A Case Report of Hemifacial Microsomia (반측안면 왜소증의 치험 1례)

  • This is a case report and review of literature that deals with hemifacial microsomia corrected by costochondral graft, Lefort I osteotomy and bilateral intraoral sagittal split ramus osteotomy. Patient, 23 years old female, had visited to treat the esthetic problem due to a deviation of jaw. On the basis of clinical and radiographic examinations, she was diagnosed as hemifacial microsomia. First, costochondral graft was performed to bridge the defect between glenoid fossa and body of mandible. After 11 months, Patient was performed a Lefort I osteotomy and bilateral intraoral sagittal split ramus osteotomy to create a symmetric jaw. Patient was satisfied with final esthetics and there have been no evidence of infection ill now.

A Therapeutic Experience of Congenital Bilateral Neuroblastoma (선천성 양측성 신경모세포종의 치료경험 1례)

  • Neuroblastoma is the most common intraabdominal malignant tumor of childhood, with 40% arising from the adrenal gland. Bilateral adrenal involvement from synchronous development or metastatic spread of tumor is rarely seen in children with neuroblastoma. The patient was born with a spontaneous vaginal delivery. Birth weight was 3,200 g. Fetal ultrasonography showed a left adrenal cystic mass. At two weeks of age, she was admitted due to a massive abdominal distension and tachypnea. Percutaneous ultrasonography guided biopsy of the left adrenal mass was performed. The result of the biopsy was neuroblastoma. Vincristine and cyclophosphamide were administerd intravenously and 450 cGy of irradiation was added. Left adrenalectomy was accomplished and postoperative course was uneventful. The patient received cancer chemotherapy with a combination of carboplatin, ifosfamide and VP-16 and is now being followed up for three months. We have experienced a case of congenital bilateral neuroblastoma and report the case with brief review of related literatures.

Efficacy of Unilateral Laminectomy for Bilateral Decompression in Elderly Lumbar Spinal Stenosis

  • Objective: The aim of our study is to evaluate the effectiveness of unilateral hemilaminectomy for bilateral decompression in elderly patients with degenerative spinal stenosis. For this purpose, we studied the co-morbid condition and clinical outcome of patients who underwent decompressive surgery using the unilateral approach technique. Methods: Thirty-four patients over 65years of age who underwent unilateral partial laminectomy for bilateral decompression from January 2000 to October 2003 were analyzed. These patients were studied for preoperative co-morbid condition and physical status according to the American Society of Anesthesiologists(ASA) classification, postoperative morphometrical change, and clinical outcomes, including visual analogue scale(VAS) score. The mean follow-up was 23months (range 6 - 48months). Results: A patient's physical status was recorded as class I, II, or III by ASA classification, which correlated to 41.2%, 44.1%, and 14.7% of patients, respectively. The cross-sectional area of the pre- and postoperative dural sac at the level of the stenosis was $52.5{\pm}19.9mm^2$ and $110.6{\pm}18.2mm^2$, respectively. The outcome was excellent in 8.8%, good in 58.8%, fair in 23.6%, and poor in 8.8% of the patients. The VAS was changed postoperatively to $3.1{\pm}1.2$. There was no operation-related transfusion yet there was no evidence of postoperative instability at the follow-up examination. Conclusion: Unilateral laminectomy for bilateral decompression, in spite of the limited exposure, can result in satisfactory decompression of the lumbar spinal stenosis and tolerable clinical outcome. This approach is thought to be appropriate for elderly patients who have a greater surgical burden.

Up-Sampling Method of Depth Map Using Weighted Joint Bilateral Filter (가중치 결합 양방향 필터를 이용한 깊이 지도의 업샘플링 방법)

  • A depth map is an image which contains 3D distance information. Generally, it is difficult to acquire a high resolution (HD), noise-removed, good quality depth map directly from the camera. Therefore, many researches have been focused on acquisition of the high resolution and the good quality depth map by up-sampling and pre/post image processing of the low resolution depth map. However, many researches are lack of effective up-sampling for the edge region which has huge impact on image perceptual-quality. In this paper, we propose an up-sampling method, based on joint bilateral filter, which improves up-sampling of the edge region and visual quality of synthetic images by adopting different weights for the edge parts that is sensitive to human perception characteristics. The proposed method has gains in terms of PSNR and subjective video quality compared to previous researches.

  • Cryptococcosis is a systemic infection caused by the yeast-like fungus Cryptococcus neoformans. Respiratory tract is the usual portal of infection but relatively few cases of pulmonary cryptococcosis have been reported. Recently the incidence of the disease are increasing because the perception to seek the disease is increasing and immunocompromised hosts are more widespread with introduction of immune-suppressants, steroids and advent of AIDS. Because of nonspecific pattern of clinical manifestation, radiologic findings, differentiating the other diseases is essential and it needs fungal culture and biopsy. We describe a patient in the state of bilateral adrenalectomy due to bilateral ACTH-independent macronodular hyperplasia who developed pulmonary cryptococcosis and was later infected with pulmonary tuberculosis.

Smartphone Based Retouching Method for Watercolor Painting Effect Using Mean Shift Segmentation (Mean Shift Segmentation을 이용한 스마트폰 기반의 수채화 효과 변환 기법)

  • We propose a retouching method that converts a photography taken by smartphone to a watercolor painting image using bilateral filtering and mean shift segmentation which are mostly used in image processing. The first step is to convert an input image to fit the screen resolution of smartphone. And next step is to weaken high frequency components of the image, while preserving the edge of image using the bilateral filtering. And after that we perform mean shift segmentation from the bilateral filtered image. We apply parameters of mean shift segmentation considering the processing speed of smartphone. Experimental result shows that our method can be applied to various types of image and bring better result.

  • Bilateral sagittal split ramus osteotomy(BSSRO) of the mandible is an essential and commonly used procedure to correct dentofacial deformities and malocclusion. The possible complications associated with BSSRO include inferior alveolar nerve injury, bleeding, temporomandibular disorder, unfavorable fractures, and clinical relapse. The incidence of facial nerve palsy after orthognathic surgery recently reported is 0.1%. The probable etiologies have included facial nerve compression, complete or incomplete nerve transection, nerve traction, and nerve ischemia from anesthetic injection. Postoperative facial palsy is one of the most serious complications because it reduces the quality of life and significantly reduces social interaction. The case of a 24-year-old patient who underwent bilateral sagittal split ramus osteotomy is described. The medical records and postoperative photographs were reviewed in detail to collect information on the clinical course, treatment, and outcomes.

The Effect of Using Smart-Phones on Neck and Shoulder Muscle Activities and Fatigue (핸드폰 사용이 목과 어깨 근육의 근육활성도와 피로도에 미치는 영향)

  • The smart-phone has become a necessity for most people. In this study, we determined that using a smart-phone for 20 minutes can cause increased neck and shoulder muscle activities and fatigue. Seventeen healthy male smart-phone users who attended Yonsei University played a smart-phone game for 20 minutes and changes in their bilateral cervical erector spinae and upper trapezius muscle activities and fatigue were measured. To assess muscle activities and fatigue, we used the following variables: the median frequency, the 50th percentile Amplitude probability distribution function (APDF) value (median load), and the discrepancy in the 90th percentile APDF value and the 10th percentile APDF value (APDF range). A paired t-test was used to compare pre-smart-phone-use status with post-smart-phone-use-status. The median frequency of the bilateral cervical erector spinae and the upper trapezius decreased significantly after 20 minutes of smart-phone use (p<.05). In addition, the 50th percentile APDF value of the bilateral cervical erector spinae and the right upper trapezius increased significantly (p<.05). The APDF range of the bilateral cervical erector spinae and the upper trapezius also increased significantly (p<.05). However, the 50th percentile APDF range of the left upper trapezius was not significantly different (p>.05). These findings suggest that using smart-phones for 20 minutes can induce muscle fatigue and increased neck and shoulder muscle activities.

Video De-noising Using Adaptive Temporal and Spatial Filter Based on Mean Square Error Estimation (MSE 추정에 기반한 적응적인 시간적 공간적 비디오 디노이징 필터)

  • In this paper, an adaptive temporal and spatial filter (ATSF) based on mean square error (MSE) estimation is proposed. ATSF is a block based de-noising algorithm. Each noisy block is selectively filtered by a temporal filter or a spatial filter. Multi-hypothesis motion compensated filter (MHMCF) and bilateral filter are chosen as the temporal filter and the spatial filter, respectively. Although there is no original video, we mathematically derivate a formular to estimate the real MSE between a block de-noised by MHMCF and its original block and a linear model is proposed to estimate the real MSE between a block de-noised by bilateral filter and its original block. Finally, each noisy block is processed by the filter with a smaller estimated MSE. Simulation results show that our proposed algorithm achieves substantial improvements in terms of both visual quality and PSNR as compared with the conventional de-noising algorithms.

The Effects of Modified Constraint-Induced Movement Therapy and Bilateral Arm Training on the Upper Extremity Performance of Individuals with Chronic Hemiparetic Stroke (수정된 강제-유도운동치료와 양측성 상지훈련이 만성 뇌졸중 환자의 상지 수행 능력에 미치는 영향)

  • Purpose: The intention of this study was to investigate the effects of modified constraint-induced movement therapy (mCIMT) with bilateral arm training (BAT) on the motor performance and daily activity performance of individuals with chronic hemiparetic stroke. Methods: Sixteen subjects one year after stroke participated in this study with a control group; the pretest-posttest method was used. The subjects were randomly allocated into two groups: combination of bilateral arm training and modified constraint-induced movement therapy (n=8), and modified constraint-induced movement therapy (n=8). The mCIMT group received therapy for 90 minutes in 3 sessions per week over a period of 4 weeks. The patients receiving a combination of mCIMT and BAT were treated for the same period and frequency. The results were evaluated using the Fugl-Meyer Assessment, Action Research Arm Test (ARAT), and Motor Activity Log-Amount of Use, and Quality of Movement (MAL-AOU, QOM) assessment tools. Results: The Fugl-Meyer Assessment showed that hand and wrist performance improved significantly more in the mCIMT group than in the Combination group (p<0.05). Result from the ARAT assessment showed greater scores for gross movement in the combined group than in the mCIMT group (p<0.05). The MAL-AOU showed that there was greater improvement in the combined group than in the mCIMT group (p<0.05). Conclusion: The forced use of the more affected side can be important for the enhancement of upper extremity performance for chronic hemiparetic stroke patients during their daily activities.
