Study According to Hematological Analysis of Sasang Constitution (사상체질의 혈액학적 분석에 따른 고찰)

  • This study was to analyze the difference between blood type and blood constitution according to Sasang constitution. The subjects were 302 men who visited blood donation house. Data collection was performed by using questionnaire that included three types of Sasang constitution. The data were analyzed as frequency, percentage, ${\chi}$2-test and ANOVA using SPSS 10.0 WIN Program. There was significant difference statistically in frequency of blood type as per Sasang constitution(${\chi}$2 =53.254, p=.000). Soyangin got most number in B type(34.7%), O type(26.7%), A type(21.8%), AB type(16.8%) in order. Taewumin got most number in O type (45.8%), A type(31.3%), B type(22.1%) and AB type(0.8%) in order, and in case of Sowumin, most in A type(54.3%), B type(24.3%), O type (11.4%) and AB type(10.0%) in order. In weight, height, fatness, hemoglobin, Hematocrit, platelet, ALT, AST and cholesterol, it appears that there is significant difference according to Sasang constitution. If proper regimen according to Sasang constitution by grasping the special features as per Sasang constitution through analyzing the difference as per Sasang constitution based on the results of above study is applied.

  • Blood products (BPs) can only be obtained through blood donation and hence represent a finite resource. BPs should therefore be used conservatively. However, BPs are being used indiscriminately without evidence. The purpose of this study was to evaluate the reasons for the use of BPs and their appropriateness. The investigation was carried out based on hemoglobin levels. Data were obtained from Nov 1, 2020, to Oct 31, 2021, from a hospital's OCS/EMR systems. The BPs were dispensed in 21,303 cases, and the number of hemoglobin levels >7.0 g/dL or higher among red blood cell drugs used by each treatment department was 1,173 (>7.0 g/dL). The misuse of blood transfusions is increasing social costs, with the adequacy of transfusion becoming increasingly important. Hence, each medical institution should review the transfusion guideline evaluation index, check the status of the release of BPs, and institute educational programs covering transfusion guidelines and continually evaluate their adequacy.

Autologous Transfusion in Pregnant Women with Significant Risk for Hemorrhage (출혈위험 임산부에서의 자가수혈)

  • Autologous transfusion, storage of one's own blood for subsequent infusion if needed, is safe and effective in a variety of scheduled operative procedures. Obstetric involvement in such programs is very limited, however, because of concern over the possibility of inducing premature labor or causing fetal distress by blood volume change or vasovagal reactions. We describe our experience with pregnant women in this program. The incidence of vagovagal reactions of autologous donation was 9.5% (2/21). After entry into this program, 17pastients received a total 37pints, which consist of 19 Autologous and 18 Homologous. Homologous transfusion was avoided in 30% of patients receiving blood. The values of the mean haematocrits before and after hpebotomy were 34.1 % and 31.8 % respectively. It was statically significant(p<0.01). We recommended that autologous blood donation by pregnant women in third trimester is safe for mothers or infants and it should be strongly encouraged for patient with placenta previa and repeated cesarean section.

Discomfort of Donors associated with Bone Marrow Donation (골수공여자들의 불편감)

  • The study was designed to provide the fundamental information for understanding discomfort of bone marrow donors and for promoting an individual comfort by comparing the difference on discomfort between unrelated donors and related donors. The subject of the study was fifty related donors and thirty unrelated donors who was in the C University Hospital. This survey had been carried out and collected from October, 1998 to March, 1999. The scale of discomfort of donors associated with bone marrow donation were assessed by the questionnaire deviced by Kim Sang Dol and amended by the researcher. Data was analyzed by $x^2-test$, t-test, two-way ANOVA, and Pearson Correlation Coefficients. The results were as follows. 1. Considering the general characteristics of bone marrow donors according to gender, male was consisted of $60\%$ and female was consisted of $40\%$. Of those related donors are consisted for $62.5\%$ and accounted for $37.5\%$ of those unrelated donors. Considering the classification to the job, employee of company were major donors which was $35\%$, and next order was student, individual businessman, and housewife. Considering the education level. college students were $48.7\%$ and students who have less than high school level were $42.5\%$. 2. According to the above the results regarding discomfort of bone marrow donation, it is especially shown that the major cause for discomfort of bone marrow donors is on physical factor. The concrete examples for physical factor are pain in the region of bone marrow harvest and pain in the injection part by fluid therapy and blood-sampling, an immovability of the body after bone marrow harvest, and difficulties on walking. Considering physiological factor, there are an uneasiness about leading to injure their health, vague fear about the hospital. and a tedium at hospital. Environmental factors for discomfort of bone marrow donors are insufficient explanation for needle gauge and procedure of bone marrow donation and difficulty on following medical schedule. Therefore. it is necessary to establish more effective and systematically organized program for nursing intervention based on the research results. An effective program is only useful in getting rid of discomfort of bone marrow donors.

Open Heart Surgery without Transfusion (수혈 없이 시행한 개심술)

  • Background: Although complications from transfusion are known to happen, transfusion is performed during most open heart surgeries. The aim of this study was to investigate the possibility of performing cardiac surgery without allogenic blood transfusion. Material and Method: Between January to August 2007, 44 consecutive patients who underwent open heart surgery with using various blood conservation methods were retrospectively enrolled. They were divided into group I (the onpump group, n=17) and group II (the offpump group, n=27). The blood conservation methods were intraoperative autologous donation, cell saver, retrograde autologous priming, conventional ultrafiltration and modified ultrafiltration. Antianemic agents were administered to all the patients postoperatively. We analyzed the possibility of bloodless operations, the causes of homologous transfusion, the serial change of the hematocrit and the postoperative chest tube drainage, and we compared the results between the two groups. If comparison between the two groups was not reasonable, then we compared two groups with the individual control groups I and II (49 patients) in 2006. Result: 40 (90.9%) of 44 patients were successfully operated on without transfusion and the success rate was 88.2% (15/17) for group I and 92.6% (25/27) for group II. There was no statistical difference between the two groups (p=NS). The causes of transfusion were 2 cases of postoperative bleedings, 1 case of intraoperative bleeding and 1 mistake of the indication for transfusion. There was no statistical difference of the total chest tube drainage (Group I: $417{\pm}359mL$, Group II: $451{\pm}237mL$) (p=NS), but the total chest tube drainages of the two groups were less than each of the control groups 1 and II (p<0.05). The lowest hematocrit level of Group I was $16.4{\pm}2%$, and this occurred just after infusion of cardioplegics and the hematocrits of both groups were recovered to the preoperative level at 2 months postoperatively. Conclusion: In this study, bloodless open heart surgery could be performed in 90.9% of the patients with intraoperative autologous donation, cell saver, retrograde autologous priming, conventional ultrafiltration and modified ultrafiltration. A combination of various blood conservation methods is the most important and bloodless cardiac surgery could be performed with meticulous bleeding control and strictly following the transfusion indications.

Epidemiological Characteristics of HIV Infected Homosexuals In Korea (1992) (국내 HIV감염 동성연애자들의 역학적 특징 (1992년))

  • In order to investigate psychological and behavioral characteristics of homosexuals and to present evidence that homosexuals are in danger of HIV infection in Korea, this study was done by self-administered questionnaire and then direct interview with the 28 (35%) HIV infected homosexual/bisexuals of 79 HIV infected persons reported in 1992. Homosexuals without heterosexual activity were 9 and the others were bisexuals. Sixty-five percent of respondents had a guilty conscience for their homosexual activty. Twenty (71%) were in twenties and 5 (18%) in thirties. Twelve(43%) were detected via health card checking by health office, 21% by blood donation, 18% by hospital visit, and 7% by partner notification. Motivations for homosexual activity were curiosity (36%), temptation or recommendation (14%) and compulsion (11%). Eighteen (72%) never used condom on anal sex. Nine of 26 respondents had experience for anal sex with foreigners. Fourteen (54%) of 26 respondents had history for sexually transmitted diseases. Fighty percent did not have sexual contact after HIV infection and the others usually used condom. It was confirmed that over 57% of the respondents were infected within 1 year before HIV diagnosis and over 82% within 2 years. These data suggest that HTV infection among homosexual group is rapidly spreading.

  • Background: Research on next-generation sequencing (NGS)-based HLA typing is active. To resolve the phase ambiguity and long turn-around-time of conventional high resolution HLA typing, this study developed a NGS-based high resolution HLA typing method that can handle large-scale samples within an efficient testing time. Methods: For HLA NGS, the condition of nucleic acid extraction, library construction, PCR mechanism, and HLA typing with bioinformatics were developed. To confirm the accuracy of the NGS-based HLA typing method, the results of 192 samples HLA typed by SSOP and 28 samples typed by SBT compared to NGS-based HLA-A, -B and -DR typing. Results: DNA library construction through two-step PCR, NGS sequencing with MiSeq (Illumina Inc., San Diego, USA), and the data analysis platform were established. NGS-based HLA typing results were compatible with known HLA types from 220 blood samples. Conclusion: The NSG-based HLA typing method could handle large volume samples with high-throughput. Therefore, it would be useful for HLA typing of bone marrow donation volunteers.

A Study on the Decrease of Platelet Count, Yield, and Efficiency after Plateletpheresis (Haemonetics MCS 3p에 의한 혈소판 성분채혈 후 혈소판 감소율, 회수량 및 효율에 대한 연구)

  • The purposes of this study were to evaluate the changes in hematologic indices after plateletpheresis and to identify the preapheresis platelet count and clinical factors (age, gender, height, and weight) that showed some influence on the percentage of platelet decrement, yield and efficiency. Plateletpheresis was performed on 101 healthy donors in Bundang CHA general hospital. The data was analyzed using the SAS program with t-test, ANOVA test and Multiple regression. The mean percentage decrease after plateletpheresis was 2.0% in hemoglobin, 1.8% in hematocrit, and 29.7% in the platelet count, while a WBC count showed an increase of 2.6%. The mean percentage decrease of hemoglobin and hematocrit were 1.7% and 1.4%, in males and 3.6% and 3.7% in females, respectively. Particularly the percentage decrease of platelet count was significantly higher in females (40.0%) than in males (27.2%). The platelet decrementage and yield were significantly higher in females, but the efficiency did not differ significantly between males and females. The yield showed the lowest levels in subjects who were 40 years old or over but the platelet decrement and efficiency did not change according to age. The platelet decrement increased as height and weight increased. Also, the platelet decrement and yield increased as the initial platelet counts increased, but the efficiency did not. From multiple regression analysis, the platelet decrement was associated with gender, weight, and initial platelet count. The yield was related to the initial platelet count, but the efficiency was not related to gender, age, weight, height or initial platelet counts. This study has a limitation of the generality of the study results since this study was conducted only in a single university hospital. Further study would be necessary to find out a subpopulation that is sensitive to the hematologic change after plateletpheresis, and to determine the standard criteria for blood donation based on the subpopulation.

Recognition and Attitudes on DNR of College Students (Focused on Comparison between Nursing and Non-Health Department College Students) (DNR에 대한 대학생들의 인식 및 태도(간호대학생과 비 보건계열대학생 비교를 중심으로))

  • The object of this descriptive survey research was to provide basic information source for building objective standards of DNR (Do Not Resuscitate) that can be clinically applied, by analyzing college students' awareness and attitude toward DNR. The participants of the study were 1,267 students from one college of Daegu, South Korea. The structured survey questionnaire was used for data collection, and the survey was conducted from 1-31 July, 2010. The error and percentage was estimated by SPSS 17.0 program, and analyzed with $x^2$-test. As a result of comparing the nursing students' and non-health care major students' awareness and attitude toward DNR, the significant differences were found in the necessity of DNR, reason for supporting DNR, reason for opposing DNR, and DNR decision-maker, among the awareness dimension; among the attitude dimension, significant differences were found in implication of family DNR and self-DNR. Comparing the nursing students' and non-health care major students' awareness toward DNR related information provision, researchers have found significant differences in the necessity of giving information on DNR, timing of the DNR information provision, result of the DNR-related information provision, and guidelines for the DNR information provision. In terms of the difference in DNR's necessity recognition by the demographic information, the significant differences existed based on the religion and the history of blood donation; in terms of the differences in attitude toward DNR decision-maker, the differences were found on the religion and the number of siblings. For the attitude toward family member's DNR, the significant differences existed for the sex, age, economic status, religion, the number of siblings, the history of familial illness and death, and experience of blood donation; the attitude toward the DNR for the self was significantly differed by the sex, economic status, the number of siblings, and the history of familial illness and death. To establish the standards for DNR based on the study, we suggest more well-designed future studies.
