실혈 후 및 혈압상승 후의 소화기 조직 혈액량 및 산소 섭취량 -제 2 편 동맥 혈압하강과 산소 섭취량 감소- (Alterations in $O_2$ Uptake Following Hemorrhage and Transfusion in Rats)

  • Total body oxygen uptake was measured in rats following hemorrhage (16 rats) and blood transfusion (7 rats) under light Nembutal anesthesia. Arterial blood Pressure measured on the tail artery decreased or increased following hemorrhage or transfusion. No direct relationship was observed between arterial blood pressure alteration and oxygen intake variation. Hematocrit ratio which changed after hemorrhage or transfusion showed a direct relationship with oxygen intake. Decrease in hematocrit ratio resulted in a decrease in oxygen intake of rats. The correlation coefficient between decrement of hematocrit ratio and decrement of oxygen intake was r=.56. The correlation coefficient between increment of hematocrit ratio and increment of oxygen intake was r=.86. Thus it was concluded that alteration in oxygen intake was limited by the systemic oxygen transport capacity of blood.

미숙아에서 수혈 후 백혈구수의 변화 (The Change of White Blood Cell Count Following Transfusion in Preterm Neonates)

  • 목 적 : 신생아 집중치료실에서 치료 중인 미숙아에서 적혈구 수혈 후의 백혈구수의 변화를 조사해 보고자 하였다. 그리고 그 정도가 신생아 감염을 의미할 만한 정도인지 알아보고자 하였다. 방 법 : 2년 1개월간 본원 신생아 집중 치료실에서 농축 적혈구를 수혈 받은 환아 33명(총 수혈횟수 48회)을 대상으로 수혈 전후의 백혈구수, 호중구수, 대호중구수를 후향적으로 비교, 분석하였다. 결 과 : 적혈구 수혈 후 최초 8시간(제 1기)에 백혈구와 호중구가 각각 평균값이 $1.33{\times}10^3/mm^3$(P=0.03), $0.55{\times}10^3/mm^3$(P=0.05 )증가하였으나, 수혈 후 17-24시간(제 3기)에는 수혈 전과 큰 차이가 없었다. 대호중구수는 모든 대상 환아에서 수혈 전후에 큰 변화가 없었다. 결 론 : 적혈구 수혈 후 일시적으로 호중구 상승에 의한 백혈구 상승이 나타나지만, 24시간경에는 수혈전 수치에 가까워지고, 초기의 그 상승 정도가 크지 않아 신생아 감염으로 혼동되지는 않을 것 같다.

  • Blood transfusion in the neonatal period remains a therapeutic act, that no one dares to consider it as perfectly innocuous and that it is advisable to make rare, considering its risks as well immunological as infectious. The present work is a retrospective study, conducted in neonatal intensive care unit, in the University Hospital MOHAMED VI, Marrakech, during the period from January 1st to December 31st, 2019. All newborns were included in this work, who received one or multiple transfusions of red blood cells (RBC). Our study covered 60 neonates with a total of birth: 794 neonates, with a prevalence of 7.55%, hospitalized for various indications (60% of newborns were at term, 31.7% premature and 8, 33% were post-mature. The majority of transfusion procedures were performed during the first week of life. This is explained by the frequency of haemolytic anemias by fœto-maternal incompatibility. The main indications for transfusion were haemolysis, anemic syndrome and haemorrhagic syndrome. The average number of transfusion episodes was 1.95 +/- 1.47 per patient. Newborns were polytransfused in 18.33% of cases. A single transfusion accident was found in our study. However, we did not observe a correlation between the maternal diseases, the state of the newborn, and the transfusional indication.

Effects of Allogeneic Blood Transfusion in Patients with Stage II Colon Cancer

  • The aim of the present study was to determine whether allogeneic red blood cell transfusions showed a deleterious effect and what might be preoperative risk factors for blood transfusion in patients with TNM stage II colon cancer. Total 470 patients who fulfilled inclusion criteria were selected for a further 10-year follow-up study. We found that there were statistical significance between non-transfused and transfused group in mortality (P=0.018), local recurrence (P=0.000) and distant metastasis (P=0.040). Local recurrence and distant metastasis between 1 to 3 units and more than 3 units group did not show any significant differences. There was no difference in survival rate between non-transfused and 1 to 3 units group (log rank=0.031, P=0.860). The difference between different blood transfusion volume in transfused patients was found (78.77% vs 63.83%, P=0.006). Meanwhile, the significant difference of survival rate was existed between non-transfused group and more than 3 units group (84.83% vs 63.83%, P=0.002 ). Univariate analysis showed the following 3 variables to be associated with an increased risk of allogeneic blood transfusions: preoperative CEA level (P<0.05), location of tumor (P<0.01) and diameter of tumor (P<0.01). Multivariate analysis revealed that location of tumor and diameter of tumor are two independent factors for requirement of perioperative transfusions. Therefore, allogeneic transfusion increase the postoperative tumor mortality, local recurrence and distant metastasis in patients with stage II colon cancer. The postoperative tumor mortality, local recurrence and distant metastasis were not associated with the blood transfusion volume. The blood transfusion volume was associated with the survival rate. Location of tumor and diameter of tumor were the independent preoperative risk factors for blood transfusion.

Analysis of necrotizing enterocolitis and transfusion of red blood cell in very low birth weight preterm infants

  • Purpose: To investigate the association between necrotizing enterocolitis (NEC) and red blood cell transfusions in very low birth weight (VLBW) preterm infants. Methods: We studied were 180 VLBW preterm infants who were admitted to the neonatal intensive care unit of CHA Gangnam Hospital from January of 2006 to December of 2009. The subjects were divided into 2 groups: an NEC group (greater than stage II on the modified Bell's criteria) and a control group (less than stage II on the modified Bell's critieria). We defined red blood cell transfusion before NEC diagnosis as the frequency of transfusion until NEC diagnosis (mean day at NEC diagnosis, day 18) in the NEC group and the frequency of transfusion until 18 days after birth in the control group. Results: Of the 180 subjects, 18 (10%) belonged to the NEC group, and 14 (78%) of these 18 patients had a history of transfusion before NEC diagnosis. The NEC group received $3.1{\pm}2.9$ transfusions, and the control group received $1.0{\pm}1.1$ transfusions before the NEC diagnosis (P=0.005). In a multivariate logistic regression corrected for gestational age, Apgar score at 1 minute, the presence of respiratory distress syndrome, patent ductus arteriosus, premature rupture of membrane, disseminated intravascular coagulopathy and death were confounding factors. The risk of NEC increased 1.63 times (95% confidence interval, 1.145 to 2.305; P=0.007) with transfusion before the NEC diagnosis. Conclusion: The risk for NEC increased significantly with increased transfusion frequency before the NEC diagnosis.

Comparison of Hemoglobin Correction Effects According to Storage Period and Other Factors in the Transfusion of Packed Red Blood Cells in Neonatal Intensive Care Unit Patients

  • Purpose: Preterm infants frequently require red blood cell (RBC) transfusions in neonatal intensive care units (NICU). Storage RBCs undergo many changes during storage periods. We aimed to compare the hemoglobin (Hb) correction effect according to the period of RBC storage and investigate the factors influencing Hb correction. Methods: This retrospective study reviewed the medical records of 289 patients who received RBC transfusion more than once in the NICU of Kosin University Gospel Hospital between February 2006 and March 2016. The subjects were classified into two storage groups: short-term (${\leq}7days$, n=88) and long-term (>7 days, n=201), according to the period of RBC storage. We checked Hb levels by complete blood cell count tests conducted within 2 days before and 5 to 9 days after the first transfusion. We compared the Hb difference between the two groups and analyzed the factors influencing Hb correction. Results: Excluding the use of an invasive ventilator, there was no significant difference between the two groups in terms of clinical characteristics. There was no significant difference in the Hb correction effect between the two groups (P=0.537). Birth weight greater than 1,500 g, higher weight at transfusion, and larger volume of transfusion were significant prognostic factors affecting greater changes in Hb. In addition, surgery experience, higher Hb level at transfusion, and additional blood tests were found to be significantly associated with less changes in Hb. Conclusion: The RBC storage period did not affect the Hb correction effect. The Hb correction effect may be diminished in infants with lower birth weight and lower weight at transfusion under unstable clinical conditions.

자가면역용혈환자에서 항-Fya 동종항체에 의한 급성용혈성수혈반응 1예 (Case of Acute Hemolytic Transfusion Reaction due to Anti-Fya Alloantibody in a Patient with Autoimmune Hemolytic Anemia)

  • 72세 남자 환자가 전신 무력감을 호소하면서 본원 혈액내과 외래로 내원하였다. 환자는 내원 2년 전 온난자가항체에 의한 자가면역용혈빈혈로 진단받고 치료 중이었다. 내원 당시 환자의 혈색소 수치는 6.3 g/dL로, 빈혈을 교정하기 위해 수혈이 의뢰되었다. 환자의 혈액형은 A형, RhD 양성이었고, 비예기항체 검사에서 범응집 소견 및 자가대조검사에서 양성 소견을 보이고 과거 교차시험에서 최소반응강도를 보이는 적혈구 3단위를 수혈받은 기왕력이 있어, ABO 동형의 적혈구와 교차시험을 하여 최소반응강도를 보이는 적혈구 1단위를 출고하였다. 환자는 수혈을 받은 후 별다른 증상 없이 귀가하였으나, 귀가 후 약 5시간 후부터 발생한 발열, 오한, 호흡곤란, 복통, 혈뇨를 주소로 수혈 다음 날 본원 응급실로 내원하였다. Polyethylene glycol을 이용한 자가흡착검사 후 획득한 상층액을 이용하여 시행한 비예기항체검사에서 항-$Fy^a$가 동정되어, 자가 항체에 의해 가려져 수혈 전에 검출하지 못한 항-$Fy^a$에 의한 급성용혈성수혈반응으로 진단하였다. 본 증례를 통해 자가항체가 동정되는 환자에 대해서 반드시 공존하는 동종항체 확인에 대한 고려가 필요하다는 것을 인지하게 되었고, 이런 자가항체를 제거하여 검사하기 위해 자가흡착검사 방법에 대해 좀 더 익숙해질 필요가 있다는 경험을 하였다.

Clinical Efficacy of Intra-Operative Cell Salvage System in Major Spinal Deformity Surgery

  • Objective : The purpose of this study was to determine the efficacy of intra-operative cell salvage system (ICS) to decrease the need for allogeneic transfusions in patients undergoing major spinal deformity surgeries. Methods : A total of 113 consecutive patients undergoing long level posterior spinal segmental instrumented fusion (${\geq}5$ levels) for spinal deformity correction were enrolled. Data including the osteotomy status, the number of fused segments, estimated blood loss, intra-operative transfusion amount by ICS (Cell $Saver^{(R)}$, $Haemonetics^{(C)}$, Baltimore, MA, USA) or allogeneic blood, postoperative transfusion amount, and operative time were collected and analyzed. Results : The number of patients was 81 in ICS group and 32 in non-ICS group. There were no significant differences in demographic data and comorbidities between the groups. Autotransfusion by ICS system was performed in 53 patients out of 81 in the ICS group (65.4%) and the amount of transfused blood by ICS was 226.7 mL in ICS group. The mean intra-operative allogeneic blood transfusion requirement was significantly lower in the ICS group than non-ICS group (2.0 vs. 2.9 units, p=0.033). The regression coefficient of ICS use was -1.036. Conclusion : ICS use could decrease the need for intra-operative allogeneic blood transfusion. Specifically, the use of ICS may reduce about one unit amount of allogeneic transfusion in major spinal deformity surgery.

Explainable Machine Learning Based a Packed Red Blood Cell Transfusion Prediction and Evaluation for Major Internal Medical Condition

  • Efficient use of limited blood products is becoming very important in terms of socioeconomic status and patient recovery. To predict the appropriateness of patient-specific transfusions for the intensive care unit (ICU) patients who require real-time monitoring, we evaluated a model to predict the possibility of transfusion dynamically by using the Medical Information Mart for Intensive Care III (MIMIC-III), an ICU admission record at Harvard Medical School. In this study, we developed an explainable machine learning to predict the possibility of red blood cell transfusion for major medical diseases in the ICU. Target disease groups that received packed red blood cell transfusions at high frequency were selected and 16,222 patients were finally extracted. The prediction model achieved an area under the ROC curve of 0.9070 and an F1-score of 0.8166 (LightGBM). To explain the performance of the machine learning model, feature importance analysis and a partial dependence plot were used. The results of our study can be used as basic data for recommendations related to the adequacy of blood transfusions and are expected to ultimately contribute to the recovery of patients and prevention of excessive consumption of blood products.

경기도 일개 종합병원에서 혈액제제 출고 현황 (Status of Blood Products Release at a General Hospital in Gyeonggi-Do)

  • 혈액제제(BPs)는 한정적인 자원이다. 혈액제제는 가장 적절하게 사용되어야 하지만 근거 기반 없이 무분별하게 사용되고 있는 것이 사실이다. 본 연구는 헤모글로빈 수치를 기준으로 혈액제제의 사용 적절성을 평가하는 것을 목적으로 하였다. 2020년 11월 1일부터 2021년 10월 31일까지 병원에서 수행된 혈액 공급 데이터는 병원의 디지털 종합의료정보시스템을 활용하였다. 총 제공 건수는 21,303건이었고, 진료과별로 사용하는 농축적혈구제제의 공급 중 헤모글로빈 수치(>7.0 g/dL)의 경우는 1,173건으로 나타났다. 수혈의 오남용은 사회적 비용을 증가시키고, 수혈의 적절성은 더욱 중요해지고 있어, 각 의료기관은 수혈지침 평가지표를 검토하고, 수혈지침의 혈액제제 방출 현황 및 수혈지침의 교육프로그램을 점검하여 적절성을 평가해야 할 것으로 사료된다.
