Purpose: This study was done to verify the effects of a self-regulation program for management of hypertension. Method: Thirty patients with hypertension registered in a community health center were selected as the experiment group, and control group were patients in another community health center, matched for age and gender. The self-regulation program included daily blood pressure checks, periodic counseling, and health education. A self-check digital device with instructions was provided for self-monitoring of blood pressure, and the participants were interviewed before they took part in the program. Results: The first hypothesis was supported: There will be a greater reduction in both systolic and diastolic blood pressure for patients with hypertension who participate in the self-regulation program compared to patients in the control group. The second hypothesis was also supported: Patients with hypertension who participate in the self-regulation program will perform self-care activities better than those in the control group. Conclusion: The findings indicate that a self-regulation program reduces systolic and diastolic blood pressure and improves self-care in patients with hypertension. It is recommended that this self-regulation program be used in community health clinics for management of hypertension and prevention of complications.
This study was to investigate the effect of calcium supplementation on the reduction of blood pressure in normotensive young adults. Fortyseven healthy college students(23 male and 24 female) were divided into Ca and placebo groups, and were orally given with calcium(1g/day, 2.5g as CaCO3) and placebo, respectively, for 20 weeks. Blood pressure was measured by Korotokoff method in seated position every two weeks. Average daily dietary calcium intakes of the subjects were 626-643mg in men and 513-552mg in women. Average initial level of serum calcium of the subjects belonged to normal range. 1. Both systolic and diastolic blood pressure(SBP and DBP) of Ca group showed significant continuously decreasing tendency from 6-8 weeks until final 20 weeks of the supplementation in both men and women. But placebo groups did not. Comparing with the basal value, reduction of SBP and DBP after 20 weeks were 6.53$\pm$4.30%(8.9mmHg) and 8.10$\pm$3.30%(7.4mmHg) in men, and 6.56$\pm$2.41%(8.1mmHg) and 7.33$\pm$3.75%(6.2mmHg) in women. The blood pressure lowering effect of calcium supplementation seemed to be greater in the subjects with higher basal SBP. 2. Serum calcium was significantly increased by calcium supplementation in both men and women, and showed significant negative correlation with SBP(r=-.213) and DBP(r=-.301) in women. Serum Ca/Mg ratio of Ca group was also elevated and showed significant negative correlation with SBP(r=-.174) and DBP(r=-.194) in total subjects. 3. Urinary excretion of Na did not show any significant changes by calcium supplementation in both men and women and showed no correlations with blood pressures.
Objectives : In order to find a possible non-invasive manipulation tool for maintenance of the cardiovascular functions in hemorrhagic shock, this study was aimed at evaluating effects of acupoints acupressure on the changes in blood pressure and heart rate from an animal model of hemorrhagic shock. Methods : In adult Sprague-Dawley rats, hemorrhagic shock was induced by a withdrawal of arterial blood from the femoral artery with volume of 0.8 ml per 100 g of body weight using peristaltic syringe pump. We applied the acupressure with a pressure oscillator to tail as a control and 2 different acupoints of sobu(HT8), youngchun(KI1) under 3 different conditions : 1) normal arterial blood pressure without bleeding, 2) at the beginning of bleeding, and finally 3) hemorrhagic shock. Results : Under normal arterial blood pressure without hemorrhage, there was a significant increase in systolic and diastolic blood pressures by the acupressure to the tail, HT8 and especially KI1 for 30 sec compared with before acupressure. Under hemorrhagic shock condition, the tail acupressure had minimal changes in cardiovascular parameters. Either the HT8 or KI1 acupressure resulted in a significant increase in arterial pressure but did not heart rate. At the beginning of bleeding, tail acupressure failed to change the reduction of arterial pressure and heart rate. However, there was a significant increase in blood pressure and heart rate following either the HT8 or especially KI1 acupressure. Conclusions : HT8 and KI1 acupressure affected cardiovascular signs but tail acupressure did not in rat model of hemorrhagic shock. These experimental data suggest that a acupressure with a pressure oscillator to HT8 or KI1 can be one of alternative emergency manipulations to ameliorate compromised cardiovascular functions under hemorrhagic shock condition.

In this paper, we discuss a method for design of an ambulance stretcher which call decrease blood pressure fluctuation caused by ambulance acceleration. Recently, a lot of stretchers which can isolate the vertical vibration to reduce body resonances (4-10 Hz) have been used during ambulance transport. However, we have found that blood pressure of a patient laying in the stretcher fluctuates when the ambulance accelerates or decelerates. Since the enforced change of the blood pressure may deteriorate the patent's condition, a stretcher to cancel head-to-foot acceleration and to decrease the blood pressure variation (BPV) is expected for safe transport. We propose a method to design a stretcher which is tilted according to an adequate angle to cancel head-to-foot acceleration by gravity when the ambulance accelerates or decelerates. A control method of the stretcher is constructed by means of simulation analysis using acceleration data measured during ambulance transport. It is confirmed that the active controlled stretcher proposed has good performance for the BPV reduction.

Hypotension is one of the potential causes of dizziness. In this review, we summarize the studies published in recent years about the electrophysiological and pharmacological mechanisms of hypotension-induced dizziness and the role of the vestibular system in the control of blood pressure in response to hypotension. It is postulated that ischemic excitation of the peripheral vestibular hair cells as a result of a reduction in blood flow to the inner ear following hypotension leads to excitation of the central vestibular nuclei, which in turn may produce dizziness after hypotension. In addition, excitation of the vestibular nuclei following hypotension elicits the vestibulosympathetic reflex, and the reflex then regulates blood pressure by a dualcontrol (neurogenic and humoral control) mechanism. In fact, recent studies have shown that peripheral vestibular receptors play a role in the control of blood pressure through neural reflex pathways. This review illustrates the dual-control mechanism of peripheral vestibular receptors in the regulation of blood pressure following hypotension.

Lead (Pb) is a metal that is generally considered to be toxic to the cardiovascular system. Pb-exposed animals display the evidence of increased oxidative stress and hypertension. The current study was designed to examine whether Korean red ginseng (KRG) has protective effects against Pb-induced hypertension and oxidative stress in Pb-exposed rats. Male Sprague-Dawley rats were randomly assigned to Pb exposure or control groups. KRG was administered in drinking water at a concentration of 100 mg/kg/day; the control group received plain drinking water. Animals in the Pb-exposed groups were provided with drinking water containing 100 ppm Pb acetate for 12 weeks. Blood pressure, plasma glutathione, blood Pb concentration, and hematologic data, such as red blood cell quantity, were determined. Pb poisoning was assessed by measuring the blood Pb concentration. Pb exposure (100 ppm) for 12 weeks resulted in a marked rise in systolic blood pressure and blood Pb concentration, as well as a significant reduction in plasma glutathione levels and red blood cell quantity. Other measurements, such as heart rate, body weight, and white blood cell quantity, were unchanged. Treatment with KRG significantly lowered blood pressure, raised plasma glutathione and increased red blood cell numbers in Pb-exposed animals; it also had no effect on heart rate, body weight, or white blood cell quantity. However, the elevated blood Pb concentration was not reduced by treatment with KRG (100 mg/kg). Taken together, these data indicate that treatment with KRG in Pb-exposed animals can reduce oxidative stress and lower blood pressure, suggesting that KRG might be protective against Pb-exposed hypertension and oxidative stress.
Increasingly nursing science is embracing the concepts and methodology derived from psycho-neuroimmunology. It has been previously shown that stress increases and immune function declines in students undergoing examinations. To date, however, no many studies have been reported on stress levels, immune function and interventions in Korean students undergoing their first clinical nursing rotation. It was proposed that nursing students during their first clinical rotation experience increase in stress because of the novelty of the situation and their lack of clinical knowledge. It was also hypothesized that biofeedback and progressive relaxation, methods of self-regulation of involuntary autonomic nervous system responses, would reduce the stress response. The purpose of this study is to test the effectiveness of progressive muscle laxation using biofeedback The effectiveness of the experimental methods was tested by measuring the degree of symptoms of stress (SOS) and the values of ephinephrine, pulse rate, blood pressure and natural killer cells. The subjects of this study were thirty nursing students divided into two groups: experimental group was progressive muscle relaxation group using biofeedback and control group. This study was conducted for 8 weeks of clinical practice. Biofeedback training was done by software developed by J&J company (1-410 form for progressive muscle training). Progressive muscle relaxation training according to Jacobson's Theory was done by messaged word from biofeedback. The data was analyzed using Chronbach' ${\alpha}$ and t-test of the SPSS program and the significance level of statistics was 5%. The results of the study were : 1) The progressive muscle relaxation training using biofeedback was effective for the reduction of symptoms of stress(t=-4.248, p<.001) under clinical practice stress conditions. 2) The progressive muscle relaxation training using biofeedback was not effective for the values of epinephrine(t=-1.294, p=.206). 3) The progressive muscle relaxation training using biofeedback was effective for the reduction of systolic blood pressure (t=-2.757, p=.01). 4) The progressive muscle relaxation training using biofeedback was effective for the reduction of diastolic blood pressure (p=-2.032, 0=.05). 5) The progressive muscle relaxation training using biofeedback was not effective for the reduction of pulse rate(t=-15, p=.988). 6) The progressive muscle relaxation training using biofeedback was effective for the maintenance of natural killer cells (t=2.381, p=02). The first clinical rotation for student nurses is a stressful experience as seen by the rise in the SOS in the control group. Biofeedback using progressive muscle relaxation were effective in preventing the rise of symptoms of stress and the blood pressure means when comparing the pre to post clinical experience, The mean natural killer cell count was depressed in the control group but not significantly different in the experimental groups, It is proposed here that stress via the hypothalamic - pituitary - adrenal axis suppressed the NK cell count whereas the relaxation methods prevented the rise in stress and the resulting immune depression. We recommend relaxation techniques using biofeedback as a health promotion technique to reduce psychological stress. In summary. the progressive muscle relaxation training using biofeedback was effective for the reduction of symptoms of stress under clinical practice stress conditions.
Yoga is an ancient Indian system of life, encompassing various practices including practices for self-discipline and also for regulating the health states of the individual, being practiced for thousands of years. The present study aims at understanding the effect of two high frequency breathing practices over autonomic nervous system. Forty healthy male volunteers of age $21{\pm}2$ years with $9{\pm}3$ months of Yoga practice experience were recruited. The two high frequency Yoga breathing practices, kapalabhati (KB) and bhastrika (BH) were given as interventions randomly on either of the two days to minimise laboratory bias. They were assessed before and immediately after the interventions for heart rate, respiratory rate, heart rate variability (HRV), blood pressure and peripheral oxygen saturation. There was a significant increase in heart rate (p<0.01; p<0.001), systolic blood pressure (p<0.01; p<0.001), NN50 (p<0.01; p<0.001) component of HRV for both KB and BH groups respectively. There was a significant reduction in respiratory rate in both the groups (p<0.001, and p<0.05, BH and KB respectively) immediately following intervention. A significant increase in LF component of HRV and reduction in Diastolic blood pressure and high frequency (HF)component following KB was also observed (p<0.05, for all comparisons). The Mean peripheral oxygen saturation remained unaltered in both the groups (p>0.05).The results suggest that high frequency yoga breathing practices induce physiological arousal immediately as evidenced by increased blood pressure and heart rate. The sympathetic arousal was more following KB session as evidenced by an increased diastolic blood pressure, LF power and a decrease in HF power of HRV as compared to the BH session.
대두발효식품인 청국장에는 다양한 항산화물질, peptide 등의 생리활성물질이 존재한다. 청국장 ethanol 추출물은 285 nm에서 0.55의 흡광도 값을 보여 주었다. 이 영역에는 phenol을 포함한 아미노산 및 peptide류가 포함되어 있다고 알려져 있다. 1,1-diphenyl-2-picrylhydrazyl (DPPH)방법으로 청국장의 항산화도를 측정했을 때, ethanol 추출물의 농도가 증가할수록 항산화도는 증가하였다. 무염 생청국장 30 g을 복용하고 2시간 마다 혈압을 측정하였다. 수축기혈압은 복용 후 6시간 지난 후, 평균혈압이 14 mmHg 떨어졌고, 이완기혈압은 8 mmHg 떨어지는 등 혈압강하 효과가 뚜렷하였다. 청국장에 존재하는 daidzein, 항산화물질, Lys-Pro과 같은 angiotensin I-converting enzyme (ACE) 억제제 등이 복합적으로 작용하면서 혈압강하에 기여할 수 있을 것이다.

Recent genomewide association studies of large samples have identified genes that are associated with blood pressure. The Global Blood Pressure Genetics (Global BPgen) and Cohorts for Heart and Aging Research in Genome Epidemiology (CHARGE) consortiums identified 14 loci that govern blood pressure on a genomewide significance level, one of which is $CASZ1$ confirmed in both Europeans and Asians. $CASZ1$ is a zinc finger transcription factor that controls apoptosis and cell fate and suppresses neuroblastoma tumor growth by reprogramming gene expression, like a tumor suppressor. To validate the function of $CASZ1$ in blood pressure, we decreased $Casz1$ mRNA levels in mice by siRNA. $Casz1$ siRNA reduced mRNA levels by 59% in a mouse cell line. A polyethylenimine-mixed siRNA complex was injected into mouse tail veins, reducing $Casz1$ mRNA expression to 45% in the kidney. However, blood pressure in the treated mice was unaffected, despite a 55% reduction in $Casz1$ mRNA levels in the kidney on multiple siRNA injections daily. Even though $Casz1$ siRNA-treated mice did not experience any significant change in blood pressure, our study demonstrates the value of $in$$vivo$ siRNA injection in analyzing the function of candidate genes identified by genomewide association studies.
