담도암에서 $^{18}F-FDG$ PET의 임상 이용 (Clinical Application of $^{18}F-FDG$ PET in Bile Duct Cancer)

  • Reports about FDG PET in biliary tumor are limited and there are almost no reports regarding its efficacy. Biliary tumor is divided to intrahepatic and extrahepatic bile duct cancer, and intrahepatic bile duct cancer can be further divided to peripheral type which occurs at lobular duct and hilar type which occurs at hepatic hilum. Surgical resection is the only curative method for bile duct tumor, and accurate staging plays an important role in deciding treatment modality. Among intrahepatic bile duct tumors, peripheral type and hilar type have the same histological characteristics, but different clinical manifestations and tumor growth pattern. On PET image, FDG uptake is also different between peripheral type and hilar type. Most of the former shows high FDG uptake at primary and metastasis site so it is very useful for determining stage and changing treatment plans. However, the later is diversified among low uptake and very high uptake. The FDG uptake pattern of hilar type is similar to that of extrahepatic bile duct cancer, and mucinous component is an important factor, which affects FOG uptake. When tumor cells are scattered in desmoplatsic stroma, then FDG uptake is low as well. In contrast, when FDG uptake is high, it is likely to be tubular type which has high tumor density. Tumor growth pattern also affects FDG uptake. Nodular type mostly takes higher FDG compared to infiltrative type. There are many cases where benign inflammatory diseases take high FDG that PET alone can not distinguish malignant lesion from benign lesion. In conclusion, studies about PET using FDG are still limited. Thus, it is hard to make accurate conclusion about the roles of PET or PET/CT in biliary cancers, but peripheral type intrahepatic bile duct cancers and mass forming hilar and extrahepatic bile duct cancers appear to be good indications performing FDG PET or PET/CT.

횡격흉막에서 발생한 고립성 섬유성 종양 (Solitary Fibrous Tumor of the Diaphragmatic Pleura)

  • 우리는 단순 흉부 방사선 촬영에서 우연히 발견한 47세 여자의 횡격흉막에서 발생한 양성 고립성 섬유성 종양 1례를 보고한다. 단순 흉부 방사선 촬영, 복부 초음파 검사, 전산화 단층 촬영과 자기공명영상으로 주변과 경계가 분명한 다엽성의 흉막 섬유성 종양으로 진단하였다. 우측 개흉 수술시 종괴는 대부분 유착이 없었으나 $5{\times}4$cm 정도의 횡격막 중앙부와 유경으로 붙어 있었다. 붙어 있는 횡격막과 함께 종괴를 완전히 절제하였다. 종양은 $23.5{\times}13.5{\times}8.0$cm 크기였으며, 병리조직학 진단은 양성 섬유성 종양이었고 함께 절제된 횡격막에는 종양세포가 없었다.

체강삼출액의 진단에 있어서 p53 단백의 유용성 (Diagnostic Value of p53 Expression in the Evaluation of Effusions)

  • The diagnostic accuracy of routine cytological preparations from effusions ranges from 60% to 70%. Immunohistochemical markers, especially tumor-associated antigens, have been successfully employed to increase diagnostic sensitivity in effusion cytology. However, more than two different antibodies in diagnosis of effusions are needed. In the view of prevalence of abnormalities of p53 gene in human malignancies we investigated the diagnostic usefulness of demonstration of p53 protein immunoreactivity in distinguishing benign changes versus malignant processes in effusions. p53 protein expression was studied immunohistochemically in 76 effusions(28 malignant and 48 benign) using anti-human p53 antibody p53 immunoreactivity was identified in 19 of 28(67.9%) malignant effusions. In contrast, no p53 immunoreactivity was observed in all benign effusions. A specificity of 100% and a sensitivity of 67.9% were observed. These results suggest that immunohistochemical detection of p53 protein seems to be helpful in distinguishing benign changes versus malignant processes in effusions, although its principal limitation is its relatively low sensitivity.

  • Benign metastasizing leiomyoma(BML) is a rare condition affecting women with a history of having undergone hysterectomy or myomectomy for a benign uterine fibroid, that is found to have metastasized to extrauterine sites, usually many years after hysterectomy. 1,2) Patient with BML almost always asymptomatic, although if the lesions are large enough, they can cause compressive symptoms. Among several hypothesis of pathogenesis, most plausible theory is that these tumors represent a true metastatic lesion but are very low-grade sarcoma. 3) Because the tumor is responsive to estrogen, menopause and pregnancy have slowed the growth of these lesion 4) and it seems reasonable to perform hysterectomy in patients with a uterine mass and, at the same time, perform oophorectomy for hormonal control. BML is an unusual cause of diffuse pulmonary nodules which should be considered in females with unexplained nodules and a history of surgery for uterine leiomyoma.

Tumor Markers in Serum and Ascites in the Diagnosis of Benign and Malignant Ascites

  • Objective: To evaluate the values of 4 tumor markers in serum and ascites and their ascites/serum ratios in the identification and diagnosis of benign and malignant ascites. Materials and Methods: A total of 76 patients were selected as subjects and divided into malignant ascites group (45 cases) and benign ascites group (31 cases). Samples of ascites and serum of all hospitalized patients were collected before treatment. The levels of carcinoembryonic antigen (CEA), alpha fetoprotein (AFP), cancer antigen 125 (CA125) and carbohydrate antigen 19-9 (CA19-9) were detected by chemiluminescence (CLIA). Results: CEA, AFP and CA19-9 in both serum and ascites as well as CA125 in ascites were evidently higher in the malignant ascites group than in the benign ascites group (P<0.01). Malignant ascites was associated with elevated ascites/serum ratios for AFP and CA125 (P<0.01). The areas under receiver operating characteristic (AUROCs) of CEA and CA125 in ascites and the ratios of ascites/serum of AFP, CEA, CA125 and CA19-9 were all >0.7, suggesting certain values, while those of ascites CA19-9 and serum CEA were 0.697 and 0.629 respectively, indicating low accuracy in the identification and diagnosis of benign and malignant ascites. However, the AUROCs of the remaining indexes were <0.5, with no value for identification and diagnosis. Compared with single index, the sensitivity of combined detection increased significantly (P<0.05), in which the combined detection of CEA, CA19-9 and CA125 in ascites as well as the ratio of ascites/serum of CEA, CA19-9, CA125 and AFP had the highest sensitivity (98.4%) but with relevantly low specificity. Both sensitivity and specificity of combined detection should be comprehensively considered so as to choose the most appropriate index. Conclusions: Compared with single index, combined detection of tumor markers in serum and ascites can significantly improve the diagnostic sensitivity and specificity.

수근부 주상골의 연골모세포종 (증례 보고) (Chondroblastoma of the carpal scaphoid (A Case Report))

  • 연골모세포종은 장골의 골단에 호발하는 상대적으로 드문 양성 종양이다. 양성 연골 모세포종은 양성 골종양의 약 1% 정도를 차지한다. 특히 수근부의 연골모세포종은 매우 드문 것으로 알려져 있다. 이에 저자들은 소파술 및 혈관부착 골이식술로 치료한 수근부 주상골에 생긴 연골모세포종의 증례를 보고하는 바이다.

  • Fine needle aspiration cytology as a diagnostic workup of parotid gland tumor is a simple and useful method. Although fine needle aspiration cytology could not predict accurate diagnosis in all cases, it is usually helpful in differentiating malignancy and benign lesions. A 35-year-old female was found to have a parotid mass for 1 year. Preoperative evaluation including computed tomography and magnetic resonance imaging were non-diagnostic, but, fine needle aspiration cytology on parotid mass showed the suspicion of a low-grade mucoepidermoid carcinoma. Superficial parotidectomy and selective neck node dissection were done based on cytology. However, final pathological examination confirmed benign pleomorphic adenoma. Here, the diagnostic accuracy and cautions in interpretation of result of fine needle aspiration cytology is discussed with respect to the case.

  • 내시경을 이용한 기도 종양 절제술은 비침습적인 방법으로 개흉수술에 따른 각종 부작용을 감소할 수 있을 뿐만 아니라 폐실질을 보존할 수 있으며, 고령 등의 고위험군의 환자에서 안전하게 이용되고 있다. 기관내 종양(양성 지방종)과 기관지 내 종양(과오종)의 2예 환자에서 국소마취상태에서 굴곡형 기관지경직시하에 폴립제거용 기구와 전기소작술을 이용하여 완전절제를 시행하였다. 기관 내 지방 종환자는 술 후 6년째, 기관지내 과오종환자는 2년 6개월 째 재발없이 추적 중이다. 저자들의 방법이 환자에게 안전하며 좋은 시야에서 시술할 수 있는 장점이 있는 것으로 생각되었다.

  • 측두하악관절부위에 발생하는 양성 외방성 증식은 골종, 연골종, 골연골종 및 활액성 연골종증 등이 있으며 증상으로는 통증, 개구제한 등을 보이며 때때로 교합변화 하악정중선변위등을 수반한다. 이러한 양성 외방성 증식은 측두하악장애와 비슷한 증상으로 인해 임상 증상만으로 진단 하는 것은 어려우며, 단층촬영, 자기공명영상을 포함한 방사선 사진을 평가하여야 하며, 확진을 위해서는 생검을 필요로 한다. 활액성 연골종증과 골연골증은 인체의 장골에 흔한 양성 종양이며 구강안면부위에 있어 발생이 비교적 드물다. 본 연구는 측두하악관절에 비교적 드물게 발생하는 활액성 연골종증과 골연골종에 대하여 보고하고 진단에 대해 다소의 지견을 얻었기에 문헌 고찰과 함께 보고하고자 한다.

Benign neurilemmoma in the infratemporal fossa involving maxillary sinus and pterygopalatine fossa

  • Neurilemmoma is a benign tumor of the nerve sheath that arises on cranial and spinal nerve roots as well as along the course of peripheral nerves. A case of a neurilemmoma that arose in the left infratemporal fossa of a 29-year-old male was presented. Plain radiographs, enhanced computed tomography scan, and magnetic resonance imaging demonstrated a large, well-circumscribed, heterogeneously enhanced mass with extension into the pterygopalatine fossa. Displaced by the large mass, bowing-in of the posterior maxillary antral wall was noted and a provisional diagnosis of a benign soft tissue tumor was made. The mass was completely excised and a diagnosis of neurilemmoma was confirmed.
