A Study on the Performance and Utilization of Results of Self-monitoring of Blood Glucose in Type Ⅱ Diabetes (일 병원 제2형 당뇨병환자의 자가혈당검사 수행 및 활용 실태)

  • Purpose: This study was conducted to investigate performance of self-monitoring of blood glucose and the utilization of results for people with type II diabetes. Method: Data was collected from 177 patients with type II diabetes using a questionnaire from August to September, 2009. Result: Most participants performed self-monitoring of blood glucose in the correct way while some misused the lancet or test strips. In the utilization of results for self-monitoring blood glucose data, 62% of participants always recorded the data, 46% always understood the cause for hyperglycemia or hypoglycemia, 25% changed their diet, 23% regulated their exercise-amount, and 11% of participants adjusted the drug dosage. There were significant differences in the performance of self-monitoring of blood glucose and the utilization of results according to the characteristics in the self-monitoring subjects such as awareness of HbA1c, target fasting glucose level, target fasting glucose level 2 hours after meal etc. Conclusion: Systemic self-monitoring of blood glucose education which includes the utilization of self-monitoring of blood glucose needs to be developed for type II diabetic patients.

Effect of Sulfonylureas Administered Centrally on the Blood Glucose Level in Immobilization Stress Model

  • Sulfonylureas are widely used as an antidiabetic drug. In the present study, the effects of sulfonylurea administered supraspinally on immobilization stress-induced blood glucose level were studied in ICR mice. Mice were once enforced into immobilization stress for 30 min and returned to the cage. The blood glucose level was measured 30, 60, and 120 min after immobilization stress initiation. We found that intracerebroventricular (i.c.v.) injection with $30{\mu}g$ of glyburide, glipizide, glimepiride or tolazamide attenuated the increased blood glucose level induced by immobilization stress. Immobilization stress causes an elevation of the blood corticosterone and insulin levels. Sulfonylureas pretreated i.c.v. caused a further elevation of the blood corticosterone level when mice were forced into the stress. In addition, sulfonylureas pretreated i.c.v. alone caused an elevation of the plasma insulin level. Furthermore, immobilization stress-induced insulin level was reduced by i.c.v. pretreated sulfonylureas. Our results suggest that lowering effect of sulfonylureas administered supraspinally against immobilization stress-induced increase of the blood glucose level appears to be primarily mediated via elevation of the plasma insulin level.

  • In order to investigate the effect of lowering blood glucose level, 27 medications of Sagal part in Donguibogam and the contraindicated medications to Sogal including Ijintang, Dodamtang, Chukdamtang and other six medications which were used to treat Diabetes in the 6th I.M of Kyung Hee Oriental Medical Center were given to the Alloxan induced diabetic mouse and blood glucose level were observed in the 2nd and 4th day. The result of this stqdy are as follows: 1. The 17 medicatlons(63% of 27 medicine) of Sogal part decreased' the glucose level significantly.(Singihwan, Baekhotang, Jowiseunggitang, Yukamijihwangwon, Palmihwan, Kamijeonssibaekchultang, Insamsukgotang, Sacngjinyanghyultang, Insambokryungsan, Hwalhyulyunjosaengjineum, Chungsinbogitang, Hwanggitang, Hwangryunjihwangtang, Ojeupokchunhwan, Yongbongwon, Joosahwangryunwon, Kagambaekchulsan) 2. Dry herbs like Banha, Namsung are the contraindicated herbs in the Sogal part of Donguibogam but Ijintang and Dodamtang which include those herbs also decreased the blood glucose level significantly. 3. The three medications-Bangpungtongsungsan, Bohyulansintang, Bogantang from the six me dications which were used to treat Diabetes in the 6th I.M. of Kyung Hee Oriental Hospital also decreased the blood glucose level significantly.

The Effects of the Application of a Glucose Control Protocol on Glycemia and Glucose Variability in Critically Ill Cardiothoracic Surgery Patients (혈당 조절 프로토콜 적용에 따른 흉부외과 중환자의 혈당 조절 상태와 혈당 변동)

  • Purpose: The study sought to determine the state of blood glucose control, and the consequent clinical effects and variation in blood glucose level, of adult patients admitted to intensive care units following cardiothoracic surgery by comparing the blood glucose levels before and after the application of a blood glucose control protocol. Methods: The protocol was developed by modifying and supplementing the Yale protocol, and was first used in 2012. The resulting blood glucose data of an experimental group (n = 314), to which the blood glucose control protocol had been applied, and a control group (n = 347), whose blood glucose levels had been controlled according to physicians'prescriptions without the protocol, were collected through the medical records. Results: The target blood glucose ratio increased significantly in the experimental group, and the low blood glucose ratio decreased significantly in the experimental group. The two groups exhibited a significant difference (p < .001) in the degree of variation in the blood glucose levels. The duration of the use of a ventilator was significantly reduced in the experimental group (p < .001). Conclusion: It is expected that the protocol can be used for the safe and effective control of critically ill cardiothoracic surgery patients' blood glucose levels.

Supplementary effect of the rice germ oil on blood glucose in Diabetic KK Mice (쌀눈기름의 급여가 KK 당뇨 마우스의 혈당 수준에 미치는 영향)

  • This study was carried out to investigate the supplementary effects of the rice germ oil compared with soy bean oil on blood glucose level of non-insulin dependent diabetic mice. Forty diabetic KK mice were fed two kinds of experimental diets with 20% lipid from soy bean oil as a control(CO) and rice germ oil(RG) for 8 weeks, respectively. Diet intake, body weight, organs weights and lipids levels of serum, liver and feces were measured. There was no significant difference in food and water intake, body weight gain and organs weights between experimental groups. The concentrations of fasting and random blood glucose were similar between CO and RG groups. There was no significant difference in blood glucose levels after glucose treatment during the glucose tolerance test between two groups. The levels of $HbA_{1c}$ as the index of blood glucose status, and insulin were similar in two groups. These results suggested that rice germ oil can't reduce blood glucose concentration of non-insulin dependent diabetic mice compared with soybean oil. But we need to investigate the hypoglycemic effect of rice germ oil by changing supplementary level and period.

Relationship of Glucose Control and Wound Infection in Diabetics after Lumbar Spine Surgery

  • Objective: The purpose of this study is to investigate the elevated blood glucose levels in the postoperative period are associated with an increased risk of deep wound infection in diabetic individuals undergoing lumbar spine surgery. Methods: Of 2896 patients who underwent lumbar spine operations by one surgeon between 1993 and 2002, 329(11.4%) were diabetics. The rate of deep wound infections in diabetic patients was 6.4%, versus 3.2% for nondiabetics. 152 patients had their operation before implementation of the protocol and 177 after implementation. Charts of the diabetic patients were reviewed. Mean blood glucose levels were calculated from documented results of finger-stick glucometer testing. Results: Twenty-one diabetic patients suffered deep wound infection. Infected diabetic patients had a higher mean blood glucose level through the first 2 postoperative days than noninfected patients($230{\pm}6.9$ versus $175{\pm}3.8mg/dL$; p<0.003) and had a long operation time($216{\pm}57.9$ versus $167.5{\pm}42.2$ minute; p<0.05). Multivariable logistic regression showed that mean blood glucose level for the first 2 postoperative days, long operation time, and use of the instrumentation(p<0.02) were all related predictiors of deep wound infection. Institution of a protocol of postoperative continuous intravenous insulin to maintain blood glucose level less than 200mg/dL was began in september 1997. This protocol resulted in a decrease in blood glucose levels for the first 2 postoperative days and a concomitant decrease in the proportion of patients with deep wound infection, from 8.3%(11/132) to 5.1%(10/195) (p<0.02). Conclusion: The incidence of deep wound infection in diabetic patients is reduced after implementation of a protocol to maintain mean blood glucose level less than 200mg/dL in the immediate postoperative period.

  • This study was performed to explore the effects of walking exercise on blood glucose level in the diabetes mellitus patients. The design of this study was a equivalent pre-post test experiment. The subjects consisted of eighteen diabetes mellitus patients. The walking exercise consists of 22.5minutes, speed of 120walking per minute. Data were analyzed with mean, standard deviation, Wilcoxon signed rank test, Spearman correlation and percentage using SAS program. The results were as follows. 1. The blood glucose level has significantly decreased after walking exercise. 2. Strength of exercise and change of blood glucose level was revealed correlation. 3. Age and strength of exercise was revealed high correlation. 4. Satisfaction after walking exercise was revealed all patients. In conclusion, the walking exercise can decrease blood glucose level. This exercise was show a positive effect on the exercise compliance.

  • Blood glucose level은, 간에 있어서의 glucose 생산과, 말초조직(뇌, 근육, 지방등)에 있어서의 glucose의 흡수 사이의 balancing에 의해 결정된다. 정상적으로는, 췌장에서 분비되는 인슐린(insulin)의 작용으로 이 균형 상태가 매우 안정적(80-85mg/㎗)으로 유지되게 된다. 즉 음식물 섭취후의 blood glucose level이 급격한 증가를 보이게 되면, insulin은 말초 조직에서의 glucose의 흡수를 촉진시켜 결과적으로 glycogen 합성을 증가시키는 한편, 간에서의 glycogenolysis와 gluconeogenesis를 저해하므로써, 이들 상승된 혈당치를 낮추게 하는 것이다. 이러한 작용은 안정된 혈당 농도를 회복시키는 이른바 glucose homeostasis를 유지하게 해 준다.

Effects of Aloe vera Treatment on Blood Glucose Level and Clinical Chemistry in Diabetic Patients (당뇨병 환자의 혈당 및 임상에 미치는 알로에의 효과)

  • In order to study the effects of Aloe vera treatment on blood glucose level and clinical chemistry in diabetic patients, eight diabetic patients were administered orally with 800 mg of Aloe vera three time a day for three months. The high levels of blood and urine glucose in diabetic patients were significantly reduced by administration of Aloe vera. The increased plasma triglyceride concentration was also significantly reduced by Aloe vera treatment. A little amount of urine bilirubin, hematuria, nitrite, urobilinogen, protein and ketone bodies were detected before treatment, but not detected after Aloe vera treatment. But other blood parameters of clinical chemistry values were not affected by Aloe vera treatment. These data suggest that Aloe vera can be effective in the treatment of the diabetic patients.
