Impact of co-transfer of embryos produced by somatic cell nuclear transfer using two types of donor cells on pregnancy outcomes in dogs

  • Objective: The present study analyzed the influence of co-transferring embryos with high and low cloning efficiencies produced via somatic cell nuclear transfer (SCNT) on pregnancy outcomes in dogs. Methods: Cloned dogs were produced by SCNT using donor cells derived from a Tibetan Mastiff (TM) and Toy Poodle (TP). The in vivo developmental capacity of cloned embryos was evaluated. The pregnancy and parturition rates were determined following single transfer of 284 fused oocytes into 21 surrogates and co-transfer of 47 fused oocytes into four surrogates. Results: When cloned embryos produced using a single type of donor cell were transferred into surrogates, the pregnancy and live birth rates were significantly higher following transfer of embryos produced using TP donor cells than following transfer of embryos produced using TM donor cells. Next, pregnancy and live birth rates were compared following single and co-transfer of these cloned embryos. The pregnancy and live birth rates were similar upon co-transfer of embryos and single transfer of embryos produced using TP donor cells but were significantly lower upon single transfer of embryos produced using TM donor cells. Furthermore, the parturition rate for TM dogs and the percentage of these dogs that remained alive until weaning was significantly higher upon co-transfer than upon single transfer of embryos. However, there was no difference between the two embryo transfer methods for TP dogs. The mean birth weight of cloned TM dogs was significantly higher upon single transfer than upon co-transfer of embryos. However, the body weight of TM dogs did not significantly differ between the two embryo transfer methods after day 5. Conclusion: For cloned embryos with a lower developmental competence, the parturition rate and percentage of dogs that remain alive until weaning are increased when they are co-transferred with cloned embryos with a greater developmental competence.

미숙아에서 경비지속기도양압과 가습고유량비강캐뉼라의 사용에 대한 비교 연구 (High Flow Nasal Cannula versus Nasal CPAP in Preterm Infants)

  • 목적: 본 연구의 목적은 미숙아에서 HHFNC를 이용한 보조환기요법의 사용 빈도와 예후에 미치는 영향을 알아보고자 하였다. 방법: 2008년 1월부터 2011년 3월까지 서울 한양대학교 신생아 중환자실에 입원한 재태주령 32주 미만의 환아들을 대상으로 의무기록을 후향적으로 분석하였다. 첫 번째 2008년 1월부터 2009년 2월까지를 Era 1, 2009년 3월부터 2011년 3월까지를 Era 2으로 구분하여 두 기간 동안 HHFNC 사용 빈도의 변화에 대해 조사하였다. 두 번째로 재태주령 25-30주 미만 환아들 중 출생 14일 이내 보조환기요법으로 NCPAP 또는 HHFNC를 적용 받았던 두 그룹의 임상경과를 비교하였다. 결과: Era 1과 비교해서 HHFNC의 사용은 10에서 55%으로 증가하였고, NCPAP의 사용은 40에서 5%으로 Era 2에서 감소하였다. 재삽관률과 BPD 그리고 산소나 인공환기기를 적용 받은 기간에서는 두 그룹에서 차이가 없었고, 기타 미숙아 합병증에서도 유의한 차이를 보이지 않았다. 그러나 완전 장관 수유까지 걸린 시간과 출생체중으로 회복되는데 걸린 시간이 HHFNC를 적용한 그룹에서 감소하였다. 고찰: HHFNC이 미숙아에서 잘 적용되었고, NCPAP과 비교하여 임상 경과에 유의한 차이를 보이지 않았으나 완전 장관 수유까지 걸린 시간과 출생체중으로 회복되는데 걸린 시간을 줄였다. 향후 전향적인 다기관 연구를 통하여 HHFNC의 안정성과 효과에 대한 연구가 필요하다.

  • Although outcomes of neonatal cardiac surgery have dramatically improved in the last two decades, low body weight still constitutes an important risk factor for morbidity and mortality. In particular, cardiac surgery in neonateswith very low birth weight (${\leq}$1.5 kg) is carried out with greater risk because most organ systems are immature. We report here on a successful case of early one-stage total repair of coarctation of the aorta and a ventricular septal defect in a 1,480 gram neonate.

임신말 모체 및 제대혈의 비타민 B(sub)12 농도와 임신결과와의 상관성 (Serum Vitamin B(sub)12 Levels of Maternal-Umbilical Cord Blood and Pregnancy Outcomes)

  • Vitamin B(sub)12(cobalamin) is an essential nutrient in human and it is particularly important during pregnancy. Nevertheless very few studies have reported, concerning vitamin B(sub)12 in relation with reproduction. This study was conducted to evaluate the vitamin B(sub)12 nutrition status of Korean pregnant women and to investigate the relationship between serum vitamin B(sub)12 levels of maternal-umbilical cord blood and pregnancy outcomes. Dietary vitamin B(sub)12 intakes of the pregnants were estimated by semiquantitative frequency questionnaire. Serum vitamin B(sub)12 levels in both maternal blood and umbilical cord blood of 30 pregnant women at delivery were measured by radioimmunoassay. Mean vitamin B(sub)12 intake was 3.3$\pm$1.4$\mu\textrm{g}$/d which was 125.8% of the Korean RDA(2.6$\mu\textrm{g}$) for vitamin B(sub)12 level of umbilical cord blood was 607.8$\pm$282.9pg/ml, more than two fold of maternal vitamin B(sub)12 level 268.6$\pm$97.8pg/ml. This finding indicates that fetal uptake of vitamin B(sub)12 in the fetus may be due to an active transport mchanism across the placenta. Umbilical cord blood vitamin B(sub)12 levels were highly correlated with maternal levels($r^2$=0.548, p<0.001), showing that fetal vitamin B(sub)12 level is affected by maternal status. However there was no significant correlation between the serum vitamin B(sub)12 levels in maternal-umbilical cord blood and the pregnancy outcomes except for the birth weight. Maternal-umbilical serum vitamin B(sub)12 levels were the highest in the group of birth weight 3.0-3.5kg, and the lowest in the group of birthweight below 3.0kg. Efficacy of intralipid administration to improve in vitro fertilization outcomes: A systematic review and meta-analysis

  • We performed a systematic review and meta-analysis to evaluate whether intralipid administration improved the outcomes of in vitro fertilization. Online databases (PubMed, Cochrane Library, Medline, and Embase) were searched until March 2020. Only randomized controlled trials (RCTs) that assessed the role of intralipid administration during in vitro fertilization were considered. We analyzed the rates of clinical pregnancy and live birth as primary outcomes. Secondary outcomes included the rates of chemical pregnancy, ongoing pregnancy, and missed abortion. We reviewed and assessed the eligibility of 180 studies. Five RCTs including 840 patients (3 RCTs: women with repeated implantation failure, 1 RCT: women with recurrent spontaneous abortion, 1 RCT: women who had experienced implantation failure more than once) met the selection criteria. When compared with the control group, intralipid administration significantly improved the clinical pregnancy rate (risk ratio [RR], 1.48; 95% confidence interval [CI], 1.23-1.79), ongoing pregnancy rate (RR, 1.82; 95% CI, 1.31-2.53), and live birth rate (RR, 1.85; 95% CI, 1.44-2.38). However, intralipid administration had no beneficial effect on the miscarriage rate (RR, 0.75; 95% CI, 0.48-1.17). A funnel plot analysis revealed no publication bias. Our findings suggest that intralipid administration may benefit women undergoing in vitro fertilization, especially those who have experienced repeated implantation failure or recurrent spontaneous abortion. However, larger, well-designed studies are needed to confirm these findings.

A meta-analysis of exposure to particulate matter and adverse birth outcomes

  • Objectives The objective of this study was to conduct a systematic review to provide summarized evidence on the association between maternal exposure to particulate air pollution and birth weight (BW) and preterm birth (PTB) after taking into consideration the potential confounding effect of maternal smoking. Methods We systematically searched all published cohort and case-control studies examining BW and PTB association with particulate matter (PM, less than or equal to $2.5{\mu}m$ and $10.0{\mu}m$ in diameter, $PM_{2.5}$ and $PM_{10}$, respectively) from PubMed and Web of Science, from January 1980 to April 2015. We extracted coefficients for continuous BW and odds ratio (OR) for PTB from each individual study, and meta-analysis was used to combine the coefficient and OR of individual studies. The methodological quality of individual study was assessed using a standard protocol proposed by Downs and Black. Forty-four studies met the inclusion criteria. Results In random effects meta-analyses, BW as a continuous outcome was negatively associated with $10{\mu}g/m^3$ increase in $PM_{10}$ (-10.31 g; 95% confidence interval [CI], -13.57 to -3.13 g; I-squared=0%, p=0.947) and $PM_{2.5}$ (-22.17 g; 95% CI, -37.93 to -6.41 g; Isquared=92.3%, p<0.001) exposure during entire pregnancy, adjusted for maternal smoking. A significantly increased risk of PTB per $10{\mu}g/m^3$ increase in $PM_{10}$ (OR, 1.23; 95% CI, 1.04 to 1.41; I-squared=0%, p=0.977) and $PM_{2.5}$ (OR, 1.14; 95% CI, 1.06 to 1.22; I-squared=92.5%, p<0.001) exposure during entire pregnancy was observed. Effect size of change in BW per $10{\mu}g/m^3$ increase in PM tended to report stronger associations after adjustment for maternal smoking. Conclusions While this systematic review supports an adverse impact of maternal exposure to particulate air pollution on birth outcomes, variation in effects by exposure period and sources of heterogeneity between studies should be further explored.

Risk Factors for Preterm Birth and Low Birth Weight Among Pregnant Indian Women: A Hospital-based Prospective Study

  • Objectives: The present study was undertaken to study the maternal risk factors for preterm birth (PTB) and low birth weight (LBW) with a special emphasis on assessing the proportions of maternal genitourinary and periodontal infections among Indian women and their association with adverse pregnancy outcomes. Methods: A hospital-based prospective study comprising 790 pregnant women visiting the obstetrics clinic for a routine antenatal check-up was undertaken. Once recruited, all study participants underwent clinical and microbiological investigations for genitourinary infections followed by a dental check-up for the presence of periodontitis. The study participants were followed up until their delivery to record the pregnancy outcomes. Infectious and non-infectious risk factors for PTB and LBW were assessed using univariate and multivariate Cox regression analysis. Independent risk factors for PTB and LBW were reported in terms of adjusted relative risk (ARR) with the 95% confidence interval (CI). Results: Rates of PTB and LBW in the study population were 7.6% and 11.4%, respectively. Previous preterm delivery (ARR, 5.37; 95% CI, 1.5 to 19.1), periodontitis (ARR, 2.39; 95% CI, 1.1 to 4.9), Oligohydramnios (ARR, 5.23; 95% CI, 2.4 to 11.5), presence of Nugent's intermediate vaginal flora (ARR, 2.75; 95% CI, 1.4 to 5.1), gestational diabetes mellitus (ARR, 2.91; 95% CI, 1.0 to 8.3), and maternal height <1.50 m (ARR, 2.21; 95% CI, 1.1 to 4.1) were risk factors for PTB, while periodontitis (ARR, 3.38; 95% CI, 1.6 to 6.9), gestational hypertension (ARR, 3.70; 95% CI, 1.3 to 10.8), maternal height <1.50 m (ARR, 2.66; 95% CI, 1.3 to 5.1) and genital infection during later stages of pregnancy (ARR, 2.79; 95% CI, 1.2 to 6.1) were independent risk factors for LBW. Conclusions: Our study findings underscore the need to consider screening for potential genitourinary and periodontal infections during routine antenatal care in developing countries.

초등학교 취학나이가 대학진학에 미치는 영향 (The Effect of Age at School Entry on Long-term Educational Attainment in Korea)

  • 본 연구는 초 중등교육법에 의해 규정된 초등학교 입학 시기와 개인별 출생월의 차이가 가져오는 학생들 간의 초등학교 취학나이의 차이가 대학진학에 미치는 영향을 두 가지 측면에서 분석했다. 첫 번째로 3월에서 12월 출생자와 이들보다 표기 연령이 1살이 어린 1, 2월 출생자의 대학진학 결과를 비교하여 취학연령의 효과를 분석했다. 두 번째로 월로 계산한 초등학교 입학당시의 나이인 취학월령의 효과를 분석했다. 최초 입시에서 1, 2월 출생자의 대학진학 성과가 좋지 않았으나, 최종 입시에서 차이가 완화되었다. 취학월령이 많을수록 최초 입시와 최종 입시에서 4년제 대학에 진학할 확률이 높았다.

임신 중 흡연에 의한 태아학대: 태아생존권 옹호를 위한 접근 (Critical Discussion on Smoking During Pregnancy as a Form of Fetal Abuse: An Approach to Advocate for Fetal Right to Life)

  • Purpose: Smoking during pregnancy contributes to the risk of negative health outcomes in mothers and babies. The purposes of this study were to review the harmful effects of maternal smoking during pregnancy on fetal and child development, to discuss if maternal smoking should be criminalized as a form of child abuse, and to explore advocating for fetal rights. Methods: A variety of published literature and legal documents including the Korean constitution, criminal laws, and children's welfare laws were reviewed and critically analyzed. Results: Women who smoke during pregnancy are more likely to experience abortion related to placental dysfunction. Their unborn risk premature birth, fetal growth restriction, low birth weight, neurobehavioral disturbances, and/or other complications and newborn babies are also at risk for complications. The advocates for fetal rights can assert that maternal smoking should be regarded as a crime. Conclusion: Findings show that maternal smoking during pregnancy is a major risk factor for many adverse pregnancy outcomes. Effective strategies and health policies for smoking cessation during pregnancy are required to protect pregnant women and their babies.

신생아 호흡곤란증후군에서 인공 폐 표면활성제 조기요법의 중요성과 필요성 (The Importance and the Need of Early Pulmonary Surfactant Therapy in Premature Infant with Respiratory Distress Syndrome)

  • Pulmonary surfactant (PS) therapy in premature infants has a remarkable impact on improving survival and outcomes in neonatal respiratory distress syndrome (RDS). Early PS therapy involves instillation of PS upon delivery of very premature infants or if there is evidence of RDS, such as an increased requirement of oxygen 2 hours after birth, especially in infants <30 weeks gestation. Early PS treatment in very premature infants results in a significant reduction in the severity of RDS, mortality, and incidence of pneumothorax, pulmonary interstitial emphysema, and bronchopulmonary dysplasia in comparison with late PS treatment. According to European and American consensus guidelines on the management of neonatal RDS, early PS instillation should be considered for infants <30 weeks gestation, infants with a birth weight <1,250 g, or if the mother has not received antepartum corticosteroids. We suggest that the Korean health insurance policy on RDS be modified so that PS can be used for better clinical outcomes of very premature infants.
