  • Excluding an operation remedy about an example of Traumatic Cervical Stenosis patient, by doing pattern identification such as blood stagnation in hospitalization for the early days and such as the stagnation of Oi for the middle days and such as Eum deficiency of liver and kidney for the last. We have done treatment by being based on method of therapy to promote blood flow to remove blood stasis and to promote the normal flow of Oi and to nourish the liver and the kidney. In result, we had remarkable conclusion and considered that we had to treat a patient like that appropriately by distinguishing & making a diagnosis correctly from now on. If we apply clinically by thinking of Oriental Medicine through more example of symptoms, we have considered that it will serve as a momentum that we can sympathize with the excellency of Oriental Medicine. In the future, we have considered that have to be necessary to make more researches and reports upon it.

Bronchopulmonary Sequestration: Report of One Case (Bronchopulmonary Sequestration: 1례 보고)

  • Bronchopulmonary sequestration is a congenital malformation characterized by a cystic portion of the lung that derives its arterial blood supply through anomalous aberrant vessel directly of the systemic circulation. An aberrant systemic vessel supplying the lung was reported by Huber in 1777. Although this lesion is uncommon disorder, there are several reports on operative death caused by exanguinating hemorrhage from the aberrant arteries to the bronchopulmonary sequestration to that it has received a great deal of attention. Two type of bronchopulmonary sequestration have been identified: Intralobar pulmonary sequestration is usually contained within the visceral pleura of a pulmonary lobe and its venous drainage to the pulmonary venous system. Extrapulmonary sequestration is usually within the pleural sheath its own and its vasculature drains into the azygos or hemiazygos system. we presented one case of intralobar pulmonary sequestration which led to motor paralysis, chylothorax and hemorrhage that are consequent on postoperative complication.

  • In order to study on Bu-ja(Oh-du) toxicosis, the outhor investigated many documents. The results obtained were as follows: 1. Toxicosis of Bu-ja(Oh-du) arised mainly from carelessness in use. 2. In the symptoms of Bu-ja(Oh-du), Heart System(心系) symptom often came out. 3. The general and local paralysis, falling of blood pressure in Heart System(心系) symptom : vertigo, cramp in Liver System(肝系) symptom : nausea, vomiting in Spleen System (脾系) symptom : difficulty of breathing in Lung System(肺系) symptom : wasting disease in Kidney System (腎系) symptom frequently originated from toxicosis of Bu-ja(Oh-du). 4. In the detoification method, there are atropin injection, carrying out a gastric lavage, and supplying a liquid medicine from the western medicine : taking ginger-licorice root decoction, taking mung beans thick decoction and getting vomited with soaked cinnamon from the oriental medicine. And also keeping warm and oxygen inhalation were used to counteract poison. 5. A pregnant woman, heat of excess type(實熱), yang sydrome(陽證 )must not be taken Bu-ja(Oh-du).

Spinal Epidural Hematoma Occuring after Bloody Tap during Epidural Catheter for Cancer Pain Control -A case report- (암성통증치료를 위한 경막외카테테르 거치술중 혈성천자로 발생한 경막외혈종 -증례 보고-)

  • We report a case of epidural hematoma occuring after bloody tap during epi-dural catheter for cancer pain control in thrombocytopenic patient. Two hours after epidurl puncture, patient experienced severe back pain and numbness of both legs. Following day, patient complained of motor paralysis and urinary difficulty. Diagnosis utilizing magnetic reasonance imaging, showed epidural hematoma extending from $T_{11}$ to $T_{12}$. Thrombocytopenia prevented surgical intervention. Therefore we restored conservative therapy with packed red cell, platelet concentration, steroid and hemostatic, which provided complete neurologic recovery, spontaneously over several days without surgical intervention.

  • Spontaneous hematomas of the iliacus muscle are rare lesions and these are seen in individuals receiving anticoagulation therapy or patients with blood dyscrasias such as hemophilia. It can cause femoral neuropathy and resultant pain and paralysis. Although there is no clear consensus for the treatment of femoral neuropathy from iliacus muscle hematomas, delays in the surgical evacuation of hematoma for decompression of the femoral nerve can lead to a prolonged or permanent disability. We report here on a rare case of a spontaneous iliacus muscle hematoma that caused femoral neuropathy in a patient who was taking warfarin for occlusive vascular disease and we discuss the treatment.

Delayed Post-Traumatic Spinal Cord Infarction with Quadriplegia: A Case Report

  • Traumatic spinal cord infarction is a rare condition that causes serious paralysis. The regulation of spinal cord blood flow in injured spinal cords remains unknown. Spinal cord infarction or ischemia has been reported after cardiovascular interventions, scoliosis correction, or profound hypotension. In this case, a 52-year-old man revisited the emergency center with motor and sensory abnormalities in all four extremities 56 hours after a motor vehicle collision. Despite the clinical presentation and imaging examination, there were no specific findings on the patient's first visit to the trauma center. Cervical spine computed tomography angiography showed a narrow vertebral artery, and diffusion-weighted imaging revealed spinal cord infarction from C3 to C5 with high signal intensity. It should be kept in mind that delayed-onset spinal cord infarction may occur in minor or major trauma patients as a result of head and neck injuries.

  • Poland syndrome is characterized by unilateral absence or hypoplasia of the pectoralis muscle and variable degree of ipsilateral hand anomalies. Mobius syndrome is a congenital neurological disorder characterized by complete or partial facial paralysis. Although the pathogeneses of these diseases are not well-characterized, diminished blood flow to the affected side is thought to play a role. A male infant weighing 2.670 g was born at 38+3 weeks of gestation with left facial paralysis, left chest wall defect with dextrocardia, and symbrachydactyly between the second and third fingers. The combination of Poland-Mobius syndrome is rare, and only 2 cases associated with dextrocardia have been reported worldwide. Here, we report the first case of Poland-Mobius syndrome associated with dextrocardia in Korea.

Associations between Physical Disorders and Suicidal Ideation in Elders (노인의 신체질환과 자살사고 간의 연관성)

  • Objectives : This study aimed to investigate the associations between physical disorders and prevalent/incident suicidal ideation in a community dwelling older population aged 65 years or over. Methods : 1204 people aged 65 years or over evaluated at baseline. Suicidal ideation was identified using the questions from the community version of the Geriatric Mental State Schedule ("GMS B3"). Reported physical disorders covering 11 common and generally chronic health problems were ascertained. Covariates included were depression, age, gender, years of education, accommodation status, past occupation, and current occupation. Of 1066 without suicidal ideation at baseline, 805 (76%) were followed 2 years later, and incident suicidal ideation was evaluated. Results : Prevalent suicidal ideation was significantly associated with 4 of 11 physical disorders: eyesight problems, persistent cough, heart disease and paralysis or weakness in one leg or arm. Incident suicidal ideation was associated with 3 physical disorders: asthma, high blood pressure and paralysis or weakness in one leg or arm. Both prevalent and incident suicidal ideation were significantly associated with increased number of physical disorders. Conclusions : Certain physical disorders were comorbid and precipitating factors of suicidal ideation in elders. And appropriate intervention and treatment of physical disorders might prevent suicidal ideation in elderly.

The Experimental Study on Anti-thrombotic Effect of Hyulbuchukeotanggamibang(HBCT) (혈부축어탕가미방(血府逐瘀湯加味方)의 항혈전작용(抗血栓作用)에 대한 실험적(實驗的) 연구(硏究))

  • Purpose: This study was performed to evaluate anti-thrombotic effects of Hyulbuchukeotanggamibang(HBCT). Methods: It was measured the effects which was given to blood flow rate through the regular volume of glass tube after the blood was diluted five times with ACD soulution. Antithrombotic effect was calculated as a percentage of the experimental animal figure protected from the paralysis of hind legs or death of the mouse that is caused from the administration of platelet aggregation regent. Results: 1. HBCT showed a safety in toxicity of liver. 2. In experiment of anti-thrombotic effect, HBCT inhibited the platelet aggregation induced by ADP and epinephrine, collagen, arachidonic acid as compared with the control group. 3. HBCT inhibited pulmonary embolism induced by collagen and epinephrine (inhibitory rate is 50%). 4. HBCT increased platelet number and fibrinogen amount significantly and also HBCT shortened PT and APTT significantly as compared with the control group in thrombus model induced by dextran. 5. HBCT increased blood flow rate insignificantly as compared with the control group in vivo. Conclusion: These results suggest that HBCT can be used for treating diverse female diseases caused by thrombosis.

The Experimental Study on Antithrombotic Effect of Honghwadangguisan (홍화당귀산(紅花當歸散)의 항혈전(抗血栓) 효과(效果)에 대(對)한 실험적(實驗的) 연구(硏究))

  • Purpose : The Purpose of this research was to investigate the effects of antithromb otic activities of Honghwadangguisan(HDS) Methods : Measured the effect which was given to blood flow rate through the regular volume of glass tube after the blood was diluted five times with ACD soulution. Antithrombotic effect was calculated as a percentage of the experimental animal figure protected from the paralysis of hind legs or death of the mouse that is caused from the administration of platelet aggregation regent. Each of the groups consisted in 8 mice, was divided into Normal, Control, and HBS. All of these 3 group were supplied a saline solution and after an hour the control group brought the dextran extravasated blood. Also the HDS group was dosed to the experimental mice with Oral Zonde one day before the experiment. After that, the mice were abstained from food. And then we gave a measured amount of it before an hour. Finally, it gave rise to dextran extravasated blood as well as the Control group. Results : The results were obtained as follows. In vitro, HDS inhibited platelet aggregation induced by ADP and epinephrine significantly as compared with the control group. HDS showed fibrinolytic activity insignificantly as compared with the control group. HDS reduced blood flow rate in significantly as compared with the control group. In vivo HDS inhibited pulmonary embolism induced by collagen and epinephrine (inhibitive rate 50%). HDS increased number of platelet, fibrinogen amount and shortened prothrombin time, activated partial thromboplastin time significantly but reduced blood flow rate insignificantly as compared with the control group in thrombus model induced by dextran. Conclusion : HDS is effective antithrombotic activity from experimental result.
