Atypical Clinical Course of after Glyphosate Herbicide Intoxication (Glyphosate 음독의 임상양상)

  • Purpose: Although glyphosate-surfactant herbicide intoxication is relatively mild toxic in humans, we encountered an atypical clinical presentation. We performed this study to understand the atypical clinical course and factors associated with severe intoxication after glyphosate ingestion. Methods: We conducted a retrospective study of 61 patients (male 43, mean age $54{\pm}18.8$ years) who were presented with glyphosate ingestion between March 1997 and March 2011. The severe intoxication group was defined as patients with systolic blood pressure less than 90 mmHg, respiratory distress needing intubation, or altered mental state. Results: Of the 61 patients, 22 patients (36.1%) had a severe clinical course, 1 patient (1.6%) had died and 1 patient (1.6%) had moribund discharge. The most common symptoms were nausea with or without vomiting which occurred in 30 patients (49.2%). Twenty-seven patients had metabolic acidosis that was the second most common medical complication. Advanced age, pH, base excess, $HCO_3$, Sat, creatinine, X-ray abnormalities and ECG abnormalities were significant factors. Hemoglobin, platelet, $pO_2$, $pCO_2$, BUN, sodium, potassium and AST levels were not different comparing the laboratory characteristics between the severe and mild intoxication groups. Conclusion: The results of this study showed that severe intoxication occurred in 22 patients (36.1%) after glyphosate intoxication. Advanced age, pH, base excess, $HCO_3$, Sat, creatinine, X-ray abnormality and ECG abnormalities were significant predictive factors for severe intoxication in patients with glyphosate surfactant herbicide poisoning.

  • Objects: This study was aimed to find out correlation between life style and cardiopulmonary function and body fat. Methods: We divided two groups by the score of behavior modification therapy questionnaire. Westudied tendency of change of cardiopulmonary function and body fat for medication of Mahuang capsule by ergogenic aids and placebo with circuit training. We got the results for Exercise stress test and Segmental Bioelectrical Impedence Analysis. Results: 1. High Score Group in Behavior Modification Therapy Questionnaire significantly(p<.05) showed elevation of Lean Body Mass, Body Fat, Waist Circumference and not significantly elevation of Body Weight, But Basal Metabolic Rate was declined. Low Score Group in Behavior Modification Therapy Questionnaire significantly(p<.05) showed elevation of Waist Circumference and not significantly elevation of Body Weight and Body Fat, Lean Body Mass but decline of Basal Metabolic Rate, Waist to Hip Ratio 2. High Score Group in Behavior Modification Therapy Questionnaire significantly(p<.05) showed elevation of $VO_{2max}$ and not significantly elevation of the rest. Low Score Group in Behavior Modification Therapy Questionnaire significantly(p<.05) showed elevation of rest Heart Rate, Diastolic Blood Pressure, and not significantly elevation of the rest except for METs Conclusion: High score group in Behavior Modification Therapy Questionnaire showed significantly elevation of body composition, but had no relationship with Cardiopulmonary function

Correlation study between Obesity and Cardiovascular Risk Factors (비만과 심혈관 질환 위험인자와의 상관관계 분석 -건강검진 대상자를 중심으로-)

  • Background: Recently obesity has become a social problem in all over the world including Korea. It is known that obesity is closely related with cardiovascular disease. Many studies are carrying out in the relation between several obesity indices, such as body mass index(BMI), waist-circumference(WC), body fat percent and risk factors of cardiovascular disease. Objective: This study was done in order to investigate correlation of obesity and cardiovascular risk factors. Method: We gathered totally 138 subjects who have taken health screening test in university hospital. With the exception of 40 abnormal cases on current illness and laboratory study, 98 healthy cases were analysed. We divided the groups as normal, overweight, obese group according to body mass index $(22.9kg/m^2\;and\;below\;/\;23{\sim}24.9kg/m^2\;/\;25kg/m^2\;and\;above)$and investigated the relationship between obesity based on body mass index(BMI) and cardiovascular risk factors. And investigated the relationship between each cardiovascular risk factor and body mass index, waist-hip ratio, body fat percent. Results: With the exception of blood pressure, other lab data such as total cholesterol, triglycerid, glucose were significantly different between the three groups divided by body mass index$(22.9kg/m^2\;and\;below\;/\;23{\sim}24.9kg/m^2\;/\;25kg/m^2\;and\;above)$. And Correlation coefficient of cardiovascular risk factors with body mass index, waist-hip ratio, body fat precent were acquired. Conclusion: We have concluded that the body mass index and waist-hip ratio body fat precent were predictive factors cardiovascular disease.

A Study about Correlations between the Interpretations of Autonomic Bioelectric Response Recorder (ABR-2000) and Diagnosis System of Oriental Medicine (DSOM) / 3D Blood Pressure Pulse Analyzer (3D-MAC) (생체전기자율반응 측정기와 한방진단시스템 및 맥진기 검사 해석의 상관성 연구)

  • Objectives: This study was performed to observe the correlations between the results of ABR-2000 and DSOM / 3D-MAC to evaluate the feasibility of ABR-2000 as a oriental medical diagnostic criteria. Methods: We studied 547 women visiting ${\bigcirc}{\bigcirc}$ hospital from December 2012 to June 2015. The subjects were categorized in two groups, 'Hypotonia' and 'Non-Hypotonia' by the result of ABR-2000 and assessed the result of DSOM, 3D-MAC for each group. The differences of pulse wave factors by group also studied. Results: 1. There was no significant difference between two groups about the output frequency of pathogenic factors in DSOM while the result showed the higher correlation in Hypotonia group in terms of the companion tendency of pathogenic factors and syndromes formed by the combination of pathogenic factors. 2. The pulse waves of Hypotonia group were mostly slow, weak, tense and stiff than Non-Hypotonia group. Conclusions: 1. In Hypotonia group, yin deficiency (陰虛) factor was frequently accompanied and consumption (虛損) of various organs based on the yin deficiency (陰虛) was observed. It means chronic and severe condition of exhaustion syndrome (虛勞). 2. The result of 3D-MAC also means pathological feature of yin syndrome (陰 證) and consumption (虛損). Besides, lower scores of Body Surface Area (BSA), body weight, and Body Mass Index (BMI) were associated with body weakness (體瘦), a symptom of exhaustion syndrome (虛勞).

A Study about Sasang Constitutional difference on Autonomous Function after Acupuncture stimulation (침자극 후 사상체질별 자율신경활동의 변화에 대한 연구)

  • 1. Objectives This paper is intended to find out Sasang Constitutional difference on Autonomous Function after Acupuncture stimulation in healthy subjects. 2. Methods of Research Nineteen healthy volunteers(Soyangin 6, Taeumin 7, Soeumin 6) had acupunctured perpendicularly into the Hapkok point(LI4) and the Taechung point(Liv3). Heart Rate Variability, Mean Heart Rate and Blood Pressure were checked 4 times(before and after Acupuncture stimulation) to evaluate Autonomous Function. 3. Results & Conclusions The conclusions were as follows 1. There was no statistically significant Sasang Constitutional difference on sympathetic activity and parasympathetic activity before Acupuncture stimulation. 2. Acupuncture stimulation caused a statistically significant increase in the parasympathetic activity in the whole subject. 3. After Acupuncture stimulation, there was a statistically significant increase in the parasympathetic activity in Soeumin and a tendency of increase in the parasympathetic activity in Soyangin. But there was a tendency of increase in the Sympathetic activity in Taeumin.

  • GABA tea which was increased of GABA by anaerobic conditions in green tea leaves) was fed in the diet to spontaneously hypertensive rats(SHR). The mean systolic blood pressure of SHR fed a diet containing 1.5% GABA tea after 3 weeks was not different with zero day as 177.3mmHg. However, 1.5% green tea and control after 3 weeks were more increased 4.2 percents and 10 percents than in zero day. The plasma levels of aldosterone of SHR fed a diat containing 1.5% GABA green tea after 3 weeks was more decreased 2.5 times than in water treatment.

Comparisons of the quality of chest compression and fatigue levels of the rescuer for different hand techniques used in cardiopulmonary resuscitation (심폐소생술 시 구조자의 hand technique에 따른 가슴압박의 질 및 피로도 비교)

  • Purpose: The purpose of this study was to compare the difference in compression quality and fatigue levels in a rescuer for three different hand techniques used in cardiopulmonary resuscitation (CPR). Methods: The participants were paramedic students at the basic life support provider level. The hands-only CPR was performed for 10 minutes for each of the three hand techniques without disruption, and the quality of chest compressions and fatigue levels were analyzed. Results: There was no difference between the sexes in the chest compression quality and the physiologic parameters before and after compression. Among the quality indexes of chest compression with each of the techniques performed for 10 minutes, the mean depth (p<.01) and mean accuracy (p=.000) of the compression were found to be higher in the five finger fulcrum technique, while the mean compression rate and relaxation accuracy showed no significant differences. Regarding fatigue levels, the five finger fulcrum technique caused lesser subjective fatigue as compared to other techniques (p<.05), although the heart rate and blood pressure revealed no difference. Conclusion: The five finger fulcrum technique was found to be better than the other techniques in terms of chest compression quality and subjective levels of fatigue, indicating that it should be used in CPR education.

A Comparative Study on the Distinction of Patients with Stroke in the Case of the First Attack and Recurrence (중풍(中風) 초발(初發) 및 재발환자(再發患者)의 특성(特性)에 관한 비교연구(比較硏究))

  • Objectives : The present study was carried out to provide basic information necessary for the prevention and efficient treatment of stroke through a comparison between thr patients of the first attack and those of recurrence. Methods : The observation f3r the current study was made on 210 cases of stroke that were confirmed through brain CT-scan. The patients were hospitalized at one of two oriental medical hospitals in Seoul during 2006. Result : The main results were as follows. First, the male-to-female ratio of stroke patients were 1:1.26, with more primary stroke far females and more recurrent stroke for males. Second, in the age distribution, seventies was the top, and sixties, fifties, and forties were next in the order of frequency. A large city was the most frequent residential site and unemployment was the most frequent occupation to have stroke. The incidence of stroke became higher as patients had a taste for spicy and salty food. Third, the most common preceding disease was hypertension. In the relationship of diastolic blood pressure with recurrence, there was significance in the test of independence. Fourth, the most important precipitating conditions at the onset of stroke were rest and steeping. The most common precedent symptoms were verbal disturbance, numbness, and dizziness. In the stroke patients with hemiparesis, male and female patients usually showed It. hemiparesis. Fifth, as a result of brain CT-scan, cerebral hemorrhage was inclined to reduce but cerebral infarction was inclined to increase due to senility and change of lifestyle. Finally, total cholesterol findings disclosed that 22.4% were hypercholesteremia, 18.6 % were hyperlipemia in triglyceride findings, and 19.0% were glycosemia in glucose findings. Conclusions : The above results suggested avoidance of meat and salty fDod and positive control of hypertension and diabetes mellitus in order to prevent stroke.

Microalbuminuria in children with urinary tract infection

  • Purpose: Microalbuminuria is defined as increased urinary albumin excretion (30-300 mg/day) or microalbumin/creatinine ratio (30-300 mg/g) in a spot urine sample. Although microalbuminuria is a predictor of clinical nephropathy and cardiomyopathy, few studies have investigated microalbuminuria in children with urinary tract infection (UTI). Therefore, we compared the spot urine microalbumin/creatinine ratio in pediatric UTI patients with that of control subjects. Methods: We investigated the correlation between the ratio in children with UTI and age, height, weight, blood pressure, glomerular filtration rate (GFR), hematuria, vesicoureteral reflux, renal parenchymal defect, and renal scar, and its predictability for UTI complications. Results: We studied 66 patients (42 boys, 24 girls) and 52 healthy children (24 boys, 28 girls). The mean microalbumin/creatinine ratio in UTI patients was statistically significantly increased compared to the control group ($340.04{\pm}321.36mg/g$ vs. $225.68{\pm}154.61mg/g$, $P$=0.0141). The mean value of spot urine microalbumin/creatinine ratio ($384.70{\pm}342.22mg/g$ vs. $264.92{\pm}158.13mg/g$, $P$=0.0341) in 1-23 months age patient group showed statistically significant increase compared to control group. Microalbumin/creatinine ratio showed negative correlation to age (r=-0.29, $P$=0.0167), body surface area (BSA) (r=-0.29, $P$=0.0173) and GFR (r=-0.26, $P$=0.0343). The presence of hematuria ($P$=0.0169) was found to be correlated. Conclusion: The spot urine microalbumin/creatinine ratio in children with UTI was significantly greater than that in normal children, and it was positively correlated with GFR. This ratio is a potential prescreening and prognostic marker in UTI patients. Further studies are required to validate the predictability of microalbuminuria in pediatric UTI patients.

In-vitro Evaluation of a Bileaflet Polymer Valve and Preliminary Animal Tests (이엽식 고분자판막의 수력학적 특성평가 및 동물실험)

  • In order to use a low cost polymer valve in our total artificial heart and ventricular assist device, we have developed a slit-type bileaflet polymer valve[BPV 1. The aim of this study is to determine the hydrodynamic effectiveness of the newly-designed BPV and its feasibility for temporary use in the blood pumps. For hydrodynamic comparison, we investigated in-vitro the pressure drop across the valve, the leakage volume, the flow rate and the flow pattern of the BPV, two mechanical valves and a trileaflet polymer valve. We employed the ventriculo-pulmonary bypassing method for in-vivo tests of the BPV's together vilh our electrohydraulic left venIn ricular assist device in mongrel dogs. The BPV showed adquate gydrodynamic performances and in the preliminary animal bests, there was no xvi dence of thrombus formation on the valve leaflets and around the struts. Detailed results obtained from the animal tests will be separately reported. This report involves the design criteria, fabrication and hydrodynamic charateristics of she BPV, and the basic merits and demerits of the valve are dis- cussed from the hydrodynamic point of view.
