Risk factors and screening timing for developmental dysplasia of the hip in preterm infants

  • Background: The delayed diagnosis of developmental dysplasia of the hip (DDH) requires complex treatment and sometimes progresses to hip osteoarthritis. Purpose: This study aimed to evaluate the risk factors and screening time for DDH in preterm infants. Methods: A total of 155 preterm infants with a gestational age <32 weeks screened for DDH with ultrasonography were enrolled in this retrospective chart review. Results: The incidence of DDH was 6.45% (10 of 155). Gestational age, birth weight, sex ratio, and breech presentation did not differ significantly between infants treated for DDH (n=10) and nontreated infants (n=145) (gestational age, 29.2±1.4 weeks vs. 29.6±2.0 weeks, P=0.583; birth weight, 1,240±237 g vs. 1,295±335 g, P=0.607; female sex, 7 of 10 (70.0%) vs. 77 of 145 (53.1%), P=0.346; and breech presentation, 5 of 10 (50.0%) vs. 43 of 145 (29.7%), P=0.286, respectively). Performing the first ultrasonography earlier than 38 weeks of postmenstrual age (PMA) increased the risk of an abnormal finding by 3.76 times compared to performing it at ≥38 weeks of PMA. These abnormal findings on ultrasonography resolved spontaneously. Breech presentation increased the risk of minor abnormal findings on the first ultrasonography by 3.11 times versus nonbreech presentation and resolved spontaneously. DDH in preterm infants did not occur predominantly on the left side or in infants born with breech presentation. Conclusion: Performing ultrasonography screening earlier than 38 weeks of PMA caused unnecessary subsequent ultrasonography and overtreatment. Breech presentation was not a risk factor for DDH in preterm infants. However, breech presentation could increase the risk of minor abnormal findings at the 1st ultrasonography compared to nonbreech presentation, which resolved spontaneously. The etiology and risk factors for DDH in preterm infants are somewhat different from those for DDH in term infants.

Serum Chemical Values of Korean Native Goats from Birth to Maturity (한국재래산양(韓國在來山羊)의 성장(成長)에 따르는 혈청화학치(血淸化學値)의 변동(變動))

  • Although considerable research has been done on the blood chemistry of domestic animals, little work has been made of the changes associated with age. Moreover, the records about physiology of the goat were not much available in Korea, and a comprehensive survey of the blood values of the Korean native goat has not been made. The object of the present investigation was to make good this deficiency and to suggest standards for the blood chemical values of Korean native goats from birth to maturity. The goats were kept under average farming conditions in Korea. Blood samples were collected from the jugular vein at birth, at one and four days, at one, two, three and four weeks, and at two, three, six, nine and twelve months of age. The results obtained in this work were summarized as follows: 1. The highest concentration of serum glucose was observed at birth and it decreased gradually until three months of age, showing a steady state thereafter. 2. The concentration of total serum protein was a little higher at one day of age than at birth and showed a steady state until seven days of age; then it decreased slightly at two weeks of age and recovered at three weeks of age and showed a steady state until twelve months of age. The concentration of serum albumin was increased a little at one day of age and revealed a steady state thereafter. The concentration of serum globulin showed three phases - a fall during the first two weeks to reach minimum, a rise to the fourth week, and a fall at two months of age, showing a steady state thereafter. The lowest value of albumin fraction was observed at birth, then the value increased gradually until three months of age and revealed a stealer state thereafter. The highest value of globulin fraction was observed at birth, then the gradual decrease of the value was seen until three months of age; thereafter the value showed a steady state. 3. Albumin/globulin ratio was the minimum at birth, then it increased to reach a maximum at two weeks of age and decreased a little thereafter. 4. The concentration of total serum cholesterol showed a gradual increase during the first three months and fell to reach adult revel at six months of age. 5. Urea nitrogen in serum decreased during the first week to reach a minimum, then it increased at three months of age, and showed a fell to mature level at six mouths of age. 6. The concentration of serum creatinine was not affected by age. 7. The concentration of total serum calcium was a little higher during the first two weeks than the other period during the first year of life. 8. The concentration of serum inorganic phosphorus showed a gradual rise to reach a maximum and a gradual fall to reach adult level at nine months of age. 9. No significant sex differences of serum chemical values were recognized.

Incidence and Associated Factors of Infantile Colic in Thai Infants

  • Purpose: Infantile colic, a common functional gastrointestinal condition, causes distress and frustration in families. Its prevalence and incidence vary from community to community. The purpose of our study was to demonstrate the incidence of and factors associated with infantile colic in Thai infants. Methods: We conducted a prospective analytic study to explore the incidence and factors associated with infantile colic in 386 Thai infants aged between one month and six months. Caregivers were interviewed using a questionnaire about infants' symptoms of colic based on the definition from the Rome IV criteria. Family background and potential precipitating factors of colic were also evaluated. Results: The incidence of colic in infants younger than 6 months was 6.5%. All infants' colic started within 12 weeks of life and lasted approximately 6 weeks. Sex, gestational age, birth weight, delivery route, birth order, family factors, and parental factors were not correlated with the occurrence of colic. Infants who were exclusively breastfed for the first 2 months of life had a lower incidence of infantile colic than those who were mixed- or formula-fed (odds ratio=3.0; 95% confidence intervals=1.3 to 7.2). Conclusion: The incidence of infantile colic in Thai infants in our study was 6.5%, which is similar to that in other reports. Being exclusively breastfed for the first two months was the only risk factor in our cohort.

Risk factors for respiratory distress syndrome in full-term neonates

  • Background: Respiratory distress syndrome (RDS) is a one of the most common cause of respiratory morbidity and mortality in neonates. This study was conducted to investigate the risk factors for RDS in full-term neonates. Methods: We conducted this retrospective study using medical records. The study group included 80 full-term neonates diagnosed with RDS and hospitalized in the neonatal intensive care unit between January 2012 and December 2016, at Yeungnam University Hospital. We analyzed sex, gestational age, birth weight, delivery method, maternal age, number of pregnancy, history of abortion, and complication of pregnancy. The control group included 116 full-time neonates who were hospitalized with jaundice during the same period. Results: The incidence of full-term RDS was more common in males (odds ratio [OR], 3.288; 95% confidence interval [CI], 1.446-7.479), cesarean section (OR, 15.03; 95% CI, 6.381-35.423), multiparity (OR, 4.216; 95% CI, 1.568-11.335). The other factors rendered no significant results. Conclusion: The risk factors for RDS in full-tern neonates were identified as male sex, cesarean section, and multiparity. Further studies involving more institutions are needed to clarify the risk factors for RDS in full-term infants.

Ear Type and Coat Color on Growth Performances of Crossbred Pigs

  • Records from a total of 202 crossbred pigs were classified by their ear type and coat color to examine the presence of interrelationships with growth performances. Crossbred pigs were F2 generations of full sib family out of ten Landrace sows bred by 5 Korean domestic boars. Heavily drooped ear type was predominant, 195 out of 202 pigs over the other two types (1 straight and 6 slightly drooped). Coat colors were classified as four categories, all white, all black, dominant white or dominant black. Ratio among coat color categories did not fall within Mendelian principle of independence regarding two loci involved. There was dependency between ear type and coat color. However, due to rarity of ear types other than heavy drooped, dependency comes from distribution of those rare ear types. Three least squares models to test the effect of ear type and coat colors on growth performances were analyzed. First model analyzed effects on birth weight, body weight at 3 and 6 weeks and ADG' before weaning and between 3 and 5 weeks of age. This model included sex in addition to ear type and coat color. Second model analyzed postweaning growth traits (initial weight, final weight and ADG between these periods) upon initiation of performance testing. This model included effects of sex, test group and start age (as a covariate) in addition. Third model was fit for fasted weight before slaughter and included the effects of sex, test group and age at slaughter (as a covariate). The effects of sex and ear type were not significant source of variation for all traits. Test group was a significant source of variation for all the postweaning traits. Effect of coat color was not significant until the initiation of performance testing and became significant then after. Least squares means of dominantly black pigs were significantly lower than the other three coat colored pigs in final weight around 195 days of age and in ADG from the start of performance test and final weight measure.

Overweight Tracking in Primary Schoolchildren and Analysis of Related Factors (초등학생의 과체중 이환율 추적과 관련요인 분석)

  • The purpose of this study was to determine the probability of overweight throughout childhood in relation to the presence of overweight at birch or in early childhood, and presence of overweight in children's parents. Weight and height measures were collected at birth and at ages of 7, 10, 12 years from 655 6th grade primary schoolchildren. Childhood overweight was defined as a body mass index at or above the 85th percentile for age and sex, and overweight in children's parents as a body mass index at or above 27. The prevalence of overweight increased with age of the children. Overweight at birth was not associated with overweight at 12 years of ages. However, overweight at 12 years old was already related to overweight at 7 years old. In comparison to non-overweight peers, overweight children at ages 7(OR = 7.64, 95% CI = 4.32-13.51) and 10 years(OR = 19.69, 95% CI = 11.42-33.94) had a higher rick of becoming overweight at 12 years of age. Among children who was overweight at age 7years, 60.7% remained overweight 5 years later, Yearly increment in BMI of overweight children was larger than that of non-obese children (1.15-1.65kg/m$^2$vs 0.50-0.71kg/m$^2$. As compared with the lower case in mealy increment of BMI, the probability of being overweight at age of 12 years was greater in higher case. BMI values at age 7years were positively correlated with BMI values at age of 10 and 127ear,i, and with annual increments in BMI. But those relationships with birth weight were not observed. Children were at greater risk for overweight if at least ogle parent was overweight. The odds ratio for child overweight associated with maternal overweight was 2.41(95% CI = 1,13-5.IS), and those associated with paternal overweight 1.70(95% CI = 0.92-3.17). And parents' BMIs were positively correlated with children's BMI values and yearly BMI increment. In conclusion, overweight at early childhood and annual inclement in BMI can be important in predicting the prevalence of overweight and the risk that overweight will remain after 7 years of age. The risk of being overweight throughout the childhood increases by the parents'overweight.

Effect of Pregnancy Rate Following Timing of Artificial Insemination after Estrus of Hanwoo Female

  • This study was conducted to investigate optimal time of artificial insemination (AI) to Hanwoo female after natural estrus. AI was occurred 12 and 24 hours after natural estrus in both heifer and multiparous recipient then pregnancy and parturition rates were estimated. Results indicated that AI performed at 24 hours after natural estrus showed significant (p<0.05) higher pregnancy rate in both heifer and multiparous recipient groups with significantly (p<0.05) higher abortion rate. However, there are no significant differences of parturition rate, twin birth and sex ratio in both heifer and multiparous recipient groups. Therefore, our results may suggest that performance of AI at 24 hours after natural estrus promise higher pregnancy rate than AI at 12 hours after natural estrus in both heifer and multiparous recipient.

쌍태임신에 관한 임상 간호학적 관찰

  • It is important to point out that the percental mortality rate of the multiple pregnancy is almost twice of the single pregnancy due mainly to their prematurity and the mothers of twins will also be suffered from the plural births to nurse and take care of them. We therefore studied the incidence, the sex ratio, the types of deliveries, the complications and the percental mortality rate of the twin deliveries in order to promote the development of our field of maternal and child nursing. We selected our materials from the 112 patients who had been admitted and delivered twins in the department of OB. & Gyn. of the S.N.U. Hospital from Jan. 1962 to Dec. 1969. The abstructed results of this study were as follows. 1. The incidence of twins among 7, 731 births was 1:69 2. The occurrence of twins from elder multigravida was 4 to 6 times higher than younger primigravida mothers under 35 years of age. 3. The generational length of 45.5% of all twins was under 38 weeks and the mean birth weight of 1st and 2nd twins were 2, 354.7gm and 2, 215.8gm respectively. 4. The sex ratio of twins was 99 males and 124 females. 5. Presentation of twins: 59 cases (52.69%) were in both vertex. 37 cases (33.03%) in vertex plus breech. 15 cases (13.39%) in both breech and one case (0.89%) in transverse presentation. 6. For the types of deliveries Spontaneous deliveries were in 90 babies (40.6%) Breech extraction in 60 babies (26.3%) Vacuum used in 33 babies (15.0%) Cesarean section in 33 babies (9.8%) Forceps in 9 babies (4.2%) and others such as version and destructive measures in 9 babies (4.1%) 7. Intervals between the 1st and 2nd delivery were analyzed and 93% of 2nd twins were delivered within minutes after 1st delivery. 8. The main complications of twin deliveries were totemic (45.5%), second was postmarital hemorrhage (19.6%) and third was hydrations (7.1%) 9. The mean percental mortality of 1st and 2nd twins was 12.52% and 16.96% respectively of which main cause of death were their prematurity.

Studies on the Safety of Artemisiae Capillaris Herba - With the Perinatal and Lactational Reproductive Toxicity - (인진의 안정성에 대한 연구 - 주산$\cdot$수유기 생식 독성을 중심으로 -)

  • Objectives: To study the effect of Artemisiae Capillaris Herba extracts, that have been used as oriental medicine to treat liver disease, on the perinatal and lactational n;)productive toxicity of SD rats when administered by oral lavage. Methods: Female SD rats were dosed from 6 days of gestation to 3 weeks postpartum. This was conducted in accordance with the recommendations of the KFDA Guidelines for Detection of Toxicity to Reproduction for Medicinal Products. Results: No Artemisiae Capillaris Herba extracts treatment-related changes in clinical signs, mortalities, implantation number, dead fetus number, loss rate of fetus, number of live young, survival rate of fetus, sex ratio of live young, external anomalies, pregnancy periods, viability index, lactational index, survival rate of litters at 4 days after birth or delivery index were demonstrated in any dosed levels in this study. However, the body weight and gains, food consumption and absolute organ weights of brain, adrenal glands, liver, spleen, kidney, ovaries and heart were significantly increased in 2000 or 1000mg/kg-dosing groups and the relative organ Weights of adrenal glands were significantly increased in 2,000mg/kg-dosing groups. Therefore, it was concluded that this increase was natural according to growth. Also, no changes of gross findings, clinical signs, mortalities, body weight and gains, physical development results, necropsy findings, organ weight, faculty test, open filed test and water-filled simple T-maze test, copulation, fertility, pregnancy indices, body weight and gains during gestation periods, necropsy findings, corpora lutea number, implantation number, implantation rate, dead fetus number, post-implantation loss rate, live young, post-implantation survival rate, sex ratio of live young, external anomalies and individual body weights of live young were demonstrated in any dosed levels in this study. Conclusions: It is considered that the NOAEL (No-Observed-Adverse-Effect Level) for perinatal and lactational reproductive toxicity of Artemisiae Capillaris Herba extracts was up to 2000mg/kg/day because no changes of other perinatal and lactational reproductive indices were demonstrated.

Effects of Method and Time of Castration on Growth and Carcass Characteristics of Omani Sheep

  • Thirty two Omani sheep with eight animals each of; intact males (INT), males castrated with rubber rings immediately after birth (RR), males castrated with a burdizzo at 8 weeks of age (BC) and intact females (IF) were fed ad libitum a concentrate diet (CP 16%) plus chopped Rhodesgrass hay (8% CP) from weaning until slaughter at 28 kg. INT lambs grew faster from 9 to 20 weeks of age (p < 0.05) thus they were significantly heavier at 20 weeks of age than BC and IF, but not RR lambs. INT consumed more total feed than other sex groups over the period from 9 to 20 weeks of age. There were no significant differences between lambs of all experimental groups in feed per gain ratio. INT lambs had lower (p < 0.01) dressing percentage (DP) than RR, BC and IF. As a percentage in the empty body weight (EBW), INT had higher proportions of head, feet, empty gut (p < 0.001), liver (p < 0.05) and genitals (p < 0.05) but lower proportions of lungs and trachea (p < 0.05) than BC and IF lambs. INT males had a significantly higher (p < 0.05) proportion of bone than RR and IF lambs but lower (p < 0.05) proportion of fat than RR and IF. As a percentage in EBW, IF had the highest protortion (p < 0.001) of total body fat (TBF) followed by BC and RR whereas INT males had the lowest proportion. There was a general trend of IF having the highest proportion of individual and total non-carcass fat (TNCF) and total carcass fat (TCF) followed by BC and RR lambs whereas INT lambs had the lowest protortions of individual carcass and non-carcass fat depots. There were only few sex or castration effects on carcass tissue distribution. IF had higher proportions of intermuscular fat in the chuck, plate, leg and flank than INT and BC. The current study demonstrated that castration of intensively-raised male Omani native sheep especially at weaning using a burdizzo retarded growth rate and reduced carcass quality by increasing fat content.
