치조골 높이가 다른 2개 임플란트 금관의 고정연결 조건에 따른 응력분석 (Stress Analysis on the Splinted Conditions of the Two Implant Crowns with the Different Vertical Bone Level)

  • The purpose of this study was to compare the stress distribution around the surrounding bone according to the splinted and non-splinted conditions on the finite element models of the two implant crowns with the different vertical bone level. The finite element model was designed with the parallel placement of the two fixtures ($4.0mm{\times}11.5mm$) with reverse buttress thread on the mandibular 1st and 2nd molars. As the bone quality, the inner cancellous bone and the outer 2 mm cortical bone were designed, and the cortical and cancellous bone were assumed to be perfectly bonded to the implant fixture. The splinted model(Model 1) had 2 mm contact surface and the non-splinted model(Model 2) had $8{\mu}m$ gap between two implant crowns. Two group (Splinted and non-splinted) was loaded with 200 N magnitude in the vertical and oblique directions on the loading point position on the central position of the crown, the 2 mm and 4 mm buccal offset point from the central position. Von Mises stress value was recorded and compared in the fixture-bone interface in the bucco-lingual and mesio-distal sections. The results were as follows; 1. In the vertical loading condition of central position, the stress was distributed on the cortical bone and the cancellous bone around the thread of the fixture in the splinted and non-splinted models. In the oblique loading condition, the stress was concentrated toward the cortical bone of the fixture neck, and the neck portion of 2nd molar in the non-splinted model was concentrated higher than that of 1st molar compared to the splinted model. 2. In the 2 mm buccal offset position of the vertical loading compared to the central vertical loading, stress pattern was shifted from apical third portion of the fixture to upper third portion of that. In the oblique loading condition, the stress was distributed over the fixture-bone interface. 3. In the 4 mm buccal offset position of the vertical loading, stress pattern was concentrated on the cortical bone around the buccal side of the fixture thread and shifted from apical third portion of the fixture to upper third portion of that in the splinted and non-splinted models. In the oblique loading, stresses pattern was distributed to the outer position of the neck portion of the fixture thread on the mesio-distal section in the splinted and non-splinted models. Above the results, it was concluded that the direction of loading condition was a key factor to effect the pattern and magnitude of stress over the surrounding bone of the fixture under the vertical and oblique loading conditions, although the type with or without proximal contact did not effect to the stress distribution.

  • Statement of problem: Standard type of ITI solid implant model in the 6.2mm thick jaw bone was axisymmetrically modelled for finite element stress analyses. Purpose: Primary objective was to investigate the influences or the characteristic design configuration of the ITI solid implant model on the bone stress with the course of osseointegration process at the bone/implant interfaces. To simulate the characteristics of the osseointegration process, five different stages of the bone/implant interface model were implemented. As load conditions, vertical load of 50N was taken into consideration. Bone at the cervical region of implant was the areas of concern where the higher level of stress were likely to take place. Results: The results indicated that rather slightly different stress level could be obtained as a function of the osseointegration conditions. Conclusion: Under vertical load, the lower level of stress was observed at the cervical cortical bone in the initial and final stages of osseointegration. Relatively higher stress level, however, was observed during the transitional stages where the osseointegration at the cancellous bone interface were yet to fully develop.

  • Clinically, proximal bone resorption in the femur is frequently seen postoperatively on the follow up X-rays after total hip replacement(THR). We developed the finite element model of cementless THR. The model is two dimensional side plate model, whereby the three dimensional structural integrity of the bone can be accounted for by a separate two dimensional mesh, a side plate. The subject of this article is the development and application of this two dimensional side plate FEM to study the reverse effect of the various degree of bone resorption of femur after THR. The results of this study indicates that two dimensional side plate model is good and simple alternative to complex three dimensional model and the severity of the proximal bone resorption has the effect of more increasing stress on the cortex at the level of femoral stem tip.

Stepwise verification of bone regeneration using recombinant human bone morphogenetic protein-2 in rat fibula model

  • Objectives: The purpose of this study was to introduce our three experiments on bone morphogenetic protein (BMP) and its carriers performed using the critical sized segmental defect (CSD) model in rat fibula and to investigate development of animal models and carriers for more effective bone regeneration. Materials and Methods: For the experiments, 14, 16, and 24 rats with CSDs on both fibulae were used in Experiments 1, 2, and 3, respectively. BMP-2 with absorbable collagen sponge (ACS) (Experiments 1 and 2), autoclaved autogenous bone (AAB) and fibrin glue (FG) (Experiment 3), and xenogenic bone (Experiment 2) were used in the experimental groups. Radiographic and histomorphological evaluations were performed during the follow-up period of each experiment. Results: Significant new bone formation was commonly observed in all experimental groups using BMP-2 compared to control and xenograft (porcine bone) groups. Although there was some difference based on BMP carrier, regenerated bone volume was typically reduced by remodeling after initially forming excessive bone. Conclusion: BMP-2 demonstrates excellent ability for bone regeneration because of its osteoinductivity, but efficacy can be significantly different depending on its delivery system. ACS and FG showed relatively good bone regeneration capacity, satisfying the essential conditions of localization and release-control when used as BMP carriers. AAB could not provide release-control as a BMP carrier, but its space-maintenance role was remarkable. Carriers and scaffolds that can provide sufficient support to the BMP/carrier complex are necessary for large bone defects, and AAB is thought to be able to act as an effective scaffold. The CSD model of rat fibula is simple and useful for initial estimate of bone regeneration by agents including BMPs.

Development of an experimental model for radiation-induced inhibition of cranial bone regeneration

  • Background: Radiation therapy is widely employed in the treatment of head and neck cancer. Adverse effects of therapeutic irradiation include delayed bone healing after dental extraction or impaired bone regeneration at the irradiated bony defect. Development of a reliable experimental model may be beneficial to study tissue regeneration in the irradiated field. The current study aimed to develop a relevant animal model of post-radiation cranial bone defect. Methods: A lead shielding block was designed for selective external irradiation of the mouse calvaria. Critical-size calvarial defect was created 2 weeks after the irradiation. The defect was filled with a collagen scaffold, with or without incorporation of bone morphogenetic protein 2 (BMP-2) (1 ㎍/ml). The non-irradiated mice treated with or without BMP-2-included scaffold served as control. Four weeks after the surgery, the specimens were harvested and the degree of bone formation was evaluated by histological and radiographical examinations. Results: BMP-2-treated scaffold yielded significant bone regeneration in the mice calvarial defects. However, a single fraction of external irradiation was observed to eliminate the bone regeneration capacity of the BMP-2-incorporated scaffold without influencing the survival of the animals. Conclusion: The current study established an efficient model for post-radiation cranial bone regeneration and can be applied for evaluating the robust bone formation system using various chemokines or agents in unfavorable, demanding radiation-related bone defect models.

임프란트와 지대주 간 내측 연결을 갖는 2종의 임프란트에서 저작압이 임프란트 주위골 내응력 분포에 미치는 영향에 관한 연구 (Study on the stress distribution around two types of implants with an internal connection by finite element analysis)

  • Adequate bone quality and stress distribution to the bone are of decisive importance for implant success. Even though the success rates of dental implants have been high, implant failures do occur. Overloading has been identified as a primary factor behind dental implant failure. The purpose of this study was to theoretically investigate the effect of two types of implants on the stress distribution in poor bone quality. Employing the finite element method, the study modeled a 4.1 mm diameter, 12.0 mm length implant placed in cortical or spongeous bone. A static loading of lOON was applied at the occlusal surface at 0, 30 degrees angle to the vertical axis of the implant. von Mises stresses concentrations in the supporting bone were analyzed with finite element analysis program. The results were as follows; 1. The stresses at the marginal bone were higher under buccal oblique load(30 degrees off of the long axis) than under vertical load. 2. Under buccal oblique load, the stresses were higher at the lingual marginal bone than at the buccal marginal bone, and the differences were almost the same. 3, Under vertical and oblique load, the stress was the highest at the marginal bone and lowest at the bone around apical portions of implant in cortical bone. 4, Under vertical load, Model 1 showed more effective stress distribution than Model 2 irrespective of bone types. On the other hand, Model 2 showed lower stress concentration than Model 1 under buccal oblique load.

  • Bone shaving for surgical correction is general method in facial asymmetrical patient with fibrous dysplasia. Therefore, decision of bone shaving amount on the preoperative planning is very difficult for improvement of ideal occlusal relationship and harmonious face. Preoperative planning of facial asymmetry with fibrous dysplasia is generally confirmed by the simulation surgery based on evaluation of clinical examination, radiographic analysis and analysis of facial study model. However, the accurate postoperative results can not be predicted by this method. By using the computed tomography based RP(rapid prototyping) model, simulation of facial skeleton can be duplicated and 3-dimensional simmulation surgery can be perfomed. After fabrication of postoperative study model by preoperactive bone shaving, preoperative and postoperactive surgical index was made by omnivaccum and clear acrylic resin. Amount of bone shaving is confirmed by superimposition of surgical index at the operation. We performed the surgical correction of facial asymmetry patients with fibrous dysplasia using surgical index and prototyping model and obtained the favorable results.

개의 비유합(非癒合) 골절(骨折) model에 있어서 동결건조골이식(凍結乾燥骨移植)의 효과(效果) (Effectiveness of freeze-dried bone grafts on the non-union fracture model of dogs)

  • To investigate the effectiveness of the freeze-dried allografts and fibrin glue in bone grafts, the status of new bone formation and union of the grafted bone were observed in three types of grafting bones; autogenic bone(AT), allogenic bone(AL), and allogenic bone particles mixed with fibrin glue(FG). These were transplanted into non-union fracture model of 7 adult dogs with 2cm defect made in the proximal metaphysis of both fibulae. The autogenic and allogenic grafting bones had been treated by a modified freeze-dried method. The serial radiogram were observed the repair process of grafted bones biweekly until 17 or 21 weeks after transplantation and the observation of histological aspects, tetracycline double labeling and microradiography in the grafted bones were undertaken at 17 or 21 weeks after transplantation. The incorporation of bone minerals to the non-union fracture models were accomplished in 4 of 5 cases grafted with AL and in 2 of 4 cases grafted with AT. None of 5 cases grafted with FG were incorporated. The process of new bone formation and resorption in the grafted bones were observed three types; resorption of the grafted bones after newbone formation(type A) in 4 cases, new bone formation after resorption(type B) in 2 cases and complete or incomplete resorption without new bone formation(type C) in 8 cases. The modified freeze-dried method used in this study contributed to inhibite the rejection in allogenic grafts but the union period of the grafted freeze-dried bone was more prolonged than that of fresh autografts. Fibrin glue did not contribute to induce a new bone formation ofbone grafts.

정규화된 형상 모델을 이용한 뼈 나이 측정 방법 (A Bone Age Assessment Method Based on Normalized Shape Model)

  • 뼈 나이 측정은 소아의 내분비계 관련 질병 진단을 위해 소아과에서 널리 사용되는 방법이다. 그러나 전문 인력이 부족하여 자동화된 측정 방법에 대한 꾸준한 요구가 있었다. 따라서 본 논문에서는 패턴 인식기법을 이용한 자동화된 뼈 나이 측정 알고리즘을 제안한다. 제안하는 알고리즘은 X-ray 영상에서 손가락뼈의 각 부분을 자동으로 분류하는 과정과 분류된 뼈 영상으로부터 정규화된 형상 모델을 추출하는 과정, 그리고 정규화된 형상 모델로부터 뼈 나이를 측정하는 과정으로 구성된다. 제안하는 알고리즘은 능동 형상 모델(Active Shape Model: ASM)을 이용하여 나이 측정에 사용되는 특정값 추출의 정확도를 향상시켰으며, 뼈 나이 분류를 위해 사용된 Support Vector Machine(SVM)의 입력으로 정규화된 형상 모델로부터 얻어진 각 뼈의 크기와 비율을 특징값으로 사용하였다. 성능 평가를 위해서 한양대학교 부속병원에서 제공한 영상에 대해 전문가가 평가한 나이와 제안한 알고리즘을 이용하여 측정된 나이를 통계적으로 비교 분석하였다. 실험을 통하여 본 논문에서 제안한 특징값과 알고리즘으로 뼈 나이를 진단한 결과, 전문가에 의한 결과와 평균 0.679살의 오차 이내의 뛰어난 뼈 나이 측정 성능을 보였다.

치과용 임플란트 지대주나사의 조임체결력에 따른 지지골과 지대주나사의 유한요소법 응력 분석 (A finite element stress analysis on the supporting bone and abutment screw by tightening torque of dental implant abutment screw)

  • Purpose: A study analysed the stress distribution of abutment screw and supporting bone of fixture by the tightening torque force of the abutment screw within clinical treatment situation for the stability of the dental implant prosthesis. Methods: The finite element analysis was targeted to the mandibular molar crown model, and the implant was internal type 4.0 mm diameter, 10.0 mm length fixture and abutment screw and supporting bone. The occlusal surface was modeled in 4 cusps and loaded 100 N to the buccal cusps. The connection between the abutment and the fixture was achieved by combining three abutment tightening torque forces of 20, 25, and 30 Ncm. Results: The results showed that the maximum stress value of the supporting bone was found in the buccal cortical bone region of the fixture in all models. The von Mises stress value of each model showed 184.5 MPa at the 20 Ncm model, 195.3 MPa in the 25 Ncm model, and 216.5 MPa in the 30 Ncm model. The contact stress between the abutment and the abutment screw showed the stress value in the 20 Ncm model was 201.2 MPa, and the 245.5 MPa in the 25 Ncm model and 314.0 MPa in the 30 Ncm model. Conclusion: The increase of tightening force within the clinical range of the abutment screw of the implant dental prosthesis was found to have no problem with the stability of the supporting bone and the abutment screw.
