The effect of various commercially available bleaching agents on the microshear bond strength of composite resin to enamel

  • This study evaluated the microshear bond strength of composte resin to teeth bleached with commercial whitening strips and compared with those bleached with home bleaching gel. Twelve extracted human central incisors were cut into pieces and central four segments were chosen from each tooth and embedded in acrylic resin. Four blocks with 12 tooth segments embedded in acrylic resin were acquired and numbered from group one to group four. Group 1 was bleached with Crest Whitestrips, group 2 with Claren, group 3 with Opalescence tooth whitening gel (10% carbamide peroxide). Group 4 was used as control. The bleaching procedure was conducted for 14 days according to the manufacturer's instructions ; the bleaching strips twice a day for 30 min and the bleaching gel once a day for 2hr. After bleaching, composite resin (Filtek Supreme) was bonded to the enamel surfaces with a self-etching adhesive (Adper Prompt L-Pop) using Tygon tube. Microshear bond strength was tested with a universal testing machine (EZ-test). The data were statistically analysed by one-way ANOVA. The study resulted in no statistical differences in microshear bond strength between the tooth segments bleached with 2 different whitening strips and bleaching gel. It can be concluded that the effect of bleaching with either commercial whitening strips or bleaching gel on enamel is minimal in bonding with self-etching adhesive to composite resin.

Impact of combined at-home bleaching and whitening toothpaste use on the surface and color of a composite resin

  • Objective: This in vitro study aimed to evaluate the effects of different whitening toothpastes on a composite resin during at-home bleaching with 10% carbamide peroxide. Materials and Methods: Sixty samples (7 mm × 2 mm) were used for color and roughness analyses, while another 60 samples (3 mm × 2 mm) were utilized to assess microhardness. The factors analyzed included toothpaste, for which 5 options with varying active agents were tested (distilled water; conventional toothpaste; whitening toothpaste with abrasive agents; whitening toothpaste with abrasive and chemical agents; and whitening toothpaste with abrasive, chemical, and bleaching agents). Brushing and application of whitening gel were performed for 14 days. Surface microhardness (SMH), surface roughness (Ra), and color (ΔL*, Δa*, Δb, ΔE*ab, and ΔE00) were analyzed. The Ra and SMH data were analyzed using mixed generalized linear models for repeated measures, while the color results were assessed using the Kruskal-Wallis and Dunn tests. Results: Between the initial and final time points, all groups demonstrated significant increases in Ra and reductions in SMH. No significant differences were found between groups for SMH at the final time point, at which all groups differed from the distilled water group. Conventional toothpaste exhibited the lowest Ra, while whitening toothpaste with abrasive agent had the highest value. No significant differences were observed in ΔL*, Δa*, and Δb. Conclusions: While toothpaste composition did not affect the color stability and microhardness of resin composite, combining toothbrushing with whitening toothpaste and at-home bleaching enhanced the change in Ra.

Development of a Piezoelectric Ultrasonic Tooth-whitening Apparatus

  • The aim of this study was to develop an ultrasonic tooth-whitening apparatus using piezoelectric transducers, which enhance bleaching efficiency by applying ultrasound, while performing a teeth whitening procedure. For this purpose, an ultrasonic transducer was designed and manufactured, and the effects of reduction in the whitening material's concentration and in the whitening treatment time through using the ultrasound cavitation phenomenon were confirmed. Also, the validity of this study was investigated by comparing the whitening performance with a commercialized optical whitener, through color comparison. The results revealed that the ultrasound whitener produced color values that were enhanced by as much as double that of the conventional LED light whitening method. Even when the operational time was reduced by half, the ultrasound method showed superior performance by over 54% compared to the conventional light whitener, revealing that the ultrasound method showed a remarkable treatment reduction effect.

Evaluation of the effects of whitening mouth rinses combined with conventional tooth bleaching treatments

  • Objectives: The aim of the present study was to evaluate the effect of whitening mouth rinses alone and in combination with conventional whitening treatments on color, microhardness, and surface roughness changes in enamel specimens. Materials and Methods: A total of 108 enamel specimens were collected from human third molars and divided into 9 groups (n = 12): 38% hydrogen peroxide (HP), 10% carbamide peroxide (CP), 38% HP + Listerine Whitening (LW), 10% CP + LW, 38% HP + Colgate Plax Whitening (CPW), 10% CP + CPW, LW, CPW, and the control group (CG). The initial color of the specimens was measured, followed by microhardness and roughness tests. Next, the samples were bleached, and their color, microhardness, and roughness were assessed. Data were analyzed through 2-way analysis of variance (ANOVA; microhardness and roughness) and 1-way ANOVA (color change), followed by the Tukey post hoc test. The Dunnett test was used to compare the roughness and microhardness data of the CG to those of the treated groups. Results: Statistically significant color change was observed in all groups compared to the CG. All groups, except the LW group, showed statistically significant decreases in microhardness. Roughness showed a statistically significant increase after the treatments, except for the 38% HP group. Conclusions: Whitening mouth rinses led to a whitening effect when they were used after conventional treatments; however, this process caused major changes on the surface of the enamel specimens.

수종 치아미백제가 구강내에서 법랑질의 미세경도에 미치는 영향 (The Effects of Tooth Bleaching Agents on Microhardness of Enamel in situ)

  • 본 연구는 치아미백용 2.6% hydrogen peroxide 부착대와 통상 home bleaching용으로 안전하다고 알려진 10% carbamide peroxide 미백젤이 법랑질 미세경도에 미치는 영향을 미백을 하지 않은 경우와 비교, 평가하기위하여 시행되었다. 최근에 발거한 사람의 상악 절치 20개를 사용하여 가로, 세로 2 mm의 정사각형 치아편을 각 치아에서 3개씩 절단, 채득한 후 교정용 bracket에 고정하여 시편을 제작하였다. 자원자는 20명을 선정하였고, 각각의 자원자의 치아에 동일 치아에서 제작한 시편이 부착된 bracket을 순면 중앙에 부착하였으며 상악 우측 측절치에 젤을 이용한 군 (Opalescence tooth whitening gel, Ultradent Product Inc, South Jordan, USA), 상악 우측 중절치에 미백을 하지 않은 대조군, 상악 좌측 중절치에 부착대를 이용한 군 (Claren, LG Household & Health Care, Seoul, Korea)을 부착 시켰다. 젤을 이용한 군은 bracket이 부착된 치아에 맞게 상악 우측 측절치부터 우측 소구치까지를 덮는 mouthguard tray를 제작하여 매일 동일시간에 2시간씩 14일 동안 미백을 시행하였으며, 대조군은 부착 28일 동안 미백 치료 없이 구강 내에 위치시켰다. 부착대를 이용한 군은 bracket의 시편을 덮을 수 있는 크기로 상악용 Claren을 절단하여 오전, 오후 각각 30분씩 하루 두 번 부착하여 14일 동안 미백을 시행하였다. 각군 모두 bracket부착 전, 미백 14일 후, 재광화 14일 후의 미세경도를 측정하였으며 측정된 값을 이용하여 미세경도 변화 양상과 변화량을 비교 평가하였다. 치아미백용 2.6% hydrogen peroxide 부착대와 젤 형태의 10% carbamide peroxide 미백제의 미백 전, 미백후, 재광화 후 미세경도 변화 양상이 미백을 하지 않은 대조군과 차이를 보이지 않았으며 (p > 0.05) 미백 전과 미백 후의 미세경도의 차이 미백후와 재광화 후의 미세경도의 차이도 유의할 만한 차이가 없었다 (p > 0.05). 따라서 시중에 판매되고 있는 whitening strip과 미백 젤은 14일 동안의 통상적인 미백과정 동안 법랑질의 미세경도에 영향을 미치지 않는 것으로 사료된다.

Evaluation of at-home bleaching protocol with application on different surfaces: bleaching efficacy and hydrogen peroxide permeability

  • Objectives: This study aimed to evaluate the bleaching efficacy and hydrogen peroxide permeability in the pulp chamber by the at-home bleaching gel in protocols applied on different dental surfaces. Materials and Methods: Forty premolars were randomly into 4 groups: control group no bleaching, only application on the buccal surface (OB), only application on the lingual surface (OL) and application in buccal and lingual surfaces, simultaneously (BL). At-home bleaching gel (White Class 7.5%) was used for the procedure. The bleaching efficacy was evaluated with a digital spectrophotometer (color change in CIELAB [ΔEab] and CIEDE 2000 [ΔE00] systems and Whitening Index for Dentistry [ΔWID]). The hydrogen peroxide permeability in the pulp chamber (㎍/mL) was assessed using UV-Vis spectrophotometry and data were analyzed for a 1-way analysis of variance and Tukey's test (α = 0.05). Results: All groups submitted to bleaching procedure showed bleaching efficacy when measured with ΔEab and ΔE00 (p > 0.05). Therefore, when analyzed by ΔWID, a higher bleaching efficacy were observed for the application on the groups OB and BL (p = 0.00003). Similar hydrogen peroxide permeability was found in the pulp chambers of the teeth undergoing different protocols (p > 0.05). Conclusions: The application of bleaching gel exclusively on the OB is sufficient to achieve bleaching efficacy, when compared to BL. Although the OL protocol demonstrated lower bleaching efficacy based on the ΔWID values, it may still be of interest and relevant in certain clinical scenarios based on individual needs, requiring clinical trials to better understand its specificities.

프라이머를 이용한 3.0% 과산화수소 미백 패치의 임상효과 (Clinical effect of 3.0% Hydrogen peroxide bleaching patch with primer)

  • Purpose The The purpose of this study was to clinically evaluate the efficacy and safety of a self-whitening patch containing a primer containing taurine and 3.0% hydrogen peroxide. Methods A double-blind randomized clinical trial was conducted on 55 subjects. The whitening patches containing 3.0% hydrogen peroxide were applied to the labial surfaces of maxillary six anterior teeth once daily for 30 minutes using a primer, and whitening efficacy was measured by △E* values before application and at 3, 5, 7, and 10 days after application. Stability was determined using the Gingival index (GI) and visual analogue scale (VAS). Results Changes in △E* values were clinically recognizable as early as day 5 after patch application, and whitening effects were visible by day 7. There was no statistically significant difference in gingival index (p=0.069). Conclusions Self-whitening patches using primer and 3.0% hydrogen peroxide applied once daily for 30 minutes showed effective whitening effect from the 5th day after application and could be used safely without significant side effects.

35% 과산화수소에 제2인산칼슘를 함유한 치아미백제가 치아의 색과 경도에 미치는 영향 (Effect of 35% Hydrogen Peroxide with Dicalcium Phosphate Dihydrate on the Tooth Whitening and Microhardness)

  • 이 연구는 전문가 미백(in-office bleaching)에 사용하는 35% 과산화수소(hydrogen peroxide, HP)를 제2인산칼슘(dicalcium phosphate dihydrate, DCPD)와 혼합하여 치아의 미백 효과와 미세경도를 평가하고자 하였다. 소구치로부터 30개의 치아시편을 제작하여 3군으로 분류하였다(n=10). 35%과산화수소에 DCPD를 0.1, 1wt% 첨가하여 실험군으로 하고 하루 60분간 치아미백을 실시하였다. 치아 미백제에 pH를 측정하였고, 미백 적용한 치아 표면에는 색과 미세경도를 측정하였다. pH는 DCPD 함유한 군에서 DCPD의 함유량이 증가할수록 함유되지 않은 군에 비해 pH 수치가 증가하였다. 색조변화량 (${\Delta}$E)을 비교한 결과, 미백 전에 비해 미백 후 색조 변화를 보였고(p<0.05), 결과적으로 색 변화에 있어 DCPD를 함유한 군과 함유되지 않은 군 사이에 통계적으로 유의한 차이는 없었다(p>0.05). 법랑질의 표면미세경도를 분석한 결과, 모든 군에서 미백 후 미세경도의 감소를 보였고(p<0.05), DCPD를 혼합한 군은 함유되지 않은 군에 비해 경도 감소의 폭이 훨씬 적게 나타났다. 이상의 결과로부터 DCPD를 함유한 35%과산화수소의 치아미백제는 치아미백 효과가 있고, pH를 상승시켜서 법랑질의 표면 경도를 덜 감소시키므로 치아미백제의 구성성분으로 실용할 수 있을 것으로 사료된다.

고농도 과산화수소와 플라즈마 아크를 이용한 미백 치료에 있어서 불소의 효과 (Effect of Fluoride Treatment after Bleaching with Hydrogen Peroxide exposed to Plasma Arc)

  • This study evaluated whether fluoride treatment can affect recovery of the irregularity of enamel surface after tooth whitening with a high concentration of hydrogen peroxide (HP) activated by plasma arc light. A total of 36 bovine teeth stained with coke were used in this experiment. The specimens were classified into following three groups (two different commercial plasma arc groups and a control group without light curing source): (1) 35% HP gel only, (2): 35% HP gel and Plasma arc A, and (3) 35% HP gel and Plasma arc B. To measure color changes and surface morphologies before and after the bleaching, colorimeter and scanning electron microscopy were used, respectively. When the specimens were bleached with hydrogen peroxide and plasma arc lights, the bleaching effect was greater than when only hydrogen peroxide gels were used (Kruskal-Wallis test, p<0.05). In addition, plasma arc B showed the more color changes than plasma arc A (Bonferroni post-hoc test, p<0.05). The surfaces of the teeth treated with fluoride gel after the whitening treatment came to be smooth. Therefore, the results of this study suggested that the fluoride application for patients who got tooth whitening therapy with a high concentration of hydrogen peroxide gels activated by plasma arc light will be effective to recover rough enamel surfaces.
