Hereditary Retinal Degeneration in a Siamese Cat (샴 고양이에서의 유전성 망막 변성 1례)

  • A 5-year-old Siamese cat was presented at the Veterinary Medical Teaching Hospital in Seoul National University with visual impairment. The cat was blind and its pupillary light reflexes were sluggish OU. While there were no remarkable findings in the anterior segment, generalized tapetal hyperreflectivity and retinal vascular attenuation were observed in the posterior segment of both eyes. Any historical significance and clinical symptoms suggestive of infection or inflammation were absent. All of the above findings strongly indicated a diagnosis of hereditary retinal degeneration in the cat. This clinical case report demonstrates the possibility of the presence of feline hereditary retinal degeneration in Korea. Awareness of the condition should be raised to prevent the spread of hereditary retinopathy in the Korean cat population.

The Production of Transgenic Mouse Harboring Mutated Pig Rhodopsin Gene (돌연변이가 야기된 돼지 로돕신 유전자를 지닌 형질전환동물의 생산)

  • It is generally known that mutations in any of several genes encoding photoreceptor-specific proteins have resulted in retinitis pigmentosa (RP), a disease characterized by losing photoreceptor function with progressive degeneration of photoreceptor cells and eventually leading to blindness. To study the procure and cure of photoreceptor degeneration, we produced transgenic mice. Transgene consisted of a 12.5kb genomic DNA fragment that contains mutated pig rhodopsin gene (Pro-347-Ser) including both the 5'-franking (4.0 kb) and the 3'-franking (2.9 kb) sequences. This gene was used for the production of transgenic mouse. The mutated rhodopsin DNA was microinjected into male pronuclei of fertilized mouse (C57BL /6]) embryos. We detected transgenic animals harboring mutated rhodopsin gene by PCR and Southern blot analysis. These transgenic mice showed stable transmission of microinjected rhodopsin gene into their offspring. Therefore these animals will provide a novel approach to study the mechanism of the photoreceptor degeneration and be provided as a disease model for the treatment of the blind in human.

The Effect of Postoperative Propacetamol on Acute Pain and Endocrine-Metabolic Response in Gynecological Surgery (부인과 수술 후 통증관리에 있어서 프로파세타몰의 모르핀 절감 및 내분비 대사에 대한 효과)

  • Background: The analgesic efficacy and safety of propacetamol, an injectable prodrug of acetoaminophen, in combination with intravenous morphine PCA were studied in 40 patients after gynecological surgery requiring lower abdominal incision. Methods: Using a double-blind, randomized, parallel-group design, the effects of four(every 6 hr) intravenous injections of 2 g propacetamol(=1 g acetoaminophen) were compared with four injections of placebo(PL) immediately after surgery. Efficacy of cumulative dose of morphine and number of boluses requested was assessed over 24 hours by automated recording on the PCA device. It was assessed on pain scores rated on a ten-point verbal scale along with vital signs, $K^+$, glucose, BUN, creatinine, PT and PTT were measured along with stress hormones(epinephrine, norepinephrine and cortisol). Results: There were no differences in demographic data between two groups. Propacetamol group demonstrated approximately 21% morphine sparing effect compared to placebo group($33.1{\pm}10.4$ mg vs $41.4{\pm}8.0$ mg). No significant differences noted in $K^+$, glucose, BUN, Creatinine, PT and PTT levels. There were significant increases in norepinephrine and cortisol in placebo group postoperatively, compared to preoperative values. At the same time, propacetamol group also showed significant changes in these hormones. Both group revealed high degree of patient satisfaction. Conclusion: Propacetamol showed significant morphine sparing effect to some degree. Side effects were much less in propacetamol group with subsequently high patient satisfaction. The secretion of stress hormone were not blocked by postoperative propacetamol injections. Authors concluded that propacetamol should be considered as an excellent adjuvant analgesics in postoperative pain control in opioid patient controlled analgesia.

Effects of Preemptive Analgesia by Epidural Bupivacaine and Fentanyl on Postoperative Pain Control in Lower Abdominal Surgery (하복부 수술에서 경막외 Bupivacaine과 Fentanyl에 의한 선행진통법이 술후 통증관리에 미치는 효과)

  • Background: Preemptive analgesia is an antinociceptive treatment that prevents the establishment of altered central processing which amplifies postoperative pain. A controversy exists over the effectiveness and clinical value of preemptive analgesia. We studied whether epidural bupivacaine and fentanyl prior to surgery could possibly affect postoperative pain and analgesic demands, as compared to administration of same at end of surgery. Methods: Forty patients scheduled for lower abdominal surgery were randomly assigned to one of two groups and prospectively studied in a double-blind method. Group 1(n=20) received epidural injection of 15 ml bupivacaine 0.25% with fentanyl 100 y g before surgery while group 2(n=20) received the same injection at the end of their surgery respectively. Postoperative analgesia consisted of basal plus patient-controlled mode of epidural bupivacaine and fentanyl from PCA system. Postoperative visual analog pain scores(VAPS), analgesics consumption, supplementary analgesics requirement and side effects were assessed for 3 postoperative days. Results: There were no significant difference in analgesics requirement and pain scores, at any time, during rest or after movement, in measurement between the groups. Conclusions: We conclude no clinical value of effectiveness in administering epidural bupivacaine-fentanyl before surgery as compared to administration after surgery.

The Pretreatment of Thiopental Sodium for Reducing Injection Pain of Propofol (Propofol 정주시 혈관통 감소를 위한 Thiopental Sodium 전처치)

  • Background: The incidence of pain on injection of propofol varies between 30 and 100%. A variety of pretreatments have been tried to alleviate this problem such as a local anesthetics, cooling and opioids. However, none of these pharmacological maneuvers were satisfactory yet. In a recent study, subhypnotic doses of both thiopental sodium and propofol decrease the acute pain. We report a comparison of thiopental sodium, lidocaine and placebo on the incidence and severity of pain on injection of propofol. Method: A controlled, double-blind study was performed to compare the prior administration of intravenous saline 2 ml(n=30, group S), lidocaine 20 mg(n=30, group L) and thiopental sodium 50 mg(n=30, group T) in alleviating the pain by propofol. Injection pain was assessed with the four-point verbal categorical scoring system. Result: The incidence of injection pain during induction was lower in group L(30%) and T(17%) than group S(77%). The incidence of injection pain was lower in group T(17%) than group L(30%), but not significant statistically. The pain scores for recall of pain in the recovery room was simlar to those pain during propofol induction. Conclusion: The pretreatment of thiopental sodium can be effective in reducing both incidence and severity of propofol injection pain and has similar effect to lidocaine to prevent propofol injection pain.

Fashion Satire in the Cartoon Magazine『Punch』 (카툰잡지『Punch』에 나타난 패션 풍자)

  • Fashion is changing and evolving everyday with an influence from and over contemporary socio-cultural factors. Cartoons expressing the phenomena of times through exchanges of mutual effects with socio-cultural factors that result from functionality and media characteristics. This study examines how fashion provides a great correlation with society-culture expressed in cartoons. The research segment of this study was conducted with literature and case studies; in addition, the UK cartoon magazine "Punch" was selected for the case study. The research findings of the fashion satire expression in cartoon were divided into 2 cases. The first case is that fashion was used as an instrument to satirize socio-cultural phenomena in cartoons. Various fashion elements (hats, dresses, words on T-shirts) were used for satiric expressions and to express periodic images related to politics, economics, society and culture. It communicated factually or criticized noteworthy phenomenon or age changes through the symbolism of fashion. The second case is that fashion itself is the object of satire in a cartoon. It satirically described the blind following and destruction of stereotype as direct objects. Fashion satire appeared in cartoons regardless of a correlation with age. Each cartoon fashion satire had meaning in both humor and criticism for satirizing the age. This study shows that fashion symbolism for satire of the reality has been used as the instrument of expression and simultaneously expressed as the object of the critique as an image and phenomenon that reflects reality. This study has significance in that it examined expressive modes of fashion satire in cartoons that escape from separating fashion from cartoon as a different area.

Determinants of health screening and its effects on health behaviors (건강검진 수검의 결정요인 및 건강증진행위 변화 효과)

  • With the burden of chronic diseases mounting among the population as a result of its aging, the importance of health examination is being stressed in order to identify and manage diseases in the early stage. Health examination in Korea is divided largely into periodic health examination provided as a national health screening program and individual physical checkups. The advantages of the former include little economic burden on the examined and those of the latter include the freedom of the individual to select various examination headings depending on the individual's characteristics and preferences. With both examinations now being expanded, empirical analyses from various standpoints are needed. This study proposes to analyze traits of the examined and non-examined as shown in the facts and figures of the 1st and 2nd Korean Longitudinal Study of Ageing (KLoSA), thereby make the determinant factors clear leading to the acceptance of the examinations, and analyze the effects of the examinations upon maintaining or moving to healthy lifestyle. It was confirmed that demographic features such as gender and age, socioeconomic features such as the level of education, place of residence and household income, physical and mental state of health such as chronic disease and dementia, and daily living habits are significantly related with whether to accept physical examination. It is also confirmed that physical examination leads to non-smoking, regular physical exercises and regular dietary habits. It is suggested that, to enhance effects of health examinations, follow-up management programs making use of results of health examinations be further expanded, and the national health screening program be more actively put into operation for the bracket lying in the blind spot of the program.

  • Recently, due to the development of mobile phone and supply of smart phone, many contents have been developed. Especially, since the small-sized cameras are equiped in mobile devices, people are interested in the image based contents development, and it also becomes important part in their practical use. Among them, the character recognition system can be widely used in the applications such as blind people guidance systems, automatic robot navigation systems, automatic video retrieval and indexing systems, automatic text translation systems. Therefore, this paper proposes a system that is able to extract text area from the natural images captured by smart phone camera. The individual characters are recognized and result is output in voice. Text areas are extracted using Adaboost algorithm and individual characters are recognized using error back propagated neural network.

Do Chamomile effect on duration, amount of bleeding, and interval of menstrual cycles?

  • Objectives: Heavy menstrual bleeding has a significant effect on women's daily life and health consequences for theirs. Attention to women's and girls' menstrual health is critical for their health. This study presents the results of a novel research on the effect of chamomile capsule on the amount of bleeding, and the duration and interval of menstrual cycles. Methods: The current study was a randomized, double-blind, clinical trial, which was carried out on 118 female students living in dorms of Guilan University of Medical Sciences. The participants were divided into two groups of 59, each receiving either chamomile capsule 250 mg or placebo three times a day, for an interval beginning from 7 days before the starting menstruation till the next onset. Higham chart is the pictorial blood loss assessment to determine the amount and duration of menstrual bleeding. Data were analyzed using SPSS version 20. Alpha was set at 0.05 for all analyses. Results: The average amount of bleeding in the chamomile group decreased after taking the capsule (p = 0.001). However, statistical tests did not show any significant difference in terms of duration and interval between two bleeding in both the experimental and control groups (p > 0.05). Conclusion: The present study showed that the chamomile capsule decreases the amount of menstrual bleeding and can be used as a therapeutic method. Production of Bovine Transgenic Somatic Cell Nuclear Transfer Embryos by Demecolcine-assisted Enucleation (Demecolcine-assisted Enucleation에 의한 소 형질 전환 핵이식란 생산)

  • 본 연구는 소 형질 전환 체세포 핵이식에서 용이한 탈핵을 위해 demecolcine을 이용할 시 탈핵율과 핵이식란의 발육능을 높이기 위한 최적의 조건을 알아보고자 실시되었다. 도축장 유래 미성숙 난자를 18시간 체외성숙 후 제1극체가 확인된 성숙 난자를 0.1, 0.2, 0.4 및 0.8 ug/ml의 demecolcine이 첨가된 배지에서 1시간 더 처리한 다음 세포막이 돌출되어 있는 난자를 체세포 핵이식에 공여하여 각 군간 배반포로의 발육능을 비교하였다. Demecolcine 처리 후 핵이 포함된 셰포막의 protrusion rates를 각 군간 비교한 결과 0.2, 0.4 및 0.8 ug/ml 군에서 0.1 ug/ml 군보다 유의적으로 높았으며 (82.8, 86.2, 90.4 vs. 70.1%), conventional blind 방법과 비교한 결과 demecolcine를 이용한 군에서 유의적으로 높은 탈핵율을 보였다(75.3 vs. 96.2%; p<0.05). 체세포 핵이식란의 발육능 비교에서는 0.1 및 0.2ug/ml 군에서 대조군과 함께 유의적으로 높은 분할율 및 배반포로의 발육능을 보였다(p<0.05). 결론적으로 소 형질전환 체세포 핵이식을 위한 탈핵시 높은 탈핵율과 배반포로의 발육율을 얻을 수 있는 demecolcine의 적정농도는 0.2ug/ml이라고 사료된다.
