Replicated Association between SLC4A4 Gene and Blood Pressure Traits in the Korean Population

  • Recent genome-wide association studies (GWAS) have identified a number of common variants associated with blood pressure homeostasis and hypertension in population. In the previous study, single nucleotide polymorphisms (SNPs) in the SLC4A4 gene have been reported to be associated with hypertension in Han Chinese population. We aimed to confirm whether the genetic variation of SLC4A4 gene influence the susceptibility to blood pressure and hypertension in Korean population. We genotyped variants in or near SLC4A4 in a population-based cohort including 7,551 unrelated Korean from Ansan and Ansung. Here, we performed association analysis to elucidate the possible relations of genetic polymorphisms in SLC4A4 gene with blood pressure traits. By examining genotype data of a total of 7,551 subjects in the Korean Association REsource (KARE) study, we discovered the SLC4A4 gene polymorphisms are associated with blood pressure and hypertension. The common and highest significant polymorphism was rs6846301 (${\beta}$=0.839, additive P=0.032) with systolic blood pressure (SBP), rs6846301 (${\beta}$=0.588, additive P=0.027) with diastolic blood pressure (DBP), and rs6846301 (OR=1.23, CI: 1.09~1.40, additive $P=1.2{\times}10^{-3}$) with hypertension. Furthermore, the SNP rs6846301 was consistently associated with both blood pressure and hypertension. Consequently, we found statistically significant SNPs in SLC4A4 gene that are associated with both blood pressure and hypertension traits. In addition, these results suggest that the individuals with the minor alleles of the SNP in the SLC4A4 gene may be more susceptible to the development of hypertension in the Korean population.

A Study on the Nutrients Intake and Blood Pressure of Korean Farmers in Summer (농업인의 여름철 영양소 섭취 및 혈압상태에 관한 연구)

  • This study was carried out to investigate the nutrients intakes, blood pressure, and correlations between these two factors in Korean rural adults(60 men, 60 women). Nutrients intakes were measured by analyzing with CAN Pro after weighing the each diets of the subjects had for 24 hours. We measured body mass index(BMI), waist to hip ratio(WHR), and fat content in the body besides blood pressure, and considered their relationship with blood pressure. This examination includes collections and analysis of serum and urine, and considers the correlations between blood pressure and serum or urinary factors. These results were as follows ; 1) The mean ages of men and women were 50.$\pm$12.6 and 46.0$\pm$12.5, respectively. The percentage of hypertensive was higher in men(43%) than in women(37%). 2) The values of WHR, body water and lean body mass were higher in men and the value of body fat was higher in women. Weight, BMI, waist or hip circumferences, and WHR were correlated with blood pressure in these subjects. 3) Though the levels of serum triglyceride, sodium, potassium and iron levels were higher in men and urinary Na was higher in women, these factors didn't showed any significant correlations with blood pressure in both sexes. 4) The values of calcium and vitamin B$_2$ intake in men and women, and of iron intake in women didn't meet the Korean recommended allowances(RDA). Animal protein and Na intakes were negatively correlated with blood pressure. These results showed that the levels of calcium and vitamin B$_2$ intake in both sexes and iron intake in women were lower than those of RDA, and weight, BMI, waist or hip circumferences and WHR were significantly correlated with blood pressure. Therefore we suggest that the farmers of this research need to increase intake of nutrients like calcium and vitamin B$_2$ and control the weight.

Implementation of Impedance Method to Estimate Blood Flow Variation with Cuff Pressure Change (커프 압력 조절에 따른 혈류량 변화 평가를 위한 임피던스법의 구현)

  • In this study, we measured the blood flow on arm by non-invasive method and implemented a system to measure variation of the blood flow by estimating bio-electrical impedance and arterial pressure according to cuff pressure. The implemented system measured impedance variation according to pressure variation applied by artificial cuff pressure on the measuring position. The system consisted of pressure measuring part and impedance measuring part using 4-electrode method. Pressure measuring part was composed of semiconductor pressure sensor and electronic circuit for signal processing of sensor output signal. In addition, impedance measuring part was composed of constant current source circuit and lock-in amplifier for detecting impedance signal. We conducted experiments of impedance measuring part using standard resistance for performance evaluation of the implemented system. In addition we experimented to estimate variation of the blood flow by measuring impedances of the experimental group. We estimated ratio of the blood flow resistance using mean arterial pressure and variation of the blood flow. As a result the ratio of the blood flow resistance and variation of blood flow were in an inverse relationship with each other and the correlation coefficient was -0.96776.

Goat Meat Does Not Cause Increased Blood Pressure

  • While there are persistent rumors that the consumption of goat meat dishes increases blood pressure, there is no scientific evidence to support this. Two experiments were conducted to clarify whether or not blood pressure increases in conjunction with the consumption of goat meat dishes. In experiment 1, 24 Dahl/Iwai rats (15 weeks old, body weight $309.3{\pm}11.1$ g) were evenly separated into 4 groups. The control group (CP) was fed a diet containing 20% chicken and 0.3% salt on a dry matter basis. The goat meat group (GM) was fed a diet containing 20% goat meat and 0.3% salt. The goat meat/salt group (GS) was fed a diet containing 20% goat meant and 3% to 4% salt. The Okinawan mugwort (Artemisia Princeps Pampan)/salt group (GY) was fed a diet containing 20% goat meat, 3% to 4% salt and 5% of freeze-dried mugwort powder. The experiment 1 ran for a period of 14 weeks during which time the blood pressure of the animals was recorded. The GS, and GY groups consumed significantly more water (p<0.01) than the CP and GM groups despite the fact that their diet consumption levels were similar. The body weight of animals in the CP, GM, and GS groups was similar while the animals in the GY group were significantly smaller (p<0.01). The blood pressure in the GM group was virtually the same as the CP group throughout the course of the experiment. In contrast, while the blood pressure of the animals in the GS and GY group from 15 to 19 weeks old was the same as the CP group, their blood pressures were significantly higher (p<0.01) after 20 weeks of age. The GY group tended to have lower blood pressure than the GS group. In experiment 2, in order to clarify whether or not the increase in blood pressure in the GS group and the GY group in experiment 1 was caused by an excessive intake of salt, the effects on blood pressure of a reduction of salt in diet were investigated. When amount of salt in the diet of the GS and GY group was reduced from 4% to 0.3%, the animal's blood pressure returned to normotensive. These results indicate that, as in the case of chicken consumption, prolonged consumption of goat meat does not cause increased blood pressure, rather the large amount of salt used in the preparation of goat meat dishes is responsible for the increase in blood pressure.

Correlations between Weight, Body Mass Index(BMI) and Risk Factors of Coronary Artery Disease in Men and Women in their Forties and Fifties (40,50대 남녀별 체중, BMI와 관상동맥질환 위험인자 간의 상관 정도 비교)

  • This study was done to examine the correlations between weight, BMI and risk factors of coronary heart disease in men and women in their forties and fifties. The subjects were 412 adults. who had regular health examinations between January and December of 1996 at S-Hospital in Seoul. The data were analyzed using ANOVA, Scheffe test, and Pearson correlation coefficient. The results are as follows : 1. The men between 50 and 59 years of age had higher levels for BMI, weight, systolic blood pressure, diastolic blood pressure, total cholesterol. LDL-cholesterol, triglyceride, fasting blood sugar, plasminogen activator-1, and hemoglobin A,C than the group of women in their forties. Yet. HDL-cholesterol was lower than in the former group. 2. In the group of men in their forties, weight was significantly correlated to diastolic blood pressure(r=.22), LDL-cholesterol(r=.20), plasminogen activator-inhibitor-1(r=.35) HDL-cholesterol(r=-.19). Their BMI was significantly correlted to systolic blood pressure(r=.27), diastolic blood pressure (r=.33), total cholesterol(r=.23), LDL-cholesterol (r=.26), plasminogen activator-1(r=.36) and HDL-cholesterol(r=-.25). 3. As for the group of women in their forties weight was significantly correlated to systolic blood pressure(r=.20), diastolic blood pressure(r=.22), triglyceride(r=.32) , plasminogen activator inhibitor-1 (r=.30) and HDL-cholesterol(r= -.37). Their BMI was significantly correlated to diastolic blood pressure (r=.25) triglyceride(r=.47), plasminogen activator-1 (r=.35), fibrinogen(r=.27) and HDL-cholesterol(r=-.47). 4. In the group of men in their fifties. weight was significantly correlated to total cholesterol (r=32), LDL-cholesterol(r=.29). plasminogen activator inhibitor-1(r=.26). Their BMI was significantly correlated to systolic blood pressure(r=.24), diastolic blood pressure (r=.22), total cholesterol (r=.34), LDL-cholesterol (r=.32), and plasminogen activator-1(r=.25). 5. In the group of women in their fifties, weight was significantly correlated to diastolic blood pressure(r=.33), total cholesterol(r=.21), LDL-cholesterol(r=.20), plasminogen activator inhibitor-1 (r=.43) and HDL-cholesterol(r=-.21). Their BMI was significantly correlated to systolic blood pressure(r=.25), diastolic blood pressure(r=.40), total cholesterol(r=.24), LDL-cholesterol(r=.24), triglyceride(r=22), and HDL-cholesterol (r=-.30). The above findings indicate that the BMI was more predictive than weight as a risk factor for coronary artery disease for men and women in their forties and fifties.

Blood Pressure, Salt Threshold, Salt Preference, Urinary Excretions and Nutrition Knowledge About Blood Pressure of Elementary School Children in Rural Area (농촌 국민학교 아동의 혈압, 짠맛에 대한 역치, 최적 염미도, 뇨 중 배설성분 및 혈압에 관한 영양지식)

  • The purpose of this study was to observe the relationship of dietary factors to blood pressure in 5th and 6th grade school children. Salt threshold, salt preference and nutrition knowlege about blood pressure were tested. Twenty-four hour urines were collected for the measurements of the volume and concentrations of sodium, potassium, chloride, calcium, phosphorus, creatinine and urea nitrogen. 1) Mean systolic and diastolic blood pressure of elementary school children were 106.8/67.6mmHg in males and 108.7/69.5mmHg in females. Seven children(4%) of total subjects were found to be hypertensive. Their mean blood pressure was 130.0/86.4mmHg. 2) Urinary excretions of creatinine and urea nitrogen during twenty-four hours were 621.1mg and 1524mg, respectively. The mean daily urinary sodium excretion was 2940mg, which is equivalent to 7.37g NaCl. 3) The daily urinary excretions of other minerals were as follows; potassium was 1301mg, chloride, 4991mg, calcium, 55.7mg and phosphorus, 700.03mg. 4) Salt preference of hypertensive children90.473$\pm$0.275) was significantly higher than those of normotensive children(0.473$\pm$0.213%), but salt threshold was lower in hypertensives(0.105$\pm$0.04%) than normotensives(0.081$\pm$0.022%) (p<0.05). INdices for assessing obesity, such as body weight, triceps skinfold, weight for heigth and body surface area, were high in hypertensive children than normotensive children(p<0.05). But there was no significant difference in energy and nutrient intakes between two groups. 5) Various anthropometric measurements had positive correlations with blood pressure, but body muscle mass(%) had a negative correlation with blood pressure. And daily energy and nutrient intakes were not related to blood pressure. 6) Blood pressures-both systolic and diastolic-were significantly correlated with urinary excretions of potassium and chloride. Daily sodium excretion was related to systolic blood pressure, but no related to diastolci blood pressure. There was no relationship of salt threshold to salt preference, and neither threshold nor preference was related to blood pressure. Results of this study suggest that nutrition education program for children including the information about desirable food habits for the prevention of hypertension should be developed.

Development of the Cardiovascular Simulator for Pulse Diagnosis Study (맥진연구를 위한 심혈관계 시뮬레이터의 개발)

  • Objectives The aim of this study is to develop a cardiovascular simulator that can reproduce blood pressure pulse and blood flow similar to those of the human body. Methods In order to design a system similar to the human cardiovascular system, the required performances were determined by investigating the hemodynamic characteristics of the heart and the arterial system. Main organ to be imitated is heart in simulator. The rest of the system was minimally designed. Also, a blood pressure and blood flow measurement system was developed for measuring the results. Results The developed system showed blood pressure pulse at similar range of the human aorta. The result waveform include primary wave caused by ventricular systole except reflected wave. Conclusions The blood pressure and blow flow patterns were replicated by the simulator. These patterns were similar to those of the human body. The system will play an important role in studying pulse diagnostics.

Estimation of Blood Pressure Diagnostic Methods by using the Four Elements Blood Pressure Model Simulating Aortic Wave Reflection (대동맥 반사파를 재현한 4 element 대동맥 혈압 모델을 이용한 혈압 기반 진단 기술의 평가)

  • Invasive blood pressure (IBP) is measured for the patient's real time arterial pressure (ABP) to monitor the critical abrupt disorders of the cardiovascular system. It can be used for the estimation of cardiac output and the opening and closing time detection of the aortic valve. Although the unexplained inflections on ABP make it difficult to find the mathematical relations with other cardiovascular parameters, the estimations based on ABP for other data have been accepted as useful methods as they had been verified with the statistical results among vast patient data. Previous windkessel models were composed with systemic resistance and vascular compliance and they were successful at explaining the average systolic and diastolic values of ABP simply. Although it is well-known that the blood pressure reflection from peripheral arteries causes complex inflection on ABP, previous models do not contain any elements of the reflections because of the complexity of peripheral arteries' shapes. In this study, to simulate a reflection wave of blood pressure, a new mathematical model was designed with four elements that were the impedance of aorta, the compliance of aortic arch, the peripheral resistance, and the compliance of peripheral arteries. The parameters of the new model were adjusted to have three types of arterial blood pressure waveform that were measured from a patient. It was used to find the relations between the inflections and other cardiovascular parameters such as the opening-closing time of aortic valve and the cardiac output. It showed that the blood pressure reflection can bring wide range errors to the closing time of aortic valve and cardiac output with the conventional estimation based on ABP and that the changes of one-stroke volumes can be easily detected with previous estimation while the changes of heart rate can bring some error caused by unexpected reflections.

A Study on the Prevalence Rate of Hypertension and Diabetes among Adults in the Urban Communities (도시지역 지하철을 이용하는 성인의 고혈압 및 당뇨병 유병률)

  • The purpose of this study is to identify the prevalence rate of hypertension and diabetes a in the urban communities and to provide the basic data for development of health promot The subjects of this study were 526 people over the age of 20, living in Seoul. Data for this collected from June 19, 2001 to September 25, 2001. The results of this study are as follows: 1. The subject group is comprised of males, $57.4\%$ and females, $42.6\%$. Their ages range from 20 to 89, and the weights from 40 to 94 kilograms. 2. In the systolic blood pressure, $36.1\%$ showed high, $56.5\%$ normal. and $7.4\%$ low. In the diastolic blood pressure, $50.5\%$ showed high, $48.9\%$ normal, and $1.7\%$. low. In the blood glucose, $70.3\%$ showed normal. $27.9\%$ high, and $1.7\%$. low. 3. There was a significant difference in the mean(SD) of the systolic blood pressure by age(p=.017) and weight(p=.005). Another significant difference was found in the mean (SD) of the diastolic blood pressure by age(p=.006) and weight(p=.007). There was a significant difference in the mean(SD) of the blood glucose by sex and age(p=.001). 4. There were significant correlations between the blood pressure and the blood glucose and the sex, age and weight. 5. The multiple regression analysis showed that the age and weight explained $9.9\%$ of the systolic blood pressure, that the weight and age explained $7.1\%$ of the diastolic blood pressure, and that the age and systolic blood pressure explained $7.0\%$ of blood glucose. The results were useful in developing health promotion programs. This study suggests that a further study be needed.

The Development of a Cuff for the Accuracy Enhancement of the Sphygmomanometer

  • The purpose of this study is to develop a new cuff to improve the accuracy of blood pressure measurement, and to evaluate the performance of the developed system. We added a small bladder to the normal cuff, which we refer to as the double bladder system. The system that we developed for blood pressure measurement was based on the oscillometric method using a double bladder. This system was developed in order to reduce the oscillation noise and to amplify the signal of pure blood pressure. An oscillometric signal database based on the developed system was evaluated according to the ANSI/AAMI/SP10-1992 standard. The correlation coefficients between the cuff of the double bladder and the normal cuff were 0.98 for systolic pressure and 0.94 for diastolic pressure. The mean differences and the standard deviations between the average blood pressure obtained from a mercury manometer and that obtained from an automated sphygmomanometer were -0.7mmHg and 4.9mmHg for systolic, and -1.4mmHg and 5.4mmHg for diastolic pressure. We conclude that the proposed double bladder-based cuff system improves the accuracy of oscillometric blood pressure measurement. The developed system reduces the range of error by about $44{\sim}62%$ for systolic pressure and about $6{\sim}21%$ for diastolic pressure compared to the most recently developed, commercially available sphygmomanometers.
