revealed better or at least equal results in detection of primary and metastatic bone lesions compared with CT and MRI. Therefore, it is obvious that $^{18}$F-Na PET/CT has potential to become new imaging modality for practical skeletal evaluation so continuous and careful evaluation of this modality and radiopharmaceutical must be required.

골격계 육종에서 관상골MR과 편평골CT의 유용성 (Skeletal Sarcomas Examined with MR in Tubular and CT in Flat Bones)

  • 목적: 골격을 침해하는 원발성 악성종양으로는 골육종이나 연골육종 같은 간엽성육종과 유윙육종이나 림프종 같은 소원형세포성육종으로 나눈다. 골격육종을 진단하기 위해 관상골 육종은 MR검사를 편평골 육종은 CT검사를 주로 이용한다. MR과 CT는 공히 골파괴병소와 연조직종괴를 잘 나타내지만 무기질침착은 MR에서 식별되기 어렵다. 이에 본저자들은 관상골 MR과 편평골 CT검사의 골파괴 소견으로 간엽성육종과 소원형세포성육종을 감별하고자 하였다. 대상 및 방법: 수술적 조직생검술에 의한 병리조직학적으로 진단되고 관상골 MR 또는 편평골 CT검사를 시행했던 간엽성육종 28례와 소원형세포성육종 26례를 대상으로 하였다. 관상골 MR검사 26례와 편평골CT검사 28례에서 골파괴 병소 소견을 각각 편심성과 중심성으로 나누어 비교 분석하였다. 결과: 관상골 MR검사에서 간엽성육종 16례중 12례(75.0%)가 편심성 골파괴 소견이였고 소원형세포육종 10례는 전례(100.0%)가 중심성 골파괴 소견이었다(p>.01). 편평골 CT검사에서 간엽성육종 12례중 10례(83.3%)에서 편심성 골파괴 소견이었고 소원형세포성육종 16례중 13례(81.3%)가 중심성 골파괴 소견을 보였다(p>.01). 결론: 관상골 MR검사든 편평골 CT검사든 골파괴 양상을 중심성과 편심성으로 나누는 방사선학적 소견은 간엽성육종과 소원형세포성육종을 감별 진단하는데 도움이 되는 소견이었다.

Camurati-Engelmann 병의 뼈스캔 소견 (Camurati-Engelmann's Disease on $^{99m}Tc$-MDP Bone Scan)

  • A 24 year-old female presented for a $^{99m}Tc$-methylene diphosphonatae (MDP) whole body bone scan due to chronic pain in the bilateral lower extremities that has aggravated since 2002. She was diagnosed with Camurati-Engelmann disease (CED) based on the clinical and radiological findings in 2002, and she re-visited our institute to evaluate disease status at this time. CED is a rare autosomal dominant type of bone dysplasia characterized by progressive cortical thickening of long bones, and narrowing of medullary cavity, and thus presents with typical clinical symptoms and signs such as chronic pain in the extremities, muscle weakness, and waddling gait. On the $^{99m}Tc$-MDP bone scan performed to evaluate disease status, intense increased uptake was seen in the skull, facial bones, bilateral scapulae, bilateral long bones, and bilateral pelvic bones, which clearly demonstrated the extent of CED involvement.

피부근염-다발성근염(皮膚筋炎-多發性筋炎)에서 골주사(骨走査)를 이용(利用)한 근염활동성(筋炎活動性)에 관(關)한 연구(硏究) (A Study on Clinical Activity of Myositis by the Use of Radioisotope Bone Scan in the Patients with Dermatomyositis-polymyositis)

  • To evaluate the diagnostic usefulness of radioisotope bone scan and clinical activity of myositis, bone scan using $^{99m}Tc-Methylene$ diphosphonate was serially done before and after treatment with prednisolone in 10 patients with well-documented dermatomyositis-polymyositis. The observed results were as follows. 1. In all 10 patients before treatment with prednisolone, the bone scans showed evidence of increased muscle uptake. Muscle uptake was markedly increased in 4 patients, moderately increased in 3 patients and minimally increased in 3 patients. 2. The site of increased muscle uptake was consistent with the site of clinically involved muscle which was weak and tender. 3. The degree of muscle uptake correlated with the severity of the muscle weakness and tenderness at the scan was done. In all 10 patients treated with high dose prednisolone, muscle uptake was decreased following therapy. Above results suggest the radioisotope bone scanning may be useful in the diagnosis and treatment of patient with dermatomyositis-polymyositis.
