This study investigated the effect of community-based health intervention program to improve metabolic disease in Jeju island. There were 50 obese local residents in the experimental group (body mass index, $BMI{\geq}25kg/m^2$). They participated in cooking therapy with nutrition education (4 times), exercise program with proprioceptive neuromuscular facilitation (PNF) and stretching training (9 times), and alternative medicine program with healing touch massage (3 times) during a 3-week period. To evaluate the effectiveness of the program, body composition, blood lipid profiles, blood glucose, and waist circumference were assessed before and after the intervention program. After the program, the value of total cholesterol (TC), glycosylated hemoglobin (HbA1C), diastolic blood pressure, percent body fat, and waist circumference were decreased, and became the normal value. Especially, HbA1C, percent body fat, and waist circumference were significantly decreased (P<0.001) in the experimental group, while HbA1C, percent body fat, and waist circumference were significantly increased (P<0.001) in the control group (N=50), who had no intervention. Our results suggest that metabolic syndrome associated markers need intervention program for improving them. In conclusion, although this study did not analyze the effect of the health intervention program and cooking therapy separately, considering the result of this 3-week, short term program, the effect will be more profuse if cooking therapy and exercise program were performed concurrently.

The antihypertensive and cholesterol- lowering effects of Monascus koji (M. koji) prepared with Monascus ruber IFO32318 were examined in spontaneously hypertensive rats (SHR). Control was fed a normal diet and groups I, II or III were fed diets containing 0.03%, 0.1% or 0.3% M. koji for 8 weeks, respectively. After 8 weeks, all animals were fed normal diets in the following 2 weeks. The blood pressure of rats fed M. koji added diets were significantly attenuated as compared with control and the diet containing high concentration of M. koji had a tendency of stronger antihypertensive effect. These differences lasted throughout the experimental period when they were fed experimental diet. For 2 weeks after the 8 weeks of experimental diet all groups were fed the same normal diet and the differences of blood pressure caused by M. koji disappeared. In rats fed the M. koji added diet, the serum total cholesterol (TC), HDL cholesterol (HDL-C), LDL cholesterol (LDL-C) were not differed compared with control, but VLDL cholesterol (VLDL-C) was significantly lowered. M koji also significantly decreased serum risk factors, both TC/HDL-C ratio and non HDL-C/HDL-C ratio. Consequently, it is suggested that M. koji may play an important role to attenuate hypertension and to improve serum lipid Profiles.

The principal objective of this study was to assess the effects of low-calorie diets(LCD) for 8 weeks of medical nutrition therapy in individuals with BMI in excess of $23\;kg/m^2$. 41 over-weight or obese individuals (male=14, female=27, age $28.6{\pm}8.9\;yrs$), none of whom were diagnosed with a medical disease, were administered MNT 4 times over the study period. Approximately $1,100{\sim}1,300\;kcal/day$ were prescribed by a dietitian, via individualized counseling. Anthropometric parameters, daily nutrient intake, and blood levels of leptin, insulin, and lipid profiles were measured prior to and after the 8 weeks of the intervention period. We noted significant reductions in body weight, body mass index (BMI), fat mass (FM), visceral adipose tissue (VAT), abdominal skin-fold thickness, and waist circumference in both men and women. The mean body weight losses in men and women were $3.2{\pm}0.6\;kg$/8 weeks and $2.8{\pm}0.6\;kg$/8 weeks, respectively. However, the % of lean body mass (LBM) in men and women was shown to increase significantly (p<0.05). The daily intake of calorie, fat, protein, vitamin E, folate, and iron were significantly reduced during the LCD period. Blood levels of lipids and glucose were in normal range, and evidenced no changes after LCD. However, the serum levels of leptin in female subjects were significantly reduced (p<0.00l) from $8.9{\pm}4.8\;ng/mL$ to $6.9{\pm}4.8\;ng/mL$. In conclusion, 8 weeks of LCD with individualized counseling by a dietitian effectively reduced body fat and visceral fat in both men and women with BMI in excess of $23\;kg/m^2$.

The purpose of this study was to investigate the effect of dietary seaweed in diabetic rats treated with streptozotocin (STZ) for 7 weeks. The rats (Sprague-Dawley male rats, 180∼200 g) were divided into 4 groups : normal rats fed control diet (C), diabetic rats fed control diet (CD), normal rats fed seaweed diet (M), and diabetic rats fed seaweed diet (MD). Diabetes was induced by single injection of streptozotocin (60 mg/kg, i.p.). Urinary levels of calcium and uric acid, and blood levels of hemoglobin, total cholesterol and low density lipoprotein (LDL)-cholesterol were not significantly different among groups. But high density lipoprotein (HDL)- cholesterol of M and MD groups were higher than that of C and CD groups. Activity of hepatic microsomal G6Pase was significantly (p<0.05) lower in C and M groups than that of CD and MD groups. Hepatic glutathione S-transferase (GST) of M, CD and MD groups were significantly lower than C group (p<0.05), glutathione peroxidase (GPX) of C, M and MD groups were higher than CD group. In conclusion, dietary seaweed may improve blood lipid profiles and GSH-related enzymes in STZ-induced diabetic rats.
The objective of this study was to assess the prevalence and clinical characteristics of anemia and to define the risk factors for anemia in older Koreans in Asan. From January to February 2002, five hundred sixty two community-dwelling older adults aged over 60 years were selected from a cross sectional study. All subjects underwent laboratory tests, which included a complete blood cell count, reticulocyte, liver and renal functional tests, lipid profiles and iron status tests. The median age was 68.6 years (range, 60-92 years). The mean levels of hemoglobin were $14.4{\pm}1.3\;g/dL$ in men and $12.9{\pm}1.0\;g/dL$ in women, and the overall prevalence of anemia was 12.5% in all subjects, 10.8% in men and 13.6% in women. The prevalence of anemia was the lowest among age group of 60-69 (10.0%) followed by 70-79 (15.5%), and the highest among age over 80 (20.7%), but the difference was significant only for men. The age difference was more distinct in men than in women (p<0.05). The mean hemoglobin level was significantly lower in the subjects aged over 80 y than those in the 60-69 y group (p<0.05). According to a logistic regression analysis, lower albumin and higher creatinine levels were identified as independent risk factors of anemia among older adults in Asan. In conclusion, the overall prevalence of anemia in our study group was 12.5% and the highest (20.7%) among those aged over 80 y.

Purpose: The aim of this study was to determine whether school-aged children with Kawasaki disease (KD) have an increased risk for early atherosclerosis. Methods: The study included 98 children. The children were divided into the following groups: group A (n=19), KD with coronary arterial lesions that persisted or regressed; group B (n=49), KD without coronary arterial lesions; and group C (n=30), healthy children. Anthropometric variables and the levels of biochemical markers, including total cholesterol, high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), apolipoprotein A, apolipoprotein B, homocysteine, highsensitivity C-reactive protein (hs-CRP), and brachial artery stiffness using pulse wave velocity were compared among the three groups. Results: There were no significant differences in blood pressure and body index among the three groups. Additionally, there was no sex-specific difference. Moreover, the levels of triglyceride, HDL-C, apolipoprotein A, and hs-CRP did not differ among the three groups. However, the levels of total cholesterol (P =0.018), LDL-C (P =0.0003), and apolipoprotein B (P =0.029) were significantly higher in group A than in group C. Further, the level of homocysteine and the aortic pulse wave velocity were significantly higher in groups A and B than in group C (P=0.0001). Conclusion: School-aged children after KD have high lipid profiles and arterial stiffness indicating an increased risk for early atherosclerosis.
The present study was designed to assess whether dietary inorganic and organic selenium (Se) could affect antioxidant enzymes, including superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and glutathione-S-transferase (GST), and the level of malondialdehyde (MDA), a marker of lipid peroxidation, in the intestine, serum, liver, and gastrocnemius muscle of Korean native goats. A total of eighteen Korean native goats was allotted into three dietary groups, consisting of basal diet (CON), or basal diet with either 0.25 ppm inorganic (IOSEL) or 0.25 ppm organic Se (ORSEL), and fed the corresponding diets for 5 wks. Growth performance, including body weight and total gain, and blood biochemical profiles, including GSH-Px, were not significantly different between the three dietary groups. Also, the specific activities of SOD, GSH-Px, and GST, and the level of MDA in the intestinal mucosa and liver from goats were not substantially affected by either inorganic Se or organic Se. However, goats fed the diet containing organic Se showed a significant increase in GSH-Px and GST activities in the gastrocnemius muscle compared with those fed the basal diet. In conclusion, increased muscle GSH-Px and GST activities suggest that dietary organic Se may affect, at least in part, the antioxidant defense system in muscle of Korean native goats under the conditions of our feeding regimen.

The purpose of this study is to evaluate the nutritional status of protein and lipids in the patients with alcoholic liver disease, to find an effective way of dietary management for patients with alcoholic liver disease and to obtain the materials for nutritional education for alcoholics. The subjects consist of 80 patients with alcoholic liver cirrhosis(ALC) and 12 patients with alcoholic fatty liver(AFL). The control group included 57 alcoholics without liver disease(A), 32 patients with viral liver cir rhosis(VLC) and 194 normal(NL). Biochemical evaluation of nutritional status was performed by ana lyzing the profiles of protein and lipids in blood samples. The results are summarized as follows: 1) The frequency of subjects below the normal range of serum total protein was 26.3% in ALC, 16.7% in AFL and 3.5% in A. Serum albumin was lower in 41.3% of ALC. 2) The alcoholics without liver disease showed significantly higher level of serum triglycerides, total cholesterol and LDL cholesterol than the other groups. The alcoholic subjects had lower HDL cholesterol than normal subjects. Overall, the protein and lipids status of the alcoholic subjects in this study was evaluated to be very poor on the basis of biochemical assessments. The results suggest that alcohol abuse and poor dietary intake could cause malnutrition. An extensive nutritional education should be emphasized for the alcohol consuming population. High quality of protein and other dietary intakes from early stage of the disease may be effective in nutritional therapy for the patient with alcoholic liver disease.
Objective: The study was carried out to evaluate the hepatoprotective and antioxidant potential of Ziziphus mucronata (ZM) fruit extract. Methods: The different types of fruit extract were prepared by soaking the dry powdered fruit in different solvents followed by rotary evaporation. Each extract was tested for its phenol content and antioxidant activities. An in vivo study was performed in Sprague-Dawley (SD) rats. Thirty adult male SD rats (aged 21 weeks) were divided into six groups of five rats each and treated as follows: The normal control (NC) received distilled water while the dimethoate control (DC) received 6 mg/kg.bw.day-1 dimethoate dissolved in distilled water. The experimental groups E1, E2, E3, and E0 received dimethoate (6 mg/kg.bw) + ZMFM (100 mg/kg.bw-1), dimethoate (6 mg/kg.bw) + ZMFM (200 mg/kg.bw-1), dimethoate (6 mg/kg.bw) + ZMFM (300 mg/kg.bw-1), and ZMFM (300 mg/kg.bw-1) only. Both the normal control and the dimethoate control groups were used to compare the results. After 90 days, rats were sacrificed, blood was collected for biochemical assays, and livers were harvested for histological study. Results: High phenol content was estimated, and 2, 2-diphenyl-1-picryl hydrazyl radical (DPPH) spectrophotometric, thin layer chromatography (TLC) and 2, 2-Azobis-3-ethyl benzothiazoline-6-sulphonic acid (ABTS) assays showed a high antioxidant activity among the extracts. The preventive effects observed in the E1, E2 and E3 groups proved that the extract could prevent dimethoate toxicity by maintaining normal reduced glutathione (GSH), vitamin C and E, superoxide dismutase, catalase, cholineasterase and lipid profiles. The preventive effect was observed to be dose dependent. The EO group showed no extract-induced toxicity. Histological observations agreed with the results obtained in the biochemical studies. Conclusion: The study demonstrated that ZM methanol fruit extract is capable of attenuating dimethoate-induced toxicity because of its high antioxidant activity.

Objectives: This study aims to investigate trends of cardiovascular disease (CVD) risk factor profiles over 17 years in percutaneous coronary intervention (PCI) patients at the Mayo Clinic. Methods: We performed a time-trend analysis within the Mayo Clinic PCI Registry from 1994 to 2010. Results were the incidence and prevalence of CVD risk factors as estimate by the Framingham risk score. Results: Between 1994 and 2010, 25 519 patients underwent a PCI. During the time assessed, the mean age at PCI became older, but the gender distribution did not change. A significant trend towards higher body mass index and more prevalent hypercholesterolemia, hypertension, and diabetes was found over time. The prevalence of current smokers remained unchanged. The prevalence of ever-smokers decreased among males, but increased among females. However, overall CVD risk according to the Framingham risk score (FRS) and 10-year CVD risk significantly decreased. The use of most of medications elevated from 1994 to 2010, except for ${\beta}$-blockers and angiotensin converting enzyme inhibitors decreased after 2007 and 2006 in both baseline and discharge, respectively. Conclusions: Most of the major risk factors improved and the FRS and 10-year CVD risk declined in this population of PCI patients. However, obesity, history of hypercholesterolemia, hypertension, diabetes, and medication use increased substantially. Improvements to blood pressure and lipid profile management because of medication use may have influenced the positive trends.
