Case Report of Chemotherapy-Induced Peripheral Neuropathy Treated with Korean Medicine (항암화학요법 유발 말초신경병증에 대한 한방치험 1례)

  • Introduction: The aim of this study is to report on the effectiveness of Korean medicine to improve symptoms of chemotherapy-induced peripheral neuropathy (CIPN). Some patients are treated with medication, but medications may have no effect in some patients. Korean medicine treatments have been used in such patients, but few reports exist. Case Presentation: A 58-year-old female patient with CIPN reported symptoms of bilateral limb pain and tingling sensations as well as cold sensations in bilateral lower legs and hands, fatigue, and bilateral limb weakness. To reduce the patient's symptoms, we provided Korean medical treatment, including herbal medicine (Uchashinki-hwan), acupuncture, and moxibation. To evaluate the results of this treatment, we used a numeric rating scale (NRS), the manual muscle test (MMT), the National Cancer Institute Common Toxicity Criteria for Adverse Events (CTCAE), and the European Organization for Research and Treatment of Cancer quality-of-life questionnaire about CIPN (EORTC-QLQ-CIPN20). After 49 days of treatment, the NRS scores decreased for bilateral limb pain and tingling, for bilateral lower leg and hand cold sensation, and for fatigue. The MMT results for the upper and lower extremities were improved. CTCAE Grade and EORTC QLQ-CIPN20 score were also decreased. Conclusion: According to these results, Korean medicine treatment may be considered an effective treatment for CIPN. Prospective studies are needed in the future to confirm and expand these findings.

  • A localized tenosynovial giant cell tumor rarely affects the bilateral knee joint compared to the diffuse form. Only a few cases can be found in the literature, and there is no case report of the bilateral localized form of giant cell tumor causing patellofemoral pain syndrome in Korea. This study experienced a case of bilateral localized giant cell tumor, causing patellofemoral pain syndrome. The tumor was excised arthroscopically and confirmed pathologically. This paper reports this case with a review of the relevant literature.

Effect of a PNF Intervention Strategy with the ICF Tool Applied to a Patient with Bilateral Total Hip Replacement Walking a Crosswalk (양측 엉덩관절 전치환술 환자의 횡단보도 걷기 개선을 위해 ICF Tool을 적용한 PNF 중재전략: 사례보고 )

  • PURPOSE: This study aimed to utilize the International Classification of Functioning, Disability, and Health (ICF) tool to identify a problem list and explore intervention effects using proprioceptive neuromuscular facilitation (PNF) for improving the crosswalk performance of patients who have undergone a bilateral hip arthroplasty. METHODS: The subject of this study was a 43-year-old male who had undergone a bilateral hip arthroplasty. To address the subject's functional status, a clinical decision-making process was carried out in the order of examination, evaluation, diagnosis, prognosis, intervention, and outcome. Patient information during the examination was collected using the ICF core set. The evaluation involved listing the items of each problem using the ICF assessment sheet and identifying the interaction between activity limitations and the impairment level. The diagnosis explicitly described the causal relationships derived from the evaluation using ICF terminology. The prognosis presented activity goals, body function, and structured goals in terms of the activity and participation levels that needed to be achieved for an individual's functional status. The intervention approached problems through the four components of the PNF philosophy, namely basic principles and procedures, techniques, and patterns, in an indirect-direct-task sequence. Results were compared before and after the intervention using the ICF evaluation display. RESULTS: The results of the study showed that the primary activity limitation, which was the walking time across the crosswalk, showed improvement, and the trunk's counter rotation and the weight-bearing capacity of both the lower limbs, which were impairment level indicators, were enhanced. CONCLUSION: This study suggests that PNF intervention strategies will serve as a positive approach for improving crosswalk walking in patients with bilateral hip arthroplasty.

Electroencephalography for Early Detection of Alzheimer's Disease in Subjective Cognitive Decline

  • Background and Purpose: Early detection of subjective cognitive decline (SCD) due to Alzheimer's disease (AD) is important for clinical research and effective prevention and management. This study examined if quantitative electroencephalography (qEEG) could be used for early detection of AD in SCD. Methods: Participants with SCD from 6 dementia clinics in Korea were enrolled. 18F-florbetaben brain amyloid positron emission tomography (PET) was conducted for all the participants. qEEG was performed to measure power spectrum and source cortical activity. Results: The present study included 95 participants aged over 65 years, including 26 amyloid PET (+) and 69 amyloid PET (-). In participants with amyloid PET (+), relative power at delta band was higher in frontal (p=0.025), parietal (p=0.005), and occipital (p=0.022) areas even after adjusting for age, sex, and education. Source activities of alpha 1 band were significantly decreased in the bilateral fusiform and inferior temporal areas, whereas those of delta band were increased in the bilateral cuneus, pericalcarine, lingual, lateral occipital, precuneus, posterior cingulate, and isthmus areas. There were increased connections between bilateral precuneus areas but decreased connections between left rostral middle frontal area and bilateral frontal poles at delta band in participants with amyloid PET (+) showed. At alpha 1 band, there were decreased connections between bilateral entorhinal areas after adjusting for covariates. Conclusions: SCD participants with amyloid PET (+) showed increased delta and decreased alpha 1 activity. qEEG is a potential means for predicting amyloid pathology in SCD. Further longitudinal studies are needed to confirm these findings.

  • We had expierienced pulmonary lymphangioleiomyomatosis(LAM) with bilateral chylothorax and chylous ascites. A twenty-one-year-old lass with chief complaint of abdominal pain was admitted through the emergency room. She received emergent pelvicoscopic surgery for the rupture of the right corpus luteum. We aspirated 1000ml of the uncoagulated blood. The bleeding point was cautherized electrically. LAM was diagnosed with tissue from the retroperitoneum. Chylous ascites and bilateral chylothorax were occurred despite of various treatments. On thoracotomy, bullous changed lung and lymphatic leakage from visceral and parietal pleura were observed. She died of respiratory insufficiency and general weakness after 6 months from admission.

  • In neuroscientific research, image segmentation is one of the most important processes. The morphology of axons plays an important role for researchers seeking to understand axonal functions and connectivity. In this study, we evaluated the level set segmentation method for neuronal axons in a Brainbow confocal microscopy image. We first obtained a reconstructed image on an x-z plane. Then, for preprocessing, we also applied two methods: anisotropic diffusion filtering and bilateral filtering. Finally, we performed image segmentation using the level set method with three different approaches. The accuracy of segmentation for each case was evaluated in diverse ways. In our experiment, the combination of bilateral filtering with the level set method provided the best result. Consequently, we confirmed reasonable results with our approach; we believe that our method has great potential if successfully combined with other research findings.

  • Color enhancement basically deals with color manipulation in digital images. Recently, the technique has become widely used as a result of the increasing use of digital cameras. Retinex-based colorenhancement algorithms are a popular technique. In this paper, retinex with bilateral filtering is proposed to improve the quality of poorly illuminated images. Generally, it consists of three main steps: first, a retinex-based algorithm with color restoration; second, transformation mapping using histogram matching; and finally, smoothing the image using a bilateral filter. The experimental results demonstrate that the proposed method can successfully enhance image contrast while avoiding the halo effect and maintaining the color distribution in the image.

  • Bayesian based Multi-Frame Super-Resolution (MF-SR) has been used as a popular and effective SR model. On the other hand, the texture region is not reconstructed sufficiently because it works on the spatial domain. In this study, the MF-SR method was extended to operate on the frequency domain to improve HF information as much as possible. For this, a spatially weighted bilateral total variation model was proposed as a regularization term for a Bayesian estimation. The experimental results showed that the proposed method can recover the texture region more realistically with reduced noise, compared to conventional methods.

  • In an edentulous situation, the dentist must make several determinations when constructing artificial teeth. These include vertical and horizontal relationships of mandible with respect to the maxilla, occlusal form and position, vertical dimension, occlusal relationships during both centric closure and eccentric excursive movements. Artificial teeth are attached to a movable base resting on movable and displaceable living tissue subject to damage. They act as a unit; therefore, they must be arranged to function as a unit. Bilateral balanced occlusion is that stability of the denture is attained when bilateral contacts ex ist throughout all dynamic and static states of the denture during function. Lateral excursion in a balanced scheme implies simultaneous working side and nonworking side contact, while occlusal contacts are maintained on both anterior and posterior teeth as the mandible moves anteriorly into protrusion.

  • In this article, we report a rare case of a 22-year-old male with bilateral pulmonary sequestration, treated with embolization and surgical resection. The initial plan involved staged bilateral lobectomy for both lungs and prophylactic embolization of feeding vessels for preventing unexpected hemorrhage during operation. Symptomatic right lower lobe was resected with video-assisted thoracic surgery after embolization, and the patient refused surgery of left lower lobe upon symptomatic relief. The two-year follow-up examination revealed that the patient was healthy and had no relevant discomfort.
