Comparison of the Values of Bone Mineral Density Between DEXAs (DEXA 측정기 간 골밀도 값 비교)

  • Statistical analysis was performed on the patients who took bone densitometry using Lunar and Hologic equipments for 5 months from January $1^{st}$ 2010 to May $30^{st}$, 2010. Patients consisted of 50 in their 40s, 100 in 50s and 50 in 60s. In addition, the results from bone densitometry were carried out on the same subject with two equipments on the same day. In case of 200 subjects, who received the bone densitometry with two equipments, the average age was 54.5 and 54.4 years old, respectively. There was no difference. The T- score of Lunar equipment was $-1.377{\pm}1.221$ and that of Hologic equipment was $-1.806{\pm}1.123$. The T-score of Lunar equipment was measured higher than that of Hologic equipment. T-test was conducted to determine the equality of the mean of two groups with 200 patients. Since the pvalue was 0.000, the value of bone mineral density was significant in two equipments. Furthermore, the patients, who were diagnosed by Lunar, showed more 'normal' and who were diagnosed by Hologic, showed more osteoporosis, which cartegorized by WHO, such as normal, osteopenia, and osteoporosis. Thus, compared results of bone densitometry on lumbar spine L1 - L4 of four normal people with the same equipment showed that T- score of Lunar equipment was $-0.4{\pm}1.2$, and T- score of Hologic equipment was $-1.1{\pm}1.5$. It showed the higher T- score was measured in Lunar equipment as well. Therefore, the correction factor should be considered to use, since T- scores are different between two equipments.

Age-Related Bone Mineral Density, Accumulated Bone Loss Rate at Multiple Skeletal Sites in Korean Men (한국 남성의 연령에 따른 부위별 골밀도 변화 및 골 소실률)

  • This study examined the age-related bone mineral density (BMD), accumulated bone loss rate (ABLR) at different skeletal sites in Korean men using the data from the Korea National Health and Nutrition Examination Survey (KNHANES)(the 1st (2010), 2nd (2010) and the year at the 5th survey). The cubic regression model was found to be the best for describing the age-related changes in BMD. The lumbar spine, total hip, femoral neck, trochanter, Ward's triangle in the bone mineral density difference were analyzed by ANOVA. The peak BMD was at 20-24 years at the lumbar spine, total hip, femoral neck, trochanter, Ward's triangle, and the 75-79 years of age group had the highest Accumulated Bone Loss Rate. Therefore, intensive management will be necessary for men over 75 years, and a diagnosis of osteoporosis in Korean men should be made according to The International Society for Clinical Densitometry; ISCD.

The Relationship between Lifetime Sports Activity Measured with MET and Peak Strain Score and Bone Measurement in College-aged Women (대사당량(MET)과 최대긴장력(Peak Strain Score)에 근거하여 측정한 스포츠 활동량과 여대생의 요골 골밀도와의 상관성)

  • Purpose: The aim of this study was to compare the relation between differently measured sports activities (metabolic equivalent [MET] and peak strain score) and distal radius bone mineral density in college-aged women. Methods: lifetime sports activity was scored in two different ways: 1) a sports activity score by multiplying the intensity (METs) and duration and 2) a sports activity score by adding up physical strain scores based on the ground reaction force of each sports activities. Bone mineral density was measured using dual energy x-ray densitometry (DTX-200) in the distal radius site. Results: In stepwise multiple regression analysis, body weight and sports activities during the college period were significant positive predictors for distal radius bone mineral density. The explained variance of sports activity measured with a peak strain score (8.8%) for distal radius bone mineral density was higher than one measured with the MET score (3.3%). Conclusion: It can be concluded that sports activity scores based on MET and peak strain scores during college are very important for determining the bone mineral density in the distal radius site in women under 30.

Bone Mineral Density and Factors influencing Bone Mineral Density in College Women (일 대학 여대생의 골밀도와 골밀도에 영향을 주는 요인)

  • Purpose: The purpose of this study was to identify the levels of bone mineral density (BMD) and to explain the factors influencing BMD among female college students in Korea. Methods: A cross-sectional study was conducted with 144 college women. Lifestyle factors were determined by self-report questionnaire. Body composition was measured by body composition analyzer and bone mineral density was measured by ultrasound bone densitometry. Data were analyzed using SPSS for windows, version 19.0. Results: The mean BMD at calcaneus site was 0.56$g/cm^2$ (mean T-score=-0.22). The incidence of osteopenia was 21.5%. Factors predicting BMD were menarche age (r=-.22, p=.009) and height (r=-.18, p=.030) with 7% of explained variance. Conclusion: These results suggest that health care professionals need to provide young women with program that is intended to affect their intention toward osteoporosis preventive behavior change.

  • Irradiation is widely used for the treatment of malignant diseases, and possibly cause the osteoporosis. The densitometry and bone scintigraphy are valuable when used to monitor the patients longitudinally to access the progression of osteoporosis and risk of osteoradionecrosis. To evaluate the osteoporosis after irradiation of Cobalt-60 gamma ray on the lumbar spines of New Zealand white rabbits, bone densitometry by dual photon absorptiometry and bone scintigraphy were performed weekly. The decrease of bone density began at the first week after irradiation, and were in the nadir at 4-6th week. The osteoblastic activity measured by bone scintigraphy decreased in the first week, and was in the nadir at 4-6th week. The severity of these changes were related to the radiation dose. In conclusion, the osteoporosis before the presentation of the osteoradionecrosis can be diagnosed early with the dual photon absorptionmetry and bone scintigraphy.

Automatic Detection Method of the Region of Interest in the Measurement of Bone Mineral Density by Ultrasound Imaging (초음파 영상에 의한 골밀도 측정에서 관심영역의 자동 검출방법)

  • In ultrasonic bone densitometry, the positioning of measurement site is decisive in precision and reproducibility. In this study, automatic Region of Interest (ROI) detection algorithm is suggested and adopted the method using the local minimum value by ultrasonic image. The preprocess before the local minimum method extracts out the bone area and calculates the geometrical information of bone. The developed ROI detection algorithm was applied to the clinical test for the subject of 305 female patients in the range of 22-88 years old. As the results, the accuracy of the algorithm was shown to be 98.3%. It was also found that bone density parameter was significantly correlated with age(r=0.85, p<0.0001).

  • Thanks to recent development of science and technology, economic standard has been improved and life expectation has been gradually increased. Accordingly, disease aspect and demand for health have been changed in the concept from conventional center of disease to health improvement As the health issue in middle and senior age has also been vital in the national health, osteoporosis, one of the physical maladies occuring in these age group, is on the rise as a matter of important concern so that the researches on the osteoporosis have being accomplished actively in several medical domains. Research has been earned out for 735 examined objects, who have received medical examination of bone mineral density in the volunteer medical service of Daegu and Kyungbuk province for recent 4 years. The result indicated that serious osteoporosis prevailed over osteoporosis for male while osteopenia was majority to osteoporosis and that abnormal bone mineral density was more prominent than normal bone mineral density for both male and female. This study is to present basic data which is indispensible to preparation of senior citizen dominated society afterward, investigating the prevalence rate of osteoporosis.

Influence of Lifetime Sports Activity Based on a Ground Reaction Force on Bone Mineral Density in Korean Adults (지면반력에 근거한 과거 신체활동정도가 골밀도에 미치는 영향 -아이오와에 거주하는 한인을 대상으로-)

  • Purpose: The purpose of this study was to -retrospectively describe the childhood sports activity level of Korean adult men and women and to determine whether a higher level of childhood sports activity was positively associated with adult bone mineral density. Methods: A cross-sectional study of 100 Korean men (n=40) and women (n=60) was completed. Participants completed a detailed lifetime sports activity questionnaire and had their bone mineral density of the femur and lumbar spine measured using dual energy x-ray densitometry (DEXA). All sports activities were classified into four categories of peak strain score on the basis of ground reaction forces (GRF). Results: During the age of high school, women and men who participated in a high intensity sports activity demonstrated higher bone density in the femur site after adjustment for the effects of body weight, fat body mass, lean body mass, the level of calcium intake, and breast feeding period than those who did not participate in sports activity at all. Conclusion: These results highlight the need to participate in high intensity sports activity during high school age as a means of increasing peak bone mass in the femur site.

Study on enhancing the ultrasonic image for bone densitometry (골밀도 측정을 위한 초음파 영상 개선에 관한 연구)

  • It is very important to obtain a high quality of bone image for an accurate ultrasonic measurement of bone mineral density. In this study, we suggested a technique to acquire an optimal image by adapting an acoustic lens and a properly selected ultrasonic probe. Also, we have applied an image processing algorithm with which automatically makes a decision of brightness and contrast of image by generating threshold level, a composition of ultrasonic data, an elimination of noise using modified median filter, and a real time interpolation. We could confirm much improved resolution of bone image with acoustic lens attached to the ultrasonic probe and with the image processing algorithm suggested in this study. Therefore, it became possible to precisely diagnose the osteoprosis using ultrasonic imaging technique.

Application of an Energy Sensitive CZT Detector to a DXA Type of Bone Densitometer

  • The accuracy of DXA(Dual Energy X-ray Absorptiometry) highly depends on the detection and separation capability of dual energy X-ray X-ray photons. In addition both of scan time and patient exposure are affected by detection efficiency. A CZT detector with a good energy resolution and high detection efficiency was evaluated for the application of bone densitometry. Its performance was compared to a photomultiplier tube with a NaI(T1) scintillator in terms of energy resolution, detection efficiency and the accuracy of bone mineral density measurement. The comparison study was performed with CZT detector and PM tube using DXA equipments(OSTEO Plus, OSTEO Prima, ISOL Technology). The energy spectrum was acquired using MCA(Multi-Channel Analyzer). The used X-ray energy ranged from 20keV to 86keV. The MCA result of the CZT detector showed a slightly sharper energy spectrum than that of NaI(T1). Detection efficiency of the CZT detector at 59.5keV was 1.4 times better. Remarkably the final results of bone mineral density measurements demonstrate only less than 1% difference. The CZT detector appears to have many benefits for the application of bone densitometry. Its excellent energy resolution can enhance the counting accuracy of dual energy X-ray spectrum. Furthermore its compactness in physical dimension and no cooling requirement will be additional benefits for a more compact and accurate bone densitometer.
