The purpose of this study was to assess the 24-hour shear bond strength of amalgam to glass ionomer cement, using five different intermediaries. The intermediaries used in this study were Scotchbond 2 (light curing dentin adhesive), Panavia (resin cement), liquid' of glass ionomer cement (chemical curing & light curing), and uncured mixture of light curing glass ionomer cement. This study was operated with 48 specimens devided into 6 groups. The experimental groups are as follows: Group 1 : Bonded Amalgam to chemical curing glass ionomer cement with liquid of chemical curing glass ionomer. Group 2 : Bonded Amalgam to light curing glass ionomer cement with liquid of chemical curing glass ionomer. Group 3: Bonded Amalgam to light curing glass ionomer cement with resin cement. Group 4: Bonded Amalgam to light curing glass ionomer cement with light curing dentin adhesive. Group 5: Bonded Amdlgam to light curing glass ionomer cement with liquid of light curing glass ionomer. Group 6: Bonded Amalgam to light curing glass ionomer cement with uncured mixture of light curing glass ionomer cement. 30 minutes after amalgam condensation, all specimens were stored for 24 hours in water at $37^{\circ}C$ and tested with Instron (1122). The following results obtained: 1. The shear bond strength of group 6 was higher than those of the other groups (46.7 kgf/$cm^2$, p<0.05). 2. The shear bond strength of resin cement intermediary group was lower than that of the group using uncured mixture of light curing glass ionomer cement. 3. The results of group 1 and group 2 were different, even though the inter-me diaries used were same. 4. Intermediary of Group 5 did not show complete set in Scanning Electromicroscopic examination. 5. Light-curing dentin adhesive did not show any bonding ability to amalgam.
The purpose of this study was to observe the tensile and bonding strength of the joined amalgam restoration. Amalgam alloys of fine-cut (F-type), spherical (S-type), and dispersed type (D-type) were selected in this study, and all specimens were divided into three groups according to the condensation methods as follows. Group I : the control group which condense the same kinds of mixed amalgam into the whole part of the mold respectively. Group II : the group which condense a mix of amalgam into one half of the mold, and then condense a new mix of amalgam into the rest half of the mold 15 minutes later. Group III : the group which condense a mixed amalgam into one half of the mold, and then condense a new mix of amalgam into the rest half of the mold 7 days later. All specimens were stored in incubator at $37{\pm}1^{\circ}C$ for seven days with immersing in saline solution before testing. The tensile and bonding strength of them were measured with Instron Universal Testing machine. The results were as follows: 1. In Group I, the order of tensile strength was F-type, S-type, and D-type. 2. In case of bonding of S-type + S-type, the difference of the bonding strength between Group II and III was not significant. (P> 0.05) 3. The bonding strength of F-type + S-type of Group II was marked the highest in value, and the lowest bonding strength was showed in bonded D-type + D-type of Group III. 4. In case of bonding with the different kinds of amalgam alloy in Group II, the specimen bonded to F-type was marked the highest bonding strength, and the specimen bonded with F-type was marked the lowest one. In Group II, the bonding strength of the specimens bonded with the same kinds of amalgam alloy was presented as the same order as that of Group I. 5. In Group III, the specimen connected with D-type marked the lowest bonding strength of all specimens. In Group III, the bonding strength of the specimens connected with the same kinds of amalgam alloy was the order of S-type + S-type, F-type + F-type, and D-type + D-type.
본 연구는 아말감 면에 광중합형 레진으로 교정용 브라켓을 접착시킬 경우, 광원의 차이(할로겐 광중합기와 light-emitting diode (LED) 광중합기)와 샌드블라스팅 표면처리 여부에 따른 접착제의 전단결합강도를 비교하고자 시행되었다. 발거된 소구치 30개를 대조군으로 이용하였으며 법랑질 표면을 산부식한 후 통상적인 방법으로 브라켓을 접착하였다. 60개의 다른 소구치에 아말감 충전을 하여 실험군으로 이용하였다. 두 군에서 할로겐 광중합기와 LED 광중합기를 이용하여 브라켓을 접착시키고 브라켓이 탈락될 때까지 힘을 가해 전단결합강도를 측정하였다. 실험 결과, 실험군의 전단결합강도는 약 3-5.5 MPa로 대조군(19MPa)보다 낮았다. 실험군에서 샌드블라스팅 표면처리를 한 경우, 할로겐 광중합기를 사용한 군이 LED 광중합기를 사용한 군보다 높은 전단결합강도를 보였으나 (P<0.05), 샌드블라스팅 표면처리를 하지 않은 경우에는 광원에 따라 전단결합강도에 차이를 보이지 않았다 (p > 0.05). 할로겐 광중합 군과 LED 광중합 군 모두에서 샌드블라스팅 여부에 따른 전단결합강도에 통계적으로 유의한 차이가 없었다 (p>005). 아말감 면에 광중합형 레진을 이용하여 브라켓을 접착할 경우 할로겐 광중합기와 LED 광중합기로 얻을 수 있는 접착제의 결합강도는 임상적으로 사용하기에는 낮게 나타나, 이의 증진 방법을 도모하기 위해 앞으로 더 많은 연구가 필요하리라 사료된다.
Composite resin and glass-ionomer cement can be used for the purpose of repair of defective amalgam restoration. The purpose of this study was to evaluate of shear bond strength of esthetic restorative materials to dental amalgam. The materials used in this study were Photo Clearfil Bright(light curing composite resin), Clearfil F II(chemical curing composite resin), Fuji II LC(light curing glass-ionomer cement), Fuji II (chemical curing glass-ionomer cement), All-Bond 2(intermediary), and Scotchbond Multi-Purpose (intermediary). A total of 120 acrylic cylinders with amalgam were divided into 8 groups After amalgam condensation, all specimens were stored for 48 hours in water at $37^{\circ}C$ and tested with Instron universal testing machine between amalgam and composite resins and glass-ionomer cements. The data were analyzes statiscally by ANOVA and Duncan test. The following results obtained ; 1. The shear bond strength of bonded composite resin to amalgam was higher than bonded glass-ionomer cement(P<.001). 2. The group 4 had highest shear bond strength with 15.45kgf/$cm^2$ and the group 5 had lowest shear bond strenght with 3.26kgf/$cm^2$(P<.001). 3. In the group 3, 4, 5, 6, the group 3, 4 with All-Bond 2 had higher shear bond strength than the group 5, 6 with Scotch bond MP both in light-curing and in chemical curing. 4. Both in composite resin and glass-ionomer cement, chemical curing materials had higher shear bond stength than light curing materials(P<.001).
많은 치과 임상가들은 아말감을 치질에 접착시키기 위해서 여러 접착재료를 이용해왔다. 이 런 술식의 장점으로는 미세누출의 감소로 인한 술후과민증의 감소, 변연파절의 감소, 삭제 후 치질의 강화, 적은 유지형태에서의 수복물의 유지등이 제시되어 왔다. 실제로 임상적 상황에서 이러한 술식의 장점을 확인할 필요가 있다. 이런 접착제가 임상적 환경에서 어떠한 잇점을 보이지 않는다면 시간과 비용을 낭비하는 이런 술식을 정당화하지 못할 것이다. 본 연구에서는 73개의 치아를 대상으로 하여 I급 와동과 II급 와동에 접착형과 비접착형 아말감으로 수복하였다. 접착제로는 Fuji I Glass Ionomer luting cement를 이용하였다. 평균 2년 후에 변연적합성을 평가하였으나 접착형 아말감과 비접착형 아말감 사이에 유의한 차이는 없었다.
The purpose of this study was to evaluate the effect on treatment methods to shear bond strength between composite resin and amalgam when the alloy surface was finished with a diamond wheel or an sandblaster. Forty round acrylic cylinders were fabricated with a diameter of 33mm and a height of 20mm to fit into the device used during shear bond strength testing. A round undercut cavity (diameter, 8mm: depth, 2.5mm) was prepared in the center of the acrylic surface and the cavity was restored using a amalgam. A total of 40 acrylic cylinders with amalgam were divided into 4 groups according to treatment method. The group treatment were as follows : Group 1 : acid etching after finishing the amalgam with diamond wheel Group 2 : no acid etching after finishing the amalgam with diamond wheel Group 3 : acid etching after sandblasting the amalgam Group 4 : no acid etching after sandblasting the amalgam The shear bond strength of composite resin bonded to amalgam of each specimen was tested with a universal testing machine at a crosshead speed of 0.5mm/min and 500kg in full scale. The results were as follow: 1. After diamond finishing, the non-acid etching group had highest shear bond strength with 7.29kg/$cm^2$ and after sandblasting, the acidetching group had lowest shear bond strength with 4.49kg/$cm^2$. 2. In both diamond finishing and sandblasting group, acid etching of the roughened amalgam surface decreased the shear bond strength. 3. The group treated with a diamond wheel had higher shear bond strength those treated with an sandblaster but there was not significanat.
소아치과 임상에서 자주 사용되는 3 종의 광중합형 GIC의 아말감에 대한 접착능을 평가 할 목적으로 60 개의 아말감 부착시편을 이용하여 중간결합제인 Scotchbond 의 사용여부에 따른 경화된 아말감에 대한 광중합 GIC의 전단결합강도를 측정하고 경계부의 파절양상을 관찰한 결과, 다음과 같은 결론을 얻었다. 1. 아말감에 대한 광중합형 GIC의 전단강도는 Fuji II LC, Vitremer, Vitrebond의 순으로 높게 나타났다 (p<0.05). 2. 중간결합제인 Scotchbond를 사용하지 않은 경우에서 Scotchbond 를 사용한 경우에 비해 전단결합강도가 높게 나타났다(p<0.05). 3. 결합파절면은 Scotchbond를 사용한 경우의 대부분에서 Scotchbond와 아말감의 경계부에서 시편의 탈락이 나타났다. 아말감과 광중합 GIC의 결합을 시도할 경우에는 Scotchbond는 사용하지 않는 것이 바람직할 것으로 사료되었다.
아말감의 단점인 건전 치질 삭제와 미세 누출을 줄이기 위한 접착 아말감(bonded amalgam)에 사용되는 치과용 접착제의 접착 강도를 측정하고 파절면을 관찰하여 콤포지트 레진과 비교 평가하였다. 발거한 우전치의 순면을 직경 8mm의 원형으로 천공하여 얻은 부분을 순면이 바깥쪽으로 향하도록 아크릴릭 레진 블록에 매몰하여 상아질과 법랑질군을 각각 55개씩 준비하였다. 1군:Superbond C&B, 2군:Panavia 21, 3군:All-Bond 2, 4군:Fuji I Glass Ionomer Luting Cement, 5군:Scotchbond Multi-Purpose(Restorative Z-100)의 5개 군으로 나누어 지름 6.3mm, 깊이 1.5mm의 구멍이 뚫린 충전용 틀(mold)을 위치시키고 표면을 접착제로 처리한 후 아말감 충전을 하였다. 5군은 레진 충전 후 40초간 광중합하였다. 각 시편은 아말감 충전 24시간 후 접착 강도 측정용 기구에 장착하고 만능 시험기에서 0.2mm/min의 cross-head 속도로 하중을 가하여 시편이 탈락할 때까지의 하중값(kg)을 측정하고, 파절면을 검사하여 다음과 같은 결론을 얻었다. 1. 상아질군에서 1, 2, 4군은 5군에 비하여 통계학적으로 유의성 있게 낮은 접착 강도를 보였다(P<0.05). 2. 법랑질군에서 4군은 5군에 비하여 통계학적으로 유의성 있게 낮은 접착 강도를 보였다(P<0.05). 3. 2군에서는 상아질군에서 법랑질군에 비하여 통계학적으로 유의성 있게 낮은 접착 강도를 보였으나(P<0.05), 다른 접착제에서는 상아질군과 법랑질군에서의 접착 강도가 유의성 있는 차이를 보이지 않았다(P>0.05). 4. Cohesive failure는 1E군과 5D군에서 관찰되었고, mixed failure는 1, 5군에서 관찰되었으며, 2, 3, 4군에서는 adhesive failure만 관찰되었다.
이 연구의 목적은 이중 중합복합레진과 수평 포스트를 이용하여 수복한 치관-치근 복합파절 치아의 수직파절 저항성을 알아보기 위한 것이다. 48개의 발치된 사람의 소구치에 실험적으로 치관-치근 복합파절을 일으킨 후 대조군 및 3개의 실험군에 (n=12) 각각 할당하였다. 대조군에서는 치아 파절편을 레진시멘트를 이용하여 접착한 후 근관치료를 시행하고, 근관와동을 이중중합 복합레진을 사용하여 충전하였다. 실험군에서는, 각각의 치아에 근관치료를 시행한 후 레진-포스트 군에서는 근관와동을 수평 포스트 및 이중중합 복합레진을사용하여 충전하였다. 레진 군에서는 근관와동을 포스트 없이 이중중합 복합레진만 사용하여 수복하였으며 아말감 코어 군에서는 근관와동을 접착 아말감을 사용하여 수복하였다. 이렇게 수복된 치아를 처음과 같은 방법으로 다시 치관-치근 복합파절을 유도하여 이때의 파절 저항성을 기록하고 이 값과 처음 파절을 유도하였을 때의 파절 저항성의 비를 계산하고 각 군별로 차이가 있는 지 검증하기 위해 Kruskal-Wallis test를 시행하였다. 수복된 치아의 파절저항성은 레진-포스트군, 대조군, 레진군, 접착 아말감 순으로 높았으며, 이 실험결과로 볼 때,치관-치근 복합파절 치아의 수복 시 수평 포스트를 사용하는 것이 재파절 방지를 위해 효과적 일 것으로 사료된다.
균열치는 저작운동과 온도자극에 대한 치아의 통증을 호소하는 환자에서 자주 발견되는 질환으로 균열의 진행정도에 따라 다양하게 분류할 수 있다. 가장 흔한 원인으로는 저작과정 중의 갑작스러운 사고로 알려져 있지만 대부분 증령, 기존 수복물 또는 의원성 응력집중과 같은 기여요인이 있을 때 발생한다. 균열의 진행정도 및 치수침범에 대한 정확한 진단이 필수적이며 저작시 통증을 확인한 후 균열을 가급적 눈으로 확인하는 것이 좋다. 이를 위해 투과광이나 염색과 같은 보조적 방법을 이용할 수 있다. 치료는 즉각적인 통증완화를 위한 교합조정이나 파절편 고정이 필요하며 치수의 상태에 따라 근관치료가 추가적으로 필요할 수도 있다. 궁극적인 치료로는 복합레진이나 접착 아말감으로 균열부위에 가해지는 충격을 완화시킨 후 주조수복물을 이용한 교두보호가 필요하다. 이 논문에서는 균열치에 대한 다양한 원인, 치료, 예후 등에 관한 문헌을 고찰할 것이다.
