The Effect on Pain of Buzzy® and ShotBlocker® during the Administration of Intramuscular Injections to Children: A Randomized Controlled Trial

  • Purpose: To investigate the effect of $Buzzy^{(R)}$ and $ShotBlocker^{(R)}$ on reducing pain induced by intramuscular penicillin injections in children. Methods: This was a randomized controlled study. A total of 150 Turkish children aged 7~12 years who presented to our pediatric emergency clinic and met the inclusion criteria were recruited. The children were randomly assigned to each group (control=50, $Buzzy^{(R)}=50$, $ShotBlocker^{(R)}=50$). Data were collected using an information form, the State-Trait Anxiety Inventory for Children, Visual Analog Scale, and Faces Pain Scale-Revised. Results: The children in the control group had significantly higher pain scores during the penicillin injection than the children in the $ShotBlocker^{(R)}$ and $Buzzy^{(R)}$ groups. The children in the $Buzzy^{(R)}$ group had significantly less pain than the children in both the $ShotBlocker^{(R)}$ and control groups (p<.001). Conclusion: $Buzzy^{(R)}$ was more effective compared with $ShotBlocker^{(R)}$ in this study.

Effect of Various Receptor Blockers on the Action of Adenosine Triphosphate on Uterine Smooth Muscle Motility in Immature Pig (미성숙 돼지 자궁 평활근의 운동성에 대한 Adenosine Triphosphate의 작용에 있어서 수종의 Receptor 차단제의 영향)

  • This study was carried out to investigate the action of ATP, which has been known as the neurotransmitter of noncholinergic- and nonadrenergic-nerve, on the motility of immature pig uterine smooth muscle. The results were summarized as follows; 1. The contraction and the contractile responses caused by ATP were increased in a dose-dependent manner between the concentration of ATP $10^{-6}M$ and $10^{-3}M$. The maximal contractile effect was appeared at the concentration of ATP $10^{-3}M$ and it was 70.2% of 100mM K contraction. 2. The contractile responses induced by ATP ($10^{-4}M$) were not blocked by the pretreatment with cholinergic receptor blocker, atropine ($10^{-6}M$). 3. The contractile responses induced by ATP ($10^{-4}M$) were not blocked by pretreatment with ${\alpha}$-adrenergic receptor blocker, phentolamine ($10^{-6}M$) and ${\beta}$-adrenergic receptor blocker, propranolol ($10^{-6}M$). 4. The contractile response induced by ATP ($10^{-4}M$) was not blocked by the pretreatment with $H_1-receptor$ blocker, pyrilamine ($10^{-6}M$) and $H_2-receptor$ blocker, cimetidine ($10^{-6}M$).

  • In this paper, we present new application for the auction method which has been based on one-chip microprocessors. We develop the portable wired terminal for market blocker. And, using that, the recommended price of products is able to push into and show the information of action status. Through our research, using EAS(Electronic Auction System), we can prevent supplier from blocker's rigging the market, because anyone who have a qualification for the action blocker is able to participate in action.

The Effects of Beta-blocker and Aerobic Exercise on Heart Rate Recovery and Exercise Capacity after Maximal Exercise in Acute Coronary Syndrome Patients

  • This study was conducted to identify the effects of intake of beta-blocker and aerobic exercise on heart rate recovery (HRR) and exercise capacity after maximal exercise in acute coronary syndrome patients (ACS). Subjects were divided into 4 groups; Beta-blocker-Exercise group (n=10), Beta-blocker group (n=10), Exercise group (n=10), Control group (n=10). Symptom-limited grade exercise test were conducted on subjects pre- and post- 6 week study, to measure maximal heart rate, heart rate at 1 minute and 2 minutes at the recovery stage, maximal oxygen uptake and maximal exercise time. Study resulted in significant improvements in heart rate recovery for BB-Ex group ($17.40{\pm}5.58$ bpm to $26.10{\pm}6.66$ bpm), maximal oxygen uptake for BB-Ex group ($30.46{\pm}8.63$ to $36.33{\pm}8.10$) and Ex group ($29.04{\pm}4.93$ to $34.31{\pm}5.19$), and total exercise time for BB-Ex group ($906.30{\pm}158.57$ to $1018.50{\pm}151.21$), BB group ($805.70{\pm}182.94$ to $897.50{\pm}160.16$) and Ex group ($870.00{\pm}145.63$ to $961.90{\pm}107.29$). Therefore it showed the biggest improvement for heart rate recovery and exercise capacity in acute coronary syndrome patients when both Beta-blocker and aerobic exercise were in effect.

  • The aims of this study were to investigate the effect of adenosine triphosphate (ATP), which has been known as the neurotransmitter of nonadrenergic, noncholinergic nerves, and the source of $Ca^{\sharp}$ in the effect of ATP on the isolated renal artery of pig. The results of this study were summarized as follows: 1. ATP caused the contraction and the contractile responses were increased in a dose-dependent manner between the concentration of ATP $2{\times}10^{-3}M$ and $10^{-2}M$ on the isolated renal artery of pig. 2. The contractile responses induced by ATP $(5{\times}10^{-3}M)$ were not blocked by pretreatment with cholinergic receptor blocker (atropine, $10^{-6}M$), $\alpha$-adrenergic recptor blocker(phentolamine, $10^{-6}M$) or $\beta$-adrenergic receptor blocker (propranolol, $10^{-6}M$), and $H_1$-receptor blocker (pyrilamine, $10^{-6}M$) or $H_2$-receptor blocker (cimetidine, $10^{-6}M$) on the isolated renal artery of pig. 3. The contractile responses induced by ATP $(5{\times}10^{-3}M)$ were not appeared in $Ca^{\sharp}$-free medium. As the concentration of $Ca^{\sharp}$ in $Ca^{\sharp}$-free medium was increased, the contractile responses induced by ATP $(5{\times}10^{-3}M)$ were enhanced but were completely inhibited by pretreatment with $Ca^{\sharp}$-channel blocker, papaverine $(5{\times}10^{-5}M)$ or verapamil $(5{\times}10^{-5}M)$ on the isolated renal artery of pig.

A Case of Gorham-Stout Disease with Life-threatening Chylothorax Successfully Treated with the Combined Therapy of mTOR Inhibitor and Beta-blocker (mTOR inhibitor와 beta-blocker 병합요법으로 성공적으로 치료된 Gorham-Stout 질환)

  • Gorham-Stout disease is a rare disorder characterized by lymphovascular proliferation and destruction of osseous matrix. The etiology of this condition remains poorly understood. Chylothorax as a consequence of lymphatic leakage in thoracic cage may cause a severe life-threatening complication, accompanying respiratory difficulty. Currently, there is no standard management for this extremely rare condition. Here we describe a patient affected by Gorham-Stout disease successfully managed by the combined treatment of mTOR inhibitor and beta-blocker. A previously healthy 11-year-old female developed dyspnea and chest pain with a massive pleural effusion. The ligation of right thoracic duct and bilateral pleurodesis temporarily decreased her pleural effusion, which was aggravated repetitively and required frequent admission and tube thoracotomies. Along with bilateral pleural adhesiolysis with thoracotomy, the combined treatment of oral beta-blocker and mTOR inhibitor was commenced. After 1 month of oral medication, her pleural effusion was not increased and she was free of respiratory difficulty on room air without chest tubes. Over eleven months of treatment, no serious adverse reaction was noted and her condition has been stable with no further admission required.

  • The efficiencies of four different solid absorbents were evaluated for the simultaneous solid-phase extraction of six ${\beta}$-blockers from biological sample. Chromosorb 107 using methanol as the eluting solvent gave the highest solid-phase extraction yield. From the breakthrough studies of ${\beta}$-blockers, the excellent sorption and desorption properties of the Chromosorb 107 column were verified. The usefulness of Chromosorb 107/methanol was examined for the simultaneous solid-phase extraction of the ${\beta}$-blockers from serum in the concentration range of 2~8ppm.

Determination of $Na^+$ Channel Blocker in Seaweed (해조류 내 $Na^+$ 챈널 차단 생리활성물질의 측정)

  • A tissue biosensor was developed for the continuous determination of $Na^+$ channel blockers. The proposed sensor was applied to the determination of Na+ channel blockers in seaweed. It was found that $Na^+$ channel blocker content displayed seasonal variation; it was high from February to April and decreased thereafter (May - August). From these results the present proposed method may be used for high sensitive determination of $Na^+$ channel blockers contained in the seaweed organisms and environments. Therefore, it may be important to monitor $Na^+$ channel blocker content of seaweed throughout the year.

Studies on the Physiological Properties of the Histamine Receptor of Ileal Smooth Muscle in Dog (개 회장 평활근에 있어서 Histamine Receptor의 생리적 특성에 관한 연구)

  • To validate the physiological properties of the histamine receptors of ileal smooth muscle in dog, the effects of adrenergic-, cholineric-, and H-receptor antagonists on the responses of ileal smooth muscle strips to histamine were investigated. The results were summarized as follows; 1. Histamine caused the contraction of ileal smooth muscle and the contractile responses were increased between the concentration of histamine $10^{-7}M$ and $10^{-5}M$ with dose-dependent manner in dog. 2. The shorter the treatment interval of histamine, the lower the contractile activity until the treatment interval extended to 40 minutes. 3. The contractile response induced by histamine was completely blocked by the pre treatment with a $H_1$-receptor blocker, chlorpheniramine and not by the pretreatment with a $H_2$-receptor blockers cimetidine. 4. The contractile response induced by histamine was not blocked by the pretreatment with a cholinergic receptor blocker, atropine. 5. The contractile response induced by histamine was not blocked by the pretreatment with an ${\alpha}$-adrenergic receptor blocker, phenoxybenzamine, or a ${\beta}$-adrenergic receptor blocker, propranolol. From these results, it was suggested that the contraction induced by histamine was elicited through $H_1$-receptor on the ileal smooth muscle in dog.

  • To elucidate the action of the cholinergic and ${\alpha}_2$-adrenergic nerve on the isolated ileal smooth muscle of the dog, effect a of electrical field stimulation were investigated on the pretreatment of the physostigmine; cholinestrase inhibitor, yohimbine; ${\alpha}_2$-adrenoceptor blocker, atropine ; cholinergic receptor blocker and phentolamine; non-selective $\alpha$-adrenoceptor blocker from physiograph. 1. The contractile response induced by electrical field stimulation was the frequency (2-40 Hz)-dependent manner. 2. The contractile response induced by electrical field stimulation was markedly increased by the pretreatment of physostigmine$(1{\mu}M)$; cholinestrase inhibitor. 3. The contractile response induced by electrical field stimulation was increased by the pretreatment of yohimbine$(1{\mu}M)$; ${\alpha}_2$-adrenoceptor blocker. These finding suggest that it was powerful excitatory action by cholinergic nerve and inhibitory action by ${\alpha}_2$-adrenergic nerve on ileal smooth muscle of the dog.
