A Study on Surgical Treatment of Thymoma -Analysis of 18 Cases- (흉선종의 외과적 치료에 대한 연구 -18례 분석-)

  • Eighteen patients with thymoma have undergone surgical treatment at Pusan National university Hospital from 1983 to 1993. The mean age of the patients was 47.3$\pm$13.3 years old and male to female ratio was 1.6 : 1. Eleven of the patients[61.1%] with thymoma had myasthenia gravis; 12 cases[66.7%] were encapsulated and 6[33.3%] invasive. Operation was complete resection in 14[77.8%] of the patients, partial resection with residual tumor in 3[16.7%], and simple biopsy in one[5.5%]. Adjuvant radiotherapy was administered in 5 of 6 invasive thymoma. Postoperatively one patient died for excessive bleeding and sepsis. A tumor relapse was observed in one patient who had invasive thymoma and underwent partial resection followed by postoperative radiotherapy. The overall 5-year survival rate was 88.2$\pm$ 7. 9%.The presence of myasthenia gravis is not considered as an adverse factor for survival but it contributes to early discovery of associated thymoma.

  • This study is aimed for investigating the influence of mortar on improved of surface shape of crushed sand, and analyzing the physical properties of fresh state and hardened state. By the test results, it was found that the flow value and bleeding ratio was increased, but the change of flow value according to time was decreased with the improved surface shape of crushed sand. Also, comparing improved of surface shape of crushed sand with not improved of surface shape of crushed sand on strength, compressive strength is about the same and flexural strength decrease in case of improved of surface shape of crushed sand.

  • The physical properties of freshly mixed steel fiber reinforced recycled concrete(SFRRC) were_investigated. Physical state and quality of fresh concrete were monitored batch by batch. the properties of freshly mixed SFRRC were determined in terms of slump, air content, superplasticizer, and bleeding ratio. Concrete mixtures were produced for three kinds of aggregate proportions. So, the experimental variables are various aggregate proportions, steel fiber contents(0, 0.5, 1.0, 1.5%) and steel length(30, 50, 60mm). From experimental results, optimum s/a by various experimental variables, variations of the slump by the air contents, and optimum superplasticizer to decrease unit water were presented.

Clinical Study of Hemoptysis in Patients with Pulmonary Tuberculosis (각혈을 동반한 폐결핵환자의 임상적 고찰)

  • We have reviewed 73 patients who experienced hemoptysis with pulmonary tuberculosis to analyze the factors contributing hemoptysis and its management The result are; 1. The age group between 30 * 50 years old was major prevalent group [59%]. 2. Sexual ratio was 3.9 to 1, as 58 men and 15 women. 3. In 58 patients [79.5 %], the number of hemoptysis in frequency was less than 5 times. 4. The quantity of hemoptysis was ranged from less than 50 ml to more than 600 ml and 20 patients [27.4%] experienced bleeding more than 600 ml at once. 5. There was no seasonal difference in hemoptysis. 6. The common prodromal symptoms of hemoptysis were coughing, fatigability, hot foreign sensation of throat in order of frequency. 7. In chest roentgenographic finding, the cavitary lesion was the most common finding, 67% [51 cases], and the multiple cavity was 19.2%[14 cases]. 8. 23 patients [31.37%] were operated for control of hemoptysis and pulmonary tuberculosis itself, 7[30.43 %] of them suffered from complication of empyema, respiratory failure, empyema with bronchopulmonary fistula. 9. One patient [4%] was died due to respiratory failure after left thoracoplasty.

  • The objective of this report is to investigate and analyze the influnce of the different base concrete in consistency for the good production of superplasticized concrete (SPC) on the basis of the experimental results. The principal conclusions are summarized as follows. 1. SPC exhibited only slight bleeding in both cases of low and medium consistency of the base concrete, compared to the conventional concrete, compared to the coventional concrete. 2. SPC lost slump and flow value at a much faster rate than the conventional concrete with an equivalent water/cement ratio and initial consistency. 3. The compressive strength of SPC was fount to be higher than that of base and conventional concrete, and the case of base concrete with medium consistency showed a little more incretment than low consistency

  • The object of this study is to compare properties of massive fly-ash concrete with plain concrete. Two concrete mixtures comprising two batch each $1.0m^3$ in volume, were made from ready mixed concrete batch plant. The water-to-cementitious materials ratio was kept constant at 51.4%. Therefore, massive concrete specimen($W800{\times}D800{\times}H800mm$) was cast from ready mixed concrete to analyze history of temperature and core strength properties. Bleeding, time of slump loss and time of setting of the fresh concrete were measured. In order to estimate the properties of massive fly-ash concrete in hardened concrete, non-destructive tests such as rebound hardness, ultrasonic pulse velocity and maturity were performed and analyzed.

Effect of S.E.C Mixing on the Properties of Concrete (S.E.C 방식에 의한 콘크리트의 혼합효과에 관한 연구)

  • Conventional concrete mixing method is to put all of the materials simultaneously into a mixer and mix for a required time. However, recently concrete researchers have reported that mixing sequence iufluences the properties of concrete. This study discusses the influence of mixing sequence and partitioning addition of mixing water. Concrete, by method of partitioning addition of mixing water, was found to have substantially stronger strength than conventional concrete with the same water-cement ratio. This means that a higher strength concrete could be obtained by using “Sand Enveloped with Cement”(S.E.C) mixing technique. Both a high bond strength between cement paste and aggregate, and elimination of bleeding both contribute to improving the strength of S. E. C concrete.

A review of the total correction of tetralogy of Fallot (Fallot 4징증의 근치수술에 대한 임상적 고찰)

  • The experience with operative treatment for total correction of Tetralogy of Fallot at the department of Thoracic and Cardiovascular Surgery, Korea University Hospital from January, 1977, through April, 1983 was reviewed. Of the 29 patients reviewed, male to female occurrence ratio was 22:7 Type of V.S.D. was type II in 26 cases[90%] and total conus defect in 3 cases[10%]. Average size of V.S.D. was 19 mm. Type of Right ventricular outflow tract stenosis was highest frequency with pulmonary valvular and infundibular stenosis combined type in 21 cases[72%], and there were 8 deaths In this group. Type of R.V.O.T. reconstruction contains 2 cases of infundibulectomy only, 1 case of infundibulectomy with valvular commissurotomy, 2 cases of pericardial patch, 23 Cases of pericardial patch with Teflon or Dacron felt reinforced and 1 case of pulmonary valved conduit reconstruction. Operative mortality was higher in outflow patch through pulmonary valve ring. Overall mortality was 31%. Major causes of death and postoperative complications were low output syndrome, complete A-V block acute renal failure, ventricular fibrillation, bleeding brain abscess, and sudden cardiac arrest.

On/Off-Design/Transient Analysis of a 50KW Turbogenerator Gas Turbine Engine (50KW 터보제너레이터용 가스터빈 엔진의 설계점/ 탈설계/과도성능해석)

  • Present paper describes on/off design performance of a 50KW turbogenerator gas turbine engine for hybrid vehicle application. For optimum design point selection, relevant parameter study is carried out. The turbogenerator gas turbine engine for a hybrid vehicle is expected to be designed for maximum fuel economy, ultra low emissions, and very low cost. Compressor, combustor, turbine, and permanent-magnet generator will be mounted on a single high speed (82,000 rpm) shaft that will be supported on air bearings. As the generator is built into the shaft, gearbox and other moving parts become unnecessary and thus will increase the system's reliability and reduce the manufacturing cost. The engine has a radial compressor and turbine with design point pressure ratio of 4.0. This pressure ratio was set based on calculation of specific fuel consumption and specific power variation with pressure ratio. For the given turbine inlet temperature, a rather conservative value of $1100^\circK$ was selected. Designed mass flow rate was 0.5 kg/sec. Parametric study of the cycle indicates that specific work and efficiency increase at a given pressure ratio and turbine inlet temperature. Off design analysis shows that the gas turbine system reaches self operating condition at N/$N_{DP}$ = 0.53. Bleeding air for turbine stator cooling is omitted considering low TIT and for a simple geometric structure. Various engine performance simulations including, ambient temperature influence, surging at part load condition. Transient analysis were performed to secure the optimum engine operating characteristics. Surge margin throughout the performance analysis were maintained to be over 80% approximately. Validation of present results are yet to be seen as the performance tests are scheduled by the end of 1998 for comparison.

Prognostic Value of Serum S100 Protein by Elecsys S100 Immunoassay in Patients with Spontaneous Subarachnoid and Intracerebral Hemorrhages

  • Objective: The serum S100 protein has been known to reflect the severity of neuronal damage. The purpose of this study was to assess the prognostic value of the serum S100 protein by Elecsys S100 immunoassay in patients with subarachnoid hemorrhage (SAH) and intracerebral hemorrhage (ICH) and to establish reference value for this new method. Methods: Serum S100 protein value was measured at admission, day 3 and 7 after bleeding in 42 consecutive patients (SAH : 20, ICH : 22) and 74 healthy controls, prospectively. Admission Glasgow coma scale (GCS) score, Hunt & Hess grade and Fisher grade for SAH, presence of intraventricular hemorrhage, ICH volume, and outcome at discharge were evaluated. Degrees of serum S100 elevation and their effect on outcomes were compared between two groups. Results: Median S100 levels in SAH and ICH groups were elevated at admission (0.092 versus $0.283{\mu}g/L$) and at day 3 (0.110 versus $0.099{\mu}g/L$) compared to healthy controls ($0.05{\mu}g/L;$ p<0001). At day 7, however, these levels were normalized in both groups. Time course of S100 level in SAH patient was relatively steady at least during the first 3 days, whereas in ICH patient it showed abrupt S100 surge on admission and then decreased rapidly during the next 7 days, suggesting severe brain damage at the time of bleeding. In ICH patient, S100 level on admission correlated well with GCS score (r=-0.859; p=0.0001) and ICH volume (r=0.663; p=0.001). A baseline S100 level more than $0.199{\mu}g/L$ predicted poor outcome with 92% sensitivity and 90% specificity. Logistic regression analyses showed Ln (S100) on admission as the only independent predictor of poor outcome (odd ratio 36.1; 95% CI, 1.98 to 656.3) Conclusion: Brain damage in ICH patient seems to develop immediately after bleeding, whereas in SAH patients it seems to be sustained for few days. Degree of brain damage is more severe in ICH compared to SAH group based on the S100 level. S100 level is considered an independent predictor of poor outcome in patient with spontaneous ICH, but not in SAH. Further study with large population is required to confirm this result.
