Nasometer 활용 바이오피드백 기법을 이용한 비인강폐쇄전환자의 치험 사례 (Speech treatment of velopharyngeal insufficiency using biofeedback technique with NM II; A case report)

  • Velopharyngeal Insufficiency(VPI); the failure of velum, the lateral wall and the posterior pharyngeal wall to separate the nasal cavity from pharyngeal cavity during speech, can be caused by congenital conditions include cleft palate, submucous cleft palate and congenital palatal insufficiency. Speech problems of VPI are characterized by hypernasality, nasal air emission, increased nasal air flow and decreased intelligibility. These speech problems of VPI can be treated with the surgical procedure, the application of temporary prosthesis and speech therapy. Biofeedback technique with Nasometer is a speech treatment method of VPI that commonly used as one component of a comprehensive procedure for improvement of speech in patients with VPI. In this article describes a case of VPI treated by biofeedback technique with Nasometer; which showed satisfactory result in nasalance and formant analysis after the speech therapy during 9 months.

  • In this paper, we present the developement of Biofeedback system. Biofeedback is a method for learned control of physiological response of the body. It can be used to control certain biological response that cause health problems. Over the past 2 decades, biofeedback has received much attention from medicine as a cure-all for a variety of disease. It has been heralded as a technique that can provide normalization of disturbed physiology without the need for drugs or surgery and their attendant negative side effects. Biofeedback system has been implemented by using PIC microprocessor. We have displayed 5-channel value on PC and managed patient information by using Delphi.

  • Increasingly nursing science is embracing the concepts and methodology derived from psycho-neuroimmunology. It has been previously shown that stress increases and immune function declines in students undergoing examinations. To date, however, no many studies have been reported on stress levels, immune function and interventions in Korean students undergoing their first clinical nursing rotation. It was proposed that nursing students during their first clinical rotation experience increase in stress because of the novelty of the situation and their lack of clinical knowledge. It was also hypothesized that biofeedback and progressive relaxation, methods of self-regulation of involuntary autonomic nervous system responses, would reduce the stress response. The purpose of this study is to test the effectiveness of progressive muscle laxation using biofeedback The effectiveness of the experimental methods was tested by measuring the degree of symptoms of stress (SOS) and the values of ephinephrine, pulse rate, blood pressure and natural killer cells. The subjects of this study were thirty nursing students divided into two groups: experimental group was progressive muscle relaxation group using biofeedback and control group. This study was conducted for 8 weeks of clinical practice. Biofeedback training was done by software developed by J&J company (1-410 form for progressive muscle training). Progressive muscle relaxation training according to Jacobson's Theory was done by messaged word from biofeedback. The data was analyzed using Chronbach' ${\alpha}$ and t-test of the SPSS program and the significance level of statistics was 5%. The results of the study were : 1) The progressive muscle relaxation training using biofeedback was effective for the reduction of symptoms of stress(t=-4.248, p<.001) under clinical practice stress conditions. 2) The progressive muscle relaxation training using biofeedback was not effective for the values of epinephrine(t=-1.294, p=.206). 3) The progressive muscle relaxation training using biofeedback was effective for the reduction of systolic blood pressure (t=-2.757, p=.01). 4) The progressive muscle relaxation training using biofeedback was effective for the reduction of diastolic blood pressure (p=-2.032, 0=.05). 5) The progressive muscle relaxation training using biofeedback was not effective for the reduction of pulse rate(t=-15, p=.988). 6) The progressive muscle relaxation training using biofeedback was effective for the maintenance of natural killer cells (t=2.381, p=02). The first clinical rotation for student nurses is a stressful experience as seen by the rise in the SOS in the control group. Biofeedback using progressive muscle relaxation were effective in preventing the rise of symptoms of stress and the blood pressure means when comparing the pre to post clinical experience, The mean natural killer cell count was depressed in the control group but not significantly different in the experimental groups, It is proposed here that stress via the hypothalamic - pituitary - adrenal axis suppressed the NK cell count whereas the relaxation methods prevented the rise in stress and the resulting immune depression. We recommend relaxation techniques using biofeedback as a health promotion technique to reduce psychological stress. In summary. the progressive muscle relaxation training using biofeedback was effective for the reduction of symptoms of stress under clinical practice stress conditions.

  • Biofeedback devices have been used successfully to improve head control and symmetrical walking in cerebral palsied children. Biofeedback of postural sway was investigated as a therapeutic technique to reduce postural abnormality in 24 selected children with cerebral palsy. Subjects were evaluated their weight distribution of both sides during their standing before and after the visual and auditory feedback training. The effectiveness of biofeedback was compared to conventional physical therapy practices in reestabilishing symmetrical standing in cerebral palsied children. Our study found biofeedback was effective in training symmetrical standing posture.

근전도 바이오피드백을 통한 PNF운동이 긴장성 요실금 환자의 증상에 미치는 영향 -사례연구- (Effects of PNF Exercise on EMG Biofeedback Symptoms of Stress Urinary Incontinence Patients -A Case Study-)

  • Purpose: The purpose of this study was to investigate changes in urinary frequency, residual urine volume, and quality of life following pelvic floor exercises using proprioceptive neuromuscular facilitation (PNF) exercise patterns and EMG biofeedback training in patients with stress urinary incontinence. Methods: The subjects were male patients diagnosed with stress urinary incontinence. This study used a single system design (A-B-C.) At baseline, the patients' symptoms prior to the treatment intervention were recorded (A section). Next, the patients performed the PNF exercise (B section). Thereafter, they performed the PNF exercise, with EMG biofeedback (C section). The subjects performed the exercises in each section for 1 week for a total of 3 weeks. Urinary frequency, residual urine volume, and quality of life of the subjects were measured. Results: The frequency of urination was 9 times in A, 8 times in A 'and B, and 7 times in C. The amount of residual urine decreased from 23.78ml in A to 21.85ml in A ', 14.85ml in B, and 14.63ml in C. The international prostate symptom score was 16 points in A, 14 points in A ', 11 points in B, and 7 points at A. The quality of life score was 4 points in A, 4 points in A ', 3 points in B, and 2 points in A. Conclusion: Both the PNF exercise and EMG biofeedback decreased urination frequency and residual urine volume and improved the quality of life of patients with stress urinary incontinence. EMG biofeedback training using the PNF technique was the most effective.

  • 건강한 사람들은 복잡한 양상의 심박동수 변이 (Heart rate variability, 이하 HRV)를 보이는데, 이것은 생체의 항상성 유지를 위한 반응을 반영한다. 0.1Hz 내외의 속도로 호흡하도록 바이오피드백 훈련을 하면 호흡 동성 부정맥(Respiratory sinus arrhythmia, 이하 RSA)이 극대화되면서 심박동수 변이의 강도가 증가한다. 이러한 효과를 극대화하려면 압반사계에 의해 자연스럽게 발생하는 것으로 추정되는 리듬과 호흡동성 부정맥(RSA)사이의 동조가 일어나는 지점까지 호흡 속도를 늦춰야 한다. 기구를 이용한 바이오피드백 훈련을 통해 각자에게 알맞은 최적의 호흡 속도를 찾을 수 있다. 본고에서는 RSA 바이오피드백 원리에 따른 훈련 과정과 지침을 요약하여 제시하였다.

목의 기능장애 환자들에게 SNAGs기법과 바이오피드백 훈련이 통합적 고유수용성 감각과 목의 기능에 미치는 효과 (The Effect of SNAGs and Biofeedback Training on the Integrative Propriocepcion and Function of the Patients with Cervical Disorder)

  • 본 연구는 SNAGs(Sustained natural apophyseal glides)기법과 시각을 융합한 바이오피드백 훈련이 목 기능 장애 환자에게 고유수용성 감각과 목의 기능장애에 미치는 영향을 알아보고자 하였다. 목 기능장애 환자 31명을 대상으로 무작위로 SNAGs기법과 바이오피드백 훈련으로 할당하였으며, 관절위치 감각과 목 기능장애 평가를 중재 전-후 실시하였다. 16명의 SNAGs 중재그룹은 멀리건 벨트를 이용하여 굽힘, 폄, 회전을 실시하였고, 15명의 바이오피드백군은 시각적 피드백을 통한 깊은목 굽힘근 훈련을 실시하였다. 두 집단은 동일한 시간, 동일한 조건에서 20분 동안 주당 2회를 4주간 실시하였다. 바이오피드백 훈련 집단이 SNAGs 집단보다 고유수용성 감각에서 좌, 우 회전 모두 유의한 향상을 보였으며(p< 0.05), 목 기능장애는 SNAGs기법을 이용한 집단이 바이오피드백 훈련 집단보다 유의한 효과를 보였다(p<0.05). 따라서 SNAGs기법은 목의 기능제한 해결에 효과적이며, 바이오피드백 훈련은 고유수용성 감각 향상에 긍정적인 중재 방법이 될 것이다. 향후 목 통증 환자에 대한 연구를 진행시, 질환의 특성에 따라 중재방법을 선택 할 수 있을 것이다.

호흡연동 방사선 치료에서 호흡생체자기제어 방식이 호흡 재현성 및 선량에 미치는 영향 평가 (The Effect of Breathing Biofeedback on Breathing Reproducibility and Patient's Dose in Respiration-gated Radiotherapy)

  • 호흡 연동 방사선 치료 시 사용되는 음성 유도 및 음성-영상 유도의 두 가지 호흡생체자기제어 방식이 호흡 재현성 및 선량에 미치는 영향을 평가하였다. 본 연구에서는 Medical College of Virginia (MCV)에서 획득한 19명의 폐암 환자에 대한 호흡 데이터를 이용하였다. 호흡 데이터는 총 5주간 1주 간격으로 자유 호흡, 음성 유도, 음성-영상 유도의 세 가지 형태로 획득하였으며 선량 평가는 MATLAB을 이용하였다. 그 결과, 자유 호흡의 경우에는 반복되는 치료에서 호흡재현성이 감소하여 CTV 선량이 약 30.0% 감소하는 것을 알 수 있었으며, 음성-영상 유도 방식을 이용할 경우 5주 후 CTV 선량이 20.0% 개선됨을 알 수 있었다. 이는 환자가 영상을 통하여 자가호흡조절 능력이 향상되기 때문으로 판단된다. 또한 음성 유도만 사용할 경우에도 호흡재현성을 유지하는 데는 효과적임을 확인하였다.

심박수 바이오피드백기법을 활용한 국가대표 후보 양궁선수의 각성조절훈련 효과 분석 (Effect of Arousal Control Training Conjugated Heart Rate Biofeedback for National Substitute Archery Players)

  • 본 연구는 국가대표 후보 여자양궁선수 3명을 대상으로 체계적인 심박수 바이오피드백기법을 활용한 각성조절훈련을 양궁현장에 적용함으로써 그 효과성을 검증하는 동시에 경기수행력을 향상시키는데 그 목적이 있다. 훈련 프로그램은 국내 외 문헌 분석과 스포츠심리전문가 1인, 양궁 지도자 3인으로 구성된 전문가 회의를 통해 최종 선정되었다. 세부 내용으로는 기초이론교육, 호흡조절법, 점진적 이완기법, 심박수 바이오피드백기법, 인지재구성, 루틴 등이며, 총 24주의 훈련기간을 기초훈련단계, 숙련 1 훈련단계, 숙련 2 훈련단계, 실전훈련단계로 구분하여 실시하였다. 심박수 바이오피드백훈련기기는 (주)스포닉스에서 개발한 심박수 측정기기(XCOACH)를 사용하였다. 훈련의 효과를 분석하기 위해 경쟁상태불안검사지, 국내양궁대회에서의 성적 및 기록, 선수 관찰 및 면담과 선수 자기평가 등이 이루어졌다. 이러한 연구 과정을 통해 분석된 결과, 인지적 상태불안요인과 신체적 상태불안요인의 평균점수는 낮아졌으며, 자신감요인은 높아진 것으로 나타났다. 양궁대회에서의 성적 및 기록 역시 현저하게 향상된 것으로 나타났다. 선수 관찰, 면담, 자기평가의 분석 결과, 본 훈련에 대한 참여도와 효과 인식도가 매우 높았으며, 훈련에 의해 적용된 각성조절방법을 시합 중에 적극 활용하여 심리적 안정과 최상의 수행력을 발휘하였다. 따라서 심박수 바이오피드백기법을 활용한 각성조절훈련은 양궁선수의 심리기술력과 경기력 향상에 긍정적인 것으로 나타났다.
