걷기 훈련이 재택 노인의 낙상방지 체력에 미치는 영향 (Effect of Walk Training on Physical Fitness for Prevention in A home Bound Elderly)

  • The purpose of this study was to determine the effect of walk training on leg strength, flexibility, postural stability, balance and gait in home bound elderly women. Eighteen elderly women of the experimental group aged between 70 and 90 years image who have normal vision, hearing and Romberg test. They participated in the 12 week walk training. The subjects of the experimental group practiced walk training 3 times a week for during 12 weeks. During the 40 minute workout, the subjects practiced 5 minutes of warming-up exercises, 30 minutes of conditioning exercises and 10 minutes of a cool-down exercise. The intensity for the conditioning phase was determined by subject' heart rates, which ranged from 60% to 70% of age-adjusted maximum heart rates. The body composition, leg strength, flexibility, postural stability, balance and gait were measured prior to and after the experimental treatment. The body fat, lean body mass, leg strength (ankle dorsiflexor, plantarflexor, inversor and eversir, knee flexor, extensior), flexibility (range of motion of ankle dorsiflexion, plantarflexion, inversion and eversion), and postural stability of the experimental group were significantly greater than those of the control group. Duration of standing on the right foot and that of standing on the left foot of the experimental group was greater than that of the control group. Total balance scores of the experimental group were significantly higher than those of the control group. Among 13 items for balance, the scores of experimental group in balance with eyes closes, turning balance, sternal nudge, neck turning, one leg standing balance and back extension were higher than those of the control group. Total scores of gait of the experimental group were significantly higher than those of the control group following the walking training. Scores of experimental group in step height, step length and walk stance while walking among 9 items for gait were significantly higher than those of the control group. The results suggest that walk training can improve physical fitness for prevention in home bound elderly women.

여성 노인의 신체활동 수준과 낙상예방 관련 체력의 상관관계 (Relationship between Physical Activity Level and Fall-Proof-related Fitness in Older Female Adults)

  • PURPOSE: This study investigated the relationship between the daily physical activity level and fall-proof-related fitness in older female adults. METHODS: This study promoted and sampled the subjects who participated in the study for 2 weeks, and developed a basic information questionnaire to select the subjects to be excluded from the research. The amount of energy expenditure through daily physical activity was examined, and the elderly physical fitness, and balance test were analyzed. The subjects were divided into group A (${\geq}1,500kcal/week$), group B (<$1,500-{\geq}1,000kcal/week$), and group C (<1,000 kcal/week) according to their daily physical activity level. RESULTS: A significant difference in the daily physical activity level (energy expenditure), Chair Stand Test (lower body strength), 8-Foot Up-and Go Test (dynamic balance), and CTSIB-M (modified Clinical Test of Sensory Interaction in Balance) was observed among groups A, B, and C (p<.5), but there was no significant difference in the Chair Sit-and-Reach Test (lower body flexibility) (p>.5). CONCLUSION: The increase in physical activity is an essential factor for preventing falls and it provides many health benefits for the elderly. On the other hand, considering that elderly people cannot access exercise programs easily in Korea, it can be predicted that increasing elderly people's physical activity in daily life rather than specific exercises may help prevent falls.

Effects of body weight-supported treadmill training combined with ball-kicking on balance and gait of subacute stroke patients

  • Objective: This study focused on subacute stroke patients who were asked to kick a ball while walking on a treadmill. The aim of the study was to determine the effect of a body weight-supported treadmill training (BWSTT) combined with ball-kicking on muscle strength, balance, and gait. Design: Single blind, randomized controlled trial. Methods: Twenty stroke patients who volunteered to participate in this study were randomly assigned to either the BWSTT combined with ball-kicking (BWSTT-BK; 10 participants) group or the BWSTT group (10 participants). Participants in the BWSTTBK group performed treadmill walking combined with simultaneous ball-kicking for 30 minutes daily for 5 weeks. Participants from the BWSTT group performed only treadmill walking. The muscular strength, balance, and gait ability were measured before and after the 5-week training. To assess for muscular strength, a digital muscle tester was used to measure hip flexor, knee extensor, and dorsiflexor strength. To assess for balance, the Berg Balance Scale (BBS) and Timed Up and Go Test (TUG) was used. To assess for gait, the 10 meter walk test (10MWT) and Functional Gait Assessment (FGA) was used. Results: The BWSTT-BK group showed significantly improved muscular strength, balance, and gait according to BBS, TUG, 10MWT, FGA, and digital muscle testing scores compared to the BWSTT group (p<0.05). In addition, within-group comparison showed significant improvement in all variables (p<0.05). Conclusions: These findings suggest that BWSTT-BK results in more favourable outcomes for stroke patients. Therefore, BWSTT-BK may be useful for the recovery of gait ability of stroke patients.

뇌졸중 환자의 웰니스를 위한 호흡기능, 하지근활성도 및 균형에 미치는 효과 - 호흡운동을 결합한 전신진동운동을 중심으로 - (Effects on the Respiratory Function, Lower Extremity Muscle Activity and Balance for the Wellness of Stroke Patients - Focused on Whole Body Vibration Exercise Combined with Breathing Exercise -)

  • 본 연구의 목적은 전신진동이 결합된 들숨근 훈련을 적용하여 뇌졸중 환자의 호흡기능 개선 및 다리근육의 기능향상을 위한 운동방법을 제시하는 것이다. 뇌졸중 환자 21명을 임상 표본추출하여 호흡운동을 결합한 전신진동운동을 적용한 집단 11명을 실험군I로, 호흡운동을 결합한 위약운동을 적용한 집단 10명을 실험군II로 각각 무작위 배치하여 5주 간, 4일/주, 1회/일, 4세트/1회 중재 프로그램을 시행하였다. 중재 전 최대들숨압 측정기로로 호흡기능을 측정하였고, 표면 근전도로 하지 근활성도를 측정하였으며, 버그발란스 검사를 사용하여 균형능력을 측정한 후, 5주 후에 사후검사를 사전검사와 동일하게 재 측정하여 분석하였다. 실험군I의 집단 내 변화 비교에서는 호흡근력, 넙다리두갈래근, 앞정강근의 활성도 및 균형에서 유의한 차이가 있었다(p<.05). 실험군II의 집단 내 변화 비교에서는 호흡근력과 균형에서 유의한 차이가 있었다(p<.05). 집단 간 변화 비교에서는 넙다리두갈래근, 앞정강근의 활성도에서 유의한 차이가 있었다(p<.01). 향후에도 신경근 기능을 향상시키기 위한 호흡운동과 전신진동에 대한 프로토콜에 대한 연구가 지속적으로 필요할 것으로 여겨진다.

Comparison of Center of Pressure Displacement during Sit to Stand to Sit and Balance Ability of Subjects with and without Chronic Ankle Instability

  • Purpose : The purpose of this study is to compare the balance ability between subjects with chronic ankle instability and normal people and the center of pressure displacement during the sit to stand and stand to sit. Methods : The subjects of this study were 63 who met the inclusion criteria and were classified into normal group (n=33) and chronic ankle instability group (n=30). The displacement of the center of pressure during sit to stand and stand to sit was measured. And the limit of stability and Y-balance tests were performed to measure the balance ability. Independent t-test was conducted to compare center of pressure displacement and balance ability between groups, and pearson correlation was conducted to analyze the correlation between the center of pressure displacement and balance ability. Results : In the case of the center of pressure displacement, there was a significant difference between the two groups during sit to stand and stand to sit. In the case of balance, both limit of stability and Y-balance test showed significant differences between the two groups. At the time of sit to stand, the center of pressure displacement showed a significant correlation with balance abilities, and at the time of stand to sit, the center of pressure displacement showed a significant correlation with Y-balance test. Conclusion : Chronic ankle instability shows that there is a lot of sway in the body due to compensation to replace the decrease in ankle joint range of motion when performing sit to stand and stand to sit due to sensory input damage such as decrease in ankle range of motion and decrease in ankle proprioception. Chronic ankle instability is expected to have a negative effect on our daily lives in life. The results of this study will serve as the basis for the dynamic approach to objective evaluation, treatment, and prevention of chronic ankle instability.

고유수용성 신경근 촉진법이 편마비 환자의 균형에 미치는 영향 -율동적 안정화와 등장성 수축 결합기법을 중심으로- (The Effects of Proprioceptive Neuromuscular Facilitation on Balance in Patients with Hemiplegia: Application of Rhythmic Stabilization and Combination of Isotonic technique)

  • Purpose : The purpose of this study was to examine the effects of Rhythmic Stabilization(RS) and Combination of Isotonic(CI) of Proprioceptive Neuromuscular Facilitation on balance in patients with hemiplegia. Methods : Ten patients with hemiplegia were trained with RS and CI for six weeks. Two Standard Scale(TSS), Functional Reach Test(FRT), Time Up and Go test(TUG) and Berg Balance Scale(BBS) were used to prove their improvement of balance. Results : Body weight bearing decreased on less affected side and increased on more affected side. Distance of forward reaching for FRT increased. Time for TUG decreased. BBS score increased. They all showed significant difference. Conclusion : This study suggests that balance training with RS and CI of Proprioceptive Neuromuscular Facilitation improved balance in patients with hemiplegia.

The Effects of Walking and Turning and Treadmill Training on Postural Balance and Walking in People with Parkinson's Disease

  • Purpose: The aim of our study was to evaluate the therapeutic effects of walking and turning plus treadmill training on the functional balance and walking ability of individuals with Parkinson's disease (PD). Methods: Twenty-four participants with Stage 1 to 3 ($2.13{\pm}0.64$) PD based on the Hoehn and Yahr scale were randomly allocated to the experimental group (EG) and control group (CG), with 12 participants in each group. The measured outcomes included the motor subscale of the unified Parkinson's disease rating scale (UPDRS-M), the Berg balance scale (BBS), the Timed Up and Go (TUG) test, the 10-meter walk test (10MWT), and the 6-minute walk test (6MWT). Results: Pre-to-post intervention improvements were noted for all the outcome measures for both groups (p < 0.05). Post intervention, there was a significant improvement in the EG compared to the CG for the following measured outcomes (p < 0.05): UPDRS-M (p = 0.021; 95% CI, 0.081-6.519), BBS (p = 0.042; 95% CI, 1.375-4.541), TUG (p = 0.034; 95% CI, -3.315--0.143), 10MWT (p = 0.011; 95% CI, -2.032--0.289), and 6MWT (p = 0.002; 95% CI, 24.39-91.273). Conclusion: Our study suggests that walking and turning plus treadmill training improves balance and walking compared to treadmill training only in patients with PD.

아급성기 뇌졸중 환자의 다리근력, 균형, 보행, 재활만족도에 대한 로봇 보조 기립경사대 훈련과 체중지지 트레드밀 훈련의 효과 비교 (Comparison of Robotic Tilt-table Training and Body Weight Support Treadmill Training on Lower Extremity Strength, Balance, Gait, and Satisfaction with Rehabilitation, in Patients with Subacute Stroke)

  • PURPOSE: This study examined the effects of Robot Tilt-table Training (RTT) on the lower extremity strength, balance, gait, and satisfaction with rehabilitation, in patients with subacute stroke (less than six months after stroke onset), and requiring intensive rehabilitation. METHODS: A total of 29 subacute stroke patients were divided into an RTT group (n = 14) and a Body Weight Support Treadmill Training (BWSTT) group (n = 15). The mean age of patients was 62 years. RTT and BWSTT were performed for four weeks, three times a week, for 30 minutes. Isometric strength of the lower extremities before and after intervention was compared by measuring the maximal voluntary isometric contraction of the lower extremity muscles. To compare the balance function, the center of pressure (COP) path-length and COP velocity were measured. Timed Up & Go test (TUG) and 10 Meter Walking Test (10 MWT) were evaluated to compare the gait function. A satisfaction with rehabilitation survey was conducted for subjective evaluation of the subject's satisfaction with the rehabilitation training imparted. RESULTS: In the intra-group comparison, both groups showed significant improvement in lower extremity strength, balance, gait, and satisfaction with rehabilitation, by comparing the parameters before and after the intervention (p < .05). Comparison of the amount of change between groups revealed significant improvement for all parameters in the RTT group, except for the 10 MWT (p < .05). CONCLUSION: Both groups are effective for all variables, but the RTT group showed enhanced efficacy for variables such as lower extremity strength, balance, gait, and satisfaction with rehabilitation, as compared to the BWSTT group.

복식호흡을 병행한 척추 안정화운동이 요통환자의 균형능력과 오스웨스트리 장애지수에 미치는 영향 (The Effects of Stabilization Exercise with Abdominal Breath on Balance and Oswestry Disability Index for Low Back Pain Patients)

  • PURPOSE: This study was to investigate the effect of stabilization exercise with abdominal breath on Balance and Oswestry Disability Index for Low Back Pain Patients. METHODS: The subjects were 18 low back pain patients in their twenties. The subjects were randomly assigned into experimental group and control group. Control group and experimental group both participated in 30 minutes of stabilization exercise for 8 weeks(5 times/week), and experimental group participated in stabilization exercise with abdominal breath. The subjects were tested balance and pain using BioRescue and Oswestry Diability Index respectively, before and after intervention. RESULTS: The experimental group showed a statistical significance in movement areas of the body's center(p<.05), but there was no significant differences between groups (p>.05). The experimental group showed a statistical significance in movement distances of the body's center with eyes open(p<.05), and there was significant differences between groups(p<.05). Both groups showed a statistical significance in ODI between pre and post test(p<.05) but there was no significant differences between groups(p>.05). CONCLUSION: As a result, the group participating in stabilization exercise with abdominal breath had a more effective improvement than the control group. Therefore, the stabilization exercise with abdominal breath may be used improving balance and pain in low back pain patients.

성공적 노화를 위한 양손 운동 듀오콕 프로그램의 적용 및 효과성 검정: 양측 운동 (Application and Effectiveness of the Two-Handed Exercise Duocock Program for Successful Aging: Bilateral Exercise)

  • Objective: This study was conducted with the aim of verifying the effectiveness of the duocock exercise, which is being utilized at the newly developed site to promote balance maintenance, basic physical strength and muscle strength among various age groups, in order to prevent fall and successfully age. Method: In this study, we conducted the duocock Exercise Program for 12 weeks (twice a week for an hour) for 10 senior citizens aged 65 or older (68.8±4.76 yrs, height: 15±6.00 cm) using Western-based health institutions in Daejeon, and compared the pre- and post- effects on basic physical strength and posture balance. All the data obtained from this study were used in statistical program SPSS 24 to perform paired t-test. The significance level for all statistical analyses was set at the level of p<0.05. Results: The 12 week duocock program showed statistically significant improvements in lower extremity muscle strength, equilibrium, agility, balance, and coordination in the basic fitness factors underlying successful aging (p<0.05). In addition, the trunk imbalance, which acts as a factor of falls and body balance, was statistically significant (p<0.05) and the posture balance of the sagittal plane also showed the effect of proper body adjustment. Conclusion: Based on the results of this study, duocock provides a new exercise program in the form of a sustainable sport as a two-handed exercise, and is very effective for the elderly to improve their basic physical strength as well as to control postural imbalances, strengthen and increase muscle strength.
