The Analysis of Patterns and Risk Factors of Newly Developed Vertebral Compression Fractures after Percutaneous Vertebroplasty

  • Objective : The purpose of this study was to investigate the patterns and the risk factors of newly developed vertebral compression fractures (VCFs) after percutaneous vertebroplasty (PVP). Methods : We performed a retrospective review of the 244 patients treated with PVP from September 2006 to February 2011. Among these patients, we selected 49 patients with newly developed VCFs following PVP as the new VCFs group, and the remaining 195 patients as the no VCFs group. The new VCFs group was further divided into 2 groups : an adjacent fractures group and a nonadjacent fractures group. The following data were collected from the groups : age, gender, body weight/height, body mass index (BMI), bone mineral density (BMD) score of the spine and femur, level of initial fracture, restoration rate of anterior/middle vertebral height, and intradiscal cement leakage, volume of polymethylmethacrylate (PMMA). Results : Age, gender, mean body height/weight, mean BMI and volume of PMMA of each of the group are not statistically significantly associated with fractures. In comparison between the new VCFs group and the no VCFs group, lower BMD, intradiscal cement leakage and anterior vertebral height restoration were the significant predictive factors of the fracture. In addition, new VCFs occurrence at the adjacent spines was statistically significant, when the initial fracture levels were confined to the thoracolumbar junction, among the subgroups of new VCFs. Conclusion : Lower spinal BMD, the greater anterior vertebral height restoration rate and intradiscal cement leakage were confirmed as risk factors for newly formed VCFs after PVP.

성인의 신체조건 및 생활습관에 따른 골밀도 변화에 대한 연구 (A Study on the Change of Bone Mineral Density(BMD) by Life Habit and Physical Condition)

  • 목 적: 성인들의 생활습관인 음주, 운동, 흡연 등의 유무와 골밀도와의 관계를 분석 해보고 연령, 성별, 신장, 체중, 체질량지수 등과 골밀도와의 상관관계를 알아보고자 한다. 대상 및 방법: 2006년 2월부터 4월까지 건강검진을 받기위해 우석대학교 한방병원을 내원한 321명을 대상으로 양 에너지 X-ray 골밀도 측정기(DEXA, DPX-IQ, Lunar. co)를 이용하여 대퇴경부 및 척추의 골밀도를 측정하였다. 결 과: 대상자중 남성은 160명 여성은 161명이었고 평균연령은 $45.10{\pm}11.54$세였다. 남성에 있어서 대퇴 경부의 골밀도는 20대에서 가장 높았고 척추는 40대에서 가장 높았으며 60대에서 대퇴경부와 척추의 골밀도가 가장 낮았다, 여성에서는 40대에서 대퇴경부와 척추의 골밀도가 가장 높았고 60대에서 대퇴경부와 척추의 골밀도가 가장 낮았다. 대상자들의 신체조건에서 신장과 체중이 클수록 골밀도가 높았다. 남성은 음주 및 운동, 흡연유무에 따른 집단별 비교에서 통계적으로 유의한 차이를 보이지 않았으나 여성은 음주집단이 비 음주집단에 비해 대퇴경부 및 척추의 골밀도가 유의하게 높았다(P<0.01). 결 론: 이는 건강한 직장인들이 건강검진을 받고자 내원한 사람들이 대부분이기에 위와 같은 결과가 나온 것으로 생각되며 앞으로 모든 성인들의 연령증가에 따른 골다공증의 위험으로부터 예방하기 위하여 골밀도에 영향을 미치는 다양한 요인들을 찾아서 많은 연구를 하고자 한다.

골다공증성 척추 압박 골절 환자에서 다열근과 척추기립근의 지방 침투율과 골다공증 관련 인자의 상관 관계 분석 (Correlation Analysis between the Factors Associated with Osteoporosis and the Fat Infiltration Rate of the Multifidus and Erector Spinae Muscles in Osteoporotic Vertebral Compression Fracture Patients)

  • 목적: 근감소증에 중요한 요소인 척추 주변 근육 감소를 수치화한 총 지방 침윤율(total fat infiltration rate, TFI rate)과 골다공증성 척추 압박 골절(osteoporotic vertebral compression fracture) 및 관련 수치와의 상관 관계를 찾는 것을 목표로 하였다. 대상 및 방법: 2012년 1월부터 2016년 12월까지 요추의 압박 골절 진료를 본 환자들 중 1) 골밀도(bone mineral density, BMD) T score -2.5 g/cm2 미만으로 골다공증을 진단받고, 2) 요추 골절로 경피적 척추성형술(vertebroplasty) 또는 경피적 풍선척추성형술(kyphoplasty)을 받았으며, 3) 요추의 한 분절만 골절된 4) 1년 이상 추시한 환자 98명을 대상으로 후향적 연구를 시행하였다. TFI는 Image J 프로그램으로 자기공명영상을 분석하여 확인하였다. 이를 바탕으로 다열근(multifidus) 및 척추기립근(erector spinae)의 TFI와 골다공증 요소들과의 연관성을 분석하였다. 결과: 다열근및 척추기립근의 평균 TFI는 14.66±10.16이었다. 척추 BMD는 고관절 BMD와 통계적으로 유의한 양의 상관 관계를 보였으나 TFI와 음의 상관 관계를 보였다. 고관절 BMD는 체질량지수(body mass index)와 유의한 양의 상관 관계를 보였다. 또한 비타민 D 수치는 척추 BMD와 고관절 BMD에서 모두 양의 상관 관계를 보였으나, TFI와는 음의 상관 관계를 보였다. 결론: 근육량은 근감소증 환자뿐만 아니라 골다공증 환자의 치료에 있어서도 도움이 되므로 주의깊게 보아야 할 것이다. 또한 비타민 D를 증가시키는 것은 근육 위축의 진행을 늦출 수 있으므로 이로써 골절을 예방하는 효과를 가져올 수 있다.

The Correlation of Metabolic Syndrome Factors and Bone Mineral Density on Postmenopausal Osteoporosis Patients with Low Back Pain under Korean Medicine Treatment

  • Objectives The relationship between metabolic syndrome causes and bone mineral density (BMD) was explored by taking 60 female chronic low back pain patients with age 61 years old or elder having metabolic syndrome and osteoporosis as study subjects. Methods Fasting blood glucose, serum total-cholesterol, triglyceride and HDL were measured by biochemical tests. Anthropometric elements and blood pressure were measured. Results Average BMD and T-score of part number 1 to 3 of lumbar vertebra were estimated by Quantitative Computed Tomography (QCT). In order to find the relationship between clinical factors and osteoporosis, correlation analysis was done on T-score. Age (r=0.679, p<0.01) had significant negative correlation and weight (r=0.342, p<0.01) and height (r=0.475, p<0.01) had significant positive correlation. Blood glucose, blood pressure, total cholesterol, triglyceride, HDL and body mass index did not have significant correlation. BMD had negative correlation with age (r=0.317, p<0.05). Regression analysis was done by taking T-score as independent variables and taking other factors as dependent variables. It was possible to know that age ($\beta$=-0.471, t=-7.050) with p<0.001, height ($\beta$=0.277, t=4.120) and weight ($\beta$=2.856, t=2.780) with p<0.05 have significant impact on osteoporosis. Conclusions Therefore, it was possible to know that T-score and BMD decrease as one gets older and T-score and BMD increase as one is taller and heavier.

칼슘보충 섭취와 운동이 중년 여성의 골밀도에 미치는 영향 (Effects of Calcium Supplementation and Exercise on Bone Mineral Density in Middle-Aged Women)

  • This study examined the effect of calcium supplementation and exercise on bone mineral density (BMD) and general characteristics, dietary intake and biochemical measurements for 45 healthy middle-aged women (40-57 years). Subjects were divided in to 4 groups; control group (G1), exercise group (G2), calcium supplementation group (G3), and calcium supplementation with exercise group (G4). The subjects were 45.8$\pm$ 0.66 years old. BMI(kg/$m^2$) was 23.31 $\pm$ 0.63 in Gl group, 22.92 $\pm$ 0.51 in G2 group, 23.64 $\pm$ 0.61 in G3 group, 23.40 $\pm$ 0.37 in G4 group, and BMI of all groups were in normal value. Energy intake was 1332.28㎉/day, 60% of RDA, so that may be unbalance of micronutrients. Especially, calcium intake was 62.8% of RDA that was very low level. Osteocalcine was not statistically significant but calcium supplementation group (G3) be showed increase. BMD was usually decreased by Aging, but in this study was increased in the all group, except control group. It showed increase of BMD for calcium supplementation and/or exercise. Overall results indicate that calcium supplementation and/or exercise increased BMD of middle-aged women, but long-term calcium supplementation and exercise will be able to more effect.(Korean J Nutrition 35(9) : 962~969, 2002)

Use of Dual-Energy X-ray Absorptiometry in Children with Inflammatory Bowel Disease: A Large Single Centre Study

  • Purpose: Low bone mineral density (BMD) is a complication in children with inflammatory bowel disease (IBD). There are limited data evaluating dual-energy x-ray absorptiometry (DXA) as a screening tool for low BMD in children with IBD. We performed a single site retrospective analysis of DXA use. Methods: Children aged 5-18 years with IBD diagnosed between 2013 to 2017 at the Royal Children's Hospital, Australia, were included. Patient demographics, measures of disease activity, DXA scores, and factors related to BMD were collected. Results: Over a median follow up of 5.1 (4-6.4) years, 72/239 (30.1%) children underwent DXA, and 28/239 (11.7%) children had a second DXA. Our DXA practice differed to consensus guidelines regarding initial screening based on height and/or body mass index (BMI) z-score (8/17 [47.1%]), and repeat surveillance (13/42 [31.0%]). Children had a median lumbar spine (LS) z-score -0.80 (-1.65-0.075). Children with LS z-score≤-2.0 (n=14) had lower weight (6.57 [1.78-23.7] vs. 51.1 [26.5-68.7], p=0.0002) and height centiles (3.62 [1.17-17.1] vs. 42 [16.9-67.1], p=0.0001), and higher faecal calprotectin (FCP) (3041 [1182-4192] vs. 585 [139-2419], p=0.009) compared to children with LS z-score>-2.0. No fractures were reported. Of 28 children who underwent a second DXA 1.6 (1.1-2.2) years following initial DXA, no significant change in z-scores occurred. Conclusion: Children with IBD had low BMD. In addition to height centile and weight centile, FCP was associated with lower BMD, and should be considered in DXA screening guidelines. Greater clinician awareness of DXA consensus guidelines is required. Future prospective studies are required.

중년여성 골다공증과 체질량지수와의 관계 분석 (Relationship Analysis of Osteoporosis and Body Mass Index(BMI) in the Middle-Aged Women)

  • 일반적으로 중년여성의 골다공증은 저체중에서 발생률이 높은 것으로 보고되었으나 비만인 경우에도 발생할 수 있다. 따라서 본 연구의 목적은 골밀도 검사에서 골다공증으로 진단받은 중년여성 120명을 대상으로 골다공증과 체질량지수의 상관관계를 비교하고자 하였다. 체질량지수가 18.5 미만을 저체중, 18.5 ~ 22.9를 정상, 23.0 ~ 24.9를 과체중, 25.0 ~ 29.9를 비만, 30.0 이상인 경우를 고도비만으로 하였다. 연구 결과 대상자의 체질량지수 분포는 저체중이 2명(1.7%), 정상이 18명(15.0%), 과체중이 19명(15.8%), 비만이 75명(62.5%), 고도비만이 6명(5.0%) 이었다. 또한 체질량지수와 골다공증의 상관관계는 체질량지수가 정상인 경우 18명 중 13명(72.2%)이 골다공증이었으며 과체중의 경우 19명 중 14명(73.7%), 비만의 경우 75명 중 63명(84%), 고도비만의 경우 6명 중 5명(83.3%)로 나타나 체질량지수가 비만인 경우에서 골다공증 발생률이 높았다. 결론적으로 중년여성의 골다공증은 체질량지수가 비만일수록 골다공증 환자가 많다는 것을 알 수 있었으며 대부분 골감소증보다는 골다공증이 높게 나타남을 알 수 있었다.

Genome-Wide Association Study of Bone Mineral Density in Korean Men

  • Osteoporosis is a medical condition of global concern, with increasing incidence in both sexes. Bone mineral density (BMD), a highly heritable trait, has been proven a useful diagnostic factor in predicting fracture. Because medical information is lacking about male osteoporotic genetics, we conducted a genome-wide association study of BMD in Korean men. With 1,176 participants, we analyzed 4,414,664 single nucleotide polymorphisms (SNPs) after genomic imputation, and identified five SNPs and three loci correlated with bone density and strength. Multivariate linear regression models were applied to adjust for age and body mass index interference. Rs17124500 ($p=6.42{\times}10^{-7}$), rs34594869 ($p=6.53{\times}10^{-7}$) and rs17124504 ($p=6.53{\times}10^{-7}$) in 14q31.3 and rs140155614 ($p=8.64{\times}10^{-7}$) in 15q25.1 were significantly associated with lumbar spine BMD (LS-BMD), while rs111822233 ($p=6.35{\times}10^{-7}$) was linked with the femur total BMD (FT-BMD). Additionally, we analyzed the relationship between BMD and five genes previously identified in Korean men. Rs61382873 (p = 0.0009) in LRP5, rs9567003 (p = 0.0033) in TNFSF11 and rs9935828 (p = 0.0248) in FOXL1 were observed for LS-BMD. Furthermore, rs33997547 (p = 0.0057) in ZBTB and rs1664496 (p = 0.0012) in MEF2C were found to influence FT-BMD and rs61769193 (p = 0.0114) in ZBTB to influence femur neck BMD. We identified five SNPs and three genomic regions, associated with BMD. The significance of our results lies in the discovery of new loci, while also affirming a previously significant locus, as potential osteoporotic factors in the Korean male population.

Comparative Analysis of Preoperative and Postoperative Muscle Mass around Hip Joint by Computed Tomography in Patients with Hip Fracture

  • Purpose: This study was conducted in order to assess changes in hip muscles by comparing results of preoperative and postoperative computed tomography (CT) in older patients who underwent surgery for treatment of hip fracture. Materials and Methods: A total of 50 patients (aged ≥65 years) who underwent surgery for treatment of intertrochanteric fractures (25 patients) and femoral neck fractures (25 patients) between February 2013 and February 2019 and underwent preoperative and postoperative pelvic CT were enrolled in the study. The cross-sectional area, attenuation and estimates of muscle mass of the gluteus medius, gluteus minimus, iliopsoas, and rectus femoris on the uninjured side were measured. Basic patient data (sex, age, height, weight, body mass index [BMI], bone mineral density [BMD], Harris hip score [HHS], and length of follow-up) were collected from medical records. Results: No significant differences in sex, age, height, weight, BMI, BMD, HHS, and length of follow-up were observed between the two groups. No significant difference in the cross-sectional areas and attenuations of gluteus medius and gluteus minimus was observed after surgery; however, a statistically significant decrease was observed in those of iliopsoas and rectus femoris after surgery. Lower estimates with statistical significance of muscle mass of the iliopsoas and rectus femoris were observed on postoperative CT. Conclusion: Muscle mass of the hip flexor (iliopsoas, rectus femoris) showed significant decreases on postoperative CT compared with preoperative CT. Based on these findings, selective strengthening exercise for hip flexor should be beneficial in rehabilitation of hip fractures.

폐경 후 여성의 골격상태에 영향을 미치는 요인분석 (Some Factors Affecting Bone Mineral Status of Postmenopausal Women)

  • Osteoporosis, the typical metabolic bone disease of the elderly, is characterized by a reduction in bone mineral density (BMD) and increased fracture risk. Genetic and environmental factors are known to play a key role in bone metabolism, and diet is also considered to be one of the important factors. The purpose of the present study was to investigate the relationship among the factors affecting BMD, including stature, body weight, age, time period since onset of menopause, and biochemical markers of bone turnover in postmenopausal women. Seventy-eight postmenopausal women who visited health promotion center for health examinations volunteered to participate in this study and they were divided into two groups according to the time period since onset of menopause : women with a time period since onset of menopause of less than 5 years (Group 1) and women with a time period since onset of menopause of 5 years or more (Group 2). The demographic characteristics and dietary intake were surveyed using a questionnaire. BMDs of the lumbar spine and femoral neck of subjects were measured by dual energy X-ray absorptiometry. Serum levels of 25-hydroxy-vitamin D and parathyroid hormone (PTH), known to be indicators of bone related hormone status, were anlyzed. Serum samples were measured for calcium, phosphorus, alkaline phosphatase, and osteocalcin as bone formation indicators, and urine was analysed for deoxypyridinoline, creatinine, calcium, and sodium as bone resorption indicators. The results are as follow : The mean BMDs of the lumbar spin and femoral neck were $1.02 \pm 0.02 g/cm^2 and 0.81 \pm 0.02 g/cm^2 respectively, and the BMD level of Group 2 was significantly lower than tat of Group 1 (p<0.01, p<0.05, respectively). The mean daily intake of energy was 1838 $\pm$ 55 kcal. When nutrient intake was compared with the recommended dietary allowances (RDA) of the subjects, only calcium, vitamin A and riboflavin intake showed means lower than the RDA. The nutrient intake did not show any significant differences between Group 1 and 2 Serum and urine levels of biochemical markers of bone turnover did not show any significant differences between Group 1 and 2, and all were within the normal range. However, the PTH and deoxypyridinoline levels showed a tendency to be higher, and the osteocalcin level to be lower in Group 2 than in Group 1. Although age and years after menopause (YAM) showed negative correlations with lumbar spine bone mineral density (LBMD) (r= -0.38, p<0.001, and r= -0.26, p< 0.05, respectively), no correlation was found with femoral neck bone mineral density (NBMD). While height, body weight and body mass index (BMI) showed a positive correlation with LBMD (r= 0.32, p<0.001, r= 0.38, p<0.001, r= 0.22, p= 0.05, respectively), only body weight and BMI showed a positive correlation with NBMD (r= 0.30, p<0.01, and r= 0.27, p<0.05, respectivley). There was no significant corealtion between BMDs and the nutrient intake of subjects, except in the case of carbohydrates (r= 0.22, p<0.05). Also, serum and urine levels of bone turnover markers showed no significant correlation with nutrient intake. On the other hand, serum osteocalcin had a positive correlation with vitamin C intake (r= 0.22, p= 0.05), and urine deoxypyridinolin showed a negative correlation with niacin intake (r= -0.22, p= 0.05). Urinary na was negatively correlated with protein intake(r= -0.23, p= 0.05). The results suggested that it is difficult to prevent the decrease in bone mass among postmenopausal women eating the usual Korean diet. However, the BMDs of the lumbar spine and femoral neck were positively related to body weight ad BMI in postmenopausal women. Therefore, this study confirmed that one of the most effective ways to minimize bone loss in postmenopausal women would be to maintain an adequate body weight with balanced nutrient intake and activity in the pre-and postmenopausal periods.
