한국성인의 사회경제적수준과 혈중 중금속 농도의 융합적 분석 (Convergent association between socioeconomic status and the blood concentrations of mercury, lead, and cadmium in the Korean adult population: based on the sixth Korea National Health and Nutritional Examination Surveys (KNHANES 2013-2015))

  • 본 연구는 사회경제적 상태의 지표인 교육수준 및 소득수준과 수은, 납, 카드뮴의 혈중 중금속 농도간의 관련성을 살펴보고자 하였다. 국민건강영양조사 2013-2015년 자료를 이용하여 성별에 따른 사회경제적상태와 혈중 중금속 농도간의 관련성을 분석하기 위해 로지스틱회귀분석을 실시하였다. 분석결과 한국성인의 교육과 소득수준이 높을수록 혈중 수은의 농도는 증가하는 경향이 나타났고, 혈중 납과 카드뮴의 농도는 감소하는 경향을 보였다 (P for trend <0.001). 혈중 중금속 농도의 유병률이 증가하고 있는 시점에서 사회경제적수준에 따른 건강불평등을 해결하기 위한 국가차원의 공중보건학적 정책이 필요할 것으로 사료된다.

아연(亞鉛)과 철분(鐵分) 동시투여(同時投與)가 백서(白鼠)의 장기내(臟器內) 연함량(鉛含量)에 미치는 영향(影響) (Combined Effects of Iron and Zinc on Accumulation of Lead in Some Organs of Rats)

  • In order to study the effects of iron and zinc on the lead poisoning of rats, lead with iron and zinc, or lead alone were administered orally to a total of 98 adult rats of Sprague-Dawley Species. The concentrations of lead, zinc, and iron were measured by atomic absorption spectrophotometer at every 20 days intervals of 20th, 40th, 60th, 80th and 120th day as a final measurement. Those datas were analysed and compared with those of control groups. The results were summarized as follows; 1. The concentration of lead in blood, bone. and liver tissues kept increasing in case of lead exposure group whereas it started decreasing at 60-80th day when concentration of zinc started increasing in case of combined exposure group. However, in kidney tissue, the concentration of lead in combined exposure group kept increasing up to the end of observation showing special high concentration at the final measurement at 120th day. 2. Concentration of zinc in blood and liver tissues had increased from 60-80th day in case of combined exposure group. 3. Concentration of iron in blood showed decreasing from $44.15{\pm}9.67\;to\;32.44{\pm}2.69{\mu}g/ml$ in case of lead exposure group, whereas it showed constant level of $47.50{\sim}45.65{\mu}g/ml$. However, in liver tissue it kept constant as control did from 40th to 60th days, but from 100th day on it started increasing to show much higher concentration than control.

모 일산화 연 제조 업체 근로자들의 연 폭로에 관한 연구 (Study on the lead exposure of workers in a litharge making industry)

  • In order to investigate the level of lead exposure of workers in litharge making industry and to evaluate how lead exposure, personal habit such as smoking and drinking affect the prevalence of lead related symptoms and other study variables, we investigate 114 workers(24 office workers and 90 lead exposed workers) in a litharge making industry. Study variables chosen were blood lead(PbB), zinc protoporphyrin in whole blood(ZPP), Hemoglobin(Hb), hematocrit (Hct), SGOT and SGPT symptom questionnaires which had 15 lead exposure related symptoms were provided to all workers and filled up by themselves and reconfirmed by physician. The results obtained were as follows; 1. The mean value of PbB, ZPP and SGOT in lead exposed group were higher than those of non-exposed group, and there were no differences of means in other study variables. 2. The smoking and drinking rate of study subjects were 65.8% and 71.0% as a whole. Smoking rates were lower in non-exposed group than exposed group, but drinking rate were not. 3. There were no differences of mean values of study variables between smoker and non-smoker in non-exposed and exposed group, but there was a difference of mean value of SGOT between drinker and lion-drinker in lead exposed group. 4. While the symptom prevalence of lead exposed group were higher in neuromuscular category than non-exposed group, those of non-exposed group were higher or same with exposed group in gastrointestinal and general symptom category. 5. The symptom prevalence of smoker were higher than non-smoker regardless of exposure. 6. The symptom prevalence of drinker were only higher in gastrointestinal symptom category than non-drinker. 7. In multiple stepwise regression analysis of lead related symptoms as dependent variable and blood lead, smoking habit, drinking habit and work duration as independent variables, drinking habit contributed to the gastrointestinal symptom category, whereas blood lead and smoking contributed to the neuromuscular symptom category. For the total symptoms work duration and smoking habit contributed significantly.

여성 근로자의 영양섭취 및 건강상태 조사 : 사무직과 납 사업장 근로자의 비교 (Nutritional and Health Status of Women Workers by Working Fields)

  • The objective of this study was to investigate the nutritional and health status of women industrial workers by working fields. One hundred forty eight (105 lead and 43 office) workers were recruited from March 2005 to October 2005. Information on age, education, smoking and drinking status were collected using questionnaire and nutrient intake and diet quality of workers were assessed by average of two-day 24 hr recall method. Biochemical indexes including blood lead level (PbB), indexes for iron status, serum calcium (Ca) and serum lipid profiles were analyzed from fasting venous blood or serum. Results showed that education level of lead workers was lower than that of office workers (p<0.05), but nutrient intake levels were not significantly different by working fields. Overall nutritional status of the subject were good except for calcium, vitamin $B_2$, C and folic acid intakes. PbB of lead workers were significantly higher than that of office workers while mean corpuscular hemoglobin concentration (MCHC) and serum Ca levels were significantly lower in lead workers. MCHC was positively correlated with zinc intake (r=0.166) and serum Ca was positively correlated with vitamin C intake (r=0.179). This study confirms that lead workers need extra care to keep their health and nutritional management especially for the nutrients known to interact with lead. Tailored nutrition education for workers at specific working fields needs to be more focused for the improvement of health status of industrial workers.

한국인의 연, 망간, 알루미늄 및 실리콘의 혈중 농도 (Blood Lead, Manganese, Aluminium and Silicon Concentrations in Korean Adults)

  • 연, 망간, 알루미늄 및 실리콘에 대하여 직업적 노출이 없는 성인 132명(남성 67명, 여성 65명)을 대상으로 혈중 농도를 정량분석하여 다음과 같은 결과를 얻었다. 평균 혈중 연량(기하평근, 기하표준편차)은 남자군 (3.49, 1.70) ${\mu}g/dL$와 여자군 (3.04, 1.65) ${\mu}g/dL$로서 남자군에서 높았으나 유의한 차이는 없었고, 연령군별로는 남, 여 모두 차이가 없었다. 평균 혈중 망간량은 $0.99{\pm}0.41\;{\mu}g/dL$이었으며, 성별, 연령군별 유의한 차이는 관찰되지 않았다. 평균 혈중 알루미늄량은 $0.59{\pm}0.35\;{\mu}g/dL$이었으며, 성별, 연령군별 유의한 차이는 관찰되지 않았다. 평균 혈중 실리콘량은 남자군 $54.11{\pm}27.64\;{\mu}g/dL$, 여자군 $43.34{\pm}23.51\;{\mu}g/dL$로서 남자군에서 유의하게 높았고(p<0.05), 남자군의 경우 연령이 증가함에 따라 유의한 증가가 관찰되었으나(p<0.05), 여자군의 경우에는 연령군별로 차이가 없었다. 본 연구는 몇 가지 금속의 참고치 설정을 위한 기초 연구의 일환으로 앞으로의 연구에 자료로서 활용함으로써 이들 물질에 의한 인체장해의 평가와 예방관리에 일조 할 수 있을 것으로 생각된다.

일부 농촌 성인 남녀의 일상식이중 납 섭취수준과 칼슘 섭취량, 혈액 수준 및 뇨중 배설량과의 관계 (The Relationship between Lead Intake and Calcium Status in Korean Rural Adult Men and Women on Self-Selected Diet)

  • This study was carried out to estimate the dally intake of lead and the relationship among dietary In take, serum level, and urinary excretion of calcium In 30 healthy adults living In rural area of Korea (12 males and 18 females). Analyses for the nutritional status of the subjects were performed by 3-day dietary intake record, duplicated diet collection, 24-hour urine collection, and venous blood sampling. The results were as follows The mean daily intake of energy was 2,176.3kcal and 1,613.9kcal in males and (tamales of 20∼49years, and 1,914.8kcal and 1,517.9kcal in males and females of 50∼59years, respectively The ratio of carbohydrate : fat : protein was 73.4:13.6:13.0 in males and 76.4:10.3:13.3 In females. The mean daily intake of lead was 277.2Pg in males and 192.0ug in females. The mean daily intake, serum level, and urinary excretion of calcium were 491.1mg, 8.9mg/dl, and 80.7mg in males and 426.7mg, 8.8mg /dl, and 80.3mg in females, respectively. No significant correlation was found between the daily lead intake and the dietary intake, serum level, and urinary excretion of calcium. It was concluded that the daily lead intake was lower than the acceptable daily lead intake of FAO/WHO but a little higher than that of other investigations. And the daily lead intake was not level that relate to calcium status seriously.

연 노출 근로자들의 ALAD genotype에 따른 연 노출지표 및 증상과의 관련성 (Relationship on the lead exposure indices and symptoms by ALAD genotype in lead worker)

  • A cross-sectional study was performed to evaluate associations between lead biomarkers, lead-related symptoms, and ${\delta}$-aminolevulinic acid dehydratase (ALAD) genotype among 598 lead workers and 144 control office workers in storage battery industries, secondary smelting and litharge making industries. Lead inhibits the second enzymes, ALAD, in the heme synthesis pathway. ALAD gene, which codes for one of three isozymic proteins (termed ALAD1-1, ALAD1-2, and ALAD2-2), seems to modify the toxicokinetics of lead. The result as follows; The percents of total workers whose genotype of ALAD1-1 and ALAD1-2 were 88.4% and 11.6%, respectively. The zinc protoporphyrin in blood (ZPP) and ${\delta}$-aminolevulinic acid in urine (ALAU) of lead workers with ALAD1-2 were significantly lower than those of lead workers with ALAD1-1, but there were no significant difference between two genotype for blood lead, age, and work duration. The proportion of ALAD1-2 genotype in control office workers was 13.2%. The proportions of ALAD1-2 genotype of lead workers were 14.0%(their mean air lead level below $0.024mg/m^3$), 10.4%($0.025-0.049mg/m^3$), 11.8%($0.050-0.099mg/m^3$), and 9.4%(above $0.100mg/m^3$), respectively. In the logistic analysis of 15 lead related symptoms, 'arthralgia'(S7) symptom of ALAD1-2 was significantly lower (OR=0.481; 95% CI=0.248-0.932) than that of ALAD1-1, but 'feeling of irritation'(S11) of ALAD1-2 was significantly higher(OR=1.636; 95% CI=1.035-2.586) than that of ALAD1-1 after controlling possible confounder (blood lead, work duration, smoking and drinking habit).

고양이의 납중독에 관한 실험적 연구 1. 임상증상 및 실험실적 평가 (Experimental Studies on Lead Toxicity in Domestic Cats 1. Symptomatology and Diagnostic Laboratory Parameters)

  • 고양이에서 납의 중독량을 결정하고 행동과 임상병리학적인 변화를 밝히고, 납 투여용량과 혈중 납농도와의 관계를 규명하기 위하여 42마리의 고양이를 대상으로 체중에 따라 0(대조군), 10, 100(저용량), 1000, 2000, 4000(고용량) ppm의 lead acetate를 경구적으로 투여하여 납독성을 평가하였다. 고투여용량(1000, 2000, 4000ppm)을 투여한 어떤 고양이는 분출성 구토, 활동항진 그리고 발작을 보였다. 모든 실험군에서 성장율은 변화가 없었다. 고양이의 정상 혈중 납농도는 사람, 개 그리고 소의 혈중 납농도 보다 낮았다. 고양이에서 준임상형 납중독의 혈중 납농도는 3~20$\mu\textrm{g}$/100$m\ell$이었고, 임상형 납중독의 혈중 납농도는 20~l20$\mu\textrm{g}$/100$m\ell$이었다. Zinc protoporphyrin 농도는 납 투여용량에 비례하였으며 50$\mu\textrm{g}$/100$m\ell$ 이상으로 ZPP농도가 유의성 있게 증가하는 경우는 임상형의 납중독을 의미하였다. Aminolevulinic acid dehydratase는 모든 납 투여용량에 역비례 관계를 보였고, 고양이의 납노출에 대한 유용한 진단적 지표로 나타났다. 오줌의 aminolevulinic acid 농도는 대개 납투여용량에 따라서 증가하지만 측정치는 개체에 따라 다양하였다. 피모의 납농도는 납투여용량에 따라 비례적으로 증가하였다. 최소한 고용량에서 납은 PMN세포와 단핵구의 화학주성을 억제하는 것으로 나타났다. 헤모글로빈, 적혈구, 백혈구, 호중구, 임파구, 단핵구 그리고 호산구에서는 투여용량에 대한 반응이 일정하게 관찰되지 않았고 또한 총단백, 혈장단백, BUN그리고 ALT치에서의 용량에 따른 일정한 변화가 없었다. 망상적혈구수는 대부분의 납투여용량수준에서 유의성 있게 증가하지 않았기 때문에 고양이 납중독의 진단적 가치는 거의 없었다. 신경전달속도의 유의성 있는 변화가 없는 것으로 보아 납섭취로 인한 대상성 탈여(Segmental demyelination)는 없는 것으로 생각된다. 고양이의 치사량은 체중 kg당 60~150mg이었다. 납중독 진단에 신뢰할 수 있는 파라미터는 혈중 납농도, ZPP. ALAD 그리고 피모의 납농도이었다.

주의력 결핍 과잉 행동장애를 종점으로 하는 혈중 납의 벤치마크 용량 하한 도출: 두 동집단 자료의 병합 (BMDL of blood lead for ADHD based on two longitudinal data sets)

  • 우리나라 환경부에서는 지난 2005년과 2006년, 매 2년마다 세 번에 걸친 추적 조사를 하여 두 개의 독립적인 자료를 구축하였고, 2010년에 두 자료를 병합하여 Children's Health and Environmental Research (CHEER) 자료라 명명하였다. 본 연구는 CHEER 자료를 이용한 Kim 등 (Journal of the Korean Data and Information Science Society, 25, 987-998, 2014)과 Lee 등 (The Korean Journal of Applied Statistics, 29, 1295-1310, 2016)의 후속 연구로서, 두 동집단을 병합한 자료에서도 기존 연구에서 보고되었던 결과가 재현되는지 확인하고 ADHD를 종점으로 한 혈중 납 농도의 95% 신뢰 하한 (BMDL) 도출하였다. Lee 등 (2016)에서 제시한 벌점화 스플라인 모형을 이용하여 모든 년도의 ADHD 척도를 통일하였고, 경시적 자료의 특성과 아동의 나이에 대한 평균으로의 회귀 현상을 반영하여 임의 기울기 모형과 AR(1) 모형을 구축하였다. 두 모형을 바탕으로 혈중 납 농도의 벤치마크 용량 하한을 공식과 모의실험을 이용하여 도출한 결과, 종전의 연구보다 작은 벤치마크 용량의 분산 값으로 인하여 벤치마크 용량 하한 값이 증가한다는 사실을 확인할 수 있었다.

환약으로 제조된 한약에 의한 급성 납중독 2례 (Two Cases of Lead Poisoning due to Herb Medicinal Pills)

  • 납중독은 가벼운 빈혈에서부터 경련을 동반한 신경장애에 이르기까지 다양한 임상증세를 나타내며, 뼈나 연부조직에 침착되어 비가역적인 장애를 남기므로 임상적으로 중요하다. 특히 소아는 소화관을 통한 흡수율이 높고 배설율이 낮아 납중독의 영향에 예민한데 혈중 농도와 개인차에 따라 심한 복통 및 지능저하, 경련 등의 비가역적인 신경계장애를 남길 수 있어 조기진단 및 치료가 중요하다. 이에 저자들은 살빼는 목적으로 비전문가에게 처방받은 환약으로 제조된 한약을 복용하여 빈혈을 동반한 심한 복통을 주소로 내원한 형제들에게 자칫 출혈성 장질환으로 여기고 지나칠뻔 하였으나 환약복용의 과거력을 단서로 혈중 납농도 및 환약의 납 성분 분석을 통해 확진한 2례를 경험하였기에 이를 문헌 고찰과 함께 보고하는 바이다.
